1.Protective Effect of Xuebijing on Lung Injury in Rats with Severe Acute Pancreatitis by Blocking FPRs/NLRP3 Inflammatory Pathway

		                        			ObjectiveTo explore the therapeutic effect of Xuebijing injection (XBJ) on severe acute pancreatitis induced acute lung injury (SAP-ALI) by regulating formyl peptide receptors (FPRs)/nucleotide-binding oligomerization domain-like receptor 3 (NLRP3) inflammatory pathway. MethodsSixty rats were randomly divided into a sham group, a SAP-ALI model group, low-, medium-, and high-dose XBJ groups (4, 8, and 12 mL·kg-1), and a positive drug (BOC2, 0.2 mg·kg-1) group. For the sham group, the pancreas of rats was only gently flipped after laparotomy, and then the abdomen was closed, while for the remaining five groups, SAP-ALI rat models were established by retrograde injection of 5% sodium taurocholate (Na-Tc) via the biliopancreatic duct. XBJ and BOC2 were administered via intraperitoneal injection once daily for 3 d prior to modeling and 0.5 h after modeling. Blood was collected from the abdominal aorta 6 h after the completion of modeling, and the expression of interleukin (IL)-1β, IL-6, and tumor necrosis factor-α (TNF-α) in plasma was measured by enzyme-linked immunosorbent assay (ELISA). The amount of ascites was measured, and the dry-wet weight ratios of pancreatic and lung tissue were determined. Pancreatic and lung tissue was taken for hematoxylin-eosin (HE) staining to observe pathological changes and then scored. The protein expression levels of FPR1, FPR2, and NLRP3 in lung tissue were detected by the immunohistochemical method. Western blot was used to detect the expression of FPR1, FPR2, and NLRP3 in lung tissue. Real-time fluorescence quantitative polymerase chain reaction (Real-time PCR) was used to detect the mRNA expression of FPR1, FPR2, and NLRP3 in lung tissue. ResultsCompared with the sham group, the SAP-ALI model group showed significantly decreased dry-wet weight ratio of lung tissue (P<0.01), serious pathological changes of lung tissue, a significantly increased pathological score (P<0.01), and significantly increased protein and mRNA expression levels of FPR1, FPR2, and NLRP3 in lung tissue (P<0.01). After BOC2 intervention, the above detection indicators were significantly reversed (P<0.01). After treatment with XBJ, the groups of different XBJ doses achieved results consistent with BOC2 intervention. ConclusionXBJ can effectively improve the inflammatory response of the lungs in SAP-ALI rats and reduce damage. The mechanism may be related to inhibiting the expression of FPRs and NLRP3 in lung tissue, which thereby reduces IL-1β and simultaneously antagonize the release of inflammatory factors IL-6 and TNF-α.

		                        			[摘  要]  目的:探讨多结节非小细胞肺癌(NSCLC)组织中的驱动基因突变情况与临床病理特征的关系,为多结节NSCLC患者治疗提供分子诊断依据。方法:本研究共纳入2018年1月至2023年10月间云南省肿瘤医院分子诊断中心检测的121例多结节NSCLC患者的253个肺结节肿瘤组织标本,以第二代测序(NGS)技术或扩增阻滞突变系统PCR(ARMS-PCR)技术检测多结节NSCLC 组织中驱动基因突变情况,分析其与患者临床病理特征的关系,比较不同结节间肺癌驱动基因的突变异质性。结果:与非“宣威”NSCLC相比,“宣威”多结节NSCLC患者驱动基因突变具有显著的地域特点,表现在“宣威”患者具有较低(20%)的EGFR敏感突变(L858R、19-del)及较高(27.26%)的EGFR少见突变(主要为G719/S768I、G719);“宣威”多结节NSCLC患者的KRAS突变率(27.27%)亦显著高于非“宣威”患者突变率(12.59%)(P<0.05)。此外,“宣威”多结节NSCLC患者驱动基因突变不一致率高达69.23%,远高于非“宣威”患者驱动基因突变不一致率(55.07%)(P<0.05)。结论:“宣威”多结节NSCLC患者具有较高的EGFR少见突变及KRAS突变率,同一患者不同病灶之间存在更高的驱动基因突变异质性,本研究将为“宣威”多结节NSCLC的诊疗策略提供更多的选择。

