1.Investigation and thinking about TCM interfering cardiovascular risk factors in obese T2DM

Objective:To approach the status quo and characteristics of traditional Chinese medicine(TCM) in intefering the related cardiovascular risk factors in obese type 2 diabetes mellitus(T2DM).Methods:The recent 10 years documents about nitric oxide(NO),endothelin(ET),C reactive protein(CRP),and etc.,eight cytokines which are involved in T2DM and its cardiovascular complications were retrieved and statistically analyzed and all related TCM were pykno-analyzed.Results:① CRP was the focal point at home and abroad,adiponectin(APN) was the hot spot in recent years.② The documents of ET at home was greatly more than that abroad,it may hint some diversity between different races.③ The related TCM were mainly characterized by cleaning heat,enriching yin and qi,activating blood circulation and removing stasis.④ ET and NO were the hot spots in the TCM fi eld.Conclusion:TCM had certain effect on the cardiovascular risk factors above in obese T2DM.
2.Effect of sevoflurane on hippocampal CaMK Ⅱ/CREB signaling pathway in aged rats

Objective To evaluate the effect of sevoflurane on hippocampal calcium/calmodulindependent protein kinase Ⅱ (CaMK Ⅱ)/cyclic adenosine monophosphate response element-binding protein (CREB) signaling pathway in aged rats.Methods Sixty pathogen-free healthy male Sprague-Dawley rats,aged 18 months,weighing 600-750 g,were divided into 2 groups (n=30 each) using a random number table:control group (group C) and sevoflurane group (group Sev).Group Sev inhaled 2% sevoflurane in the mixture of 50% air and oxygen (2 L/min) for 4 h.Group C inhaled the mixture of 50% air and oxygen (2 L/min) for 4 h.Morris water maze test was performed on 6 days before anesthesia and 1 day after anesthesia.The escape latency,swimming distance,frequency of crossing the original platform and time of staying at the platform quadrant Ⅱ were recorded.On 1,3 and 7 days after anesthesia,the rats were sacrificed,and the hippocampus was obtained for determination of the expression of CaMK Ⅱ,phosphorylated CaMK Ⅱ,CREB and phosphorylated CREB by Western blot.Results Compared with group C,the escape latency and swimming distance were significantly prolonged,the frequency of crossing the original platform was decreased,and the time of staying at the platform quadrant Ⅱ was reduced on 5th day of training and 1 day after anesthesia,and the expression of CaMK Ⅱ,phosphorylated CaMK Ⅱ,CREB and phosphorylated CREB was down-regulated after anesthesia in group Sev (P< 0.05).Conclusion Sevoflurane leads to cognitive decline through inhibiting hippocampal CaMK Ⅱ/CREB signaling pathway in aged rats.
3.MR angiography study on morphology of the Willis' circle in healthy Chinese male adults with variation of A1 segment of anterior cerebral artery

Objective To investigate the variations of multi-segments of the Willis' circle in cases with ACA-A1 variation.Methods Retrospective comparative analysis was applied to images of 2 246 healthy cases which were excluded from abnomalities of physical and biochemistry examination and angiostenosis caused by angiosclerosis.We divided variant group and contrast group according to variation of ACA-A1 segment.Several aspects were carried to comparison:the variation between bilateral ACA-A1 segment within the variant group was studied by x2 test; the proportion of FTP and developing of PCA-P1 were put into test.Finally we use trend x2 test to compare the variation degree of ACA-A1,ipsilateral FTP formation and PCA-P1 variation.Results Variants of ACA-A1 contains 634 cases whereas normal cases were 1 612.A prominence in variation revealed in right side of ACA-A1 (x2=26.19,P<0.01) which revealed by 429 cases in right side versus 205 cases in lefi side respectively.Parallelled with contrast group,the variant group showed immensely advantages in high proportion of FTP (x2=14.165,P<0.01) which indicated by variant group 22.56% (143/634)and contrast group 18.30% (295/1 612); but crippled in PCA-P1 formation (x2=4.32,P<0.05)which illustrated by proportions of by dysplasia and deficiency compared by variant group and contrast group was 17.35 % (110/634),14.21% (229/1 612).AC A-A 1 normal,slight variation,dysplasia and deficiency together with FTP dyformation was 9.15% (58/634),15.62% (52/333),16.50% (34/206),23.16% (22/95) respectively.Within the variation group,a pattern between ACA-A1 and FTP which fluctuated simultaneously with significant statistical discrepancies (x2=51.117,P<0.01; linear by linear x2=13.340,P<0.01)which demonstrated by ACA-A1 normal,slight variation,dysplasia,deficiency together with FTP formation was 4.57% (29/634),12.91% (43/333),16.02% (33/206),20.00% (19/95) respectively,as far as ACA-A1 variation was concerned,The variant group were more prone to develop PCA-P1 dysplasia or deficiency (x2=45.87,P<0.01; linear-by-linear x2=43.474,P<0.01).Conclusions There were more multi-variation of Willis' circle in the cases with ACA-A1 variation,MRA can value the anatomy and variation of the Willis' circle.MRA can also guide the clinic diagnosis,treatment and prognosis of the cerebrovascular diseases.
4.Development and application of automatic dental crown remover

