1.Effects of Nurses' Self-leadership and Team Trust on Organizational Commitment.

PURPOSE: The purpose of this study was to investigate the association between self-leadership, team trust, organizational commitment and identify the influencing factors on organizational commitment. METHODS: The subjects in the study were 296 conveniently selected general nurses at 2 general hospitals in Chungbuk Province. The measurements included the self-leadership, organizational trust inventory (OTI), and organizational commitment. RESULTS: The mean score for self-leadership was 3.40, the mean score for OTI was 3.61, and the mean score for organizational commitment was 3.07. There were significant correlations among self-leadership, team trust and organizational commitment. Self-leadership had significant correlation with team trust (r=.25, p<.001). Also, self-leadership (r=.34, p<.001) and team trust (r=.31, p<.001) showed significant correlation with organizational commitment. In stepwise multiple regression analysis, age, self-leadership and team trust were included in the factors affecting organizational commitment of clinical nurses. These variables explained 20% of organizational commitment. CONCLUSION: The results confirmed that nurses' self-leadership and team trust affects significant influences on organizational commitment. Accordingly, in order to enhance nurses' organizational commitment, it is necessary to build effective strategies to enhance self-leadership and team trust for nurses. Such strategies will be able to improve the goal achievement of nursing unit.

2.Unusual Manifestation of Immunoglobulin G4-Related Disease Involving the Retroperitoneum:A Case Report

Immunoglobulin G4 (IgG4)-related disease is a systemic disease characterized by dense lymphoplasmacytic infiltrates with abundant IgG4-positive plasma cells and fibroblast proliferation. The retroperitoneal involvement of IgG4-related disease usually appears as a soft-tissue mass covering the abdominal aorta or entrapping the ureters, resulting in hydronephrosis. Here, we present a case of IgG4-related disease with retroperitoneal involvement in a 75-yearold woman with an unusual manifestation. A preoperative computed tomography (CT) scan revealed an irregular infiltrative retroperitoneal mass invading the normal anatomic barriers, raising the suspicion of malignancy or inflammation. Contrast-enhanced CT revealed a homogeneous progressive enhancement of the mass.
3.Multiloculated Cystic Type Renal Epithelioid Angiomyolipoma Mimicking Renal Cell Carcinoma: A Case Report

Renal epithelioid angiomyolipoma (EAML) is a rare variant of angiomyolipoma (AML), with a prominent epithelioid component. EAML usually presents as a large heterogeneous soft tissue lesion with intratumoral hemorrhage and variable necrosis or cystic changes. We present a case of multiloculated cystic renal EAML mimicking renal cell carcinoma in a 64-year-old female.Intracystic massive hemorrhage, hyperattenuating wall and septa on an unenhanced study, and enlarged intratumoral vessels can be helpful imaging features for distinguishing renal EAML from renal cell carcinoma.
4.Is It Possible to Discontinue Tumor Necrosis Factor Antagonists after Psoriasis Remission?

Tumor necrosis factor (TNF) antagonists are highly effective treatments for psoriasis. These agents provide the opportunity to improve disease activity and achieve clinical remission. Despite its efficacy, long-term use of biologics is associated with high financial costs and possibly life-threatening adverse events. Recently, there has been an increasing interest in discontinuing TNF antagonists in patients with psoriasis who have achieved a positive clinical response. However, there is a paucity of data and clinical guidelines concerning the cessation TNF antagonists in psoriasis treatment. Several factors, including cost, subsequent treatment efficacy, relative risks, and tolerability, should be considered before the decision is made to discontinue TNF antagonists. Well-designed clinical trials are necessary to identify factors that may trigger disease exacerbation after medication discontinuation in order to recognize the potential disadvantages of discontinuing treatment in patients who are previously successfully managed on TNF antagonists.

7.Influence of Nurses' Self-leadership on Individual and Team Members' Work Role Performance.

