2.Effect of health education on farmers' intention of joining in the new rural cooperative medical system

Objective:To evaluate the impact of health education intervention on promoting rural residents to join new rural cooperative medical system(NCMS)and their intention of joining NCMS based on Health Belief Model in project areas in Henan and Jilin Provinces.Methods:Quasi-experiment study was used to evaluate intervention impact.Following the evidence-based approach,according to needs assessment,a half-year health education intervention was implemented among farmers in the experimental counties in Henan and Jilin Provinces respectively.A questionnaire survey was conducted among farmers in intervention and control counties before and after intervention,and intervention impact was evaluated by comparing the indicators' changes in intervention and control counties.Results:After health education intervention,the knowledge level of farmers in two intervention counties increased by 29.0% and 37.8% respectively,their scores of perceived threatens of health risk and perceived barriers of joining NCMS among the respondents were decreased.Meanwhile,their score of perceived benefit of joining NCMS were increased,and the rate of willingness to join NCMS increased remarkably in both intervention counties.Conclusion:Health education was effective and helpful in increasing farmer's knowledge,understanding and cognitive level of NCMS,and it should play an important role for the sustainable development of NCMS.
3.Value of serum procalcitonin in diagnosing bacterial lower respiratory tract infections in people with exacerbation of Chronic Obstructive Pulmonary Disease

Objective:To investigate the changes and clinical implications of serum procalcitonin in exacerbation of chronic obstructive pulmonary disease(COPD).Methods:We have evaluated PCT measurement in 45 patients with an exacerbation of COPD(group A) and 25 patients with stable COPD(group B),quantitative sputum culture was performed,too.PPMs were only regarded as significant if they reached a growth of ≥10~7cfu/mL,indicating the presence of bacterial infection.Results:In patients with an exacerbation,15 patients,sputum yielded a high(≥10~7cfu/mL) bacterial load(group A_1),30 patients,sputum yielded a low(0.05).Using a cut-off point of 0.155?g/L for PCT,the sensitivities and specificities for bacterial infection in patients with an exacerbation of COPD were 93.3% and 60% respectively.Conclusion: Serum procalcitonin measurements in patients of an exacerbation of chronic obstructive pulmonary disease play a role in the diagnosis of bacterial infection.
4.Change of airway anaphylatoxin C5 a in patients with asthma

Objective:To investigate the role of anaphylatoxin C 5 a in patients with asthma .Methods:A prospective study was performed between September 2006 and February 2007.A total of 33 patients with acute exacerbation of asthma and 13 healthy subjects were recruited into the study .The patients with acute exacerbation of asthma were also studied when they returned to the remission state .Levels of lung function, levels of C5a in induced sputum and cell differential count in induced sputum were determined . Results:The level of C5 a in induced sputum was significantly higher in patients with acute exacerbation of asthma [0.85(0.68-2.13) μg/L] than that in patients with stable asthma [0.45(0.26-0.88)μg/L, Z=-2.193, P=0.013];Sputum C5a levels in stable asthma patients were significantly higher than those in healthy controls [0.14(0.06-0.45) μg/L, Z=-2.141, P=0.015].The level of C5a in patients with severe exacerbation [2.21(1.27 -9.0) μg/L] was significantly higher than those in patients with mild exacerbation [0.34(0.17-0.63) μg/L] and moderate exacerbation [0.85(0.55-1.67) μg/L,χ2 =12.330, P=0.001].The level of C5a in induced sputum was positively correlated with the number of total cells count (r=0.797, P=0.004), neutrophils (r=0.504, P=0.032) and macrophages ( r =0.424, P=0.036 ) in acute exacerbation of asthma .Conclusion: C5a levels in induced sputum could be identified as an important prognostic biomarker , which involved in asthma ’ s pathogenesis .
5.Application of Plasma Exchange Therapy on Critical Diseases in Children

Objective To explore continuous renal replacement therapy(CRRT) machine for plasma exchange in critical disease in children.Methods Retrospective study of 8 patients(8 month to 14 years,mean 5.7 years) and 32 plasma exchange treatments,after(adowble) lumen catheter inserted into the subclayian venous,using the Baxter BM25 machine with commercially available plasma filters.Results Five patients(3 ABO-incompatibility in bone marrow transplantation,1 thrombotic thrombocytopaenic purpura TTP,1 sepsis) gained full recovery.One systemic lupus erythematosus(SLE) and 1 sepsis experienced moderate improvement while 1 case of acute disseminated encephalomyelitis failed PE treatment.The average total exchange volume was 80-100 mL/kg,achieved at a blood flow rate of 5-10 mL/(kg?min) and a turnover rate of 60-120 mL/(kg?h) over a 3-hours duration.Thirty-one PE treatments were finished smoothly,one of which experienced the serious complication involving plasma filter.Conclusion Plasma exchange therapy is a safe and effective procedure for severe autoimmune abnormalities and pathogen removal in children.
6.Effects of atorvastatin on blood pressure and left ventricular remodeling in patients with both essential hypertension and hypercholesterolemia

