2.Clinical Observation of Peiyuan Shengjin Tongqiao Decoction for Treatment of Childhood Asthma Complicated with Allergic Rhinitis

Objective To investigate the effects of Peiyuan Shengjin Tongqiao Decoction, a Chinese herbal recipe with the actions of supplementing kidney to strengthen lung and dredging orifices , on traditional Chinese medicine (TCM) symptoms and lung function of childhood asthma complicated with allergic rhinitis. Methods Fifty-five childhood asthma complicated with allergic rhinitis were randomized into treatment group(N=28) and control group (N = 27). The two groups were treated with Budesonide and Formoterol Fumarate Powder for Inhalation, and the treatment group was given oral use of Peiyuan Shengjin Tongqiao Decoction additionally. After treatment, the clinical efficacy of the two groups was evaluated, and TCM symptom scores and lung function indexes [ the percentage of forced expiratory volume in the first second to the expected value (FEV1%) and the percentage of peak expiratory flow to the expected value (PEF%) ] were also observed before and after treatment. Results (1)After treatment for 3 months, the total effective rate of the treatment group was 92.86%, being superior to that of the control group (81.48%), and the difference was significant (P<0.01). (2) After treatment, the scores of cough plus asthma, rhinitis symptom scores and overall TCM symptom scores of the two groups were decreased (P < 0.01 compared with those before treatment) , and the rhinitis symptom scores and overall TCM symptom scores of the treatment group were lower than those of the control group (P < 0.01). (3) After treatment, PEF% and FEV1% were both improved in the two groups (P < 0.05 compared with those before treatment) , and the improvement in the treatment group was superior to that in the control group (P <0.05). Conclusion Peiyuan Shengjin Tongqiao Decoction is effective for the control of childhood asthmacomplicated with allergic rhinitis by improving rhinitis symptom scores, overall TCM symptom scores and lung function.
3.Effect of ginkgolide B on TLR4 and inflammatory protein expression in high glucose treated human umbilical vein endothelial cells

Aim To investigate the effect of ginkgolide B on TLR4 expression in glucose-treated endothelial cells.Methods Human umbilical vein endothelial cells (HUVECs)were stimulated by high concentra-tion of glucose.TLR4,inflammatory protein expression and Akt phosphorylation were analyzed by Western blot.Transcription factor NF-κB nuclear translocation was analyzed by immunofluorescence.Results The expression of TLR4 and PAF receptor was increased in high glucose-treated HUVECs. In contrast, both ginkgolide B and CV3988 dose-dependently decreased TLR4 and PAF receptor expression in high glucose-treated cells,respectively.Ginkgolide B decreased in-flammatory protein ICAM-1 ,VCAM-1 expression.Mo-reover,ginkgolide B potently abolished Akt phospho-rylation and NF-κB p65 nuclear translocation.Conclu-sion Ginkgolide B can reduce TLR4,PAF receptor, ICAM-1 and VCAM-1 expression in high dose of glu-cose-treated HUVECs,the mechanism might be linked to inhibition of Akt phosphorylation and NF-κB activa-tion.
4.The expression of macrophage migration inhibitory factor in PBMC,serum and lacrimal fluid of patients with atopic dermatitis and its clinical significance

Objective To investigate the expression of MIF in PBMC, sera and lacrimal fluid samples in patients with AD, and study the diagnostic significance of MIF in AD. Methods Forty-three AD patients (11 mild AD patients,23 moderate AD patients, 9 severe AD patients classfied by SCORAD index)and 31 age- and sex-matched healthy control subjects were recruited. Real-time RT-PCR was employed to analyze the expression of MIF mRNA in PBMC. ELISA was performed to detect the concentrations of MIF in sera and lacrimal fluid samples. Results AD patients had significantly higher levels of MIF mRNA in PBMC than normal controls [7.46 (3.38-8.90) vs 1.67 ( 1.24-2.45 ), Z=-6.141, P < 0.05]. The concentrations of MIF in sera and lacrimal fluid samples in AD patients were also markedly higher than those of normal controls [serum 36. 32( 11.89-43.80) μg/L vs 7.89(6.13-9.54) μg/L, Z = -6.180,P <0.05; lacrimal fluid 12.66(2.01-20.12) μg/L vs 0.85(0.77-1.06) μg/L, Z = -4.118,P <0.05]. MIF mRNA levels were 2.35 ( 2.12-2.49 ) , 7.83 ( 6. 54-8.90 ) and 8.76 ( 8.22-9.73 ) in mild, moderate and severe AD respectively, and the expression was higher in moderate and severe AD than in normal controls (Z= -6.237, -4.520,P <0.05). MIF serum concentrations were 8.98(7.90-10.51) μg/L, 36.50 (29.78-43.23) μg/L and 45.70(41.27-48. 84) μg/L in mild, moderate and severe AD respectively, and the differences were significant in moderate and severe AD compared to normal controls ( Z = - 6.238,- 4.521, P < 0.05 ). The lacrimal fluid MIF oncentrations were 1.10 ( 0.83-1.35 ) μg/L, 12.66 ( 9.76-15.87) μg/L and 24. 65 ( 19.29-30.94) μg/L in mild, moderate and severe AD respectively. Similarly,they increased significantly in moderate and severe AD compared to normal controls (Z = -4.062,- 3.372, P < 0.05 ). In moderate and severe AD, MIF mRNA levels in PBMCs and MIF concentrations in sera and lacrimal fluid samples were all positively correlated with the severity of AD ( r = 0.395, 0.404,0.515, P < 0.05 ). Conclusions The expression of MIF in PBMCs, sera and lacrimal fluid samples is higher in different course of AD. MIF can serve as a useful laboratory parameter for evaluation of AD activity and severity.
5.Recombinant human epidermal growth factor and basic fibroblast growth factor promote the proliferation of human corneal epithelial cells