Objective To develop a dental crown remover with the strength provided constantly and adjusted freely.Methods The remover was composed of a shell, a crown hook, a conduction mechanism, a strength generating mechanism and a percussion mechanism. The spring had the explosive force formed in the crown head to destruct the cohesion of the cement.Results The remover could remove the crown, with one hand freed for the protection of abutment and mucous membrane.Conclusion The remover has simple structure and easy operation, and is worth popularizing clinically.
5.The performance of pediatric risk of mortality score in pediatric patients with severe hand,foot and mouth disease

Objective To comment the severity of severe hand,foot and mouth disease(HFMD)by pediatric risk of mortality score(PRISM),and assess the performance of PRISM in predicting mortality or complication probability in HFMD.Methods Four hundred and twenty-four severe HFMD pediatric patients were recruited in the study from 1th Jan 2010 to 31th June 2013.Information on the outcome and the varia-bles required to calculate PRISM score were collected.The logistic regression model developed in the learning sample was evaluated in the test sample by calculating the area under the receiver operating characteristic (ROC)curve to assess discrimination pneumorrhagia and death.Calibration across deciles of risk was evalua-ted using the Hosmer-Lemeshow goodness-of-fit χ2 test.Results The area under the ROC curve were 0.87 (95%CI 0.80~0.94 )for PRISM in predicting pneumorrhagia probability.The area under the ROC curve were 0.87(95%CI 0.80~0.95)for PRISM in predicting mortality probability.The PRISM in observed and expected pneumorrhagia did not demonstrate good calibration at ten mortality risk intervals (χ2 =36.66, P<0.001 ).The PRISM in observed and expected mortality did not demonstrate good calibration at ten mortali-ty risk intervals(χ2 =41.11,P<0.001).Conclusion The PRISM score is demonstrated good discrimination of pneumorrhagia and death in HFMD pediatric patients,but the performance of calibration is not good.
6.Influencing factors for blood pressure control in elderly patients with hypertension in Hangzhou

Objective:
To understand the situation of blood pressure control and its influencing factors in elderly patients with hypertension in Hangzhou, and to provide basis for the management of elderly patients with hypertension in community.
Methods:
The subjects of this study were hypertension patients aged 60 years and over in Hangzhou community health management of basic public health services. Demographic data and life behaviors were collected by a questionnaire survey, physical examination and laboratory tests were carried out. The multivariate logistic regression model was used to analyze the influencing factors for blood pressure control in elderly patients with hypertension.
Results:
A total of 109 583 people were investigated, with 50 500(46.08%) males and 59 083(53.92%) females. The control rate was 47.70% ( 52 273/109 583 ). After adjusted for age and gender, regular medication ( OR=0.874, 95%CI: 0.838-0.912 ) was the protective factor, obesity ( OR=1.291, 95%CI: 1.260-1.324 ), abnormal fasting plasma glucose ( OR=1.218-1.344, 95%CI: 1.178-1.410 ), the number of unhealthy lifestyles ( OR=1.271-1.292, 95%CI: 1.231-1.344 ), the items of dyslipidemia ( OR=1.047-1.253, 95%CI: 1.017-1.311 ), and the number of cardiovascular risk factors above ( OR=1.254-2.109, 95%CI:1.175-2.281 ) were the risk factors for blood pressure control in elderly patients with hypertension.
Conclusions
The control rate of elderly patients with hypertension in Hangzhou is 47.70%, which is associated with irregular medication, unhealthy lifestyle, obesity, dyslipidemia, abnormal fasting plasma glucose and clustering of these factors.
7.Effect of oxiracetam on sevoflurane-induced cognitive dysfunction in mice: role of PI3K/Akt signaling pathway