Objective To compare the changes in serum levels of high sensitive C-reactive protein (hs-CRP)and uric acid(UA)before and after atorvastatin treatment for the patients with both essential hypertension and hypercholesterolemia,and to evaluate its effects on blood pressure and left ventricular remodeling.Methods One hundred and twenty-six hypertensive patients complicated with hypercholesterolemia were randomized into group A with amlodipine 10 mg/d(n = 65)and group B with amlodipine 10 mg/d plus atorvastatin 20 mg/d(n = 61),for three months continuously.Serum levels of total cholesterol(TC),low-density lipoprotein-cholesterol(LDL-C),high-density lipoprotein-cholesterol (HDL-C),triglyeerides(TG),hs-CRP and UA,as well as blood pressure,were determined for both groups before and after treatment.Left ventricular posterior wall thickness(LPWT)and interventricular septum thickness(IVST)were measured by echocardiography and left ventricle mass index(LVMI)was calculated.Results Serum levels of TC,LDL-C,HDL-C,TG,hs-CRP and UA decreased significantly in gr6up B after three-month treatment with atorvastatin,while serum level of HDL-C increased significantly. With the in-depth study of related substances and the development of consistency evaluation of generic drugs, relative correction factors are gaining increasing attention. By analyzing the domestic and foreign literature on correction factors in recent years, this paper describes the correction factor component, the current measurement method and its application. The rules and key points of use of an impurity correction factor and its determination and application are described, and some problems in its determination and application are discussed, providing a reference and basis for the standardization of research on impurity correction factors in the future.

Background:Reports on the aquaporin-4 immunoglobulin G (AQP4-IgG) status for cognitive performance
and neuroimaging correlations are limited in neuromyelitis optica (NMO) and multiple sclerosis (MS)
literature. Methods: Cognitive results of 19 MS and 15 NMO patients were compared with 47 agematched
controls. Apparent diffusion coeffi cient (ADC) values were used to delineate gray matter
and white matter damages and correlate with neuropsychological results. Results: Verbal memory test
showed signifi cant differences between MS and NMO in the late registration, early and delay recall
(p<0.05), while their retention rates were even. In MS, ADC values were signifi cantly elevated in the
dorsolateral prefrontal and occipital gray matter which was in contrast with NMO group that showed
elevation in the dorsolateral prefrontal gray matter and parieto-occcipital white matter. AQP4-IgG
status exerted a limited effect on ADC values and neuropsychological results.
Conclusions: Verbal memory test might be helpful in differentiating NMO and MS. ADC values
can be used as a surrogate marker for tissue injury in NMO and MS since they were in line with the
cognition scores. Anatomical regions with elevated ADC values were different in NMO and MS.

10.Rapid determination of dopamine and its metabolites during in vivo cerebral microdialysis by routine high performance liquid chromatography with electrochemical detection.

OBJECTIVETo determine dopamine and its metabolites during in vivo cerebral microdialysis by routine high performance liquid chromatography with electrochemical detection.
METHODSMicrodialysis probes were placed into the right striatum of Wistar rat brains and perfused with Ringer's solution at a rate of 1.5 microL/min. A reverse phase HPLC with electrochemistry was used to assay DA, DOPAC, and HVA after cerebral microdialysates were collected every 20 minutes from awake and freely moving rats. In order to identify the reliability of this method, its selectivity, linear range, precision and accuracy were tested and the contents of DA, DOPAC, and HVA in rat microdialysates were determined.
RESULTSThe standard curve was in good linear at the concentration ranging from 74 nmol/L to 1.5 micromol/L for DOPAC (r2=0.9996), from 66 nmol/L to 1.3 micromol/L for DA (r2=1.0000) and from 69 nmol/L to 1.4 micromol/L for HVA (r2=0.9992). The recovery of DOPAC (0.30, 0.77, 1.49 micromol/L), DA (0.26, 0.69, 1.32 micromol/L), and HVA (0.27, 0.71, 1.37 micromol/L) was 82.00+/-1.70%, 104.00+/-4.00%, 98.70+/-3.10%; 92.30+/-1.50%, 105.30+/-2.30%, 108.00+/-2.00%; 80.00+/-7.80%, 107.69+/-8.00%, and 108.66+/-3.10%, respectively at each concentration. Their intra-day RSD was 3.3%, 3.4%, and 2.5%, and inter-day RSD was 4.2%, 2.3%, and 5.6%, respectively. The mean extracellular concentrations of DOPAC, DA, and HVA in rat brain microdialysates were 10.7, 2.4, and 9.2 micromol/L (n=6), respectively.
CONCLUSIONThe findings of our study suggested that the simple, accurate and stable method can be applied to basic researches of diseases related to monoamines neurotransmitters by cerebral microdialysis in rats.
