Objective To study the effect and mechanism of the Fosinopril combined with Fenofibrate on the prevent?ing of diabetic retinopathy. Methods A total of 150 viripotent ICR mice(100 male mice, 50 female mice) were randomly di?vided into five groups(n=30), including A group (Sham group), B group (Model group), C group [Fosinopril prevented group, 20 mg/(kg·d)], D group [Fenofibrate prevented group, 400 mg/(kg·d)] and E group (Fosinopril combined with Fenofibrate pre?vented group). The expression levels of Bax and Bcl-2 gene mRNA were determined by RT-PCR method. TUNEL staining method was used to detect the apoptosisi of retinal cells. Results The Bcl-2 mRNA of A group, Bax mRNA of B group were higher than those of other four groups. Bcl-2 mRNA of E group was higher than that of C group and D group, while the Bax mRNA was lower than those of two groups(all P<0.05). 2.Isolation and characterization of cancer stem cell-like sphere-forming cell subpopulation from the human nasopharyngeal cell line CNE-2

Objective: The present study aimed to isolate and characterize a cancer stem cell-like sphere-forming cell subpopula-tion. Methods: By using a spheroid culture stem cell-conditioned medium, we isolated a subgroup of cancer stem-like cells from naso-pharyngeal cancer cell lines. Chemotherapy resistance was analyzed by using methyl thiazolyl tetrazolium, and clone-forming capabili-ty was determined by using softer agar clone formation assay. Finally, we verified the expression of the stemness-specific gene andβ-catenin by using immunocytochemistry staining and RT-PCR. Results: The lower-differentiated nasopharyngeal cancer lines con-tained more sphere-forming cells, whereas sphere-forming cells were not observed in the high differentiated nasopharyngeal cancer line CNE-1. Compared with CNE-2, CNE-2S (sphere-forming cells derived from CNE-2) exhibited higher chemotherapy resistance and clone-forming ability. Interestingly, the stemness genes BMI-1, ABCG2, and ALDH1 exhibited higher expression in CNE-2S than in CNE-2. β-catenin, a vital transcription factor of the WNT pathway related to stem cells, exhibited higher expression in CNE-2s cellular nucleus and plasma than in CNE-2. Conclusion: We isolated a subgroup of stem-like nasopharyngeal cancer sphere-forming cells. This discovery paves the way for the development of therapeutic strategies aimed at eradicating tumorigenic subpopulations in nasopharyn-geal cancer.
3.Effects of Exenatide on Body Weight, Blood Glucose, Blood Lipids and Proteinuria in Obese and Overweight Young and Middle-aged Patients with Type 2 Diabetes

Objective: To investigate the effects of exenatide on body weight, blood glucose, blood lipids and proteinuria in obese and overweight young and middle-aged patients with type 2 diabetes to provide reference for better controlling the macro-vascular and micro-vascular complications in the patients.Methods: Totally 60 obese and overweight young and middle-aged patients with poorly controlled type 2 diabetes were chosen and randomly divided into exenatide group and novolin N group with 30 ones in each.The patients maintained the previous oral hypoglycemic drugs, and exenatide group was treated with exenatide, Novolin N group was treated with protamine biosynthetie human insulin(NovolinN), and the treatment course was 3 months.The body weight, BMI, fasting plasma glucose (FPG), 2-hour postprandial blood glucose (2hPG), glycated hemoglobin (HbA1C), fasting C-peptide (FCP), 2-hour postprandial C-peptide (2hCP), blood lipids, plasma homocysteine (HCY) and urine microalbumin (UMA) were compared before and after the treatment.Results: After 3 month treatment,the FPG,zhpG and HbAlc were significantly decreased both in exenatide group and NwoCinN group(P<0.05),and were no significant differences.were found in these parameters between the two groups(P>0.05).In exenatide group, the levels of FCP and 2hCP were higher than those before the treatment (P<0.05), and the body weight, BMI, TG and UMA were significantly lower than those before the treatment (P<0.05).However, in Novolin N group, all the above-mentioned indices were not significantly changed after the treatment (P>0.05).Conclusion: Exenatide can effectively control blood glucose, improve β-cell function, reduce body weight, lower blood lipids and decrease urine protein.
4.Effect of Jinan injection on ultrastructure of lung cancer cell lines

