Modern Clinical Nursing 2015;(12):7-11
Objective To explore the correlations of perceived social support and burnout with general well-being among caregivers in nursing homes. Methods One hundred nursing workers were selected from three nursing homes in Guangzhou. Perceived social support scale ( PSSS ) , Maslach burnout inventory ( MBI ) and general well-being schedule ( GWBS ) were used to investigate the status of their support, burnout and general well-being and analyze the correlations between them. Results The scores on perceived social support, job burnout and general well-being were (59.00 ± 9.00), (2.49 ± 0.88), (77.19 ±9 .90), respectively, all at moderate level. The scores on emotional exhaustion and depersonalization were significantly higher than the norms (both P<0.001). Job burnout together with its three dimensions was negatively correlated with general well-being (all P<0.01), while perceived social support was positively correlated with general well-being ( P < 0 . 01 ) . The exogenous family support , emotional exhaustion and personal accomplishment influenced caregiver′s general well-being (all P<0.01). Conclusions Perceived social support, job burnout and general well-being are all at moderate level in caregivers of nursing homes. The higher the perceived social support, the lower the job burnout, the higher the passion and the general well-being in care workers. The general well-being of caregivers of the nursing homes was not high. The managers should pay more attention to them and measures should be taken to promote their physical and mental health.

Objective:To sum up nursing experience of a multi-drug resistant patient with upper gastrointestinal hemorrhage who underwent Lateral skull base. Methods:One patient with multi-drug resistant was admitted to our hospital in October, 2019. To observe and prevent the postoperative complications of lateral skull base and to prevent the rupture bleeding of carotid artery. The early detection and treatment the symptoms of upper gastrointestinal bleeding is effective and necessary. Intervention for patient receiving nutrition therapy is promoted. Results:By giving psychological care to improve patient compliance. The patient had no postoperative complications of lateral skull base, and the bleeding symptoms of upper gastrointestinal hemorrhage was controlled in time. Conclusion:The patient was discharged from the hospital with a tracheal tube in a stable condition.

Bacterial antibiotic resistance has become a serious problem facing the world today. The rapid spread of mobile genetic elements (MGEs) carrying antibiotic resistant genes between the same or different bacterial species has accelerated the formation of antibiotic resistance, posing a huge challenge to clinical treatment of bacterial infections. The insertion sequence (IS) is the simplest MGE, which encodes a transposase gene and has reverse terminal repeat sequences on both sides. The IS91 family is one of the 26 known insertion sequence family, with 34 members to its name. Recent studies have shown that many members of the IS91 family are strongly associated with antibiotic resistance. Therefore, this article will review the discovery and genetic characteristics of IS91 family, similarities and differences between IS91 and insertion sequence common region(ISCR), as well as their relationship with drug resistance and virulence gene transmission, so as to provide reference for future research on bacterial drug resistance.

Objective:To understand the willingness and cognition of nurses in Guangzhou and Foshan to "Internet + Nursing Service", and analyze the relevant factors that affect nurses' willingness so as to provide a reference for the formulation of related policies. Methods:From June to August 2019, convenience sampling was used to selected 625 nurses from 8 hospitals in 2 Class Ⅱ hospitals and 2 Class Ⅲ hospitals in Guangzhou and Foshan respectively as research subjects. All nurses were investigated with the self-designed Nurses' Willingness to "Internet + Nursing Service" Questionnaire. A total of 625 questionnaires were issued and 612 valid questionnaires were returned. Results:Among 612 nurses, only 31.21% (191/612) nurses were willing to carry out "Internet + Nursing Service"; 56.70% (347/612) nurses took a neutral attitude; 78.76% (482/312) nurses were willing to develop "Internet + Nursing Service" with the biggest motivation of increase in economic income. The biggest concern of 90.03% (551/612) nurses was their practice risk and personal safety. Univariate analysis showed that there were statistically significant differences in the willingness of nurses of different ages and working hours to develop "Internet + Nursing Service" (χ 2=11.04, 26.27; P<0.01) . Conclusions:Most nurses take a wait-and-see attitude towards "Internet + Nursing Service". We should improve relevant laws and regulations for nurses as soon as possible to protect the rights and safety of nurses, and attract junior nurses to carry out "Internet + Nursing Service" in a targeted manner.

Objective To explore the effect of a phased rehabilitation training programme to relieve shoulder dys-function in patients after neck dissection and to provide effective solutions for postoperative shoulder joint function recov-ery of patients. Methods This study has been reviewed and approved by the Ethics Committee, and informed consent has been obtained from patients.A phased rehabilitaiton training programme for the shoulder after neck dessection was developed through literature review and discussion, and 70 eligible patients from Hospital of Stomatology,Sun Yat-sen University from December 2020 to April 2021 were selected and randomly divided into the test group and control group(35 patients in each group). The control group underwent motor rehabilitation training from 6 weeks postoperative to 1 year after surgery, such as shoulder mobility and coordination training and small range of motion training of the neck, while the test group took part in a rehabilitation training program that included familiarization maneuver training, protective rehabilitation, exercise rehabilitation, and resistance training in the following four stages:preoperative,postop-erative general anesthesia and awake until the removal of stitches, the removal of stitches until 6 weeks after surgery, and 6 weeks after surgery until 1 year after surgery. The frequency of training in both groups was at least 3 days per week, and the length of each training session was 10-15 min.The intensity of exercise was 2-3 points on the Borg Conscious Ex-ercise Intensity Scale(i.e.,mild-to-moderate tachypnea or fatigue). The neck dissection injury index(NDII)was used to evaluate the quality of life related to shoulder joint function at four time points:preoperative, postoperative 3 months, postoperative 6 months, and postoperative 12 months. The higher the score, the better the quality of life. Results 28 cases in the test group and 32 cases in the control group completed a one-year follow-up.At 3 and 6 months postopera-tive,the NDII of the test group was significantly higher than that of the control group[3 months postoperative:test group(93.48±9.36)vs.control group(80.00±11.34)(P<0.001),6 months postoperative:test group(98.21±4.76)vs.control group(90.70±9.12)(P<0.001)];12 months after surgery, the NDII of the test group(97.23±4.88)was still higher than that of the control group(96.33±4.49),but the difference was not statistically significant(P=0.458).The difference in NDII scores among subjects at 3,6,and 12 months after surgery was statistically significant in each group(P<0.001).Conclusion The application of the phased rehabilitation training method in neck dissection patients has a feasibility and could improve the quality of life of patients'shoulder joint function within 6 months after surgery.
