Objective Discuss recovering guidance of effect of coronary heart disease and angina Patients. Methods 120 patients with coronary heart disease and angina were randomly selected for recover self health at home recovering guidance. The process includes data collection, evaluation, action and analyzing the feedback of the home practice in the recovering guidance application within two years when the patient is out of the ward. Results Recover self health at home Recovering Guidance for coronary heart disease and angina patients is effective on preventing and reducing the disease relapse, the neopathy of the brain, kidney,liver and mortality.Conclusions Recover self health at home Recevering Guidance for coronary heart disease and angina patients could help to short patient’s in-ward period, speed in and out ward circulation, relieve his distress and treatment cost of the patients, strengthen the communication between the patient and nurses. It is also a way to improve the comprehensive nursing ability of the nurses.

Objective To assess the value of joint detection of serum cysteine proteinase inhibitors C (sCys-C),urinary kidney injury molecule 1 (uKIM-1),urinary neutrophil gelatinase-associated lipocalin(uNGAL) and urinary interleukin 18 (uIL-18) for early diagnosis of acute kidney injury (AKI) in critically ill patients.Methods A total of 256 adult patients who stayed Intensive Care Unit for 24 hours in the Third People's Hospital of Liaocheng between Aug 2011 and Dec 2012 were enrolled.According to Kidney Injury Net(AKIN) work,the patients were divided into non-AKI group and AKI group (including state 1,2 and 3).The concentrations of urine NGAL,KIM-1,IL-18 and serum sCys-C were measured.The diagnosis value of four biomarkers joint detection and single detection for AKI were analyzed with the receiver operating characteristic (ROC) curve and the area under curve (AUC).Results (1) The levels of uNGAL,uKIM-1,uIL-18 and sCys-C were higher in patients with AKI than the patients with no AKI (P ＜ 0.01).(2) The area under curves of uNGAL,uKIM-1,uIL-18,sCys-C and joint detection were 0.742,0.871,0.803,0.703,0.925 respectively.(3) The sensitivity and specificity of parallel tests and serial tests of four biomarkers were 97.9％,62.8％,64.3％ and 96.2％ respectively.There were significant differences of sensitivity or specificity between single test and joint tests.Conclusions The urine NGAL,KIM-1,IL-18 and serum Cys-C are sensitive indexes for the early diagnosis of acute kidney injury.Joint detection has high value for early diagnosis of AKI.

Objective To compare the influence of different end-tidal concentrations of sevoflurane on transcranial electrical four-limb muscle motor evoked potential (MEP) monitoring. Methods Twenty ASA Ⅰ-Ⅱpatients aged 23-62 years undergoing craniotomy were enrolled. Triangular muscle, biceps brachii muscle, triceps brachii muscle, brachioradialis muscle, extensor digitorum communis muscle, abductor pollicis brevis abductor digiti minimi muscle, rectus femoris muscle, tibialis anterior muscle, gastrocnemius muscle and abductor hallucis were selected for MEPs recording. Sevoflurane was introduced at 0.5, 0.75, 1.0 and then 1.3 MAC (15 min each), and the effects on MEPs were studied. Results ①Maximum MEP amplitude was observed at abductor pollicis brevis muscle in upper limb and abductor hallucis muscle in lower limb at baseline and 0.5 MAC. Up to 1.0 MAC, there was no significant difference in MEP amplitude among extensor digitorum communis muscle, abductor pollicis brevis and abductor digiti minimi muscle. ②The success rate of MEP recording from abductor pollicis brevis muscle and abductor hallucis muscle was 100% during the administration of 0.5-1.0 MAC sevoflurane. ③The MEP amplitude was decreased and the latency was increased in a sevoflurane dose-dependent manner. Conclusions Abductor pollicis brevis muscle and abductor hallucis muscle were suitable for MEP monitoring during the administration of 0.5-1.0 MAC sevoflurane.

Objective To compare the effects of sevoflurane,isoflurane and desflurane on transcranial electrical motor evoked potentials(MEPs)in patients undergoing neurosurgery.Methods Sixty ASA Ⅰ or Ⅱ patients aged 18-64 yr undergoing neurosurgery were randomly divided into 3 groups(n = 20 each): sevoflurane group,isoflurane group and desflurane group.BIS value and MEPs were monitored.The end-tidal concentrations of sevoflurane,isoflurane and desflurane were adjusted and maintained at 0.50,0.75,1.00 and 1.30 MAC respectively for at least 15 min during 6 stimuli delivered at 1000 Hz and 300 V lasting for 75 μs.The amplitudes and latency of MEPs and BIS value were recorded before administration(baseline)and the each stable state (T1-4).The failure rate of MEPs was also recorded.Results The amplitude of MEPs and BIS value were significantly decreased at T1.2 and the latency of MEPs was prolonged at T1-4 in desflurane group compared with sevoflurane and isoflurane groups(P＜0.05).There was no significant difference in the amplitudes and latency of MEPs and BIS value between group sevoflurane and isoflurane(P＞0.05).The failure rates of MEPs were 0 at baseline,T1 and T2,0,5% and 20% at T3 ,and 5%,20% and 45% at T4 in sevoflurane,isoflurane and desflurane groups respectively.The failure rate of MEPs was significantly higher in desflurane group than in sevoflurane and isoflurane groups(P＜0.05).Conclusion Desflurane has greater suppressive effect on MEPs than sevoflurane and isoflurane.1.00 MAC of sevoflurane and isoflurane,while 0.75-1.00 MAC of desflurane may be the suitable end-tidal concentration for MEP monitoring.