AIM: To investigate the effects of Chinese medicine, Jinan injection, on ultrastructure and mitochondria in cultured lung cancer cell lines. METHODS: The cultured lung cancer cell lines PG and PAa were used and divided into 4 groups: control (C), cisplatin (DDP), Jinan (JA) and Jinan in combination with cisplatin (DJ), respectively. The changes of morphology and mitochondria membrane potential, intracellular Ca~(2+) and pH in every group were observed by inverted microscope and electronic microscope as well as by using flow cytometry, staining by rhodamine, Fluo-3 and BCECF, respectively. RESULTS: Degeneration cells showed chromatin condensation and peripheral congregation. In cytoplasm autophage lysosome increased and myelinoid body was seen easily. In mitochondria structure, where the space between the inner and outer membranes of these organelles expanded as the matrix was compressed. The electron-dense or swelled was observed as vacuole degeneration and its matrix showed electron-lucent. Compared to control, mitochondria membrane potential increased in every group after 24 h and 48 h treatment. DDP increased intracellular calcium ion in PG cells, however, in PAa cells, JA and DJ decreased it. Intracellular pH got lower at 24 h and higher at 48 h in PG and PAa cells. Objective:
To discuss the impact and practicability of pluralistic management when puerpera with hepatitis B virus (HBV) in hospital.
Methods:
By review researching and convenient sampling, two groups of puerpera with HBV were studied comparatively by the index of the time of breast milk, the breastfeeding rate and the satisfaction rate in hospital.The control group (411 cases, January to December, 2017) carried on the normal regulations nursing and the observation group (411 cases, January to October, 2018) carried on the pluralistic management in the Obstetrics and Gynecology Department of the Fifth Medical Center of PLA General Hospital.
Results:

Objective:
To elucidate the mechanism of melatonin combined with cisplatin in promoting cell apoptosis of rat pancreatic cancer AR42J cells.
Methods:
Rat pancreatic cancer AR42J cells were divided into control group, 1 mmol/L cisplatin treated group (cisplatin group), 1 mmol/L melatonin treated group (melatonin group), 1 mmol/L cisplatin combined with 1 mmol/L melatonin treated group (combined group), 1 μmol/L cisplatin combined with melatonin treated group after 1 μmol/L PBN pretreatment for an hour (PBN+ combined group) and 1 μmol/L cisplatin combined with melatonin treated group after PBN solvent pretreatment for an hour (solvent+ combined group). MTT and annexin V-FITC/PI were used to detect the cell proliferation rate and cell apoptosis rate, respectively. The protein expression of caspase-3 was detected by Western blot. DCFH-DA was used to detect the level of ROS. ROS level and caspase-3 expression in AR42J cells pretreated with ROS antagonist PBN for 24 hours were detected.
Results:
The cell proliferation rate of control group, cisplatin group, melatonin group and combination group after 24-hour culture was (96.29±3.49)%, (81.38±6.01)%, (80.72±3.68)% and (42.26±6.35)%, respectively. The cell apoptosis rate was (16.42±4.15)%, (56.47±9.06)%, (52.94±6.57)% and (87.36±6.48)%, respectively. The percentage of ROS positive cells was (1.33±1.53)%, (46.67±7.64)%, (45.67±5.13)% and (83.33±7.64)%, respectively. The expression of cspase-3 was 100%, (150.64±7.70)%, (147.00±7.27)% and (190.04±5.07)%, respectively. The cell proliferation rate of cisplatin group and melatonin group was significantly lower than that of the control group. The apoptotic rate, the proportion of ROS positive cells and the expression of caspase-3 were significantly higher than those in control group. 7.Effects on Various Indicators of Exenatide in Young and Middle-aged Patients with Type 2 Diabetes and Non-alcoholic Fatty Liver Disease

Objective: To investigate the effect of exenatide on body weight, blood glucose, blood lipids, insulin resistance and the degree of fatty liver in young and middle-aged patients with type 2 diabetes and non-alcoholic fatty liver disease (NAFLD). Meth-ods: Totally 70 young and middle-aged patients with poorly controlled type 2 diabetes and NAFLD were chosen and randomly divided into observation group and the control group with 35 cases in each group. The patients were treated with exenatide or isophane prota-mine biosynthetic human insulin injection for 3 months on the basis of original oral hypoglycemic drugs. The body weight, BMI, waist circumference, HOMA-IR, plasma glucose (FPG), 2 hour postprandial blood glucose (2hPG), glycated hemoglobin (HbAlc), blood lipids, liver function indices (ALT, AST and GGT), and the severity of fatty liver were compared before and after the treatment. Re-sults: After the 3-month treatment, there was no significant difference in glycemic control between observation group and the control group. After treatment, the body weight, BMI, waist circumference, HOMA-IR, TG, ALT, AST and GGT in observation were signifi-cantly decreased than those before the treatment(P<0. 05), and were superion to those in control group(P<0. 05). While in the control group, all the above indices were not significantly changed before and after the treatment (P>0. 05). The total efficiency for treating NAFLD in observation group was significantly higher than that in the control group (P<0. 05). Conclusion: Besides reducing blood glucose effectively, exenatide can also obviously reduce body weight and TG level, improve insulin resistance, decrease liver en-zymes, and significantly ameliorate the degree of fatty liver. The results suggested that exenatide might be a new therapeutic option for young and middle-aged patients with type 2 diabetes and NAFLD.
8.Clinical study of oxidative stress and inflammatory factors in patients with diabetic retinopathy

