1.Mechanism of the reversibility of chronic cyclosporine nephrotoxicity after drug withdrawal

Objective To explore the mechanism of the reversibility of chronic cyclosporine (CsA) nephrotoxicity in rats after long-term drug withdrawal. Methods Chronic CsA nephrotoxicity was induced in Sprague-Dawley rats by administering CsA(15 mg . kg-1. d-1) for 5 weeks,and then the effect of drug withdraw for 5 and 10 weeks was observed. Body weight,systolic blood pressure,and renal function were monitored. In addition,renal histopathology(arteriolopathy,ED-1-positive cells,and tubulointerstitial fibrosis) and expression of osteopontin(OPN) and transforming growth factor(TGF) -?1 mRNA was examined. Results Compared with the control rats,CsA-treated rats showed loss of body weight,deterioration in renal function and development of typical histopathology. All of these above parameters were significantly reversed,meanwhile,the upregulation of OPN and TGF-?1 mRNA expression decreased significantly at 5th and 10th week after CsA withdrawal. Of note,the decreased OPN and TGF-?1 mRNA expression was positively correlated with the reduction in the tubulointerstitial fibrosis score. Conclusion The established chronic CsA nephrotoxicity is reversible after long-term CsA discontinuation,and the mechanism may be associated with the down-regulation of OPN and TGF-?1 mRNA expression.
2.The clinical study on the relationship between mechanism of miRNA-126-3p and pulmonary hypertension

Objective To investigate the corelation between miRNA-126-3p and pathogenesis of pulmonary hypertension . Methods Totally 25 patients with congenital heart disease ,including 11 cases with pulmonary hypertension and 14 control cases , were recruited .The expression of miRNA-126-3p was detected by qRT-PCR ,and we used starBase to predict the biological informa-tion miRNA-126-3p ,and then verified it by the level of mRNA and protein .Results There was no statistical difference in age ,gen-der ,and biochemical examination(P> 0 .05) ;the expression of miRNA-126-3p in the pulmonary hypertension cases was significantly higher(P< 0 .01) ;starBase found that miRNA-126-3p may participate in binding protein ,signal transduction ,cell differentiation and regulation of cell morphology ,the regulation of MAPK and insulin receptor signaling pathways ,etc .Its target genes mainly contain VEGFA ,SPRED1 ,PIK3R2 ,etc ;the mRNA and protein level of VEGFA in PH group were statistically significantly than control group(P< 0 .01) ;the miRNA-126-3p and VEGFA showed positive correlation(P< 0 .01) .Conclusion miRNA-126-3p may partici-pate in the occurrence of pulmonary hypertension by regulating VEGFA .
3.Effect of preoperative prophylactic parenteral nutrition on prognosis of advanced gastric cancer

Objective To investigate the effect of preoperative prophylactic parenteral nutrition on prognosis of advanced gastric cancer.Methods A total of 159 patients with advanced gastric cancer in our hospital were collected,and randomly divided into experimental group and control group.80 cases in experimental group were treated with prophylactic parenteral nutrition,and 79 cases in control group were treated with traditional nutrition.The partial indexes of liver function,complications and mortality rate were compared between the two groups before and after surgery.Results The levels of aspartate aminotransferase (AST),alanine aminotransferase (ALT),total bilirubin (TBIL) and direct bilirubin (DBIL) were much lower in experimental group than in control group at 8 days after surgery (t=6.542,6.460,5.634,5.275,all P<0.001).The levels of ALB and PAB were much higher in experimental group than in control group at 2 and 8 days after surgery (t=12.580,12.471,6.757,12.821,all P<0.001).The incidence of complications and mortality rate were much lower in experimental group than in control group (x2 =23.409,4.818,P<0.001 or 0.05).Conclusions Preoperative prophylactic parenteral nutrition can speed up the recovery of liver function and reduce the complications and mortality rate after surgery in patients with advanced gastric cancer.
4.Identification of two peaks in HPLC chromatogram of Erigeron Injection

AIM:To study the chemical constituents of Erigeron Injection by HPLC. METHODS: Under the guidance of HPLC analysis,two constituents were separated and purified by the aid of column chromatography with silica gel,and identified by UV、 IR、 MS、 NMR respectively and documentary data. RESULTS: Two compounds from semi-finished products of Erigeron Injection were identified as caffeic acid(Ⅰ) and chlorogenic acid(Ⅱ). CONCLUSION: Two main peaks in HPLC chromatogram of Erigeron Injection are Ⅰ and Ⅱ.
5.Study on the regulatory effects of mechano growth factor on soft tissue repair.

