1.Effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function in patients with hypothyroidism

Objective To investigate the effects of monocyte CD141 and platelet CD36 on peripheral vascular endothelial function (VEF) in patients with hypothyroidism (PWH). Methods The number of mono-cyte CD141and platelet CD36 from 52 patients wtih PWH and 40 healthy persons (control group) was calculated by flow cytometry. VEF was detected in terms of brachial artery flow-mediated dilatation (FMD) using Doppler ultrasonography. The differences of expressions of monocyte CD141 and platelet CD36 were compared between the two groups, so was FMD. The association of monocyte CD141 and platelet CD36 with FMD (%) was analyzed by Pearson analysis. Results Expression of monocyte CD141 (%) was significantly higher in the PWH group than in the control group (21.79 ± 9.45 vs. 18.84 ± 9.64, P < 0.001), so was expression of platelet CD36 (%) (34.26 ± 10.23 vs. 28.65 ± 9.53, P < 0.001). FMD% was lower in the PWH group than in the control group (8.65 ± 1.97 vs. 11.25 ± 2.72, P < 0.001). CD141 was negatively correlated with FMD% (r = -0.657, P < 0.01), so was CD36 (r = -0.557, P < 0.01). Conclusions Both monocyte CD141 and platelet CD36 are significantly higher in the PWH group than in the control group. CD141 and platelet CD36 are negatively related with VEF.
2.Qualitative research about the influence of communication disabilities caused by dialects on the safety of ophthalmic surgery and its solution

Objective To explore the influence of dialect-based communication obstacle on the safety of ophthalmic operation room and to provide the basis for improving the safety of ophthalmic operation room. Methods An in-depth interview was taken on 10 medical staff by qualitative research. The information was arranged and analyzed by phenomenology method. Results The interview with the surgical doctors on the communication barriers were extracted to three themes, including lower surgical efficiency, decreased patient satisfaction, increased risk of surgical complications, the interview with the surgical nurses on the dialects leading to communication barriers were extracted three themes, including surgical verification difficult to carry out, affect the perioperative care, hinder the postoperative mission to carry out, the extracted themes about the impact on patients include increased stress and anxiety affect the surgical outcome, poor intraoperative experience reduced satisfaction, the extracted themes about solution include the organization of medical staff to learn the dialect of the common language, recruit staff diversification, to provide professional translation, standardized body language to instruct the patient. Conclusions In this study, the experience of communication disorder caused by doctors and nurses in the ophthalmic operating room reflects the shortcomings of the safety management of the ophthalmic operating room. These deficiencies can be reduced by the independent study of the medical staff.
3.Professor's experience in the treatment ofsyndrome with fire needling therapy at lower-sea points.

Based on the syndrome differentiation of meridians and collaterals andtheories, Professorproposed the fire needling therapy at the lower-sea points for five types ofsyndromes. Professorstresses on the flexible application of fire needling therapy forsyndrome at the lower-sea points and innovated the unique five techniques of fire needling therapy, named lifting and threating needle of small amplitude at the lower-sea points, the stimulation along the relevant meri-dians before fire needling, detecting the sensitive sites by gentle pressing the relevant lower-sea points, combining the movement of affected upper limb or joint during fire needling, or combining with contralateral needling technique, and controlling the depth of fire needling based on the duration of sickness and depth of affected site. He pointed out that Shangjuxu (ST 37) is for skinsyndrome, Zusanli (ST 36) for musclesyndrome, Yanglingquan (GB 34) for tendonsyndrome, Weizhong (BL 40) for bonesyndrome and Xiajuxu (ST 39) for vesselsyndrome.

In an answer to the challenge of enzymatic instability and low oral bioavailability of proteins/peptides, a new type of drug-delivery vesicle has been developed. The preparation, based on sodium dodecyl sulfate (SDS) and β-cyclodextrin (β-CD) embedded in chitosan gel, was used to successfully deliver the model drug-insulin. The self-assembled SDS/β-CD vesicles were prepared and characterized by particle size, zeta potential, appearance, microscopic morphology and entrapment efficiency. In addition, both the interaction of insulin with vesicles and the stability of insulin loaded in vesicles in the presence of pepsin were investigated. The vesicles were crosslinked into thermo-sensitive chitosan/β-glycerol phosphate solution for an in-situ gel to enhance the dilution stability. The in vitro release characteristics of insulin from gels in media at different pH values were investigated. The insulin loaded vesicles-chitosan hydrogel (IVG) improved the dilution stability of the vesicles and provided pH-selective sustained release compared with insulin solution-chitosan hydrogel (ISG). In vitro, IVG exhibited slow release in acidic solution and relatively quick release in neutral solutions to provide drug efficacy. In simulated digestive fluid, IVG showed better sustained release and insulin protection properties compared with ISG. Thus IVG might improve the stability of insulin during its transport in vivo and contribute to the bioavailability and therapeutic effect of insulin.
