2. Preparation and in vitro properties of montmorillonite/matrine nanocomposite

OBJECTIVE: To use montmorillonite as the carrier of matrine. METHODS: Urea was used to insert into the montmorillonite interlayer by solid-phase grinding method. After this process, the matrine was loaded by the solution blending to obtain montmorillonite/matrine nanocomposite. RESULTS: XRD and FT-IR showed that the urea was intercalated into the montmorillonite layers, and the interlayer spacing of montmorillonite was enlarged from ca. 1.2 to 1.7 nm. With the help of the urea, the matrine was successfully loaded into the montmorillonite layers, at the same time the interlayer distance of the montmorillonite was further increased to 1.86 nm. In vitro release experiments displayed that the release of montmorillonite/matrine composite accorded with the Higuchi e-quation, indicating that the montmorillonite could sustain the release of matrine. MTT measurement showed that the montmorillonite/matrine nanocomposite has significant cytotoxicity on human gastric cancer cells (SGC7901). CONCLUSION: This research suggests the feasibility of montmorillonite as the sustained release carrier for the matrine and other alkaloids drug, and provides the evidence for montmorillonite in the pharmaceutical field as drug carriers.

ObjectiveTo study the reliability to asses s interstitial fibrosis in myocardium of advanced age spontaneous hypertension rats(SHR) by acoustic densitometry. 4.Ribavirin combined with dioctahedral-smectite in treatment of rotavirus enteritis

Objective:To analyze the curative effect of ribavirin combined with dioctahedral-smectite(smecta,DS) on ro-tavirus enteritis. Methods:Totally 168 children with rotavirus enteritis, aged from 6 month to 2 years, were randomly divided into 4 groups (3 treatment groups and a control group). All cases were treated with liquid infusion and other supportive treatment. Group A(n = 41) was treated with ribavirin intravenous injection, group B(n = 40) was treated with DS orally, and group C(n = 45) was treated with ribavirin and DS. The effective rate, symptom amelioration time, average hospital stay and side effects were analyzed. 5.Missed diagnosis of hiding posterior marginal fracture of ankle with pronation-external rotation type and its treatment.

OBJECTIVETo analyze causes of missed diagnosis of hiding post-malleolar fractures in treating ankle joint fractures of pronation-external rotation type according to Lauge-Hansen classification and assess its medium-term outcomes.
METHODSAmong 103 patients with ankle joint fracture of pronation-external rotation type treated from March 2002 to June 2010,9 patients were missed diagnosis,including 6 males and 3 females,with a mean age of 35.2 years old (ranged, 18 to 55 years old) . Four patients were diagnosed during operation, 2 patients were diagnosed 2 or 3 days after first surgery and 3 patients came from other hospital. All the patients were treated remedially with lag screws and lock plates internal fixation. After operation,ankle joint function was evaluated according to American Orthopaedic Foot and Ankle Society (AOFAS).
RESULTSAll the 9 patients were followed up, and the duration ranged from 14 to 30 months (averaged, 17 months). No incision infection was found, and all incision healed at the first stage. At the latest follow-up, AOFAS was 83.0 +/- 4.4, the score of 4 patients diagnosed during operation was 85.0 +/- 2.9, and the score of 5 patients treated by secondary operation was 81.0 +/- 5.3. All the patients got fracture union observed by X-ray at a mean time of 2.2 months after operation. There were no complications such as internal fixation loosing, broken and vascular or nerve injuries.
CONCLUSIONAnkle joint fracture of pronation-external rotation type may be combined with hiding post-malleolar fractures. So to patients with ankle joint fracture of pronation-external rotation type, lateral X-ray should be read carefully, and if necessary, CT or MRI examination should be performed. If adding lateral X-ray examination after reduction of exterior and interior ankle joint fixation, the missed diagnosis may be avoided.