Objective By measuring the level of serum advanced oxidation protein products (AOPP), vascular endothelial growth factor (VEGF) and pigment epithelium-derived factor (PEDF) in patients with diabetic retinopathy (DR), and to explore its possible role in the pathogenesis of DR. Methods 90 type 2 diabetes mellitus (T2DM) patients were divided into three groups:30 patients with non-DR (NDR), 30 patients with back ground-DR (BDR), 30 patients with prolifer-ative-DR (PDR). At the same time, 30 healthy controls were selected. The concentration of serum AOPP was measured by modified Wikto-Sarsat, while VEGF and PEDF by ELISA assay. Results The serum level of AOPP increased gradu ally along with the development of DR, and the difference was statistically significant (P<0.05). Serum levels of VEGF in T2DM groups were significantly increased compared with the control group (P<0.05), and VEGF in BDR and PDR groups were significantly higher than that in NDR group (P<0.05), but the serum level of VEGF in PDR group was sig-nificantly lower than that in BDR group (P<0.05). The serum level of PEDF in each group was gradually decreased, and the difference was statistically significant (P<0.05). Pearson correlation analysis showed that the serum level of VEGF was negatively correlated with PEDF(r=-0.734, P<0.05). The serum level of AOPP was positively correlated with VEGF (r=0.816, P<0.05), and negatively correlated with PEDF (r=-0.905, P<0.05). Conclusion The increase of AOPP and VEGF level in serum of patients with diabetes, and the decrease of PEDF may promote the development of DR.
9.Evaluation of the applicability of the Zhang′s diagnostic criteria for atopic dermatitis in adolescents and adults from southern Jiangsu

Objective:To evaluate the applicability of the Zhang′s diagnostic criteria for atopic dermatitis (AD) in adolescents and adults from southern Jiangsu.Methods:A total of 1 769 patients were collected, who were newly diagnosed with eczema or AD in departments of dermatology of 7 hospitals in southern Jiangsu from May 2019 to May 2021. A standardized survey interview was conducted with patients by dermatologists using a self-designed questionnaire, which included patient′s personal information, relevant medical history, clinical characteristics, laboratory tests, etc. Using Hanifin-Rajka criteria as the gold standard, the sensitivity and specificity of the Williams criteria, Zhang′s criteria and Japanese Dermatological Association (JDA) criteria were evaluated, separately.Results:Among the 1 769 patients, there were 759 (42.9%) males and 1 010 (57.1%) females, aged 32.2 ± 8.2 years (range, 12 - 79 years) . Pruritus was the most common clinical feature among the AD patients (883/913, 96.7%) . When using the Hanifin-Rajka criteria as the gold standard, 913 (51.6%) subjects were diagnosed with AD; the sensitivity of Zhang′s criteria reached 92.6% (845/913) , and its specificity was 73.2% (627/856) ; the sensitivity of Williams′ criteria was 87.8% (802/913) , and its specificity was 81.3% (696/856) ; the sensitivity of JDA criteria reached 96.9% (885/913) , and its specificity was 68.9% (590/856) . The consistency in the diagnosis of AD was moderate between the Zhang′s criteria and Williams criteria (Kappa = 0.61, P = 0.009) , was relatively high between the Zhang′s criteria and JDA criteria (Kappa = 0.85, P = 0.001) , and was moderate between the Williams criteria and JDA criteria (Kappa = 0.51, P = 0.013) . Conclusions:Compared with the Hanifin-Rajka criteria, the Zhang′s criteria exhibit good sensitivity and specificity for the diagnosis of AD among adolescents and adults in southern Jiangsu. However, pruritus remains important for the diagnosis of AD.
10.Expression and significance of Nrf2 and caspase-3 in non-small cell lung cancer

Objective To study the expression and significance of Nrf2 and caspase-3 in non-small cell lung cancer (NSCLC).Methods The specimens from 82 cases of NSCLC in our hospital from September 2015 to December 2017 were selected for observation.All patients were surgically resected to obtain NSCLC specimens.The expression of Nrf2 and caspase-3 in NSCLC was detected by immunohistochemistry and its clinical significance was analyzed.Results The positive expression rate of Nrf2 in NSCLC (63.4%) was significantly higher than that in normal tissue (30.0%),with statistically significant difference (P < 0.05).The analysis of clinical characteristics showed that the expression of Nrf2 in NSCLC was related to tumor diameter,tumor node metastasis (TNM) stage,lymph node metastasis and distant metastasis (P < 0.05).The expression of Nrf2 in NSCLC was not significantly associated with gender,age,smoking history,case classification,and histological differentiation (P > 0.05).The positive rate of caspase-3 in NSCLC (40.2%) and the positive rate of caspase-3 in normal tissues (40.0%) were not statistically different (P > 0.05).Analysis of clinical characteristics showed that the expression of caspase-3 in NSCLC was related to TNM stage,lymph node metastasis,distant metastasis,and the difference was statistically significant (P < 0.05);the expression of Nrf2 in NSCLC was not significantly associated with gender,age,smoking history,case classification,tissue differentiation,and tumor diameter (P > 0.05).Conclusions The abnormal expression of Nrf2 and caspase-3 is correlated with the progression of NSCLC and the malignant biological behavior.Active detection of Nrf2 and caspase-3 levels can clinically determine the clinical stage,lymph node metastasis and distant metastasis of NSCLC,providing information to facilitate clinical development of relevant interventions.