BACKGROUND:The preparation of recombinant human epidermal growth factor (rhEGF) and basic fibroblast growth factor (bFGF) has been clinical y used in the repair of ocular surface trauma. However, the concentration of these growth factors that achieve the maximal healing effect and the comparison of two kinds of growth factors on promoting wound healing are stil controversial.
OBJECTIVE:To investigate the effect of rhEGF and bFGF on the cloning of human corneal epithelial cells.
METHODS:The human corneal epithelial cells cultured in vitro were interfered with rhEGF and bFGF under different concentrations. The proliferation of human corneal epithelial cells was detected using MTT assay after 3, 5, 7 days of growth factors treatment. Plate clone formation assay was applied to observe the morphology of cellclone and analyze clone formation rate of human corneal epithelial cells.
RESULTS and CONCLUSION:MTT value shows that 10μg/L rhEGF and 10μg/L bFGF on day 5 were the most powerful concentration. The clone formation rate of human corneal epithelial cells after treated with 10μg/L rhEGF was higher than that with 10μg/L bFGF (P=0.02). The results confirmed that both rhEGF and bFGF could promote the proliferation and increase clone formation ability of human corneal epithelial cells. 10μg/L rhEGF for 5 days achieves the best effect on promoting clone formation of human corneal epithelium cells.
6. Celastrol promotes apoptosis of human osteosarcoma HOS cells through endoplasmic reticulum stress pathway

Objective: To investigate the effect of celastrol on the apoptosis of human osteosarcoma cell line HOS, and to explore the underlying mechanism. Methods: After treatment with different concentrations (0, 1.5, 2.5, 3.5 and 4.5 μmol/L) of celastrol for 24 h, the proliferation of HOS cells was detected by CCK-8 assay to determine the optimum experimental concentration (3.0 μmol/L). The cell morphology was observed by inverted phase contrast microscopy, and the apoptotic morphology was detected by fluorescence microscopy after Hoechst33258 staining. The apoptotic rate and Ca2+ concentration were analyzed by flow cytometry. The expressions of binding immunoglobulin protein (BIP), calnexin (CNX), endoplasmic reticulum oxidoreductin 1-L alpha (Ero1-Lα), protein disulfide isomerase (PDI), inositol-requiring enzyme 1alpha (IRE1α), protein kinaselike endoplasmic reticulum kinase (PERK), phosphorylated-PERK (p-PERK), CCAAT/enhancerbinding protein homologous protein (CHOP), cleaved-caspase-12 (c-caspase-12) and c-caspase-3 were determined by Western blotting. Results: The different concentrations of celastrol inhibited the proliferation of HOS cells in a dose-dependent manner (all P < 0.05). After treatment with 3 μmol/L celastrol for 24 h, the morphology of HOS cells was obviously atrophic and disordered, the floating cells increased, and the typical morphological changes of apoptotic cells such as nuclear pyknosis and fragmentation appeared. As compared with the untreated control group, the apoptotic rate and Ca2+ concentration in HOS cells treated with 3 μmol/L celastrol for 24 h were significantly increased (both P < 0.05), and the expressions of endoplasmic reticulum stressrelated marker proteins BIP, CNX, Ero1-Lα, PDI, IRE1α, p-PERK and endoplasmic reticulum stress-mediated apoptosis-related proteins c-caspase-12, CHOP and c-caspase-3 were upregulated (all P < 0.05). Conclusion: Celastrol can induce the apoptosis of human osteosarcoma HOS cells, and the mechanism may be related to endoplasmic reticulum stress-mediated apoptosis pathway.
7.Ginkgolide B inhibits apoptosis in high glucose-stimulated human umbilical vein endothelial cells

