1.Relationship between acute radiation pneumonitis after intensity modulated radiation therapy in patients with non-small cell lung cancer and parameters of dose-volume histogram

Objective To observe the incidence of acute radiation pneumonitis (RP)after intensity modulated radiation therapy,and to explore the relationship between the incidence of RP and the parameters from dose-volume histogram (DVH)in the patients with non-small cell lung cancer (NSCLC).Methods By Pinnacle 7.6c TPS, 46 patients with NSCLC were treated by IMRT.The values of the percentages of the lung volumes receieved 5, 10,20,and 30 Gy radition to the total lung volumes (V5 ,V10 ,V20 ,V30 )and mean lung dose (MLD)were observed,and its correlation with acute RP was analyzed.Results The incidence of acute RP of the patients with NSCLC was 37.0% (17/46),among then there were 29 cases (63.0%)of Grade 0 (non-pneumonitis group), 12 cases (26.1%)of Grade 1,5 cases (10.9%)of Grade 2,and there were no Grade 4 and 5 RP.The values of V5,V10,V20,V30,and MLD of lung were 53.34 %,43.12%,24.15%,15.36 %,and 16.02 Gy.The values of V5,V10,V20,V30,and MLD in pneumonitis group were 57.81%,48.91%,31.34%,17.83%, 21.71 Gy,and they were 49. 81%,39.78%,21.82%,13.12%,and 13.71 Gy in non-pneumonitis group, there were significant differences between two groups (P<0.05).Conclusion The intensity modulated radiation therapy in treatment of NSCLC can protect the lung tissue,and the parameters of DVH such as V5 ,V10 ,V20 , V30 and MLD can predict the occurrence of RP.
2.Clinical efficacy of Tanreqing Injection in treatment of acute irradiationmucosal injury in patients with head and neck cancer

Objective To observe the therapeutic effect of Tanreqing Injection in treatment of acute irradiation oropharyngeal mucositis in the patients with head and neck cancer, and to provide reference for its clinical application.Methods Fifty patients with head and neck cancer were randomly divided into treatment group and control group(n=25).The patients in treatment group received concurrently intravenous infusion of Tanreqing Inj ection during irradiation course.The patients in control group received daily regular oral nursing;oropharyngeal mucositis was evaluated according to the protocol proposed by RTOG.Results The patients in treatment group with Ⅰ and Ⅱ oropharyngeal mucosal injury accounted for 76%,and the patients in control group with Ⅲ,Ⅳoropharyngeal mucosal injury accounted for 60%,there was statistically significant difference between two groups (P<0.05).The scores of the patients with oropharyngeal pain in treatment group were 0-3 (mild),in control group they were 4- 6 (moderate).The patients with mild,moderate,and severe oropharyngeal pain in treatment group and control group after radiotherapy accounted for 72%,24%,4% and 12%,64%,24%;there were statistically significant differences between two groups (P<0.05).Conclusion Tanreqing Injection has a good therapeutic effect on acute radiation stomatitis and layngopharyngitis, and it has the effects of relieving oral and pharyngeal mucosa inj ury,anti-inflammatory and analgesic and accelerating wound healing.
3.Applicability of capacity of sexual self-defense assessment scale in psychiatric patients without mental retardation.

OBJECTIVE:
To explore the applicability of Capacity of Sexual Self-defense Assessment Scale (CSSAS) in psychiatric patients without mental retardation.
METHODS:
Eighty-seven cases required for assessment of the capacity of sexual self-defense were collected and evaluated by trained professionals according to CSSAS, and the results were compared to the experts' opinions.
RESULTS:
The scores of three grades of capacity of sexual self-defense (none, limited and whole) were 5.97 +/- 5.83, 32.22 +/- 10.15 and 61.60 +/- 13.02, respectively, which were statistically significant among them (P < 0.01). The correlation coefficients between items and total score were 0.59 to 0.91, the Cronbach's alpha value was 0.96, the spilt half Cronbach's alpha value were 0.94 and 0.91, respectively, and the relation coefficient between them was 0.94. According to the original cut off scores, the Kappa value between scale results and experts' opinions was 0.32 (P < 0.01). By redefining the cut off scores, the Kappa value was improved to 0.84 (P < 0.01). Two factors were extracted by means of the factor analysis. The explanation rates of variance were 46.15% and 28.93%, respectively. The accuracy of retrospective cumulative square was 94.30%.
CONCLUSION
The CSSAS could be applied in the psychiatric expertise of the capacity of sexual self-defense in psychiatric patients without mental retardation; however, the cut off scores and the factor weight should be redefined.

4.Relevance between writing characteristic and therapeutic effect in schizophrenia.

OBJECTIVE:
To explore the relevance between writing characteristic and therapeutic effect in schizophrenia and to discuss the influence of aggressive behavior on writing characteristic.
METHODS:
Recoding the casual and fixed writing in admission, one week, two weeks, four weeks, eight weeks after treatment and rating Positive and Negative Syndrome Scale (PANSS) and Modified Overt Aggression Scale (MOAS). Choosing two characteristics, "relationship between font and grid lines" and "having big strokes or not", and comparing before and after treatment.
RESULTS:
Eight weeks after treatment, the score of PANSS decreased. The condition of patients and the writing characteristic improved as well. The differences of writing characteristics were statistically significant in patients with aggressive behavior before and after treatment (P < 0.05).
CONCLUSION
The writing characteristic has relation with therapeutic effects and improved with therapeutic effects in aggressive patients.