PURPOSE: The purpose of this study was to examine correlations between nurses' self-leadership and individual work role performance and correlations between self-leadership in nursing units and team members' work role performance. METHODS: Participants were 202 conveniently selected general nurses from 5 general hospitals in Korea. The study was carried out on 35 nursing units. Data were collected during February 2015 with self-report questionnaires. RESULTS: For factors affecting individual work role performance, self-expectation, self-goal setting, constructive thought, clinical career in the present nursing unit and marital status accounted for 44.0% of proficiency, while self-expectation, self-goal setting, constructive thought, and marital status accounted for 42.3% of adaptivity. Self-expectation, self-goal setting, constructive thought, self-reward, clinical career in the present nursing unit and position accounted for 26.4% of proactivity. In terms of team members' work role performance, self-reward and self-expectation in nursing units explained 29.0% of team members' proficiency. Self-reward and self-expectation in nursing units explained 31.6% of team members' adaptivity, and self-reward in nursing units explained 16.8% of team members' proactivity. CONCLUSION: The results confirm that nurses' self-leadership affects not only individual self-leadership but also team members' work role performance. Accordingly, to improve nurses' work role performance in nursing units of nursing organizations, improvement in nursing environment based on self-leadership education is necessary and nurses' tasks rearranged so they can appreciate work-autonomy and challenges of work.

8.Development and Validation of the Korean Patient Safety Culture Scale for Nursing Homes.

PURPOSE: The purpose of this study was to develop a tool to evaluate patient safety culture in nursing homes and to test its validity and reliability. METHODS: A preliminary tool was developed through interviews with focus group, content validity tests, and a pilot study. A nationwide survey was conducted from February to April, 2011, using self-report questionnaires. Participants were 982 employees in nursing homes. Data were analyzed using Cronbach's alpha, item analysis, factor analysis, and multitrait/multi-Item analysis. RESULTS: From the results of the analysis, 27 final items were selected from 49 items on the preliminary tool. Items with low correlation with total scale were excluded. The 4 factors sorted by factor analysis contributed 63.4% of the variance in the total scale. The factors were labeled as leadership, organizational system, working attitude, management practice. Cronbach's alpha for internal consistency was .95 and the range for the 4 factors was from .86 to .93. CONCLUSION: The results of this study indicate that the Korean Patient Safety Culture Scale has reliability and validity and is suitable for evaluation of patient safety culture in Korean nursing homes.

9.Prediction Model for Demands of the Health Meteorological Information Using a Decision Tree Method.

PURPOSE: Climate change affects human health and calls for health meteorological services. The purpose of this study is to find the significant predictors for the demands of the health meteorological information. METHODS: This study used a descriptive design through structured self-report questionnaires. Data from 956 participants who were at least 18 years old and living in Busan, Korea, were collected from June 1 to July 31, 2009. The data was analyzed using a decision tree method, one of the data mining techniques by SAS 9.1 and Enterprise Miner 4.3 program. RESULTS: Two hundred and ninety participants (30.3%) demanded the information, and 505 of them (52.8%) perceived the necessity of health meteorological information. From the decision tree method, the predictors related to the demands of the health meteorological information were determined as "the perception of the necessity of health meteorological information," "the coping to the weather warnings" and "the importance of the weather forecasting in daily life." In Particular, the significant different variables in the perception of the necessity of health meteorological information were "female," "aged over 40" and "environmental diseases." Thus, the model derived in this study is considered for explaining and predicting the demands of health meteorological information. CONCLUSIONS: It can be effectively used as a reference model for future studies and is a suggested direction in health meteorological information service and policy development. We suggest health forecasting as a nursing service and a primary health care network for healthier and more comfortable life.

10.Measuring Patient Safety Culture in Korean Nursing Homes.

PURPOSE: The purpose of this study was to investigate the patient safety culture in Korean nursing homes using the Nursing Home Survey Patient Safety Culture (NHS-PC), a valid tool, provided by the Agency for Healthcare Research and Quality (AHRQ) and to compare the results with AHRQ data. METHODS: Administrators and staff (N=151) of six nursing homes in Seoul, Busan, Kyeonggi Province and Gyeongsangnam Province completed the survey in July, 2010. The data were analyzed using descriptive statistics, positive response rate, t-test, ANOVA, DUNCAN, Cronbach's alpha. RESULTS: The total mean (SD) positive response rate for patient safety culture was not significantly different from the AHRQ data. For composite levels, the results of 'handoffs' were significantly higher, and the results of 'feedback and communication about incidents' and 'nonpunitive responses to mistakes' were significantly lower than the AHRQ data. CONCLUSION: More effective strategies related to nonpunitive responses to mistakes and management activities for patient safety are needed to improve patient safety culture in nursing homes.
