1.The influences of ischemia-reperfusion injury in liver transplantation on intraoperative lung injury and postoperative pulmonary complications

Objective To study the effects of ischemia-reperfusion in liver transplantation on the pathophysiological changes of the lung and mechanisms of lung injury. Methods We studied 23 patients who received liver transplantation at Fuzhou General Hospital of PLA. We cut a small piece of the right lung for pathological study and for L-1β and TNF-a immunohistochemistry studies at 5 minutes after the beginning of operation (Ta), 5 minutes before the portal vein was opened (Tb) and three hours after the new liver was transplanted (Tc). We also collected peripheral blood to study the concentration of IL-1β and TNF-a in the plasma at the beginning of operation (T1), the portal vein 5 minutes before opening, the portal vein (T2) ten minutes after the opening (T3) , and one hour after the new liver was transplanted (T4), three hours after the new liver was transplanted (T5), and 12 hours after operation (T6). Results The cytokines TNF-a and IL-1β in peripheral blood were not obviously increased in the portal vein before it was opened, but were significantly increased after the portal vein was opened. Comparison of T1 and T2 separately with T3, T4 and T5 showed significant differences (P<0. 01). In light and electron microscopy, the structures of the lung tissues were normal at Ta and did not change significantly at Tb. There were significant abnormalities at Tc. The average positive points of TNF-a and IL-1β expressions in the lung tissues at Tc were significantly higher than Ta and Tb(P<0. 01). Conclusion Ischemia-reperfusion in liver transplantation led to a serious systemic inflammatory syndrome,and acute lung injury. TNF-a and IL-1β were involved in acute lung injury.
2.Evaluation of postoperative short·-term survival rate after liver transplantation for benign end-stage hepatopathy by model for end-stage liver disease score and serum sodium concentration and ascites condition

Objective To investigate the efficiency of model for end-stage liver disease(MELD)score,serum sodium concentration and aseites condition in the evaluation of short-term survival rate of patients with benign end-stage hepatopathy after liver transplantation.Methods The clinical data of 98 patients with benign end-stage hepatopathy who had undergone liver transplantation in Fuzhou General Hospital from January 1999 to February 2007 were retrospectively analyzed.The relationship between serum sodium concentration.ascites condition and the prognosis of patients with the same MELD score was analyzed.Kaplan-Meier survival curve was drawn.The 1-year survival rate of the patients was analyzed by chi-square test.The mortality of patients with the same MELD score at the end of the third month after operation was analyzed by Fisher's exact test.Results MELD score of aIJ patients was 15-25 or>25.The postoperatire 3-month mortality rates of patients with serum sodium concentration≥130 mmol/L were 5%and 15%.which were significantly lower than 33%and 55%of those with serum sodium concentration<1 30 mmol/L.The difference upon 1-year survival rates between them had statistical significance(x2:12.88,P<0.05).The postoperative 3-month mortality rates of patients without ascites were 5%and 8%.which were lower than 35%and 57%of those with aseites.and the difference upon 1-year survival rates between them had statistical significance(X2=15.26.P<0.05).Conclusions It is more accurate to evaluate the short-term survival rate after liver transplantation for benign end-stage hepatopathy by combining the MELD score with serum sodium concentration and ascites condition.
3.Exploration of carotid artery elasticity in children with simple obese by echo tracking imaging technique

Objective To explore the elasticity of carotid artery in children with simple obese by echo tracking imaging technique.Methods Intima-media thickness (IMT), resistent index (RI), pulsatility index(PI ), the ratio of systolic phase and diastolic phase(S/D ) of common carotid artery (GCA) were detected differently in 50 simple obese children (obese group) and 50 normal children (control group) by CDFI.Internal systolic diameter (Ds), internal diastolic diameter (Dd),pressure-strain elastic moduhs(E ρ ), stiffness parameter ( β ), arterial compliance (AC), augmentation index (AI), pulse wave velocity ( PWV β ) were calcuhted automatically by means of echo tracking system.Results There was no significant difference in Ds,Dd,RI, PI,S/D and AI'between two groups (P >0.05).In obese group, IMT was significantly thicker,E ρ,β, PWV β were significantly higher,and AC was significantly lower thon those in control group (P < 0.05).Conclusions It has alteration of early atherosele in carotid artery of simple obese children.Echo tracking imaging technique is a quick, noninvasive, accurate and quantitative evaluating method for the elasticity of carotid artery.
4.Genotype and phenotype correlation in patients of tuberous sclerosis complex-associated renal angiomyolipomas

Objective To study the correlation of genotype and phenotype in patients of tuberous sclerosis complex-associated renal angiomyolipomas (TSC-RAML).Methods The clinical and gene mutation data from 39 patients with TSC-RAML diagnosed in Peking Union Medical College Hospital were collected from January 2014 to December 2015.The clinical phenotype and genotype correlation associated with TSC-RAML was analyzed.A total of 39 patients (15 male,24 female) were enrolled,with mean age of 28.4 years (range,13 ~48).Results Five cases showed TSC1 gene mutation,thirty-two patients presented with TSC2 gene mutation and two cases didn't find any mutations.The mutations ratio of TSC1:TSC2 was 1∶6.4.Comparing to those with TSC1 gene mutation and no mutation identified,patients with TSC2 gene mutation had significantly higher frequency with hypomelanotic macules and dental enamel pits.The maximum diameter and clinical stage of patients with TSC2 gene mutation was significantly larger than patients with non-TSC2 gene mutations,along with higher frequency of AML bleeding risk and accepted invasive treatment.Conclusions The patients with TSC2 gene mutations presented higher frequency with hypomelanotic macules and dental enamel pits than those with non-TSC2 gene mutations.Comparing to those with TSC1 gene mutation and no mutation identified,the frequency of AML bleeding risk and loss of renal function was higher in patients with TSC2 gene mutations.Therefore,more active surveillance and treatment should be given to the patients of TSC-RAML with TSC2 gene mutations.
5.Application of targeted second-generation sequencing in the genetic diagnosis of tuberous sclerosis complex associated with renal complications

