3.In Vitro Antibacterial Activity of Meropenem and Its Pharmacokinetics in Patients with Burn Injury

OBJECTIVE: To study the antibacterial activity of meropenem in vitro and the pharmacokinetics of which in patients with burn injury. METHODS: The minimal inhibition concentration (MIC) of meropenem against 136 clinical isolates was determined with double agar dilution method. 28 patients with burn injury were given 0.5g of meropenem by iv gtt, with the blood concentration of meropenem and the recovery of meropenem in urine were determined at different time. The pharmacokinetics parameters were computed with 3p97 software. RESULTS: Meropenem showed a strong antibacterial activity against K.peneumoniae, pneumonococcus and Enterobacter cloacae. MIC90 was below 0.0 075~0.25?g/ml. Pharmacokinetics parameters of meropenem were the following: t1/2? was (0.35?0.12) h,t1/2? was (2.10?0.71) h,AUC was (44.62?12.95) ?g/ml,Vc was (10.60?3.93) L; CLs was (12.00?3.04) L/h. 6.5 hours later, the mean blood concentration was (1.01?0.53)?g/ml; and the recovery of meropenem in 0h~12h urine was (58.06?16.83) %. CONCLUSIONS: Meropenem showed a very strong antibacterial activity in this study,there were significant differences regarding the in vivo pharmacokinetics parameters between the healthy volunteers and the patients with burn injury.
4.A survey on chronic heart failure management in community physicians in Chengdu

A questionnaire survey on chronic heart failure management was conducted in 110 physicians from Xindu,Supo,Jiajiang and Xinhua community hospital in Chengdu from January 2007 to June 2010.Results showed that 77.3% (85/110) of community physicians lacked knowledge about prevention,diagnosis and treatment of chronic heart failure,and the diagnostic accuracy rate was only 51.3% (40/78) in these community hospitals.There was lack of awareness of the guideline of chronic heart failure:the rate of β blockers use was 15.0% (6/40) and only 5.0% (2/40) used the target dose; the rate of angiotensin converting enzyme inhibitor use was 17.5% (7/40) and only 7.50% (3/40) used the target dose.In addition,90% (99/110) of community doctors lacked the education and management for patients with heart failure.The survey suggests that the current situation of chronic heart failure management in community physicians in Chengdu is unsatisfactory.It is necessary to strengthen the training of community physicians.
5.Isolation and characterization of the conotoxins of Conus textile from the South China Sea

Objective To isolate new conotoxins of Conus textile from the South China Sea,provide new conopeptides for drug discovery and design, and investigate the effects of living environment on toxin secretion by comparing the compositions of the conotoxins of this species from different areas. Methods Crude conotoxins were purified by Sephadex G-25 and RP-HPLC, the amino acid sequences of the purified peptides were determined. Results More than 10 conotoxins were isolated from Conus textile, 4 conotoxins were firstly isolated and 6 conotoxins had the similar sequence as the reported peptides. Conclusion Many M-family conotoxins were isolated from Conus textile, the main compositions of the venom of Conus textile from the South China Sea were similar to those from Cebu and Marinduquc island in Philippines.
6.Determination of the Content of Ceftazidime in Renal Dialysate Effluent in Burn Patients

OBJECTIVE:To determine the content of ceftazidime in renal dialysate effluent in burn patients.METHODS:HPLC was used.The sample was separated on Bondapak-C18 analysis column with a mobile phase of mixture of methanol-ammonium acetate(17∶83,V/V).The flow rate was 1.0ml/min.The UV detector wavelength was set at 254nm.RESULTS:A linearity was obtained from 0.5~100?g/ml with a good correlation(r=0.9 999).The RSDs of inter-day and intra-day precision were not more than 3.The average recovery was 100.30%.CONCLUSION:The method is simple,rapid and accurate.
7.Effect of Dynamic Contractions on sEMG Co-contraction Ratio Taken from Antagonist Muscles of Knee Joint in Stroke Patients at Convalescence Stage

Objective To observe the influence of dynamic contractions on the sEMG co-contraction ratio taken from the antagonist muscles of the knee joint of stroke patients.Methods 11 stroke patients and 14 healthy subjects were measured the dynamic contractions in both knee flexion and extension with the surface electromyography(sEMG).Results There was no significant differences of the median frequency(MF),mean power frequency(MPF),average EMG(AEMG)and the sEMG co-contraction ratio within the stroke patients and the healthy(P>0.05).There was no significant difference of the MF between the patients compared with the healthy(P>0.05),but the sEMG co-contraction ratio of the MPF increased significantly and that of AEMG decreased significantly(P<0.01).Conclusion The MPF,AEMG and the sEMG co-contraction ratio taken from the stoke patients may reflect the characters of the low limb muscle and the MF may not.
8.Characteristics of Dynamic Contractions on Surface Electromyography Single of Stroke Patients Induced from Low Limb Muscle When Exercising Passively, Exercising Initiatively with Assitant and Against Resistance

