1.Influence of case management on short-term intensive insulin therapy effect in newly diagnosed type 2 diabetic patients in outpatient clinic

Objective To explore the influence of case management on short-term intensive insulin therapy effect,including treatment compliance,safety and glucose metabolic index in newly diagnosed type 2 diabetic patients in outpatient clinic. Methods A total of 100 newly diagnosed type 2 diabetic patients in outpatient clinic were enrolled in this study and randomly divided into 2 groups:intervention group (n=52)and control group (Con,n = 48 ). The intervention group received standard insulin injection case management provided by a team composed of doctors and nurse case manager. The control group received routine insulin injection education in outpatient. Attitudes and compliance of insulin treatment,incidence of hypoglycemia,fasting blood glucose (FBG) and glycated haemoglobin A1c (HbA1 c ) were compared between the two groups at baseline and after 3 months follow up. Results After intervention,the total score and scores by dimension of insulin treatment attitude were higher in intervention group than in Con group[(76.30 ± 4.06 )vs (60.53 ± 3.18 )score,P < 0.01 ]. Insulin treatment compliance was better in intervention group than in Con group (82.7% vs 64.6%,P < 0.05 ). The hypoglycemia incidence was lower in intervention group than in Con group (19.2% vs 31.3%,P >0.05),although without statistical difference. FBG and HbA1 c were all lower in intervention group than in Con group [(6.79 ± 1.41 )vs (7.51±1.15)mmol/L,(6.62±0.69)% vs (7.15±0.75)%,P <0.05]. Conclusion Case management could effectively improve the insulin treatment attitude and compliance,and then optimize glycemic control innewly diagnosed type 2 diabetic patients treated with short-term intensive insulin therapy in outpatient clinic.

3.Establishment of quantitative PCR assay technique for plasma miRNA

Objective To establish a specific,stable and reliable real-time fluorescence quantitative PCR for detecting plasma mi-croRNAs(miRNAs).Methods The plasma samples from 10 healthy individuals were collected,and miRNAs was extracted using mirVanaTM PARIS kit.Exogenous cel-miR-39 and cel-miR-238 and endogenous plasma miRNAs were reversely translated by spe-cific stem-loop primers and quantified by real time fluorescence quantitative PCR.Results cel-miR-39,cel-miR-238 and miR-342-3p were amplified and quantified specifically in RNA preparations isolated from plasma samples of healthy individuals.The amplifica-tion products of cel-miR-39,cel-miR-238 and miR-342-3p showed a single melting peak at 81.44,81.62 and 82.71 ℃,respectively, without primer dimer peak or non-specific peak in all 10 cases of healthy individual plasma samples.The standard deviation(SD)of intra-assay and extra-assay of miR-342-3p was 0.13-0.20,and the coefficient of variation(CV)was 0.42%-0.66%,which sug-gesting that this detection method has a good repeatability.The levels of miR-342-3p were detected in a same plasma sample,each experiment was repeated for 5 times,and normalized by cel-miR-39 and cel-miR-238.The SD and CV of ΔCt was 0.22,1.68%,re-spectively,which indicating that cel-miR-39 and cel-miR-238 could be taken as the stable exogenous reference for the plasma miR-NAs detection by real-time fluorescence quantitative PCR.Conclusion Real-time fluorescence quantitative PCR could serve as a good platform for plasma microRNA research.

5.Expression of matrix metalloproteinase-2 and tissue inhibitor of metalloproteinase-2 in endometriosis

Objective To investigate the role of matrix metalloproteinase-2 (MMP-2) and tissue inhibitor of metalloproteinase-2 (TIMP-2) in the pathogenesis of endometriosis. Methods Immunohistochemical S-P method was employed to detect the expression of MMP-2 and TIMP-2 in the ectopic tissues and eutopic endometrium of 22 patients with endometriosis and 20 normal controls. Results The expression of MMP-2 in glandular cells was significantly higher than those of uterine endometrium from women with and without endometriosis (P0.05). The expressions of MMP-2 and TIMP-2 were not related to the degree of endometriosis. Conclusion The increased expression of MMP-2 and the decreased expression of TIMP-2 in the ectopic endometriotic tissues result in the higher ratio of MMP-2/TIMP-2 that can make ectopic endometrial tissues have a greater capacity to invade and play an important role in the pathogenesis of endometriosis.
6.Analysis of 5 cases of coexistence of ankylosing spondylitis and Sjgren′s syndrome

