5.Therapeutic effect of rehabilitation intervention on patients with diabetes mellitus complicated tubercu-losis
Chinese Journal of cardiovascular Rehabilitation Medicine 2015;24(4):359-362
Objective:To explore therapeutic effect of rehabilitation intervention on patients with diabetes mellitus (DM)complicated tuberculosis (TB).Methods:A total of 108 DM + TB patients treated in our hospital from Jan 2008 to Aug 2013 were enrolled.According to random number method, they were randomly and equally divided into rehabilitation intervention group and routine treatment group. The intervention course was eight weeks, and then rehabilitation effect was compared between two groups. Results:Compared with before treatment, after treatment, there were significant reductions in fasting blood glucose (FBG)and 2h postprandial blood glucose (2hPBG)in both groups,P <0.01 all;compared with routine treatment group, after treatment, there were significant reductions in levels of FBG [(6.77±1.82)mmol/L vs.(5.54±1.46)mmol/L]and 2hPBG [(7.94±1.83)mmol/L vs.(6.87 ±1.42)mmol/L],P <0.01 both, and significant rise in percentages of significant reduced or disappeared pleural effusion (51.85% vs.83.33%)and significant reduced or closed TB focus (44.44% vs.72.22%)in rehabilitation intervention group,P <0.01 both. Conclusion:Rehabilitation intervention can make patients get better control of blood glucose, promote absorption of pleural effusion and pulmonary tuberculosis focus.
6.Modified posteromedial and posterolateral approaches for the treatment of posterior tibial plateau fractures
Chinese Journal of Orthopaedic Trauma 2012;14(11):945-949
Objective To observe the clinical outcomes of modified posteromedial and posterolateral approaches for the treatment of posterior tibial plateau fractures. Methods Between January 2006 and October 2011,25 patients with posterior tibial plateau fracture were identified. They were 17 men and 8 women, aged from 22 to 76 years (average,46.4 years).According to the AO/OTA classification, there were 4 cases of type 41-B1,6 cases of type 41-B2 and 15 cases of type 41-B3.According to Luo's three-column classification, they were all posterior column fractures.Ten patients with posterolateral tibial plateau fracture were treated by the modified posterolateral approach and the other 15 patients with posteromedial tibial plateau fracture by the modified posteromedial approach. The posterior fractures were fixated with a buttress plate. The knee function was evaluated at the last follow-up by the The Hospital for Special Surgery (HSS) Score. Results All the 25 cases were followed up for a mean time of 13.2 months (from 10 to 24 months).By the radiological Rasmussen criteria for fracture reduction, the mean score immediately after surgery was 16.5 points (from 13 to 18 points),with 20 excellent cases,4 good cases and one fair case. All patients were pain-free with full range of motion.Radiologically, bone union was achieved in all cases after a mean time of 13.9 weeks (from 12 to 18 weeks). No surgery-related complications were found. The mean HSS score was 91 points(range,74 to 97 points) and the good to excellent rate was 100% (17 excellent and 8 good cases) at the last follow-up.The average range of knee motion ranged from 0° to 125°.Conclusion In the treatment of posterior tibial plateau fractures, because the modified posteromedial and posterolateral approaches can result in excellent fracture reduction under direct visualization and allow for posterior buttress plating, soft tissues can be well preserved and good functional results can be expected.
7.Descending necrotizing mediastinitis: report of two cases and literature review
Chinese Journal of General Practitioners 2009;8(4):262-264
We reported 2 cases of descending neerotizing mediastinitis (DNM) and reviewed the related literatures. The etiology, clinical manifestations, imaging manifestations, diagnostic criteria and therapeutic strategies of DNM were discussed. It is concluded that descending necrotizing mediastinitis is a life-threatening disease with high mortality and is prone to be misdiagnosed. Early diagnosis, prompt application of sensitive broad-spectrum antibiotics and abscess drainage with proper surgical medality may improve the prognosis of DNM.
8.Employment analysis of graduates in biomedical engineering specialty
Chinese Medical Equipment Journal 2015;(9):144-146,152
Mass data is absent for the employment of the graduates in biomedical engineering specialty in China. Questionnaire was carried out in the graduates from 1989 to 2013 in School of Biological Sciences & Medical Engineering of Southeast University, and more than 1 500 questionnaires on the graduate employment were classified, summarized and analyzed to provide instructions for graduate employment guidance and student education in biomedical engineering specialty.
9.Progresses of ultrasonography in diagnosis of biliary atresia
Chinese Journal of Medical Imaging Technology 2017;33(4):628-631
Biliary atresia (BA) is the most common cause of persistent jaundice in infant,it can rapidly advance to biliary cirrhosis and death.Surgical treatment in time can decrease mortality,so it is essential to be differentiated from other diseases as early as possible.Main diagnosis methods of BA include clinical feature of patients, liver biopsy, and high-frequen cy ultrasonography,MR cholangiopancreatography (MRCP),hepatobiliary scintigraphy.High-frequency ultrasonography is superior to other methods with high diagnostic specificity and sensitivity. The progresses of ultrasonography in diagnosis of BA were reviewed in this article.
10.Visual prognosis factors analysis of scleral buckling for primary macula-off rhegmatogenous retinal detachment
Chinese Journal of Experimental Ophthalmology 2011;29(3):261-264
Background Many surgical techniques are used to improve the postoperative visual acuity for rhegmatogenous retinal detachment(RRD)during recent decade, and scleral buckling surgery is one of these operations.Whether the visual function after operation can be rescued is an important issue. Objective The aim of this study was to analyze the risk factors of influencing the postoperative vision following scleral buckling surgery for primary macula-off RRD. Methods The clinical and follow-up data from consecutive series of 116 eyes of 116 patients received scleral buckling surgery for primary maeula-off RRD were retrospectively reviewed. The relationship of multiple factors, including age, preoperative best corrected visual acuity(BCVA),duration of disease, refractive error, location of retinal hole, number of retinal hole, area and height of retina detachment, management of subretinal fluid and intravitreal gas injection, with visual acuity were analyzed respectively by χ2 test. The correlations among statistically significant factors with postoperative vision were analyzed by multivariate Logistic regression analysis.Written informed consent was obtained from any patient before surgery. Results The postoperative vision outcome was found with significantly difference among different preoperative vision groups(P=0.002)and different course (P=0.009).There were significant differences between the groups with different preoperative BCVA(P=0.002)and duration of disease(P=0.009).Multivariate Logistic regression analysis showed that the preoperative BCVA was the only variable affecting postoperative visual result(r=0.400,P=0.009).Considerable linear correlation wag seen between preoperative vision and postoperative vision(r= 0.400,P=0.000).The probability with postoperative vision of t>0.4 in the eyes with preoperative≥0.05 was 3 folds more than that of preoperative<0.05(OR=2.992).The better visual outcome after scleral buckling surgery was seen in the eyes with the course≤7 days. Conclusion Preoperative BCVA and duration of disease are the key factors associated with the postoperative BCVA.Scleral buckling surgery should be performed within the first week for primary macula-off RRD.
