Objective To investigate the effect of acetylcysteine on the inflammation and oxidative stress in elderly patients with chronic obstructive pulmonary disease(COPD).Methods Sixty patients with COPD were divided into two groups by random mechanical sampling method (30 cases each group).The control group was treated with conventional therapy and the treatment group added acetylcysteine for 8 weeks.The levels of tumor necrosis factor- t (TNF- α ),interleukin-6 (IL-6),interleukin -8 (IL-8),superoxide dismutase (SOD),malondialdehyde (MDA) were monitored and analyzed before and after treatment.Results The levels of SOD,MDA were (51.08 ± 7.80),(75.09 ± 8.03) μ U/L and (7.13 ± 0.89),(4.51 ±0.61 ) μ mol/L before and after treatment in the treatment group,and (52.12 ± 7.31 ),(65.16 ± 8.01 ) μ U/Land (7.11 ± 0.87),(6.21 ± 0.78 ) μ mol/L in the control group.There were significant differences before and after treatment in two groups (P＜0.01 or ＜0.05),and there were significant differences after treatment between two groups (P ＜ 0.05).The levels of IL-6,IL-8 and TNF- α were significant differences before and after treatment in the treatment group [ ( 18.25 ± 7.24) ng/L vs.(29.02 ± 6.70) Ng/L,( 115.28 ± 13.76 ) ng/Lvs.(148.99 ± 16.61) ng/L,(20.43 ±3.92) ng/L vs.(32.32 ± 8.18)ng/L] (P ＜0.05).But there was nosignificant difference before and aftertreatment in the control group [ (25.25 ± 7.55 )ng/L vs.(28.82 ± 7.54)ng/L,( 136.27 ± 12.97) ng/L vs.( 150.21 ± 17.52) ng/L,(28.43 ± 3.92) ng/L vs.(32.56 ± 8.78) ng/L] (P＞0.05),there were significant differences after treatment between two groups (P ＜0.05).Conclusions Acetylcysteine can inhibit inflammation and e hminate free radicals in plasma.Acetylcysteine has beneficial effects on COPD.

Objective To investigate the possible mechanism of Nrf2 in sepsis development and its effect in the septic acute liver injury ,and derive the effect of curcumin on Nrf2 expression ,which provide theoretical basis for clinical prevention and treatment of sepsis .Methods Divided putting 72 male Sprague-Dawley rats into 3 groups:control group ,experimental group and intervention group .After operation ,3 groups were further divided into subgroups 6 hours later ,12 hours later ,24 hours later and 48 hours later respectively .The septic rats model with acute liver injury was reproduced by method of cecal ligation and puncture (CLP) .The ex-perimental group and the intervention group took CLP .The rats in the intervention group were rejected with curcumin in the abdo-men at a dosage of 100 mg/kg an hour after operation .Detected the expression of Nrf2 in the livers of each group used Western blot and measure alanine aminotransferase(ALT ) in serum by drawing the rats′heart blood ;observed the pathological changes in the liver with HE coloration .Results ALT :the values of ALT in the experimental group was significantly increased compared with the control group(P<0 .05);while in the intervention group ,it was markedly improved compared with the experimental group (P<0 . 05) .The expression of Nrf2 :Nrf2 exists in control group rat′s hepatic tissue .In the experimental group ,it was progressively in-creased since the 6 h time point and decreased after the 12 h time point which shown significant decreased compared with the control group(P<0 .05) .In the intervention group ,it was gradually increased since the 6 h time point and decreased after the 24 h time point which was obviously higher than the experimental group(P<0 .05) .Pathological changes :There was no obvious abnormalities in the control group rats′liver structure ,while a lot of inflammatory cells gather and liver cells swell in the experimental group .It was obvious improved after curcumin intervention .Conclusion As Nrf2 is generally lower in the experimental group than in the comparative group ,it shows that Nrf2 directly participates in the occurrence and development of sepsis ,while severe infection blow damages the endogenou s protection system .Decreased activity of Nrf2 causes ignificantly inhibition of anti-oxidative stress and nat-ural immune response ,which may exacerbate acute liver injury by oxidative stress in sepsis .In case of sepsis ,curcumin may increase the level of Nrf2 and the antioxidant enzyme′s activity in the hepatic tissues ,enhancing the general antioxidant ability and alleviating the oxidative stress which points out that curcumin prevents the septic acute liver injury .