5.Different reaction patterns and influencing factors among family caregivers of patients with advanced gynecological malignancies: a latent profile analysis

		                        			Objective:To explore the different reaction patterns among family caregivers of patients with advanced gynecological malignancies, and to analyze the influencing factors of different profiles.Methods:A cross-sectional study was conducted with 210 family caregivers of patients with advanced gynecological malignancies from Obstetrics and Gynecology Hospital in Shanghai from January 2022 to December 2022. Data were collected by Caregiver Reaction Assessment, Mishel's Uncertainty in Illness Scale-Family Member Form and Perceived Social Support Scale. Latent profile analysis was used to explore the different reaction patterns among family caregivers of patients with advanced gynecological malignancies. The influencing factors of caregiver reaction were identified by multivariate Logistic regression.Results:Totally 208 questionnaires were effectively collected. The family caregivers among patients with advanced gynecological malignancies included 163 males and 45 females, aged (53.89 ± 12.61) years old. The reaction characteristics of 208 family caregivers among patients with advanced gynecological malignancies were divided into three categories: low burden and high benefit group (24.5%, 51/208), moderate burden and benefit group (30.8%, 64/208), and high burden and low benefit group (44.7%, 93/208). Compared to the low burden and high benefit group, caregivers with lower levels of social support were more likely to be classified as moderate burden and benefit group, high burden and low benefit group ( OR = 0.563, 0.407, both P<0.01). Caregivers with moderate burden and benefit group, high burden and low benefit group had higher levels of disease uncertainty ( OR = 1.328, 2.064, both P<0.01). The caregiver′s age, monthly family income, education level, and co-caregivers were also influencing factors regarding to care reaction among family caregivers of patients with advanced gynecological malignancies ( OR values were 0.207-6.422, all P<0.05). Conclusions:The care reaction of family caregivers among patients with advanced gynecological malignancies has obvious categorical features. Healthcare professionals should implement targeted nursing interventions according to their reaction characteristics, so as to reduce the care burden of family caregivers and improve the quality of care for patients with advanced gynecological malignancies.
		                        		
		                        		
		                        		
		                        	
6.Research on constructing talent training objectives and courses of undergraduate education of tropical medicine by Delphi method

		                        			Objective:To construct talent training objectives and courses for undergraduate education of tropical medicine.Methods:Two rounds of questionnaire consultation were conducted among 15 experts by Delphi method. SPSS 26.0 software was used for statistical analysis, and the recovery rate, expert authority coefficient, mean of importance score, full score ratio, coefficient of variation and Kendall coordination coefficient were calculated respectively. Kendall's rank correlation test was used to analyze the degree of expert coordination, and the "boundary value method" was used to screen the indicators.Results:The effective recovery rates of the two rounds of consultation were all 100.00% and the expert authority coefficient was 0.815. The coordination coefficient was 0.25, 0.32, and 0.27, 0.36 respectively, and the significance test showed P<0.001. Finally, 11 talent training objectives and 7 courses for undergraduate education of tropical medicine were formed. Conclusions:The talent training objectives and courses for undergraduate education of tropical medicine are reasonable and reliable, which can provide theoretical support for tropical medicine talent training and have certain guiding value.
		                        		
		                        		
		                        		
		                        	
7.Early gait analysis after total knee arthroplasty based on artificial intelligence dynamic image recognition