Mechano growth factor (MGF) is an autocrine/paracrine factor and sensitive to mechanical stimulation. MGF can be highly expressed in various soft tissues under physical stimuli, biochemistry stimuli or in damaged situation. MGF may "compensate" the stress for tissue in the processing of tissue repair. MGF can effectively accelerate the repair of the soft tissue by promoting the proliferation, migration and differentiation of cells. This paper summarizes the MGF expressions in different soft tissues and their functions in soft tissue repair. The paper also discusses current problems and challenges in using MGF to repair the soft tissue.

6.Study on perinatal outcome of 178 fetuses with increased nnchal translucency

Objective To investigate the perinatal outcome of fetus with increased nuchal translucency (NT) at first trimester.Methods The thickness of NT above 95th percentile of the fetuses with same crown-rump length (CRL) was set as the criteria of increased NT.The outcomes of fetuses with increased NT during early pregnancy from Jan.2008 to Dec.2009 in Guangzhou Women and Children's Medical Center were followed up.The information of ultrasound at second trimester,pregnant complications and delivery outcome were collected.All infants were followed up for 3 months after birth and were divided into four groups according to their different thickness of NT.The relationship between NT thickness and perinatal outcome were analyzed with single factor analysis of variance and multiple comparison method.Results Among the 178 cases we followed up,there were 2 spontaneous fetal losses and 19 terminations whose reasons were Down syndrome (n=6),severe a thalassemia (n =5),fetal malformations (n =7) and social factor (n =1).Among the 157newborns delivered,one was found with congenital heart disease.The rate of abnormal infants was 11.8% (21/178) and the detection rate of abnormal infants was 9 5.2% (20/21).Healthy living rate of fetus with NT thickness between 95th percentile and 2.9 mm was 96.1% (122/127); 82.4% (28/34) for those with NT thickness between 3.0 mm and 3.9 mm; and 35.3% (6/17) when NT≥ 4.0 mm.Conclusions Increased NT might have close relationship with poor pregnant outcome.The thicker the NT,the lower the healthy living rate of the fetus.The pregnant outcome is very poor if NT≤4.0 mm.
7.One stage reconstruction of limb injuries caused by high energy in 55 patients

Objective To report the clinical results of one stage reconstruction of high-energyinjured limbs. Methods From January 2001 to July 2006,one stage reconstruction was done on 55patients suffered from massive limb trauma,severe open fractures or spoiled limbs caused by high energy.There included 16 patients with Gustilo Type-Ⅲ C fractures,with mangled extremity severity score (MESS)of(6.25 ±2.53)points.All patients were treated with transplantation of free flaps or pedicle skin flaps,fracture fixation,nerve reconstruction or functional reconstruction. Results All limbs were saved and all flaps were transplanted successfully.without protracted course to obtain soft tissue coverage.The hospitalization was mean 24 days(12-63 days)and follow-up ranged from 12 months to 50 months (averaged 23 months).Complications including wound infection,bone nonunion,necrosis and morbidity were significanfly less likely to appear than conventional ways and the ultimate functional results of the saved limbs were at least better than that of artificial ones. Conclusions In comparison with traditional methods,one stage reconstruction of severe high energy-injured limbs has advantages of easier justification of tissue structure,better match of donor and recipient blood vessels,easier operation design,shorter hospitalization and better functional recovery.Definitive wound closure with vascularised tissue transfer can facilitate local blood supply.decrease bone infection and hence promote bone healing.
8.Consistency and stability analysis of two types of the pubococcygeal line in evaluation of the anterior and apical pelvic organ prolapse with dynamic MRI