Aim Toinvestigatetheeffectofginkgolide B on apoptosis in high glucose-treated endothelial cells.Methods Humanumbilicalveinendothelial cells(HUVECs)were used in the present study.The level of transmigration of HUVECs was analyzed by Tr-answell experiment.Apoptosis was detected by flow cy-tometry.Reactive Oxygen Species (ROS ) was meas-ured by immunofluorescence kit.The protein expres-sionwasanalyzedbyWesternblot.Result Highglu-cose treatment resulted in a reduction in transmigration of HUVECs and ginkgolide B recovered the phenome-non in glucose-treated endothelial cells.The level of ROS generation was increased in high glucose-treated group,whereas ginkgolide B inhibited ROS genera-tion.Immunofluorescence data showed high glucose in-creased apoptosis,whereas ginkgolide B inhibited ap-optosis in high glucose-treated HUVECs.Moreover, the expressions of Bax and caspase-3 were increased and Bcl-2 was reduced in high glucose-treated group. In contrast,ginkgolide B abolished the expressions of Bax and caspase-3 and increased Bcl-2 expression. Moreover,high glucose enhanced the expression and phosphorylation of p53,while ginkgolide B suppressed the expression and phosphorylation of p53 induced by highglucose.Conclusions GinkgolideBcaninhibit apoptosis and improve transmigration function in high glucose-treated HUVECs.Ginkgolide B has protection against high glucose-induced endothelial cell injury.
8.Antitumor Mechanism of Metformin via Adenosine Monophosphate-activated Protein Kinase (AMPK) Activation

Metformin, as a traditional oral hypoglycemic agent, is commonly used in the clinical treatment for type 2 diabetes. Recently, a large number of epidemiological researches have shown that metformin could reduce the tumor morbidity of type 2 diabetes, moreover, it has also been indicated that metformin could inhibit the growth, proliferation and transformation of cancer cells in metabolic pathways, cell cycle, oxidative stress and cancer/tumor stem cells transformation via AMPK pathway activation. But the antitumor effect of metformin via AMPK activation still exists arguments, and the deifnite mechanism remains to be further investigated and conifrmed by extensive clinical trials.
9.Positioning errors assessed with kV cone-beam CT for image-guided prostate radiotherapy

Objective To assess set-up errors measured with kilovoltage cone-beam CT (KV-CBCT), and the impact of online corrections on margins required to account for set-up variability during IMRT for patients with prostate cancer. Methods Seven patients with prostate cancer undergoing IMRT were enrolled onto the study. The KV-CBCT scans were acquired at least twice weekly. After initial set-up using the skin marks, a CBCT scan was acquired and registered with the planning CT to determine the setup errors using an auto grey-scale registration software. Corrections would be made by moving the table if the setup errors were considered clinically significant ( i. e. , > 2 mm). A second CBCT scan was acquired immediately after the corrections to evaluate the residual error. PTV margins were derived to account for the measured set-up errors and residual errors determined for this group of patients. Results 197 KV-CBCT images in total were acquired. The random and systematic positioning errors and calculated PTV margins without correction in mm were:a) Lateral 3. 1,2. 1,9. 3;b) Longitudinal 1.5, 1.8, 5. 1 ;c) Vertical 4. 2,3.7, 13.0. The random and systematic positioning errors and calculated PTV margin with correction in mm were:a) Lateral 1.1,0. 9, 3.4;b) Longitudinal 0. 7, 1.1, 2. 5;c) Vertical 1.1, 1.3, 3.7. Conclusions With the guidance of online KV-CBCT, set-up errors could be reduced significantly for patients with prostate cancer receiving IMRT. The margin required after online CBCT correction for the patients enrolled in the study would be appoximatively 3-4 mm.
10.Organizing and financing urban employees' supplementary medical insurance

Objective To explore appropriate organization and financing models of urban employees supplementary medical insurance under the conditions of the market economy. Methods Employee surveys on willingness to participate in, willingness to pay for, and actual enrolment in supplementary medical insurance were conducted in Shanghai, Wenzhou and Chengdu and nationally different operational models were compared so as to analyze prospectively coverage and operational models of feasible supplementary medical insurance options. Results Characteristics of the employees needs for various supplementary medical insurance options were garnered and appropriate coverage and operational models were put forward. Conclusion It is suggested that the role of the government in the development of supplementary medical insurance be clearly defined, the development of supplementary medical insurance be promoted through policy support and guidance, and the supervision of supplementary medical insurance be strengthened.
