1.Improved endovascular stent-graft exclusion for Debakey Ⅲ type aortic dissections

BACKGROUND: Conventional endovascular stent-graft exclusion for Debakey Ⅲ type aortic dissection is complicated and the complication rates are relatively high. Moreover, their operating techniques have increased the risk of rupture of laminated break. OBJECTIVE: To evaluate the efficacy and safety of improved endovascular stent-graft exclusion for Debakey Ⅲ type aortic dissection by the innovative methods of interventional therapy. DESIGN, TIME AND SETTING: A retrospective case analysis. Cases were selected from Department of Cardiology, Affiliated Hospital of Gannan Medical College between October 2006 and March 2009. PARTICIPANTS: A total of 30 male patients with Debakey Ⅲ type aortic dissection were selected from Department of Cardiology, Affiliated Hospital of Gannan Medical College, aged 43-70(53.5±12.8) years. The time of onset was 5-45 days. METHODS: All patients underwent improved endovascular stent-graft exclusion for Debakey Ⅲ type aortic dissection by the innovative methods of intervsntional therapy. Fine wire bundled endovascular stent-graft were implanted by the innovative transmission and delivery methods in all patients, of which branch stent were implanted when the distance from the break of descending aorta to left subclavian artery was less than 10 mm. MAIN OUTCOME MEASURES: After the success of operation, aortic angiography was performed to detect stent-graft position, shape, break closure, internal leakage, and patency of the aortic arch branches. The situation of post-operative complications were observed. All cases were followed up by MRI to understand the complications such as aneurysm rupture, internal leakage and stent migration. RESULTS: A total of 33 fine wire bundled endovascular stent-graft were successfully implanted in 30 patients, including 4 branch stents. After the operation, 5 patients were verified endoleak, 3 of them were resolved by repeated stent distension and 2 were treated by implantation of another stent. Three months after the operation, 1 patient showed new rupture at the distal stent and then was successfully implanted a new stent. Three cases showed numbness of right lower extremity and 1 case showed intermittent claudication. In follow-up of 3-32 months, MRI scanning showed the reduction of the false cavity with the formation of intraluminal thrombus, the enlargement of true cavity, and no complications such as tumor rupture, internal leakage or stent displacement were found. CONCLUSION: Fine wire bundled endovascular stent-graft and the innovative methods of interventional therapy for endovascular thoracic aorta repair is an effective, less invasive and safe surgery with fast postoperative recovery, high success rate and few complications for patients with Debakey Ⅲ type aortic dissection, especially for high-risk patients.
2.Comparison of balloon dilation and YAG laser endouretertomy in treatment of secondary ureteric stricture

Objective To compare the efficacy of balloon dilation and YAG laser endouretertomy in treatment of secondary ureteric stricture . Methods 32 patients with secondary ureteral stricture were randomly divided into balloon dilatation group and holmium laser cut group, 16 cases in each, respectively. The two kinds of treatment efficacy was compared after two years postoperative follow-up. Results The clinical results of the two groups showed no statistical differences at the third, sixth and twelfth month follow-up (P > 0.05), and there were no statistical differences in the overall effective rates (P > 0.05). The clinical results of the two groups showed no statistical difference at the twenty-fourth month follow-up (P < 0.05), and the overall effective rates had statistical difference (P < 0.05). Conclusions Balloon dilation and YAG laser endouretertomy in treatment of secondary ureteral stricture have the same short-term clinical effects. But the middle-term efficacy of YAG laser endouretertomy is superior to that of balloon dilation.
3.Efficacy and Safety of the Generic and Original Preparation of Atorvastatin:A Systematic Review