Objective To study the patients with ankylosing spondylitis (AS) complicated with Sjgren′s syndrome (SS).Method Five patients were diagnosed and clinical features were analyzed.Results The 5 patients (5 AS/SS) had a mean age 46 8 years (range 33~63) at onset of the disease, 1 male and 4 females.The main clinical features of patients with both AS and SS were axial inflammatory back pain,sacroiliitis,dry eyes and/or dry mouth.HLA B27 in 3 patients and RF in 2 patients was positive.Conclusion The coexistence of AS and SS can be seen in clinical works.
7.Treatment of traumatic brain injury by Panax notoginseng saponins combined with bone marrow mesenchymal stem cell transplantation

BACKGROUND:Studies have shown that Panax notoginseng saponins (PNS) has extensive pharmacological basis for the treatment of cerebral ischemic injury, and animal experiments for treatment of brain injury by bone marrow mesenchymal stem cel transplantation are ongoing. However, little is reported about the combined use of PNS and bone marrow mesenchymal stem cel transplantation.
OBJECTIVE:To investigate the effect of PNS combined with bone marrow mesenchymal stem cel transplantation on traumatic brain injury in rats.
METHODS:Traumatic brain injury models were made in 60 Sprague-Dawley rats by hydraulic shock method. Then, model rats were randomized into model group, cel transplantation group receiving bone marrow mesenchymal stem cel transplantation and combined treatment group undergoing PNS combined with bone marrow mesenchymal stem cel transplantation. Nerve function recovery of the rats and protein expression of nerve growth factor in the brain tissue were evaluated and detected by Bederson scoring and western blot methods, respectively. Morphological changes of the brain tissue and apoptosis in cortical neurons were observed and detected by hematoxylin-eosin staining and TUNEL, respectively.
RESULTS AND CONCLUSION:Highest Bederson score was found in the model group, fol owed by the cel transplantation group and combined treatment group (P<0.05), while the protein expression of nerve growth factor was ranked as fol ows:the combined treatment group>the cel transplantation group>the model group (P<0.05). Inflammatory infiltration and brain edema in the brain were relieved markedly in the combined treatment group compared with the other two groups, and the number of apoptotic neurons was significantly reduced as wel . Our findings suggest that PNS combined with bone marrow mesenchymal stem cel transplantation can promote neurological recovery from traumatic brain injury by increasing the expression of nerve growth factor and reducing neuronal apoptosis.
8.Clinical evaluation of intermittent low-dosage cyclophosphamide in the treatment of 12 eases with lu-pus nephritis related chronic renal insufficiency

Objective To explore the efficacy and safety of intermittent low-dosage intravenous (Ⅳ) cyelophosphamide (CTX) in treating lupus nephritis (LN) related chronic renal insufficiency. Methods Twelve patients with chronic renal insufficiency with azotemia caused by lupus nephritis received intermittent low-dosage eyclophosphamide therapy (200-400 mg/2~4 weeks) for 2 month to 5 years. We monitored their clinical outcomes and laboratory parameters changes. Results SLEDAI significantly decreased in all cases. Renal function was improved in varying grees. Nine cases had their renal function tests returned to normal range and another 3 cases were still in azotemia. No severe adverse effects occurred. Conclusion The treatment is effective and safe. But it is necessary to carry out large samples and long-term follow-up clinical studies to evaluate its effects on renal function changes.
9.Preparation of ?-cyclodextrin Inclusion Compound of Volatile Oil from Danfu Tongmai Granules

Objective To study the inclusion conditions for volatile oil from Danfu Tongmai Granules.Methods With the utilization rate of volatile oil and yield rate of inclusion as indexes,the optimum inclusion conditions were selected by orthogonal design,and the inclusion compound was identified by TLC.Results The optimal inclusion conditions for volatile oil were as follows:a proportion of 1 ∶7(mL ∶g) for oil to ?-cyclodextrin,stirring for 4 hours at 40 ℃.TLC results showed that inclusion process was successful and the main components of volatile oil were similar before and after inclusion.Conclusion This methods can be used for producing ?-cyclodextrin inclusion compound on a large scale.
10.The correlation of miR-106a expression with clinical characteristics and prognosis of colon cancer

Objective To detect the correlations of miR -106 a expression with clinical features of pa-tients with colon cancer ,and to explore the significance of miR -106 a as a prognostic factor .Methods One hun-dred and five patients with primary colon cancer were retrospectively enrolled in this study including their clinical information and slices of FFPE.miR-106a expression was detected by qRT -PCR,and analyzed the correlation between the level of miR -106a and clinical features and survival .Results The level of miR -106a in tumor tissue was higher than adjacent normal tissue (1.142 vs.0.685,P<0.001).miR-106a was correlated with TNM stage and lymphnode metastasis .The higher miR -106a expression,the poorer survival patients were .The hazard risk was increased 3.390 folds(95%CI for HR:1.028 ~11.178)when compared high expression group with the low.Conclusion The aberrant expression of miR -106a may be associated with colon cancer .It will po-tentially be a therapeutic target and prognostic factors .
