1.The study of quantitative evaluation of hypoxia of the patients with pulmonary heart disease

Objective To find a grading method for assisting in evaluating hypoxia degree of patients with pulmonary heart disease non-invasively. Methods To design a quantitative method to score degree of hypoxia of 67 patients with pulmonary heart disease by using acute physiology and chronic health evaluation Ⅱ (APACHE Ⅱ) as a frame and to make comparative analysis between the grading result and artery blood gas to find the correlation. Results There was a negative correlation between evaluation mark and pH、PaO2、SaO2 and a positive correlation between the evaluation mark and PaCO2, according to statistics. Conclusions Although the grading table of hypoxia evaluation can not substitute blood gas analysis, acting as a non-invasive method to assist in evaluating the hypoxia degree of patients with pulmonary heart disease, it can not only relieve patients' repeated punctuation pain, but also achieve the goal of estimating hypoxia.Consequently the non-invasive method to evaluate hypoxia is of clinical value.

3.Clinical and epidemiological characteristics of severe measles in 62 infants

Objective To explore the clinical and epidemiological characteristics of severe measles in infants. Methods Clinical data of 62 infants with severe measles were analyzed retrospectively.Results Of 62 infants with severe measles,42 (67.74%)were aged < 9 months,of whom 37(88.10%)were not vaccinated against measles. The onset months were February-May,41 cases (66.13%)were found in March-April.All patients had fever and skin rash,the rates of other symptoms and complications were as follows :oral leukoplakia 80.65%(n=50),Ca-tarrh symptom 77.42%(n=48),conjunctivitis 79.03%(n=49),history of choked water cough 75.81%(n=47), pneumonia 95.16%(n=59),acute laryngitis 35.48% (n=22),electrolyte disorder 20.97%(n=13),acute respir-atory distress syndrome 9.68%(n=6),liver function damage 9.68%(n=6),pneumothorax 8.06% (n=5),myo-cardial damage 4.84% (n=3),respiratory failure 3.23% (n=2),toxic encephalopathy 3.23%(n=2),measles en-cephalitis 1.61% (n= 1),and pleural effusion 1.61% (n = 1 ).Of all cases,41 cases were cured,19 cases im-proved,1 case died,and 1 case gave up treatment.Conclusion These severe measles cases occurred mainly in in-fants aged<9 months and were not vaccinated against measles;infants had history of choked water cough;the main onset months were March-April ;pneumonia was still a predominant complication of infant measles.
4.Relationships between CA9 and recurrence in clear cell renal carcinoma:a prelimina-ry investigation

Objective To study the expression of carbonic anhydrase 9(CA9) in clear cell renal carcinoma(ccRCC), and the relationship between the expression and recurrence .Methods Paraffin-embedded tissues from 53 ccRCC cases were collected during the past 3 years,and immunohistochemistry was used to detect the expression of CA 9.Fifty-three cases were followed up to evaluate the disease-free survival ( DFS) and recurrence .Association of CA9 expression with recurrence was then analyzed .Results Localized RCC recurred in eleven cases , the median DFS was 10 months, the integral optical density ( IOD) of the recurrent cases was 230 879 .84 ±64 878 .72 while the IOD of the thirteen nonrecurrent cases was much higher,444 993.31 ±146 946.27(P<0.01).Conclusion There might be a positive correlation between the low expression of CA9 and the recurrence of ccRCC .CA9 may help predict the ccRCC patients ’ prognosis.
5.Searching of anatomy teachers' participating in neurosurgery case-based teaching

Neuroanatomy knowledge is the key to improve the teaching quality of neurosurgery and anatomy teachers' participation in neurosurgery teaching can obviously improve teaching effect. Combined with case-based teaching, we enabled medical students to master the system knowledge from the basic to the clinical , cultivating their clinical thinking ability and the ability to solve clinical problems. Cooperation between clinical medicine teachers and basic medicine teachers can promote the update of knowledge, work out teaching plans together, and improve the quality of teaching.
6.Effect of vancomycin artificial bone on the treatment of elderly patients with chronic osteomyelitis and the influence to TNF-α

Objective To evaluate the the effect of vancomycin artificial bone on the treatment of elderly patients with chronic osteomyelitis and the influence to TNF-α.MethodsIn Yongkang orthopaedic hospital, 48 cases of elderly patients with chronic osteomyelitis were selected as the object of study, and were randomly divided into the control group and the experimental group, 24 cases in each group.This is a retrospective study.Three days before operation, the control group were given cloxacillin 2g/d by 4 times intravenous drip.The experimental group were given Norvancomycin Hydrochloride 0.8g/d by two times intravenous drip.In the process of operation, the control group were given bone graft substitutes 5-10g and the experimental group were given Norvancomycin Hydrochloride 1g that is added to the water to form a paste.After operation, the control group were given cloxacillin 2g/d by 4 times intravenous dripand the experimental group were given Norvancomycin Hydrochloride 0.8g/d by 2 times intravenous drip.The two groups were treated continuously for two weeks.The effect,CRP, ESR, TNF-α, NF-κB and adverse drug reactions were compared between the two groups.ResultsThe effect in the experiment group was 95.45%,is significant higher than that in the control group 81.82%(P<0.05).The serum level of CRP、ESR、TNF-α and NF-κB decreased post-treatment in the experiment group much lower than that in the control group (P<0.05).The adverse drug reactions was not statistically different between the two groups.ConclusionVancomycin artificial bone on the treatment of elderly patients with chronic osteomyelitis was effective with high safety and it can decrease the level of TNF-α.
7.Basic and clinical studies of non-myeloablative allogeneic hematopoietic stem cell transplantation