5.Study on Enhancing Efifcacy and Reducing Toxicity of Total Flavonoids, Total Polysaccharides inTaxus mairei Compatibility with Taxol

This article was aimed to study enhancing efficacy and reducing toxicity on anti-tumor from total flavonoids and total polysaccharides in Taxus mairei compatibility with taxol. MTT assay was performed to analyze the cell proliferation and apoptosis of 4T1, A549, MCF-7 tumor cells after treatment of total flavonoids and total polysaccharides in T. mairei combined with taxol at different concentrations. Isobolograms was used to investigate the behavior of combination therapy. Therapy experiments were conducted on ICR mice bearing S180 sarcoma. The tumor weight was measured to calculate inhibition rate according to uniform design. Blood cells indicators were detected by blood cell analyzer. The results showed that total polysaccharides and total flavonoids can enhance the inhibition effect on 4T1, A549 cells by taxol. Total polysaccharides can enhance the inhibition effect of taxol to MCF-7 in vitro, and S180 sarcoma-bearing mice in vivo. It reduced the toxic effects of anti-tumor effect of taxol in vivo. It was concluded that taxol can achieve the effect of enhancing efficacy and reducing toxicity through combining with total flavonoids and total polysaccharides in T. mairei.
6.Strategy of postoperative TR -Band tourniquet to stop bleedingin coronary interventional treatment

Objective To observe the different methods of hemostasis and comfort of the TR -Band com-pression hemostasis device after coronary intervention and to find the best strategy for TR -Band to stop the bleeding. Methods 150 patients with coronary artery disease were randomly divided into three groups according to the digital table method.The initial time of TR -Band compression hemostasis of group A was 3h,(once per hour,2mL per time,total compression time was 10h),while that of group B was 2h(once in every two hours,3mL per time,total com-pression time was 8h),and that of group C was 1h(once per hour,2mL per time,total compression time was 6 h). Thumb oxygen saturation,complications and comfort level in the three groups of patients were observed and compared. Results The differences between the three groups of oppression hemostasis before and after 2 h thumb oxygen saturation were normal,the three groups had no statistical significance (P >0.05).lesions,hematoma,bruising and radial artery occlusion occurred rate of group A were respectively 4,3,5,2 cases,those of group B were 3,4,6, 1 cases,and those of group C were 2,5,6,0 cases.Lesions,hematoma,bruising,radial artery occlusion of local com-plication incidence of the three groups showed no statistical significance (P >0.05).The comfort level (numbness, swelling,pain)of the three groups had statistically significant differences (Fnumbness =8.215,Fswel ing =7.657,Fpain =7.985,all P <0.01).Conclusion TR -Band compression hemostasis device can achieve effective hemostasis in the treatment of coronary artery intervention.And the reasonable adjustment of different bleeding time,compression time and the amount of bleeding can affect the comfort of patients.
7.Medial and lateral plating treatment for Schatzker type V tibial plateau fractures with dual incisions

0.05).The clinical outcomes were evaluated according to improvement HSS knee joint function scoring,and the score showed an average of 87.5 points (range,71-97).[Conclusion]The dual-incision technique resulted in accurate fracture reduction with low tissue complication rate,and the medial and lateral plating treatment achieved improved stabilization which enabled mobilization of the knee joint to start immediately after surgery.Medial and lateral plating treatment with dual incisions is a safe and effective technique for Schatzker type Ⅴ tibial plateau fractures.

Objective To explore the effects of ingestion of purified drinking water from water treatment devices on health of rats. 10.Clinical features of 70 patients with primary biliary cirrhosis

Objective Clinical features of primary biliary cirrhosis (PBC) were reviewed in order to improve its diagnosis in our clinical practice. Methods Clinical data of 70 patients with PBC were reviewed including the clinical manifestation, laboratory tests, pathological findings and the response to therapy. Results Sixty-two patients were females (88.6%), eight patients were males (11.4%), and the mean age at diagnosis was (53.1?10.4) years. The most frequently complained symptoms were pruitus (60.0%), fatigue (42.9%), anorexia (41.4%) and jaundice (45.7%). Serum alkaline phosphatase (ALP) and ? glutamyl transpeptidase (GGT) levels were markedly elevated in all patients, while alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels were mildly elevated. The serum total bilirubin level increased in sixty-five patients (92.9%). Forty-four patients (62.9%) had elevated serum immunoglobin M (IgM), and 98.6% of patients (69/70) were antimitochondrial antibody (AMA) /AMA-M2 positive. Forty-one patients underwent pathological examinations, 82.9% in early stage (Ⅰ,Ⅱ stage) and 17.1% in advanced stage (Ⅲ,Ⅳ stage).Conclusions PBC most frequently affects middle-aged women and the main clinical manifestations are pruritus, fatigue, anorexia and jaundice. The elevated level of ALP, GGT, IgM and positive AMA/AMA-M2 may be crucial to diagnosis of PBC.
