Autoimmune polyendocrinopathy syndrome type Ⅰ( APS-Ⅰ) is a rare autosomal recessive disorder caused by mutations in autoimmune regulator gene( AIRE) . A number of mutations have been described in the AIRE gene of patients with APS-Ⅰ, including nonsense mutation, missense mutation, silent mutation, splice site mutation, insertions and deletions mutation, et al. The mutation characteristics of the APS-Ⅰ pathogenic gene have been reviewed in the article.
2.Resveratrol attenuates LPS-induced cell injury and TLR4/NF-κB activation in human periodontal ligament cells

Objective:To investigate the protective effect of resveratrol on lipopolysaccharide (LPS)-induced cell injury in human periodontal ligament cells(hPDLCs).Methods:hPDLCs were cultured and identified.The cultured hPDLCs were divided into 5 groups:control group and LPS(10 μg/ml) + RES(0/30,60 and 90 μmol/L respectively) groups.The cell proliferation was detected by MTI assay.The secretion of TNF-α and IL-6 of hPDLCs was detected by ELISA kit.The expression of TLR4/NF-κB mRNA and protein was determined by PCR and Western blot analyses,respectively.Results:The cultured cells were negative for cytokeratin and positive for vimentin staining.Compared with the control group,cell proliferation was decreased,the secretion of TNF-α/IL-6 levels and the expression of TLR4/NF-κB mRNA and protein were increased after treatment with LPS.Whereas,with 30-90 μmol/L resveratrol pretreatment,the proliferation ability of hPDLCs was enhanced(P < 0.05),the secretion levels of TNF-α and IL-6 and the expression of TLR4/NF-κB mRNA and protein were reduced (P < 0.01) in a dose-dependent manner.Conclusion:Resveratrol may attenuate LPSinduced cell injury by inhibiting TLR4/NF-κB pathway in hPDLCs.
3.Incidence and risk factors of macular edema after cataract surgery in patients with type 2 diabetes mellitus

Objective To investigate the incidence and risk factors of macular edema (ME) after cataract surgery in patients with type 2 diabetes mellitus (T2DM),and to provide a reference for clinical treatment.Methods From March 2014 to April 2017,128 cases of cataract phacoemulsification and intraocular lens implantation in Jincheng Ophthalmology Hospital were retrospectively analyzed,of whom 36 cases(36 eyes) without diabetes were A group,41 patients(41 eyes) with no retinopathy were B group,51 cases(51 eyes) of T2DM patients with retinopathy were C group.The incidence of ME,the BCVA and the CSMT of the three groups were compared,and the Logistic multiple factor regression analysis was used to analyze the risk factors of DME after T2DM cataract surgery.Results The incidence rates of ME in A,B and C group were 5.56%,29.27% and 50.98%,respectively,and the difference was statistically significant (x2 =6.567,P < 0.05),and that of C group > B group > A group (x2 =4.419,19.951,all P <0.05).The BCVA after operation in A group,B group and C group,was (0.26 ±0.08),(0.19± 0.04) and (0.08 ± 0.01),respectively,the difference was statistically significant (F =9.352,P < 0.001),and that of group A > group B > group C (t =4.950,2.030,P < 0.05).The CSMT after operation in A group,B group and C group was (246.35 ± 23.73) μm,(249.52 ± 20.95) μm and (289.31±43.82) μm,respectively,the difference was statistically significant (F =9.352,P < 0.001).Logistic multivariate regression analysis showed that preoperative retinopathy [3.672 (0.261-0.961),P =0.000],preoperative ME [3.862 (0.136-0.769),P =0.001] were independent risk factors for the occurrence of ME after T2DM.Conclusion The incidence of ME is higher after T2DM cataract surgery,and the risk of ME is higher in patients with preoperative ME and preoperative diabetic retinopathy.
4.Hospital Infection in Respiratory Intensive Care Unit:Clinical Analysis and Prevention and Control Measures