OBJECTIVE:To systematically review the efficacy and safety of the generic and original preparation of atorvas-tatin,and provide evidence-based reference for clinical treatment. METHODS:Retrieved from PubMed,EMBase,Cochrane Li-brary,CJFD,Wanfang Database,VIP and CBM,related randomized controlled trials(RCT)about generic preparation of atorvas-tatin(test group)versus original preparation of atorvastatin(control group)were collected. Meta-analysis was performed by using Rev Man 5.3 software after data extract and quality evaluation. RESULTS:Totally 16 RCTs were included,involving 2 077 pa-tients. Results of Meta-analysis showed,compared with control group,there were no significant differences in reducing total choles-terol (TC) [MD=-0.06,95%CI(-0.14,0.01),P=0.11],triglyceride (TG) [MD=-0.00,95%CI(-0.08,0.08),P=0.99], low-density lipoprotein cholesterol (LDL-C) [MD=-0.07,95%CI(-0.16,0.01),P=0.09],increasing high-density lipoprotein cholesterol(HDL-C)[MD=-0.00,95% CI(-0.03,0.03),P=0.96] and the incidence of major adverse cardiac events(MACE) [OR=1.18,95%CI(0.71,1.97),P=0.52] in test group;in terms of safety,compared with control group,there were no significant differences in the resulting in alanine aminotransferase (ALT) increased [OR=1.08,95%CI(0.51,2.30),P=0.83],the incidences of myalgia [OR=2.46,95%CI(0.70,8.65),P=0.16] and gastrointestinal adverse reactions [OR=1.11,95%CI(0.64,1.95),P=0.71]. CONCLUSIONS:Both the generic and original preparation of atorvastatin can effectively reduce blood lipid levels,with sim-ilar safety.
4.The value of real time three-dimensional ultrasound in differentiating benign and malignant breast tumors

Objective To evaluate the value of real time three-dimensional ultrasound in differentiating benign and malignant breast tumors. Methods One hundred and fifty-three patients with breast tumors were examined with real time three-dimensional ultrasound. The results were compared with post-operative pathological results. Results A sign of malignancy on the coronal plane was observed by three-dimensional ultrasonography,named “sun-like symptom”,because the sign looked like a shining sun:striated hyperechoic or iso-echoic images scattered radially around cancers,together with the distortion of normal tissue.The accuracy of the sign in differentiating benign and malignant breast tumors was 86.93 %( 133/153 ), the sensitivity was 78.38 %( 58/74 ), and the specificity was 94.94 %( 75/79 ). Conclusions Real time three-dimensional ultrasound is a valuable and new technique in differentiating benign and malignant breast tumors.

ObjectiveToestablisharatmodelofatherosclerosiscombinedwithsyndromeofintermingledphlegm and blood stasis , to observe the inflammatory reactions and the treatment effect of prescription ( Danlou tablet ) on the rat model.Methods Thirty-two healthy male SD rats were randomly and equally divided into four groups , namely, normal control group, model control group, atorvastatin group (ATV group), and Danlou group (DLP group).The normal control group was given basic forage , while other three groups were given high fat forage plus intraperitoneal injection of vitamin D 3 and balloon injury of the left common carotid artery to build rat atherosclerosis model combined with syndrome of intermingled phlegm and blood stasis , and then received intragastric administration of saline , atorvastatin suspension and Danlou tablets suspension for 4 weeks, respectively.After intervention, both serum lipid and hs-CRP, IL-6, TNF-α, and LP-PLA2 levels were determined by ELISA , pathological alterations in the thoracic aorta was analyzed using HE staining , the expressions of IL-6, TNF-αand LP-PLA2 mRNA in the thoracic aorta tissue were assessed by real-time fluorescent quantitativePCRtechnology.Results ①Comparedwiththenormalcontrolgroup,thereweresignificantincreasesin serum TC, LDL-C, hs-CRP, IL-6, TNF-α, and LP-PLA2 levels (P<0.05 for all) and decrease of serum HDL-C levels in the model group (P<0.05).The expressions of IL-6, TNF-α, and LP-PLA2 mRNA in the rat aorta were significantly increased ( P<0.05 for all ) .In the model control group , HE staining showed altered arrangement of aortic endothelial cells, irregular intimal thickening , broadened subendothelial space , and accumulation of foam cells and lipid deposition , forming typical atherosclerotic plaques .② Compared with the model control group , the ATV and DLP groups showed significantly decreased serum TC , LDL-C, hs-CRP, IL-6, TNF-α, and LP-PLA2 levels ( P<0.05 for all) as well as the expressions of IL-6, TNF-α, and LP-PLA2 mRNA in the thoracic aorta (P<0.05 for all).There were no significant differences between the ATV and DLP groups (P>0.05).There were also reduced aortic intimal hyperplasia , macrophage infiltration and plaque area compared with those of the model group .Conclusions Rat model of atherosclerosis combined with syndrome of intermingled phlegm and blood stasis can be established by high fat diet feeding combined with the intraperitoneal injection of vitamin D 3 and balloon injury of carotid artery .The prescription ( Danlou tablet ) can inhibit the inflammatory reaction and ameliorate atherosclerotic changes in the rat models .
7.Retrograde HRP tracing after transplantation of BDNF genetically modified NSCs at spinal cord injury in rat