Objective To prepare a new type of micropore porcine acellular dermis matrix with the aid of laser (LPADM),and to validate the healing effect of the LPADM through the phrase Ⅰ composite transplanting on the back of the full- thickness skin defects in SD rats.Methods In vitro,the allogeneic fibroblasts were separately cultured with the LPADM (LPADM group) or the non-pore PADM (non-pore LPADM group),while fibroblasts cultured by pure medium were used as control.After culture of 1 day,3 days and 5 days,the contents of IL-10,IL-6,TGF-β1,LN,VEGF expressed by fibroblasts were determined by double-antibody sandwich ELISA method.In vivo,the phrase Ⅰ transplantations of LPADM graft with split-thickness autologous skin were carried on the backs of the full-thickness cutaneous defects of SD rats.The healing condition was observed and analyzed by histological tests.Results The differences of the absorbance value between the LPADMgroup,PADM group and control group in each day were not statistically significant (F=0.050-1.763,P＞0.05).The transplantation of LPADM graft with split-thickness autologous skin graft resulted in high rate of surviving without signs of rejection 3 weeks later.After 1-month of transplantation,the regenerated skin was well enough to be lifted without any serious scars.Conclusions The phrase Ⅰ transplantation of LPADM graft with split-thickness autologous skin graft can accelerate the healing process of full-thickness skin wounds with high biological safety.

Objective To investigate the application value of enhanced recovery after surgery(ERAS) in laparoscopic pancreaticoduodenectomy (LPD).Methods The retrospective cohort study was adopted.The clinical data of 64 patients who underwent LPD from January 2014 to January 2016 in the Shangjin Hospital of West China Hospital of Sichuan University were collected.Of the 64 patients,41 patients managed with ERAS program between March 2015 and January 2016 were allocated into the ERAS group,23 patients managed with traditional perioperative treatment between January 2014 and Febuary 2015 were allocated into the traditional group.The following indexes were observed:(l) intraoperative status:operation time,volume of intraoperative blood loss,conversion to open surgery,pylorus preservation.(2) Postoperative status:the time to out-of-bed activity,time to postoperative flatus,time of drainage tube removal,postoperative complications (pancreatic leakage,bile leakage,hemorrhage,delayed gastric emptying,abdominal infection,cardiovascular complications),duration of postoperative hospital stay,death within the postoperative 30 days.(3) Follow-up status:incidence of complications after discharge and survival of patients.The follow-up including incidence of complications after discharge and survival of patients was conducted by outpatient examination and telephone interview up to March 2016.Measurement data with normal distribution were presented as (x) ± s and analyzed by t test.Count data were analyzed using the chi-square test or Fisher exact probability.Results (1) Intraoperative status:the operation time,volume of intraoperative blood loss,number of patients with conversion to open surgery and pylorus preservation were (377 ± 38) minutes,(164 ± 48) mL,1,40 in the ERAS group and (392 ± 53) minutes,(152 ±31)mL,2,21 in the traditional group,showing no statistically significant difference between the 2 groups (t =5.02,8.43,x2=1.29,1.29,P ＞ 0.05).(2) Postoperative status:the time to out-of-bed activity,time to postoperative flatus,time of drainage tube removal and duration of postoperative hospital stay were (1.7 ± 0.6)days,(2.5 ± 0.6) days,(5.3 ± 1.7) days,(9.1 ± 1.3) days in the ERAS group and (2.1 ± 0.9) days,(3.8 ±1.2) days,(8.2 ± 2.6) days,(11.9 ± 1.8) days in the traditional group,showing statistically significant differences between the 2 groups(t =-5.28,-7.01,-16.20,-10.67,P ＜ 0.05).The numbers of patients with pancreatic leakage in stage A,B and C,bile leakage,hemorrhage,delayed gastric emptying,abdominal inflection,cardiovascular complications and death in the postoperative 30 days were 8,0,0,0,1,3,0,1,1 in the ERAS group and 5,1,0,1,1,3,2,1,0 in the traditional group,respectively,showing no significant difference between the 2 groups (x2=0.37,1.81,0.18,0.57,3.68,0.18,P ＞0.05).(3) Follow-up status:the 64 patients were followed up for a median time of 11 months (range,1-25 months).During the follow-up,number of patients complicated with diabetes,local tumor recurrence,liver metastasis and death were 5,4,1,0in the ERAS group and 2,5,2,3 (2 died of tumor recurrence and 1 died of myocardial infarction) in the traditonal group.Conclusion Application of ERAS in the perioperative management of LPD is safe and effective,meanwhile,it can accelerate the recovery of patients who underwent LPD and shorten the duration of hospital stay.