Objective To investigate the serum procalcitonin (PCT) levels in sepsis caused by the bacteria,virus and mycoplasma and explore the role of PCT in etiology diagnosis of sepsis in children.Methods Three hundreds and thirty critically ill children with sepsis caused by bacteria,virus and mycoplasma admitted in PICU of Hunan Children' s Hospital from Feb 1,2011 to Sep 1,2012 were reviewed and analyzed.The PCT levels were measured at admission and day 3.The differences in accidence of sepsis caused by bacteria,viruses and mycoplasma according to different serum PCT levels were analyzed.The differences of PCT levels at admission and day 3 in sepsic children caused by bacteria,viruses and mycoplasma were analyzed.Results The level of serum PCT in sepsis caused by bacterial infection were distinctly increased,caused by virus and mycoplasma infections was not obvious but the increases of serum PCT [0.71 (8.14)ng/ml,0.15 (1.68) ng/ml,0.28 (1.89) ng/ml].According to various PCT levels(0.05 ～ ng/ml,0.5 ～ng/ml,2 ～ ng/ml,10 ～ 300 ng/ml),the differences of accidence of sepsis caused by bacteria,virus and mycoplasma were also statistically significant(x2 =84.50,P ＜ 0.01).The PCT level of septic children caused by bacterial infection in day 3 was significantly decreased compared with that at admission [0.32 (5.68) ng/ml vs 0.71 (8.14) ng/ml] (U =19.34,P ＜0.05).Conclusion PCT plays a certain role in etiology diagnosis of sepsis in children.The increased PCT levels which can be reduced by anti-inflammatory treatment indicate the likelihood of bacterial infection and sepsis.The increase of PCT induced by viral and mycoplasma infections is not obvious,but bacterial infection can not be completely ruled out.

Objective To evaluate the ability of two sets parameters of the volome rendering technich(VRT)with multislice spiral CT(MSCT)to display the normal inner ear.Methods Fifty-eight inner ears of twenty-nine healthy volunteers were scanned and reconstructed with VRT.The images of VRT were recombinated with two sets(A,B)of parameters.Results The semicirculares、cochlea,vestibule,fenestra cochleae and vestibuli of inner ear could be visualized clearly by VRT images with"A"group's parameters.The vestibuli was displayed unsharply by VRT images with"B" group's parameters.Conclusions The double-threshold segmentation matching with manuleditor of VRT was successfully used to separate the inner ear from the temporal bone.And the semicirculares,cochiea,vestibule,fenestra cochleae and vestibuli of inner ear could be displayed clearly by VRT images of"A"group's parameters(threshold:256～765,935～1019,brightness:42,opacity:70).

Objective To explore the reasonable dose of hydromorphone hydrochloride in the elderly patients after hip surgery. Methods 120 cases of elderly patients who underwent total hip arthroplasty in Affiliated Hospital of Jiangsu University from April 2016 to July 2016 were divided into six groups randomely,with 20 cases each group,they were 0.020,0.035,0.050,0.065,0.080 and 0.095mg/mL group,the corresponding concentrations of HYD were given to each group after surgery,blood rheology, inflammatory cytokine, VAS and Ramsay sedation scores were observed at different time points, Complications were recorded. Results One day and three days after operation,whole blood viscosity and whole blood reduced viscosity level of six groups were significantly increased (P<0.05),with the increase of the concentration of HYD,whole blood viscosity and whole blood reducing viscosity of each group decreased,compared with 0.020-0.050 mg/mL group,the 0.065-0.095 mg/mL group was lower (P<0.05).One day and three days after operation,the levels of inflammatory cytokines were increased in six groups (P<0.05),with the increase of the concentration of HYD,the level of inflammatory factors decreased,compared with 0.020-0.050 mg/mL group,the 0.065-0.095 mg/mL group was lower (P<0.05).The VAS scores of six groups were decreased in different degree with the prolongation of time, with the increase of the concentration of HYD,VAS at different time after operation decreased to some extent (P<0.05),compared with 0.020-0.035 mg/mL group,the 0.050-0.095 mg/mL group was lower (P<0.05). Ramsay sedation scores increased in 0.020-0.035mg/mL group and decreased in 0.065-0.095mg/mL group,but no significant change in 0.050mg/mL group. The incidence of adverse reactions was the lowest in the 0.050 mg/mL group. Conclusion HYD can be used for postoperative analgesia in elderly patients undergoing hip joint surgery,the concentration of HYD was correlated with the analgesic efficiency, and the recommended concentration was 0.050 mg/mL.

Aim To study the pharmacokinetics of curcumin-pvp solid dispersion ig administration in mice in comparison with the free curcumin suspension.Methods Drugs were administered at a dose of 300 mg?kg-1 via ig.The plasma concentration of curcumin was determined by HPLC,the pharmacokinetics were calculated by DAS ver1.0 program.Results The curcumin pharmacokinetics conforms to a two-compartment open model after a single ig dose of curcumin solid dispersion in mice.The parameters were as follows:T12? and T12? were 16.4 and 266.1 min,respectively.AUC was 89.6 mg?L-1?min-1,Vd was 763.9 L,V1 was 51.0 L,and CL was 1.99 L?min-1.The absorption rate of curcumin solid dispersion was 6.75 times as much as curcumin suspension.Conclusion The curcumin solid dispersion improves the absorption of curcumin in vivo in mice evidently.