OBJECTIVE To determine the distribution of pathogens of bacterial infection in respiratory intensive care unit (RICU) to provide reference for the prevention of hospital infection. METHODS To collect specimens of the patients received endotracheal intubation from Jan 2006 to Dec 2006 in our RICU,to identify pathogens and drug sensitivity test. RESULTS We isolated 105 pathogenic bacteria from 626 specimens of the 58 patients,the G-bacilli accounted for 64.76%,G+ cocci accounted for 20.95%,fungi accounted for 12.38%; Acinetobacter baumannii was one of the main G-bacilli accounted for 19.05%. Staphylococcus aureus of G+ cocci for 9.52%,and Candida albicans of fungi for 7.62%; from 105 pathogens,65 from the respiratory tract,15 from the for urinary tract,eight from a catheter tube,six from the digestive tract,five from the blood,four from the various drainage tubes and two from the incision secretions. CONCLUSIONS Hospital infection pathogens exist in the respiratory tract infection in hospitalized patients of respiratory ICU with risk factors of age,underlying diseases,time in the intensive care unit,ventilator time and the use of invasive procedures in a certain relationship.
5.Laparoscopy Combined with Hysteroscopy for the Diagnosis and Treatment of Female Infertility:Report of 145 Cases

Objective To study the clinical application of laparoscopy combined with hysteroscopy to female infertility.Methods From January 2007 to March 2008,145 cases of female infertility were examined and treated by laparoscopy combined with hysteroscopy in our hospital.We analyzed the clinical records of the patients to determine the value of the method.Results By using laparoscopy combined with hysteroscopy,136 cases of pelvic disease(93.8%,136/145) were detected in this series.The most common causes of infertility were pelvic inflammation(58.6%,85/145) and endometriosis(29.0%,42/145).The incidence rate of pelvic inflammation in secondary infertility patients was significantly higher than in those with primary infertility(P
6.Clinical observation of Chinese medical gargle on gingivitis of the patients during fixed orthodontic treat-ment

Objective:To observe the clinical efficacy of Chinese medical gargle on gingivitis of the patients wearing fixed appliance. Methods:60 gingivitis patients with fixed orthodontic treatment were randomly divided into chlorhexidine gargle group( CG group) and Chinese medical gargle group(CMG group)(n=30). Gingival index(GI) and sulcus bleeding index(SBI) in all patients were tested, then the patients were treated by scaling and curettage, and guided to use gargling with corresponding gargle 3 times a day for 14 d. GI and SBI were tested after 7 and 14 days respectively. Data were analysed. Results:After 7 d treatment, GI and SBI were decreased( P<0. 05) and there was no obvious difference between the 2 groups(P<0. 05). After 14 d treatment GI and SBI in CMG group de-creased more than those in CG group(P<0. 05). Conclusion:Chinese medical gargle is effective in the treatment of gingivitis of the patients during fixed orthodontic treatment.
7.Detection of serum antibodies to polymorphic membrane proteins (Pmp) in patients with urogenital Chlamydia trachomatis infection

Objecfive To detect serum antibodies to Pmp in patients with urogenital Chlamydia trachomatis infection and to assess the relationship between Pmp and urogenital C.traehomatis infection.Methods Twenty healthy adults and 77 patients with urogenital C. trachomatis infection were recruited into this study.A 3-month foilow-up was carried out in 43 patients,who were classified into persistent infection group(n=19)and negative-conversion group(n=24).Western-blot was performed to detect serum antibodies to Pmp in all subjects.Results The positivity rate of anti-Pmp antibodies was 90.20% (71/77) in patients,significantly higher than that in the normal controls[20% (4/20),P<0.05].All the 9 types of anti-Pmp antibodies were detected in patients with a varying positivity rates,which were 61.04% (47/77),88.31% (68/77),63.63% (49/77),28.57% (22,77),63.63% (49/77),75.32% (58/77),62.34% (48/77),77.92% (60/77)and 70.13% (54/77) for antibodies against PmpA,PmpB,PmpC,PmpD,PmpE,PmpF,PmpG,PmpH and PmpI respectivelyThe prevalence was highest for anti-Pmp B antibodies and lowest for anti-Pmp D antibodies.There was no significant difference in the positivity rate of anti-Pmp antibodies between persistent infection group and negativeconversion group.Conclusions Anti-Pmp antibodies could be generated in patients infected with C. trachomatis.The immunogenicity of different Pmps is different,and the immunoprotective activity of Pmps is rather weak.Individual differences exist in serum anti-Pmp antibodies among patients.Nine types of Pmps are expressed in patients with urogenital C. trachomatis infection.
8.Clinic anatomy and design of island skin flap in the lower leg

