1.Comparison of acute ischaemic stroke adult patients with atrial fibrillation to without atrial fibrillation in the effect on renal dysfunction

Objective To find out whether inpatients with acute ischaemic stroke and atrial fibrillation (AF) have higher rate of renal dysfunction than without AF,and analyze risk factors associated with renal dysfunction.Method 374 patients with acute ischaemic stroke and AF were enrolled,which cases have complete data.500 patients were randomly selected as the non-AF group.To compare the prevalence of renal dysfunction [eGFR < 60 mL/(min· 1.73 m2)] of two groups.Non-conditional Logistic regression analysis was used to detemmine the factors associated with renal dysfunction.Results 374 patients in AF group,114 (30.5%) were renal dysfunction;500 patients in non-AF group,75 (15%) were renal dysfunction (P =0.000,OR =2.485).The eGFR of AF and non-AF group was (77.75 ± 39.89) mL/(min· 1.73 m2) and (96.93±39.14) mL/(min · 1.73 m2).In Logis tic regression analysis,heart dysfunction (OR =2.057),hypertension (OR =1.826),diabetes (OR =1.897),hypevuricemia (OR =3.161) were found to be associated with renal dysfunction.Conclusions Adult patients with acute ischaemic stroke and AF have a higher rate of renal dysfunction than acute ischaemic stroke without AF.Heart dysfunction,hypertension,diabetes,hyperuricemia were factors associated with renal dysfunction.

Objective To investigate the early predictive indexes of curative effect of acute respiratory failure(AFR) treated with noninvasive bilevel positive airway pressure(BiPAP) ventilation.Methods Sixty-eight patients with AFR were treated with BiPAP ventilation.According to the therapeutic outcome,patients were divided into success group and failure group,and the distributions of ages,acute physiology and chronic health evaluation(APACHEⅡ) score,consciousness level as well as arterial blood gas(ABG) measurement before and after the treatment with BiPAP ventilation were compared between the two groups.Results There was statistical difference in the ABG levels of pH and PaCO_2 after 1 hour's treatment with BiPAP ventilation(P0.05).Conclusion The levels of pH and PaCO_2 after 1 hour's treatment with BiPAP ventilation are valuable markers for curative effects in early stadge.If the levels of pH and PaCO_2 fail to improve,the treatment with intubation may be necessary.
4.Comparative study of Scrophulariae and Aconite in inhibiting myocardial hypertrophy in rats and mice

OBJECTIVE: To explore the effects of Scrophulariae of cold nature and Aconite of hot nature on myocardial hypertrophy and neuroendocrine factors in rats and mice. METHODS: A mouse model of myocardial hypertrophy was established by hypodermic injection of isoproterenol. Sixty myocardial hypertrophy mice were randomly divided into five groups: normal control group, untreated group, metoprolol-treated group, Scrophulariae-treated group and Aconite-treated group. A rat model of myocardial hypertrophy was established by peritoneal injection of L-thyroxin. Fifty rats were randomly divided into five groups: normal control group, untreated group, captopril-treated group, Scrophulariae-treated group and Aconite-treated group. After 7-9 days of treatment with intragastric administration of the corresponding drugs, the effects of Scrophulariae and Aconite on left ventricular weight index (LVWI) and heart weight index (HWI) were determined. The concentrations of cyclic adenosine monophosphate (cAMP) in plasma and angiotensin II (Ang II) in myocardium were detected through radio-immunity method. Cardiocyte cross-section area was determined by using HE staining. RESULTS: Scrophulariae of cold nature could significantly reduce the LVWI, HWI and cardiocyte cross-section area, and could decrease the content of cAMP and Ang II. However, Aconite had no such effects. CONCLUSION: Scrophulariae of cold nature can inhibit myocardial hypertrophy through restraining the activity of sympathetic nervous system and decreasing the level of Ang II. The inhibition of Aconite of hot nature on cardiac hypertrophy is not significant.
5.Ezrin-associated signal transduction in tumor invasion and metastasis

Ezrin is an important membrane-cytoskeleton linker protein.It is highly related with tumor invasion and metastasis and indicates poor prognosis.It is reported that aberrant Ezrin expressed in many carcinomas,such as osteosarcoma and breast carcinoma.However,the complex mechanisms of Ezrin in tumor invasion and metastasis remain unclear.It is involved in several different tumor associated signal pathway based on different tumor types,including adhesion molecule signal transduction,Rho and Akt signal transduction.Therefore,research on the signal transduction of Ezrin has great significance for the understanding of cancer progression and Ezrin is probable to be a new treatment target.
6.SURGICAL TREATMENT OF PERIPHERAL NERUOFIBROMA

