3.Ipsilateral cerebral abscess and endophthalmitis from an infected external carotid artery coil

Simultaneous endopthalmitis and ipsilateral cerebral abscesses are uncommon. This is the case report of
a 68 years old man with left peritonsillar squamous cell carcinoma, who had coils placed in the external
carotid artery to induce arterial thrombosis and slow tumor growth. The patient subsequently developed
left sided endopthalmitis and left brain abscess. This shows that the coils are able to serve as a nidus
of infection resulting in secondary endopthalmitis and brain abscess in an immunocompromised host.

Merkel cell carcinoma (MCC) is an unusual primary cutaneous tumor, occasionally found con-current with other malignancies. A case of MCC with coexisting squamous cell carcinoma (SCC) was studied histologically, immunohistochemically and ultrastructurally. The MCC and SCC occured at the same site, but each preserved its identity and transition between the two was not identified.

7.A Case of Multiple Basal Cell Epitheliomas Arising in An Epidermal Nevi.

Basal cell epithelioma arising from epidermis and its appendages is a common skin tumor. But multiple occurence of basal cell epitheliomas and that of arising in an epidermal nevi are extremely rsre. Since the first report by Toyama and Kojima in 1937, only few cases were reported in the world literatures. We observed a case of multiple basal cell epitheliomas arising in an epidermal nevi in a 54-year-old female patient who had rhomboidal verrucous patch on the right side of the cheek and arciform erythematous verrucous eruption from the right side of the eyebrow to the lower eyelid. Clinical features were not compatible with any type of basal cell epithelioma. But histological finding revealed basal cell epithelioma with epidermal nevi. Literatures were briefly reviewed.

8.Effect of Z,E-butylidedephthalide on experimental choroidal neovascularization in rat and ocular blood flow in rabbits

AIM: To investigate the effect of Z,E-butylidedephthalide (Bdph) on laser-induced experimental choroidal neovascularization (CNV) in rat model and choroid blood flow in rabbits'eyes.METHODS: Male Brown Norway rats were treated with Nd:YAG laser to break Bruch's membrane. Thirty mg/kg and 15 mg/kg Bdph were given daily through intraperitoneal injection for 4 weeks after laser treatment. Fluorescein angiography (FA) and choroidal flat mount were used to measure the development of CNV. Female New Zealand white rabbits' eyes were instilled with 10g/L Z,E-butylidenephthalide solution,and ocular blood flow was measured with colored microsphere technique. RESULTS: The intensity of fluorescein leakage, indicating the ocular lesion, decreased significantly in group Bdph 30mg/kg and 15mg/kg, as compared to the control at P<0.01.The area of neovascularization checked by FA in both groups of Bdph, at 30mg/kg and 15mg/kg decreased significantly compared to the control group at P<0.05. On the choroid flat mount, the areas of CNV were also smaller in both Bdph groups than in control group. One percent Z,E-butylidenephthalide solution instilled into rabbits' eyes could improve the choroid blood flow at 30 and 60 minutes after drug instillation (P<0.05).CONCLUSION: Z,E-butylidedephthalide could inhibit the development of CNV in the rat eyes and increase the choroid blood flow in the rabbit eyes. These results suggest that Z,E-butylidedephthalide may be a good agent for the treatment of age-related macular degeneration(ARMD).
9.Application of the TRISS method to evaluate trauma care.

The TRISS (Revised Trauma Score and Injury Severity Score) method of trauma care evaluation was applied to 476 consecutive trauma patients admitted to our medical center over a 6-month period. Male to female ratio was 8:1, with a mean age of 24.7 years. Penetrating injury was the most common mechanism of injury (62%), with the chest as the most common region injured (36%). Mean probability of survival of 476 patients was 0.9802 and a predicted mortality of 9.4 patients. The overall mortality was 5.4 per cent with 26 actual deaths. As 93 per cent of patients had injuries to isolated anatomic regions, using the TRISS method, assigning numerical values to noninjured anatomic regions mathematically increased their probability of survival, even though the isolated injury was life-threatening. Consideration should be taken before adapting the TRISS method as the gold standard in evaluating trauma care in the local setting

10.Femoral lengthening: Report of 2 Cases

Reduction in the extremity length of more than three to four centimeters has significant effect on both gait disturbance and aesthetic change. The method of Anderson has been one of the most successful procedures of surgical leg elongation but requires long period of bed rest and immobilization. Also. the lengthening occurs the tibia, although shortening occurs twice as frequently in the femur as in the tibia. Wagner in 1971 reported his experience with the technique that allows correction in both the femur and tibia yet does not require prolonged immobilizaton. This involves a two stage procedure with rigid external fixation and lengthening of 1.5mm per day. The second stage is for osteosynthesis and may be suppleniented by bone graft. We began using the Wagner's technique of leg lengthening at Chonju Presbyterian Medical Center about 18 months age. Two patients are presented with 7 and 9 month follow-ups after femoral lengthening of 5 cm each. Both femurs united well and the leg length discrepancy was corrected. These cases indicate that this method is a good addition to the armamentarium of treating leg length discrepancy. A subsequent review of more cases and their follow-up using this tretment program shall be forthcoming.