Objective:To evaluate the effect of oxiracetam on sevoflurane-induced cognitive dysfunction in mice and the role of phosphatidylinositol 3-kinase/serine/threonine protein kinase (PI3K/Akt) signaling pathway.Methods:Eighty adult Kunming mice, half male and half female, weighing 35-55 g, were divided into 4 groups ( n=20 each) using a random number table method: control group (group C), sevoflurane group (group S), oxiracetam plus sevoflurane group (group OS), and LY294002 plus oxiracetam plus sevoflurane group (group LOS). Group S inhaled 2% sevoflurane for 6 h. A 2 h before sevoflurane anesthesia, oxiracetam 105 mg/kg was injected via the tail vein in group OS, oxiracetam 105 mg/kg and LY294002 0.3 mg/kg were injected via the tail vein in group LOS, and the equal volume of normal saline was injected in group S. The apoptosis rate of hippocampal neurons was detected using TUNEL.The expression of PI3K, phosphorylated PI3K (p-PI3K), Akt and phosphorylated Akt (p-Akt) was determined by Western blot.Cognitive function was assessed using Y-maze at 14 days after the end of anesthesia. Results:Compared with group C, the apoptosis rate of hippocampal neurons was significantly increased, the total number of times and the number of errors required for 10 times of correct responses in Y-maze test were increased, and the expression of PI3K, Akt p-PI3K and p-Akt in hippocampal tissues was down-regulated in group S ( P<0.05). Compared with group S, the apoptosis rate of hippocampal neurons was significantly decreased, the total number of times and the number of errors required for 10 times of correct responses in Y-maze test were decreased, the expression of PI3K, Akt, p-PI3K and p-Akt in hippocampal tissues was up-regulated in group OS ( P<0.05), and no significant changes were found in the parameters mentioned above in group LOS ( P>0.05). Compared with group OS, the apoptosis rate of hippocampal neurons was significantly increased, the total number of times and the number of errors required for 10 times of correct responses in Y-maze test were increased, and the expression of PI3K, Akt, p-PI3K and p-Akt in hippocampal tissues was down-regulated in group LOS ( P<0.05). Conclusion:Oxiracetam can alleviate sevoflurane-induced cognitive dysfunction in mice, and the mechanism may be related to activating PI3K/Akt signaling pathway and inhibiting apoptosis in neurons.
8.Comparison of Rehabilitation of Extremity Motor Function between the Eldcrly and the Middle-Aged or Youthful Stroke Patients

Faciliysyion techniques were used to help improve the extremity motor functions of 43 elderly and 79 middle aged or youthful stroke patients. The Fugl-Meyer scale was employed for thc assessment of the functional status. The results showed that the motor function of upper extremty of thc elderly patients was not improved. but the motor function of lower extremity was improved significantly. The motor functions of upper and lower extremities were all improved significantly in the middle-aged or youthful patients. In early rehabilitation group the Increased motor function marks of the upper extremity of the elderly stroke patients were less than that of the middle-aged or youthful stroke patients. the increased motor function marks of the lower extremity of The elderly stroke patients were less significantly than that of the middle-aged or youthful stroke patients also. In late rehahilitation group the increased motor function marks of the lower extremity in elderly stroke patients were more than that in the middle-aged or youthful stroke patients.
9.Risk factors for mortality in diabetic peritoneal dialysis patients