7. Surgical treatment of multiple primary hypopharyngeal cancer and thoracic esophageal cancer: A report of 7 cases

Objective: To investigate the clinicopathological characteristics, diagnosis and surgical treatment of multiple primary hypopharyngeal cancer and thoracic esophageal cancer. Methods: Seven male patients with multiple primary hypopharyngeal cancer and thoracic esophageal cancer underwent surgical treatment during Jan. 2002 to Jan. 2008. Their clinical data, including the adjuvant examination method, clinical staging, pathological types, and operative methods, postoperative complications, and long-term outcomes, were retrospectively analyzed. Results: Six patients were confirmed to have the disease before operation, and one patient was pathologically confirmed after operation. All patients had squamous cell carcinoma, with all the thoracic esophageal lesions and four hypopharyngeal lesions being well-differentiated, and three hypopharyngeal lesions being moderately differentiated. All cases underwent total laryngectomy, total hypopharyngectomy and total esophagectomy, and were reconstructed with gastropharyngostomy. There was no perioperative death and the swallowing function recovered satisfactorily after operation in all cases. One patient had postoperative pleural effusion and one had postoperative wound infection, which were cured after conservative treatment. All patients were followed up. One patient survived tumor-free for five years, two patients died two years after operation due to recurrence, and one died 10 months after operation due to local recurrence and bleeding. Conclusion: Multiple primary hypopharyngeal cancer and thoracic esophageal cancer have similar characteristics, and various adjuvant examinations should be used routinely to avoid missed diagnosis. Total hypopharyngectomy and total esophagectomy combining reconstruction with gastropharyngostomy are easy to perform; they can be the first choice for the condition.
8.Studies on the Change of the Content of Polysaccharides in Crude and Processed Fructus Corni

Objective To investigate the change of the content of polysaccharides in crude and processed Fructus Corni,thus to approach the process mechanism of Fructus Corni.Methods The content of polysaccharides in the crude and processed Fructus Corni was determined by phenol-sulfuric acid method.Results After processing with wine,the content of polysaccharides of Fructus Corni decreased from 10.12 %(content of total polysaccharidesbeing 51.41 %) to 5.91 %(content of total polysaccharidesbeing 53.10 %),decreased by 41.60 %as compared with that in the crude Fructus Corni.Conclusion After processing with wine,the content of polysaccharides decreases markedly.The results provide certain evidence for approaching the process mechanism of Fructus Corni.
9.RESEARCH ON DETECTABILITY OF HUANKAI COLIFORM BACTERIA DETECTIVE PLATE FOR DINNER AND DRINKING SETS

Based on the counting results from nutrition agar in our study, and compared with another two kinds of coliform bacteria detective plates for dinner and drinking sets which were provided by two manufactures and inspected by China National Health Bureau , we accomplished the detectability test of the Huankai coliform bacteria plates. In our study, 3kinds of cell density were adopted: 10cells/mL, 50cells/mL and 100cells/mL . The results shew that the detectability of Huankai plates basically conformed with that of nutrition agar , and was no evident difference from that of another two kinds of plates. The conclusions were that Huankai plates can completely be used in the inspection and test for conform bacteria.
10.Ultrasound-guided PTCD combined with ERCP for the treatment of malignant obstructive jaundice:initial experience in 10 cases

Objective To evaluate the clinical effect of ultrasound-guided percutaneous transhepatic cholangial drainage (PTCD) combined with endoscopic retrograde cholangio-pancreatography (ERCP) in treating malignant obstructive jaundice, and to discuss its technical points. Methods A total of ten cases with malignant biliary obstruction were enrolled in this study. After the failure of ERCP treatment, the patients had to be treated with ultrasound-guided PTCD immediately. The guide-wire was inserted into the duodenum through intrahepatic bile duct and common bile duct to connect with ERCP, which was followed by the biliary stent implantation or the removal of physical factors causing obstruction. The clinical results were analyzed. Results Technical success was obtained in all 10 cases. In the patients who underwent a successful guide-wire docking with ERCP the postoperative serum bilirubin was significantly decreased. The main complications were fever, elevation of amylase and transient bloody bile. Conclusion With the help of docking technique the combination use of ultrasound-guided PTCD and ERCP is a new tentative treatment for malignant obstructive jaundice after the failure of initial ERCP treatment. This technique carries promising value in clinical practice as it can significantly increase the success rate of ERCP.