Non-myeloablative allogeneic hematopoietic stem cell transplantation has been extensively applied in patients with hematologic malignancies who are ineligible for conventional hematopoietic stem cell transplantation because of age or medical comorbidities. Non-myeloablative regimens lead to an initial state of mixed hematopoietic chimerism which can produce a marked effect of graft versus tumor to treat the diseases. Compared with the conventional hematopoietic stem cell transplantation, non-myeloablative allogeneic hematopoietic stem cell transplantation has a lower transplant-related mortality and incidence rate of graft-versus-host disease. The improvement of non-myeloablative regimens and the prophylaxis of diseases associated with transplantation can improve the therapeutic efficacy. Though many therapies have been introduced and proved to be successful in animal models, we still need to investigate the research trend and the problem on human body.
8.Ma Yinglong Musk Hemorrhoids Ointment combined with psychological intervention in the treatment of perianal eczema and its effect on quality of life

Objective To investigate the effect of Musk Hemorrhoids Ointment Combined with psychological intervention in patients with perianal eczema and the effect on the quality of life of patients. Methods 70 patients with perianal eczema were randomly divided into control group (n=35) and observation group (n=35). The control group using triamicinolone acetonide acetate and miconazole nitrate neomycin sulfate cream with the treatment, the observation group with musk hemorrhoids ointment combined with psychological intervention treatment by SF-36 life quality scale of two groups before and after treatment to assess the quality of life, compared 2 groups of clinical curative effect and influence on the quality of life of patients. Results The observation group after treatment itching, exudation and lesion area and the symptoms and signs improved time shorter than the control group (P<0.05); quality of life score of SF-36 in observation group after treatment were higher than those in control group (P<0.05); the observation group after 2 weeks of treatment, the clinical efficiency and control group had significant difference (P<0.05). Conclusion Patients with perianal eczema with musk hemorrhoids ointment combined with psychological intervention treatment effect is ideal, it is worthy of popularization and application.
9.STUDY ON 10 668 CASES OF SECOND-TRIMESTER MATERNAL SERUM SCREENING

Objective To evaluate the clinical value of prenatal screening by analyzing the second -trimes-ter maternal serum screening results and prognosis .Methods The second-trimester maternal serum screening in-cluding alpha-fetoprotein (AFP) andβ-human chorionic gonadotrophin (β-HCG) was tested by time-resolu-tion immunofluorescence .2T-Risks software was used to evaluate fetal risk of three kinds of defects , such as trisomy 21, trisomy 18 and neural tube defects (NTD).For those of pregnant women with high -risk screening results, am-niotic fluid, umbilical cord blood karyotype analysis or four -dimensional color Doppler ultrasound scan can be rec-ommended to confirm the diagnosis .Results This research included the analysis of 10 668 cases of pregnant meta-phase prenatal screening results .677 cases were in high-risk, which took up 6.35%.Among those 677 cases, 501 cases had high-risk 21-trisomy Syndrome , and 104 cases had high-risk NTD.72 cases with high-risk 18-tri-somy Syndrome had been found , and the percentage of those cases above were 4.70%, 0.97%and 0.67%, respec-tively.As for the 677 follow-up pregnant women with high risks , the results indicated that during those 356 high-risk women, who had performed prenatal diagnosis , 2 cases of 21-trisomy Syndrome, 3 cases of NTD, and 4 cases of structural abnormalities had been found .In addition , there were 19 cases of spontaneous abortion and stillbirth , as well as 10 cases of other abnormalities .There are 40 abnormal cases in total , which took up an abnormal percentage of 5.91%.In those 9 991 follow-up pregnant women with low risks , there are 57 cases (0.57%) presented abnor-mal, in which had 1 cases with 21-trisomy Syndrome, 2 cases with NTD, 3 cases with deformity, 23 cases of spon-taneous abortion and stillbirth and 28 other abnormal cases .Conclusion The second -trimester maternal serum screening plays an important clinical role in the prediction of abnormal fetus and prevention of birth defects .
10.Effects of CO_2 pneumoperitoneum on pulmonary functions in rabbits with chronic pulmonary failure:An experimental study

Objective To investigate the effects of CO 2 pneumoperitoneum during laparoscopic cholecystectomy (LC) on pulmonary functions in chronic pulmonary failure rabbits and their mechanisms. Methods A total of 50 healthy male rabbits ( oryctolagus cuniculus ) were randomly divided into 4 groups: normal control group (N 0: n=5, no pneumoperitoneum), experimental control group (T 0: n=5, no pneumoperitoneum), 10 mmHg experimental group (T 10 : n=20, 10 mmHg pneumoperitoneum) and 15 mmHg experimental group (T 15 : n=20, 15 mmHg pneumoperitoneum). After the successful establishment of emphysema rabbit models, CO 2 pneumoperitoneum was conducted and maintained for 2 hours at the pressure of 10 mmHg (1.33 kPa) and 15 mmHg (2.00 kPa), respectively. Pulmonary functions (total respiratory resistance, central resistance and total airway resistance) of the 4 groups were measured by pulse oscillation technique before and after the pneumoperitoneum, respectively. Results Total respiratory resistance, central resistance and total airway resistance increased after the establishment of emphysema models.