		                        			Objective To explore early postoperative gait characteristics and clinical outcomes after total knee arthroplasty(TKA).Methods From February 2023 to July 2023,26 patients with unilateral knee osteoarthritis(KOA)were treated with TKA,including 4 males and 22 females,aged from 57 to 85 years old with an average of(67.58±6.49)years old;body mass in-dex(BMI)ranged from 18.83 to 38.28 kg·m-2 with an average of(26.43±4.15)kg·m-2;14 patients on the left side,12 pa-tients on the right side;according to Kellgren-Lawrence(K-L)classification,6 patients with grade Ⅲ and 20 patients with grade Ⅳ;the courses of disease ranged from 1 to 14 years with an average of(5.54±3.29)years.Images and videos of standing up and walking,walking side shot,squatting and supine kneeling were taken with smart phones before operation and 6 weeks after operation.The human posture estimation framework OpenPose were used to analyze stride frequency,step length,step length,step speed,active knee knee bending angle,stride length,double support phase time,as well as maximum hip flexion angle and maximum knee bending angle on squatting position.Western Ontario and McMaster Universities(WOMAC)arthritis index and Knee Society Score(KSS)were used to evaluate clinical efficacy of knee joint.Results All patients were followed up for 5 to 7 weeks with an average of(6.00±0.57)weeks.The total score of WOMAC decreased from(64.85±11.54)before op-eration to(45.81±7.91)at 6 weeks after operation(P<0.001).The total KSS was increased from(101.19±9.58)before opera-tion to(125.50±10.32)at 6 weeks after operation(P<0.001).The gait speed,stride frequency and stride length of the affected side before operation were(0.32±0.10)m·s-1,(96.35±24.18)steps·min-1,(0.72±0.14)m,respectively;and increased to(0.48±0.11)m·s 1,(104.20±22.53)steps·min-1,(0.79±0.10)m at 6 weeks after operation(P<0.05).The lower limb support time and active knee bending angle decreased from(0.31±0.38)sand(125.21±11.64)° before operation to(0.11±0.04)s and(120.01±13.35)° at 6 weeks after operation(P<0.05).Eleven patients could able to complete squat before operation,13 patients could able to complete at 6 weeks after operation,and 9 patients could able to complete both before operation and 6 weeks after operation.In 9 patients,the maximum bending angle of crouching position was increased from 76.29° to 124.11° before operation to 91.35° to 134.12° at 6 weeks after operation,and the maximum bending angle of hip was increased from 103.70° to 147.25° before operation to 118.61° to 149.48° at 6 weeks after operation.Conclusion Gait analysis technology based on artificial intelligence image recognition is a safe and effective method to quantitatively identify the changes of pa-tients'gait.Knee pain of KOA was relieved and the function was improved,the supporting ability of the affected limb was im-proved after TKA,and the patient's stride frequency,stride length and stride speed were improved,and the overall movement rhythm of both lower limbs are more coordinated.
		                        		
		                        		
		                        		
		                        	
8.CDK4/6 inhibitors in the first-line treatment of HR+/HER2-advanced stage breast cancer:a rapid health technology assessment

		                        			Objective To evaluate the efficacy,safety and economy of cyclin-dependent kinase 4/6(CDK4/6)inhibitors for the first-line treatment of hormone receptors positive(HR+),human epidermal growth factor receptor 2 negative(HER2-)advanced breast cancer(ABC)by rapid health technology assessment,and to provide evidence for clinicians and policymakers.Methods PubMed,Cochrane Library,Embase,CNKI,WanFang Data,VIP databases and the official website of health technology assessment(HTA)agency were electronically searched to collect clinical evidence and literature of CDK4/6 inhibitors in the treatment of HR+/HER2-ABC from the inception to December 31,2023.Two reviewers independently identified studies,extracted data,assessed the quality of included studies,and descriptively analyzed and summarised the results.Results A total of 33 articles were included,including 9 systematic reviews/Meta-analyses,15 pharmacoeconomic studies and 9 HTA reports.In terms of efficacy,compared with endocrine therapy alone,the addition of CDK4/6 inhibitors significantly improved progression-free survival(PFS)and overall survival(OS)in patients with HR+/HER2-ABC(P<0.05),but there was no significant difference in efficacy among palbociclib,abemaciclib and ribociclib(P>0.05).In terms of safety,more adverse events were observed in patients treated with CDK4/6 inhibitors when compared with endocrine therapy(P<0.05).There was a difference in the incidence of adverse effects between the different CDK4/6 inhibitors,with palbociclib having higher incidence of haematological adverse effects(P<0.05),and abemaciclib being more likely to cause gastrointestinal adverse reactions such as diarrhoea(P<0.05).The economic evaluation results were variable due to differences in healthcare costs,analysis perspectives,willingness-to-pay thresholds,and study duration in different countries.Conclusion CDK4/6 inhibitors have similar efficacy in the first-line treatment of HR+/HER2-ABC patients,but there are some differences in aspects such as safety and economy.
		                        		