Objective To study the blood supply of the flap accompanying vessels of the cutaneous nerves in the lower leg, and to design the reversed flap for clinical reference. Methods Anatomic observation was performed on 30 adults’ lower extremity specimens perfused via pressure with red latex through femoral arteries. Results Superficial peroneal nerves, sural nerves and saphenous nerves were all nourished by their accompanying arteries which, anastomosed with the cutaneous perforating branches of other arteries, also nourish the corresponding skin areas. Conclusion The blood supply of the reversed island flap accompanying vessels of the cutaneous nerves in the lower leg is reliable and so it is possible to design long flaps along the cutaneous nervous axis.
9.Quantificational assessment of classification of liver cirrhosis in patients with hepatocellular carcinoma by multi-slice spiral CT perfusion imaging

Objective To measure hepatic blood flow of the normal liver and cirrhotic liver with hepatocellular carcinoma using multi-slice spiral CT(MSCT),and to evaluate the feasibility of the quantitative judgement of classification of liver cirrhosis with CT perfusion imaging.Methods CT dynamic perfusion imaging was performed in 53 subjects,including 32 patients of liver cirrhosis with hepatocellular carcinoma and 21 patients without liver disease(control subjects).The patients of liver cirrhosis with hepatocellular carcinoma were divided into three groups by CT morphologic classification :17 were classified as light liver cirrhosis group,8 as moderate group,7 as severe group.The parameters of CT perfusion included blood flow(BF),blood volume(BV),mean transit time(MTT),hepatic arterial fraction(HAF)and impulse residue function to(IRF To).The parameters were analyzed by the CT perfusion 3 software package(GE)with deconvolution method,and the correlation of the CT morphologic classification of liver cirrhosis with Child-Pugh functional classification was further evaluated.100 g-1,(17.81±6.59)ml/100 g,(9.52±3.08)s,0.20±0.08 and(4.38±2.35)s,respectively,while 0.27±0. 09 and (4. 14±2. 16)s, respectively. The mean BF, BV and HAF between normal livers and liver cirrhosis were statistically significant (P<0. 05), and there were significant statistical differences in BF, BV and HAF among light, moderate and severe liver cirrhosis (P<0. 05). The CT morphologic classification of liver cirrhosis was correlated positively with Child-Pugh classification (r=0. 935, P=0. 001 ). Conclusions Hepatic perfusion with MSCT can reflect the change of blood flow of cirrhotic liver, and MSCT perfusion can quantitatively evaluate the liver cirrhosis degree of patients with hepatocellular carcinoma.
10.Relationship between Functional Gastrointestinal Disorders and Psychological Factors and Related Treatment

Background:Functional gastrointestinal disorders(FGIDs)is a common disease of digestive system which severely affects the quality of life. Recently,more and more attentions have been paid to the relationship between FGIDs and psychological factors. Aims:To investigate the relationship between FGIDs and psychological factors and related treatment. Methods:A total of 270 patients with FGIDs admitted from Sept. 2013 to Sept. 2014 at Anhui No. 2 Province People’s Hospital were enrolled. Hamilton Anxiety Scale(HAMA)and Hamilton Depression Scale(HAMD)were used to evaluate the mental state of patients,and symptom severity of digestive system was scored. Patients with psychological abnormalities received conventional therapy to relieve digestive symptoms,and those who failed from conventional therapy were assigned into refractory group and others were assigned into response group. Patients in refractory group were further divided into conventional therapy group and combined therapy group for receiving paroxetine in addition to conventional therapy. Results:The rates of anxiety and depression were 61. 5%(166 / 270)and 57. 0%(154 / 270),respectively. The scores of HAMA and HAMD were positively correlated with the score of digestive symptoms(r = 0. 585,P < 0. 01;r = 0. 584,P <0. 01)in patients with anxiety and depression in response group. The scores of HAMA,HAMD and digestive symptoms in response group after conventional therapy were significantly decreased(P < 0. 05),while the scores of HAMA and HAMD in refractory group after therapy were significantly higher than those in response group before therapy(P < 0. 05). The scores of HAMA,HAMD and digestive symptoms in combined therapy group were significantly lower than those in conventional therapy group(P < 0. 05). Conclusions:Relationship is found between FGIDs and psychological factors,and antidepressants might contribute to the amelioration of symptoms of FGIDs.