PURPOSE To investigate the indications for surgical treatment of peripheral neurofibroma and the cause of postoperative recurrence.METHODS Analysis of the clinical data of 15 cases of limb neurofibroma in different locations,whichhave been operated 16 times.RESULTS Neurofibroma normally grows in two ways:along nerve cord and in plexiform.The former can be excised along tumor edge and the latter should be removed extensively.Pathological studies were performed postoperatively,and confirmed that it was neurofibroma.13 of 15 cases have been followed up on average 3.4 years.No cases had recurrence except 1 case after a half year which was reoperated and has not recurred for 8 months up to now.CONCLUSION If the patients have pain or discomfort,limb malfunction or disfigurement,operation should be done in the early stage.Different operative methods were chosen due to different types of neurofibroma.Rccurrence is related to the extent of operation

Objective To investigate effect of anthracyclines of VEGF-C, VEGFR-3, ER and E-cadherin in serum of patients with breast cancer.Methods 43 patients with breast cancer were selected and randomly divided into experimental group and control group, 40 cases in each group.According to the experimental program based on the treatment, serum VEGF-C, VEGFR-3 levels and breast tissue ER and E-cadherin were detected after the end of the treatment.Results Compared with the control group, VEGF-C, VEGFR-3 levels of the experiment group were lower( P<0.05),ER level was lower(P<0.05) and E-cadherin level was higher(P<0.05).Conclusion the ER, VEGFR-3 and VEGF-C levels in breast cancer patients can significantly decrease the E-cadherin level, and it has a guiding significance for clinical.

Objective To investigate expression of TPS, VEGF of beast cancer patients and relationship with clinical pathological.Methods 68 cases of breast cancer were selected as experiment group, 36 cases with breast fibroma were selected as control group.TPS and VEGF were detected by elisa of patients before and after treatment.Serum TPS and VEGF levels and TNM staging were compared after operation.Results TPS and VEGF of the experiment group were higher than control group(P<0.05).TPS and VEGF of experiment group after operation were higher than before operation.TPS and VEGF increased with the grade of tumor, but there was no significance between grade Ⅰ and 0.The expression of VEGF in tumor diameter≤2 cm group was significantly lower than >2 cm group(P<0.05).TPS and VEGF of axillary lymph node metastasis group and axillary lymph node metastasis had significant difference(P<0.05).The concentration of TPS was significantly higher than that of estrogen and progesteronepositive group in estrogen receptor negative, progesterone receptor negative (P<0.05).Conclusion TPS, VEGF levels in the serum of breast cancer patients showed high expression, the expression of TPS, VEGF levels related to axillary lymph node metastasis, estrogen receptor, progesterone receptor.
10.Establishment of posttraumatic acute diffuse brain swelling with sinus balloon compression method in rabbits

Objective To establish a rabbit posttraumatic acute diffuse brain swelling (PADBS) model and investigate the mechanism of action.Methods Fifty New Zealand rabbits were assigned to control group (n =10) and model group (n =40) according to random number table.The animal model of sinus balloon compression was established under intracranial pressure monitoring by using intracranial pressure probe.The model group was subdivided equally at 1.5 hours after compression,1.5 hours after decompression,3 hours after decompression and 4.5 hours after decompression,for which intracranial pressure,brain water content,pathological mechanism and ultrastructure were measured dynamically.Results The success rate of modeling was 83% (33/40).Intracranial pressure was (4.9 ± 0.8)mmHg in control group,(50.1 ± 4.3) mmHg in 1.5 hours after compression group,(45.2 ± 1.7) mmHg in 1.5 hours after decompression group,(48.6 ± 2.2) mmHg in 3 hours after decompression group,and (59.1 ±2.5)mmHg in 4.5 hours after decompression group (P <0.05).Brain water content was (75.0 ± 0.6) % in control gorup,(76.7 ± 0.8) % in 1.5 hours after compression group,(77.3 ± 0.5) % in 1.5 hours after decompression group,(78.5 ± 0.6) % in 3 hours after decompression group,and (79.4 ± 0.7) % in 4.5 hours after decompression group (P < 0.05).Vasogenic brain oedema was seen 1.5 hours after decompression.Cytotoxicity brain swelling generated with brain tissue destroyed 3 hours after decompression.The vicious cycle of high intracranial pressure and brain tissue destruction occurred 4.5 hours after decompression.Conclusion Under intracranial pressure probe monitoring,the rabbit model of PADBS by sinus balloon compression has stable pressure of the sinus balloon and has good reliability and repeatability,which provides a reliable evidence for further study on the possible mechanism and treatment methods of PADBS.