		                        		
		                        		
		                        	
9.Introduction to the revision of Diagnostic Standard for Occupational Medicamentose-like Dermatitis due to Trichloroethylene

		                        			With the development of clinical related disciplines, the update and establishment of relevant standards/guidelines at home and abroad, GBZ 185-2006 Diagnostic Criteria for Occupational Medicamentose-like Dermatitis due to Trichloroethylene (hereinafter referred to as “GBZ 185-2006”) was unable to meet clinical needs. Therefore, the GBZ 185-2006 was revised based on the principles of evidence-based medicine, in accordance with relevant laws/regulations and relevant standards/guidelines in combination with review of research data on occupational medicamentose-like dermatitis due to trichloroethylene (OMDT) home and abroad, and the development of clinical practice and clinical related disciplines. The main modifications include: adding terms and definitions of OMDT, modifying the description of clinical manifestations of the diagnostic principles, adjusting the description of latency, deleting the diagnostic requirement of the incidence probability, adding the specific allergen patch test as the etiological diagnostic index, standardizing the application scope, operating procedure and precautions of the specific allergen patch test. In addition, the relevant content of “Basic Characteristics and Clinical Types of Skin Damage of Medicamentose-like Dermatitis due to Trichloroethylene” in Appendix A is improved, the treatment principles are revised, and the content of new progress in treatment, artificial liver application, are added. The revised GBZ 185-2024 Diagnostic Standard for Occupational Medicamentose-like Dermatitis due to Trichloroethylene is more scientific and practical, and can provide technical basis for the standardized diagnosis and treatment of OMDT in medical and health institutions. 
		                        		
		                        		
		                        		
		                        	
10.Effect and its mechanism of esketamine on anxiety and depression in mice

		                        			To explore the effect of esketamine on anxiety-depressive-like behavior in mice and its rela- tionship with inflammation.
		                        		
		                        			Methods    :
		                        			SPF  grade  healthy  adult male C57BL/6J mice,weighing 20 -24  g,were  used in the exprement.The random number table method was used to divide into 5 groups  (n = 8) : control  group  ( Con group) ,esketamine group  (ESK  group) ,model group  ( LPS  group) ,model + esketamine prevention  group  (LPS + ESK1 group) and model  +  esketamine treatment group  ( LPS + ESK2  group) .An inflammation-induced  anxiety-depression model was prepared by intraperitoneal injection of LPS 0. 83 mg / kg.The ESK group was injec- ted with esketamine 10  mg / kg ; LPS  group was injected with  LPS 0. 83  mg / kg ; LPS + ESK1  group  was  injected  with LPS 0. 83 mg / kg before 24 hours intraperitoneal injection of esketamine 10 mg / kg ; and the LPS + ESK2 group  was injected with LPS 0. 83 mg / kg and 30 minutes later with esketamine 10 mg / kg.24 h after intraperitoneal injec- tion of LPS,the anxiety-depression-like behaviors of mice were measured using behavioral experiments.At the end  of behavioral experiments,the spleen was taken immediately ; hippocampal tissues were taken and enzyme-linked  immunosorbent assay  (ELISA) was used to detect the contents of interleukin-1 β  (IL-1 β) ,tumor necrosis factor al- pha  (TNF-α) and interleukin 6  (IL-6) and neuronal pathological changes in hippocampal tissues were observed by  HE staining.  
		                        		
		                        			Results    :
		                        			Compared with the Con group,mice in the LPS group showed increased anxiety and depres- sion-like behavior  (P<0. 05) ,increased  spleen  weight / body  weight  (P <0. 05 ) ,increased  hippocampal  tissue  concentrations of IL-1 β , TNF-α and IL-6  (P<0. 05) ,and increased neuronal degeneration necrosis,there was no  statistically significant difference in these indicators in the ESK group compared with the Con group.Compared with  the LPS group,mice in the LPS + ESK1 and LPS + ESK2 groups showed reduced anxiety-depression-like behavior (P<0. 05) ,decreased splenic weight / body weight  (P <0. 05) ,hippocampal  tissue  IL-1 β , TNF-α , IL- 6  con- centrations were reduced  (P<0. 05) ,and neuronal degeneration necrosis was reduced.Compared with the LPS +  ESK1 group,the LPS + ESK2  group  showed an increase in the distance travelled in the central area of the open  field experiment and the distance into the open arm of the elevated cross maze experiment  (P<0. 05) ,a decrease  in IL-6 and TNF-α concentrations  (P<0. 05) ,and a reduction in the degree  of neuronal damage.
		                        		
		                        			Conclusion