Abstract: Objective To investigate the dynamic changes and clinical significance of specific antibodies in patients with coronavirus disease 2019 (COVID-19). Methods A retrospective study was conducted to collect 141 adult COVID-19 survivors who were followed up in the Eighth Hospital affiliated to Guangzhou Medical University from February 6, 2020, to March 24, 2021. The patients were divided into severe group (severe and critical) and non-severe group (light and ordinary) according to the diagnosis at discharge. The antibody changes of the two groups were compared and analyzed at 1 week, 2 weeks, 1 month, 3 months, 6 months and 1 year after discharge. Results After discharge from hospital, the positive rate of IgG in the severe group was 95.00% after 1 week and 100.00% in the following year, in the positive rate of IgG in the non-severe group was 59.50% after 1 week, 90.08% in 6 months and 76.03% in one year. The level of serum IgG in the severe group was significantly higher than that in non-severe group (Z=-2.441, P=0.015). One-year follow-up: the serum IgG in the severe group was significantly higher than that in the non-severe group (Z=-3.410, P=0.001). The serum IgM level of the severe group after one year follow-up was lower than that of the six months follow-up, the difference was statistically significant (Z=-2.259, P=0.024). The serum IgG and IgM level of the non-severe group after one year follow-up was lower than that of the six months follow-up, the difference was statistically significant (Z=-7.37, P<0.01; Z=3.850, P<0.01). Conclusion The level of serum protective antibody in COVID-19 patients remained high within 6 months after discharge, and remained stable within 1 year after discharge. The antibody titers in the severe group were significantly higher than those in the non-severe group and lasted for at least one year. COVID-19 survivors receive 1 year of natural immune protection, and patients with critical conditions receive immunity for longer periods of time.

Objective The morphological and particle size distribution of Fritillaria unibracteata under different preparation conditions and the physicochemical property before and after grinding were studied, which provides the experimental basis for the particle size control and application of ultra-fine powder. Methods The single-factor experiment was carried out. The ultra-fine powder was prepared using ball milling method under different milling time, the weight ratio of ball to material, and rotating speed conditions. The powder morphology was observed by scanning electron microscopy. The particle size and particle size distribution of the powder was determined by laser particle size analyzer. The particle size, particle size distribution, expansion degree, total alkaloid content, and other properties of F. unibracteata powder before and after milling were compared. Results The optimum conditions for powder preparation were as follows: The grinding time was 0.5 h, the weight ratio of ball to material was 12:1, the rotating speed was 250 r/min. The volume average particle size of ultrafine powder was (26.92 ± 0.12) μm, and D90 was (45.15 ± 0.18) μm. After grinding, the particle size decreased, the particle size distribution became narrower, the expansion degree and angle of repose increased, the hygroscopicity of powder did not change significantly, the bulk density decreased, and the alcohol-soluble extract content and total alkaloid content increased greatly. Conclusion Moderate grinding can significantly reduce the size of F. unibracteata powder, improve the particle size distribution, increase the total alkaloid content, and help to improve bioavailability.

Objection To observe the change rules of quantity and species of diatom sin Hunhe River in Shenyang and to provide technology and scientific evidence for drow ning identification and the location of drow ning in forensic investigation. Methods In 2011, different locations for collecting water sam pleswere chosen in Hunhe River in Shenyang. Water sam pleswere collected and variation of quantity and species of diatom swere observed every month. And variation of dom inant species of diatom swas ob-served every week. Results The quantity, speciesand dom inant species of diatom sin Hunhe River in Shenyang varied with different tim eand locations. The quantity and species of diatom swere lowest from Decem ber to February and gradually increased, reaching peak in May and second peak in October, and then gradually decreased. The dom inant species of diatom s varied significantly adjacent two weeksat same location from April to N ovem ber, but had little changesat different locationsin same week from July to August. Conclusion The change rules of quantity and species of diatom sare com plicated and affected by various factors such as environm ent and hydrology. The change rules of speciesand quantity of diatom s should be considered in forensic investigation of drow ning identification and the lo-cation of drow ning.