Objective To investigate the risk factors of all-cause mortality in diabetic patients on peritoneal dialysis (PD).Methods As a single-center retrospective cohort study,all incident PD patients who were catheterized at the First Affiliated Hospital of Nanchang University between November 1,2005 and February 28,2017 were included.Patients were divided into diabetes mellitus group (DM group) and non-diabetes mellitus group (NDM group).Outcomes were analyzed by Kaplan-Meier method.Multivariate Cox proportional hazards models were utilized to assess the risk factors of all-cause mortality.Results A total of 977 patients were enrolled.Compared with NDM group,patients in DM group were older (47.5±14.4 vs 59.3±11.3,P < 0.01),had more cardiovascular disease (CVD) (7.5% vs 20.3%,P < 0.01),higher levels of serum hemoglobin (78.2±17.2 vs 82.3±14.6g/L,P < 0.01),and lower levels of serum albumin (36.1±5.0 vs 32.7±5.6 g/L,P < 0.01).The one-,three-and five-year patient survival rates of DM and NDM group were 89.7%,56.0%,31.9% and 94.7%,81.3%,67.4%,respectively.Survival rate was significantly lower in DM group than in NDM group (x2=63.51,P < 0.01).Stratified analysis showed that DM group had significant lower survival rate than NDM group in patients younger than 70 years old (x2=73.35,P < 0.01),while survival rate was similar between the two groups patients older than 70 years old (x2=0.003,P=0.96).Multivariate Cox proportional hazards model analysis showed that DM (HR:1.74,95%CI:1.27-2.38,P < 0.01),age (HR:1.05,95%CI:1.04-1.06,P < 0.01),leukocyte (HR:1.06,95%CI:1.00-1.12,P=0.04) and triglyceride (HR:1.19,95%CI:1.07-1.32,P < 0.01) were all independent risk factors for all-cause mortality of PD patients.However,age (HR:1.05,95%CI:1.04-1.07,P< 0.01) and alkaline phosphatase (HR:1.01,95% CI:1.00-1.01,P=0.02) were independent risk factors for all-cause mortality of diabetic patients.Conclusions Long-term survival rate was lower in diabetic PD patients than in non-diabetic PD patients.DM,age,leukocyte and triglyceride were independent risk factors of mortality in PD patients.Age and alkaline phosphatase were independent risk factors of mortality in diabetic patients.
10.The risk factors of mortality in peritoneal dialysis patients in different periods

Objective To explore the risk factors and characteristics in patients with peritoneal dialysis who died in different periods.Methods The clinical data of new peritoneal dialysis patients in the Department of Nephrology and Peritoneal Dialysis Center of the First Affiliated Hospital of Nanchang University from November 1,2005 to February 28,2017 was retrospectively analyzed.The patients were divided into two groups according to the time of death:those who died within one year and died after one year.The risk factors of mortality between the two groups were analyzed by Cox regression model.Results A total of 997 patients were enrolled and 244 patients died.There were 69 patients (28.3%) died within one year and 175 patients (71.7%) died after one year.Cardiovascular and cerebrovascular disease was the dominating reason of death in both groups,accounting for 59.4% (died within one year group) and 51.4% (died after one year group) respectively.Cox regression analysis showed that for died within one year group,old age (HR=1.035,95% CI:1.016-1.055,P< 0.001),low blood total calcium (HR=0.167,95% CI:0.053-0.529,P=0.002),low albumin (HR=0.899,95%CI:0.856-0.943,P < 0.001) and low apolipoprotein A1 (HR=0.274,95%CI:0.095-0.789,P=0.016) were risk factors associated with mortality.However,for died after one year group,old age (HR=1.053,95%CI:1.038-1.069,P < 0.001),combined with diabetes (HR=2.181,95%CI:1.445-3.291,P < 0.001) and hypertriglyceride (HR=l.204,95%CI:1.065-1.362,P=0.003) were risk factors associated with mortality.Conclusions The risk factors of mortality for peritoneal dialysis patients of different periods were not exactly the same.For died within one year patients,old age,low blood total calcium,low albumin and low apolipoprotein A1 were independent risk factors for mortality.However,for died after one year patients,old age,combined with diabetes,and high triglycerides were independent risk factors for mortality.
