1.Evaluation of effect of two different enema methods in congenital megacolon children

Objective To explore the effect of two different enema methods in congenital megacolon children. Methods 70 cases of congenital megacolon children were randomly divided into the experimental group and the control group with 35 cases in each group respectively. The control group used traditional reflux-enema; The experimental group was implemented systemic and modified back reflux-enema (negative pressure drainage vessel to reflux-enema plus the padded-fixation of anal canals, cooperated with manual massage of lower abdomen along the direction of colon, if necessary, changed body position). The average enema time, the postoperative hospital stay and the effect of cleaning the intestine in the two groups were observed. Results Compared with the control group, the average preoperative enema time and hospitalization time was significantly shorter, and intestine cleaning effect was better in the experimental group. Conclusions Modified reflux-enema could increase preoperative enema time and postoperative hospitalization time, and had good effect on cleaning intestine.
2.The effect of extended care on behavior mental state of patients with Hirschsprung disease

Objective To explore the effect of extended care on behavior mental state of patients with Hirschsprung disease.Methods 40 cases of patients with Hirschsprung disease underwent surgery in our hospital from January 2009 to June 2010 were set as the control group.42 cases of patients with Hirschsprung disease underwent surgery in our hospital from July 2011 to June 2012 were set as the experimental group.The patients in the control group received conventional guidance after Hirschsprung disease surgery before discharge,which included dietary guidance,anus functional exercise,regular anal expansion,follow-up upon discomfort and regular re-visit.The patients in the experimental group not only received conventional guidance after Hirschsprung disease surgery before discharge,but also received systematic extended care plan.The nursing effect was compared between two groups.Results Conduct problems and hyperactivity index of patients in the experimental group were compared with those in the control group,the differences were significant.The scores of learning problems,psychosomatic disorders,impulsivity-hyperactivity and anxiety were lower,and compared with those in the control group,the differences were not significant.Conclusions Implementation of extended care for patients with Hirschsprung disease surgery can improve behavior mental state of them.
3.Prospective Study on the Relationship between Social Support and Internet Addiction

Objective: To explore the relationship between social support and internet addiction in undergraduates. Method: 382 undergraduates were divided into three groups according to their social support score and perceived social support score at end of the first term in their freshmen year. They were followed for 4.5 years to collect the incidence of internet addiction. Results: The group with less social support or less perceived social support had higher incidence rate (15.7%, 7.9% and 2.4% with social support score 0~60, 61~73 and higher than 73 respectively, ?2=9.49,P=0.009). There was obvious dose-effect relationship between social support and incidence of internet addiction. Conclusion: Less social support is a risk factor of internet addiction.

5.Investigation of Fluoride Source in Coal-burning Endemic Fluorosis Areas in Hunan Province

Objective To explore the origin of fluoride in the coal-combustion endemic fluorosis areas in the middle part of Hunan Province and to provide basic data for making preventive measure in the regions. Methods In November, 2006, the prevalence rate of dental fluorosis of children aged 10-12 years were examined, the villages were divided into three groups based on the prevalence rate of dental fluorosis. The fluoride content in the drinking water, coal, briquettes, briquette ash, clay and indoor air was determined by fluoride ion selective electrode. The quantity of fluoride release was calculated. Results There was no significant difference in the concentration of fluoride in water among the three groups, and it was below 1.0 mg/L in fluorosis and non-fluorosis areas. The fluoride levels in the coal, briquette ash and indoor air showed no statistical difference among groups. [Objective]To explore the effect of tacrolimus on expression of hepatocyte growth factor(HGF)in rat spinal cord following peripheral nerve injury.[Method]Fifty male rats were randomly divided into normal group,injury group and treatment group.Models of peripheral nerve injury were established by transection of bilateral sciatic nerve at 0.5 cm distal to piriform muscle.Then treatment group received subcutaneous injection of tacrolimus(1 mg/kg)at the back of the neck,while injury group received 0.9 % saline.The L4~6 spinal cords were harvested at various time points after surgery.Western blotting and immunofluorescence staining were used to detect the level and position of HGF in the spinal cord.[Result]HGF-positive neurons were located in anterior horn,intermediate zone and posterior horn of gray matter in normal spinal cord.There was no significant difference in the expressions of HGF between injury group and normal group,while the expression of HGF was significantly higher in treatment group than that in injury group at 7 and 14 days after surgery.[Conclusion]Peripheral nerve injury does not up-regulate the expression of HGF in spinal cord,while tacrolimus can induce high expression of endogenous HGF after injury to protect neurons and promote axonal outgrowth.
7.Intelligence Impact of Children with Endemic Fluorosis Caused by Fluoride from Coal Burning

Objective To explore the adverse effect of endemic fluorosis caused by fluoride from coal burning on the intelligence of children. Methods Four primary schools were selected respectively from slight, moderate, severe endemic fluorosis areas and the control area in Xinhua county of Hu'nan Province, and twenty children of grade 5 were randomly chosen in each school. The seriousness of dental fluorosis was examined by Dean's method and intelligence level was evaluated by CRT-RC. The morning urine was collected and the fluoride content in urine was measured by fluoride ion selection electrode method. 9.Long term effects of end-to-end suture on repairing acute closed Achilles tendon ruptures

Objective To retrospectively evaluate the clinical effects of end-to-end suture in repairing acute closed Achilles tendon ruptures.Methods Twenty-eight consecutive patients(20 males and 8 females,aged from 19 to 48 years with a mean of 36.5) with acute closed Achilles tendon rupture admitted from Nov.2000 to Jun.2006 were involved in present study.All patients were produced to have complete Achilles tendon rupture by MRI,and they received surgical procedure within 4 days after injury.An incision of 1cm long was made medial to the tendon,sparing the lesser saphenous vein and sural nerve.The tendon stumps were minimally debrided and end-to-end sutured using the Kessler or Bunnell techniques.The patients were followed-up for 12 to 36 months(mean 20 months),the therapeutic effects were evaluated by Arner-Lindholm scoring system,and postoperative complications were documented during the follow-up period.Results Of the 28 patients,19 got excellent therapeutic effect and 9 got good as evaluated by Arner-Lindholm scoring system,the satisfactory rate was 100%.All the patients regained normal gait,and rehabilitative training was initiated 3 months after operation.The postoperative complications included superficial wound infection in one case and skin edge necrosis in one case,resulting in a 7.1% incidence of complication.No peripheral nerve injury or Achilles tendon re-rupture occurred during the follow-up period.Conclusions An excellent therapeutic effect and minimal postoperative complication may be expected with end-to-end suture for repairing acute closed Achilles tendon rupture.
10.Dynamic Changes of CT Scan of Focal Brain Edema After Head Injury

Objective To study the dynamic changes of posttraumatic focal brain edema. Methods 120 patients who underwent multiple CT scans after head injury with hematoma or brain contusion in our hospital from 1998 to 2002 were enrolled in this study. Their CT image parameters and clinical data were recorded, and statistically analyzed. Results 96.7%(116/120) patients had perihematoma or pericontusion edema. Edema appeared 7 hours after head injury and reached its climax at the ninth day. Edema shape trended taper during its development, and cortex escaped from edema. Conclusion Focal brain edema was very common in the patient with head injury. Taper shape's formation and escape of cortex may mainly attribute to the difference in construction topology and blood supply between cerebral white and gray matter.