Objective To investigate the characteristics of the dynamic contractions on the surface electromyography (sEMG) single of stroke patients induced from the low limb muscle when exercising passively, exercising initiatively with assistant and against resistance.Methods 24 stroke patients with hemiplegia and 17 normal subjects were tested with sEMG under a dynamic contractions in coxa and knee flexion and extension passively, initiatively with assistant and against resistance. The myoelectric signals were collected and processed by linear time and frequency domain method.Results The values of median frequency (MF) and mean power frequency (MPF) of stroke group were significantly lower, but the value of average EMG (AEMG) was higher ( P<0.001). The values of MF and MPF in activity side were lower than that in non-activity side ( P<0.001). The values of MF and MPF when exercising passively were higher than that when exercising with resistance ( P<0.05). The value of AEMG when exercising with resistance was highest. The values of MF and MPF in the synergist muscle were higher. The values of AEMG in the antagonistic muscle and synergist muscle were higher than that agonist and synergist muscle ( P<0.01). The values of MF and MPF in non-paretic exercising side were higher significantly, but in paretic exercising side and non-paretic silent side were lower. The values of MF and MPF in exercising side from vastus lateralis (VL) were the highest. The values of AEMG in exercising side and non-exercising side from biceps femoris (BF) were the highest. The values of MF and MPF in low limb of stroke group reduced, that in rectus femoris (RF) from paretic side was the lowest; that in BF from non-paretic side was the lowest ( P<0.01). The value of AEMG in low limb of stroke group was high significantly, especially in BF from the low limb of the non-paretic side in stroke patients. The values of AEMG in four group muscles gradually were higher following the higher exercising load, and that in the BF was the highest, and that in vastus medialis (VM) rose significantly.Conclusion The values of MF and MPF of stroke patients with hemiplegia reduce significantly, but the value of AEMG is higher. The values of MF and MPF in exercising side are lower than that in non-exercising side and non-paretic exercising side rising significantly, but that in paretic exercising side and non-paretic silent side reduce significantly. The values of MF and MPF in assistant exercise are higher than that in passive exercise and resistance exercise, but the value of AEMG in resistance exercise is higher than that in assistant exercise and in passive exercise. The values of MF and MPF in synergist muscle rise, but the values of AEMG in antagonist and synergist muscle are higher than that agonist and synergist muscle.
9.Efficiency of second trimester screenings for Down syndrome using alpha-fetoprotein and β-human chorionic gonadotropin duplex in 32 188 cases in south Zhejiang

Objective To evaluate the efficiency of second trimester screenings for Down syndrome using alpha-fetoprotein and β-human chorionic gonadotropin duplex.MethodsPregnant women of south Zhejiang were screened for Down syndrome fetuses by maternal alpha-fetoprotein and β-human chorionic gonadotropin duplex during second trimester.The high-risk women underwent prenatal diagnosis by amniocentesis,cell culture and chromosome analysis.The newborns followed up by the maternal and child tertiary health care network and suspected to have Down syndrome were diagnosed by peripheral blood chromosome analysis.Statistical analysis was performed using two-sample t test and x2 test.Risk probability of Down Syndrome was calculated by random screening software. Results From Oct.2007 to May 2009,1130 of 32 188 singleton pregnant women in second trimester received prenatal screening were discovered with high risk(≥1 ∶ 270).Prenatal diagnosis was performed in 90.79% cases (1026/1130) of high risk women and seven fetuses were diagnosed as Down syndrome by amniotic fluid chromosome analysis,and the pregnancies were terminated.Among the other 104 cases without prenatal diagnosis one Down syndrome baby was delivered.Six of 31 058 pregnancy women with low risk delivered Down syndrome babies with the incidence of Down syndrome of 0.19‰ (6/31 058).Detection rate of second trimester screenings for Down syndrome using alpha-fetoprotein and β-human chorionic gonadotropin duplex was 57.14%(8/14).False positive rate was 3.48% (1122/32 188).Positive predictive value was 7.08‰(8/1130).During the same period,there were 23 813 pregnant women who didn't receive screening and 15 fetuses with Down syndrome were diagnosed after birth.There was no statistical difference in the prevalence rate of Down syndrome between those pregnant women who received prenatal screening or not [0.43‰ (14/32 188) vs 0.63‰ (15/23 813),x2 =1.004,P>0.05].The prevalence of Down syndrome was 0.52‰ (29/56 001) in this area. ConclusionsThe prenatal screening and diagnosis could reduce the birth rate of Down syndrome patients.However,detection rate,false positive rate and positive predictive value of which were lower than reports in other studies.It's possible that the reference data might be not suitable for Chinese.
10.Investigation of a wild mushroom poisoning incident

Objective To investigate a poisoning caused by wild mushrooms and to identify the toxin in these mushrooms.Methods Epidemiological investigation,blood test and mushroom toxin were analyzed.Results This incident was taken place in one family,and all family members were dead.Multiple organ damage was observed in all patients;amatoxins and virotoxins were detected in both mushrooms and the soup,but were not detected in blood samples because of dialysis.Conclusion The incident was caused by wild mushrooms and public education shoud be strenthened to urge people to avoid eating wild mushrooms and go to the hospital immediately if poisoning takes place.