Objective To investigate the consistency and stability of two types of pubococcygeal line (PCL) determined by dynamic MRI used in evaluating pelvic organ prolapse (anterior and apical compartments).The first type of PCL was measured from the inferior pubic symphysis to the tip of coccyx (PCLtip) and the second was to the sacrococcygeal joint (PCLjnt).Methods Dynamic MRI changes of 50 female patients who were diagnosed with pelvic organ prolapse by pelvic organ prolapse quantification were retrospective reviewed.Chi-square test was used to compare the staging of each pelvic compartment (anterior,apical) with the two PCLs.The lengths and the degree of the oblique angle of the two PCLs during the rest and straining were compared using a paried t test.Results Agreement of PCLjnt with PCLtip was 96% (48/50) for anterior compartment and 94% (47/50) for apical compartment.There was no difference between the two PCLs in staging of each pelvic compartment (anterior,apical)(x2 values were 2.000 and 3.000,P values were 0.368 and 0.223).The length of the PCLtip at rest and straining was (10.1±0.8),(10.2± 0.8) cm respectively and the result was statistical significance (t=-2.339,P=0.023).Twenty patients (40%) in the 50 pelvic organ prolapse patients demonstrated a shortening of the PCLtip,while the rest including 30 patients (60%) was longer.The oblique angle of the PCLtip at rest and straining was 22°±6° and 18°±11° respectively(t=3.490,P=0.001).The length of the PCLjnt at rest and straining were (11.2±0.8) and (11.2± 0.8)cm respectively(t=-1.845,P=0.071).The oblique angle of the PCLjnt at rest and straining were 29°±6° and 26°± 10° (t=2.836,P=0.007),but the degree of PCLjnt's oblique angle had a mild fluctuate compared with the PCLtip.Conclusions PCLjnt and PCLtip have the equal level in staging of anterior and apical pelvic organ prolapse.Meanwhile the oblique angle and the length of PCLjnt illustrated the better the stability.
9.Effect of Notch1 gene expression on the proliferation and apoptosis of T-cell acute lymphoblastic leukemia SupT1 cells and the mechanism

Objective To explore the relationship between T-cell acute lymphoblastic leukemia and the Notch signaling pathway.Methods Human T-cell acute lymphoblastic leukemia SupT1 cells were infected with the lentiviral vector made up specific Notch1-shRNA gene and nonspecific Notch1-shRNA gene.The inhibitive rate of SupT1 cells was detected by CCK-8.The rates of early apoptotic cells (Annexin V+/ 7-AAD-) and late apoptotic cells (Annexin V+/7-AAD+) were analyzed by flow cytometry and the expression levels of Notch1 receptor gene and downstream target genes were assessed by quantitative reverse transcription and polymerase chain reaction (QT-PCR).Results The cell inhibition rates of Notch1 interference group,control group and empty vector group at 96 h were 0.902±0.013,0,and 0.486±0.084,respectively,and it was increased obviously in Notch1 interference group (both P < 0.05).The cell early apoptosis rates of the three groups were (15.27±0.31) %,(5.57±0.25) %,(5.80±0.20) %,respectively.The cell early apoptosis rate of Notch1 interference group was increased obviously compared with the control group and empty vector group (both P < 0.05).While the cell late apoptosis rates had no significant difference among the three groups (P > 0.05).The mRNA expression levels of Notch1 receptor gene and its target genes (Hesl,c-myc,NF-κB) at 48 h,72 h and 96 h were higher than those in the control group and empty vector group (all P < 0.05).Conclusions The specificity of Notch1-shRNA can effectively decrease the Notch1 mRNA expression,and reduce the expression level of downstream target genes.Notch1 cut can inhibit the proliferation of SupT1 cells,and promote the early apoptosis.
10.Nasal intermittent positive pressure ventilation versus after extubation:a retrospective analysis

Objective To compare the curative effect of nasal intermittent positive pressure ventilation (nIPPV ) and nasal con-tinuous positive airway pressure (nCPAP) method .Methods Retrospective study was took on 196 neonatal infants ,all cases were assigned into nIPPV group (n=102) and nCPAP group (n=94) according to mode of ventilation from January 2011 to December 2012 in a neonatal intensive care unit of this hospital .The ratio of requirement for endotracheal ventilation and the outcome were in-vestigated .Results Infants treated initially with nIPPV need less endotracheal ventilation and ventilation time than those treated with nCPAP (11 .8% vs .24 .5% ,P<0 .05) .Conclusion Compared with nCPAP ,nIPPV decreased the requirement for endotrache-al ventilation in neonatal infants after extubation .
