Objective To assess the diagnostic value of the single-photon-emission computed tomography (SPECT) and the magnetic resonance imaging (MRI) for Alzheimer's disease (AD) and vascular dementia (VD), and to explore the correlation between cerebral atrophy and cerebral regional blood flow and the degree of cognitive impairment in AD patients. Methods The temporal and frontal lobes and hippocampus were quantitatively detected by MRI and the cerebral regional blood flow in these regions was analysed semi-quantitatively by SPECT for 36 patients with AD, 32 with VD and 30 sex-matched non-demented elderly controls (NC). Results The hippocampal atrophy of AD and VD groups detected on MRI was significantly different from that of the NC group. Left hippocampal height was (9.27?3.43)mm in AD, (11.62?5.72)mm in VD and (12.13?2.97)mm in NC(P0.05, compared to that of NC), showing that hippocampal atrophy could not be detected by MRI. However, RAR of the left hippocampus by SPECT detection was 0.61?0.07 (P

Background and purpose:Stromal tumor is one of the common gastrointestinal mesenchymal tumors. There is certain understanding about the typical cases. However, the diagnosis of those occurring in rare location or with rare imaging findings is often difficult. This research summarized this kind of cases,in order to increase the radiological knowledge of the disease. Methods:This study retrospectively analyzed clinical, radiological and pathological data from 550 patients who had stromal tumor conifrmed by pathology in our hospital. Those with incomplete data were eliminated. Forty-nine patients were selected for further study according to the typical imaging findings. Results:Among these 49 patients, 9 were pathologically confirmed to have extra-gastrointestinal stromal tumor, while 40 patients had gastrointestinal stromal tumor. Among the patients with gastrointestinal stromal tumor, 22 were found in rare locations, 12 in retroperitoneal space, 3 in omentum majus and mesenterium, 5 in esophagus, and 2 in prostate. Obvious cystic degeneration was found in 16 patients. Bulky calciifcation, such as lfake or annulus, was found in 7 patients. The analysis result of risk-stratiifcation showed 19 patients were conifrmed as high-grade among the patients with tumors found in rare locations, 15 as high grade among those with obvious cystic degeneration, and 7 as high-grade among those with extra-gastrointestinal stromal tumor. Conclusion:Rare stromal tumor often occurs in the locations, such as retroperitoneal space, omentum majus and mesenterium. Obvious cystic degeneration and bulky calciifcation can be seen. The risk-stratiifcation of these patients often showed high-grade. Comprehensively analyzing its clinical features and imaging ifndings can help improve the diagnostic accuracy.

OBJECTIVETo explore the effect of half circle external fixation for the treatment of open tibial fractures.
METHODSFrom March 2005 to March 2011, 94 patients with open tibiofibula fractures were treated by closed manipulative or Kirschner-wire poking reduction with half circle groove external fixation including 63 males and 31 females with an average age of 39 years old ranging from 17 to 65 years old. Among these patients, 5 cases were cross shaped fractures, 19 were oblique form and spiral fractures, 70 cases were comminuted fractures. According to the fracture Gustilo classification, 49 cases were type IIIA, 45 were type IIIB. The incidence of wound infection, fracture healing time were observed. The function was evaluated according to the Johner-Wruhs standard.
RESULTSAll patients were followed up for 14 to 63 months (averaged 29 months). The wound healing time was from 16 to 39 weeks (means 21.4 weeks). No fracture nonunion, osteomyelitis and calf compartment syndrome occurred. The wounds of 81 cases were healed at the first period, deep wound infection occurred in 2 cases. According to the function Johner-Wruhs evaluation criteria:the result was excellent in 52 cases, good in 37 cases, fair in 5 cases.
CONCLUSIONClosed manipulative or Kirschner-wire poking reduction and half circle groove external fixation can reduce the infection rate of open tibiofibula fractures. For open tibiofibula comminuted fractures, after the half circle groove external fixation for 3 to 6 weeks, a middle Kirschner-wire was removed according to fracture end healing situation to make fixation dynamic and promote fracture healing.

Aim To explore the effects of propofol on learning and memorizing ability and the effects of com-bination of propofol and phenobarbital sodium on epi-leptic rats. Methods Thirty-six epileptic rats were di-vided into epileptic model group (EP),normal saline group (NS),lipid emulsion +epileptic group (LE), phenobarbital sodium +epileptic group (PB),propofol+epileptic pattern (Prof),and combination of propo-fol and phenobarbital sodium +epileptic group. Each group had 6 rats.Tests of Morris water maze were giv-en to the rats to evaluate their learning and memorizing ability. The protein expression of toll-like receptor 4 (TLR4 )was examined by ELISA.Results There were no effects of saline and lipid emulsion on learning and memorizing ability and the expression of TLR4 pro-tein in hippocampus in epileptic rats (P >0.05 ). Propofol could increase the incubation period in epilep-tic rats obviously, shorten the plateau period, and in-crease the expression of TLR4 protein in hippocampus (P <0.05 ).Phenobarbital sodium could shorten the plateau period in epileptic rats, and increase the ex-pression of TLR4 protein in hippocampus (P <0.05), but it had no effect on the incubation period (P >0.05).Compared with PB,combination of propofol and phenobarbital sodium +epileptic group had a lon-ger incubation period and a shorter plateau period with an increase of the expression of TLR4 protein in hippo-campus (P <0.05 ).Compared with propofol group, combination of propofol and phenobarbital sodium +ep-ileptic group had a shorter plateau period (P <0.05) with an obvious increase in the expression of TLR4 pro-tein in hippocampus (P <0.05),but it had no effect on incubation period (P >0.05 ).Conclusions Propofol damages the learning and memorizing ability of epileptic rats.Phenobarbital sodium had no obvious effect on the learning ability in epileptic rats, but harms the memorizing ability in epileptic rats.Combi-nation of propofol and phenobarbital sodium affects the learning and memorizing ability of epileptic rats.Hip-pocampus TLR4 may be involved in the effect of propo-fol and phenobarbital sodium on the learning and mem-orizing ability of epileptic rats.

Objective To summarize the effect of laparoscopic repair of acute gastric perforation. Methods Patients with acute gastric perforation underwent laparoscopic repair and abdominal drainage under intravenous- inhalation general anesthesia after a histological biopsy. Results Forty-six patients with acute gastric perforation were successfully operated.Of which, 6 patients with pylonic perforation, 37 patients with perforation in the front wall of gastric antrum, 3 patients with perforation in stomach body. Thirty-three cases were received frozen section biopsy, only 1 case showed a cancer in the lesser curve of stomach body. There was no complications occurred and no operative mortality in this series. Of the 45 benign ulcers patients,31 patients were discharged 7 days after the operation, 14 patients were discharged 2 weeks after medication treatment.Three months after operation the symptoms of the 45 patients disappeared totally, they had normal diet, and got body weight 1-3kg. Pathological examination showed that no malignant cell was found in 45 benign ulcer patients. The cancer patient was discharged 9 days after operation and refusing to further treatment. Conclusions Laparoscopic repair is a safe mini-invasive method, and it isn′t breach the surgical principles,it allows the patients recover quickly and with less complications. The intraoperative biopsy can prevent misdiagnosis of cancer. It is an effective treatment for acute gastric perforation and worthy to be widely used.