Objective To evaluate the value of targeted second-generation sequencing (NGS) in the genetic diagnosis of tuberous sclerosis complex (TSC) associated with renal complications.Methods The clinical data of 43 patients (with 33 patients of definite diagnosis and 10 patients of possible diagnosis)with tuberous sclerosis complex associated with renal complications were analyzed.There were 26 females and 17 males with a mean age of 28 (10 ~ 48) years ranging from 10 to 48 years old.All patients had renal complications,including 39 renal angiomyolipomas,3 renal cysts and 1 renal cell carcinoma.Written informed consents were signed,and 5ml peripheral blood was drawn for DNA extraction.Mutations of TSC1 and TSC2 genes were detected by the NGS technology,and then confirmed by Sanger sequencing.Results TSC1 or TSC2 pathogenetic mutants were identified in 39 patients (90.7%),which were consistent with clinical phenotypes and two non-significant mutations were identified in two individuals,while no mutation was detected in the other two cases.Fourteen novel mutations were reported for the first time,including 8 frameshift mutation,3 deletion mutation,2 nonsense mutation and 1 splicing mutation.Conclusion NGS is an accurate and less time-consuming technology in detecting TSC1 or TSC2 gene mutation,which has great value in genetic diagnosis of tuberous sclerosis complex associated with renal complications.
6.Effect of Gusuibu Zonghuangtong on level of TNF-? and IL-6 in macrophagocyte

[Objective]To study the effects of Gusuibu Zonghuangtong on level of TNF-? and IL-6 in macrophagocyte stimulated by PMMA particles.[Method]The rabbits were divided into six groups.Group A:only monoculear cells;group B:monoculear cells+PMMA particles;group C:monoculear eells+PMMA particles +2mlcontrol-serum;group D:monoculear cells +PMMA particles +2 ml drug-serum;group E:monoculear cells + PMMA particles +3 ml drug-serum;Group F:monoculear cells + PMMA particles +5 ml drug-serum.Each group was cultured for 4 hours,and then the TNF-? and IL-6 of each groupwas determined.[Result]The levels of TNF-? and IL-6 were much higher in Group B than that of Group A,much lower in Group D,E,F than that of Group C(P0.05).[Conclusion]The PMMA particles could stimulate macrophagocyte to excrete TNF-? and IL-6,which can be inhibited by Gusulbu Zonghuangtong.Gusuibu Zonghuangtong is hoped to have clinical utility in the prevention and treatment of aseptic loosening of prosthesis.
7.MiRNA-106a Induces Multidrug Resistance of Gastric Cancer Cells by Targeting RUNX 3

Objective To explore the effect of miRNA‐106a(miR‐106a) expression on multidrug resistance(MDR)of gastric cancer(GC)cells and the involvement of runt‐related transcription factor 3 RUNX3.Methods The expression of miR‐106a was detected in two human gastric adenocarcinoma cell lines with MDR by immunoblotting and apoptosis assay. The sensitivity of GC cells to anticancer drugs was observed by detecting the expression of miR‐106a by using immunoblotting and PCR ,and the relationship between miR‐106a and RUNX3 was determined by luciferase activity assay.Results miR‐106a was significantly in‐creased in GC cells with MDR ,and it suppressed the sensitivity of GC cells to anticancer drugs. It could modulate MDR by tar‐geting RUNX3.Conclusion miR‐106a can induce the MDR by targeting RUNX3 in GC.

9.Pharmacokinetic analysis of tacrolimus in infants subject to living related liver transplantation and cardiac death liver transplantation

Objective To analyze and compare the dosage,blood concentration and metabolic characteristics of Tacrolimus (Tac) for pediatric patients who underwent living related liver transplantation (LRLT) or donation after cardiac death liver transplantation (DDLT).Methods The clinical data of 75 liver transplantation pediatric patients from October 2012 to August 2015 were retrospectively analyzed.According to the different source of donors,the recipients were divided into two groups:LRLT group (40 cases) and DDLT group (35 cases).Results (1) Under the condition of same initial Tac dosage,the Tac dosage in LRLT group was less than in DDLT group during the first 28 days post-transplantation (P> 0.05).However,the Tac dosage in DDLT group was significantly higher than in LRLT group on the second and third months after sugery (P =0.000).(2) Correlation analysis revealed that graft-recipient body weight ratio (GRWR) was correlated with Tac dosage (mg·kg-1 ·d-1) on the 14th day postoperative (LRLT group:r=0.579,P<0.05;DDLT group:r =0.583,P<0.05) and Tac concentration/dosage ratio (LRLT group:r =-0.607,P<0.05;DDLT group:r=-0.680,P<0.05).Conclusion Tac has a satisfactory anti-rejection effect on liver transplantation pediatric patients while the metabolism varied with each individual.There is a positive correlation between the early Tac dosage and the GRWR in both groups.It is necessary to set individualized Tac administration regimen according to the metabolic characteristics and GRWR.