Objective To investigate the nerve recanalization and the motor function of hind legs after transplantation of BDNF genetically modified neural stem cells(NSCs) at spinal cord injury site in rat. Methods After L4 spinal cord transection of rat, BDNF genetically modified NSCs were transplanted immediately. Retrograde HRP tracing through sciatic nerve were practiced at 1 week, 1 month, 2 month, 3 month after transplantation of BDNF genetically modified NSCs. The morphological changes were observed at section of spinal cord and the motor functions of both hind legs of rat were detected. Results The morphology of the injured spinal cord sections turned better. Retrograde HRP tracing through sciatic nerve showed some HRP positive neurons and nerve fibers at the site of near rostral end of the nearly injured part at one month after transplantation and increased with the time going by. Motor function of hind legs of rats recovered significantly in all transplantation groups. Conclusion BDNF genetically modified NSCs have repairing effect on spinal cord injury in rat.
8.The gene expressions at the spinal cord injury site of rat after transplantation of BDNF genetically modified NSCs

Objective To observe the changes of the gene expressions at spinal cord injury site of rat after transplantation of BDNF genetically modified neural stem cells(NSCs) so as to provide basic data for the repair of spinal cord injury. Methods The Wistar rats were randomly divided into 4 groups: control group, operation group, NSCs transplantation group, BDNF NSCs transplantation group. Four time points(7 day, 1 month, 2 month, 3 month) were divided for each group. The expressions of ? galactosidase and BDNF, GFAP, NF 200 at the site of spinal cord injury were observed by cell transplantation, X gal histochemistry, immunocytochemistry, in situ hybridization, etc. Results After transplantation of BDNF genetically modified NSCs, some X gal positive cells were found at the sections of spinal cord injury. The expressions of BDNF were strong, especially at 1 week and 1 month post transplantation in transplantation group. The GFAP and NF 200 positive cells were also found at each time point in each group. Conclusion BDNF genetically modified NSCs can survive at the site of spinal cord injury and can strongly express BDNF, suggesting that BDNF genetically modified NSCs can be used as the material for the repair of spinal cord injury.
9.Reverse hybridization applied in detection on human papillomavirus infection of twenty-three subfamilies

Objective To establish a method of reverse hybridization to detect five subfamilies of low risk Human Papillomaviruses(HPV6,11,42,43 and 44)and eighteen subfamilies of high risk HPV (HPV16,18,31,33,35,39,45,51,52,53,56,58,59,66,68,73,83 and MM4)in one reaction.Methods Special probes for twenty-three HPV subfamilies were fixed on nylon membrane bars,biotin labeled general primers mediated polymerase chain reaction(GP-PCR)were applied in HPV DNA amplification.PCR amplified DNA fragments were reversely hybridized with special probes that were fixed on the membranes. All samples(136)detected by reverse hybridization method were paralleled with the methods of Hybridization Capture Ⅱ(HC-Ⅱ)and sequencing.Results Positive rate of the 136 samples detected by reverse hybridization was 41.9%,while HC-Ⅱ 42.6% and sequencing 40.4%.Reverse hybridization detection indicated coherence with the other two methods(Kappa 0.8644 and 0.9089,respectively).While sequencing was lab standard for DNA test,the sensitivity was 96.36%,specificity was 95.06%,accuracy was 95.59%.Conclusions Method of reverse hybridization is adaptable to 23 kinds of HPV subfamilies, which can confirm the exactly subfamilies of HPV infection.This method is adaptable in clinical detection of HPV,with high sensitivity,high specificity,simply and convenient operation and the results are easily to be read.
10.EXPRESSION OF AROMATASE CYCOCHROME P - 450 IN THE NEURAL STEM CELLS AND THE EARLY DIFFERENTIATED CELLS

Objective To explore the gene expression in the neural stem cells as well as the cells after the clone was differentiated. Methods Neural stem cell culture, Immunocytochemistry and RT-PCR technique were used. Results There was Aromatase expression in the neural stem cells, after the stem cells were differentiated, the Aromatase strongly expressed in the neruons and weakly expressed in the astrocytes. The aromatase gene in these cells was expressed by means of the brain-specific promoter 1. 4.Conclusion It may provid a new clue for the source of estrogen in the central nerve system.
