1.Urachal Adenocarcinoma -Report of Two Cases-.

Urachal carcinoma is an uncommon neoplasm associated with a poor prognosis. We have encountered two another cases of urachal adenocarcinoma recently 5 years after our first report. The common sign of urachal cancer was painless hematuria. We diagnosed the specific site of origin preoperatively by means of computerized tomography. One patient was performed partial cystectomy and another was total cystectomy with double barrel ureterocutaneostomy. A brief review of the literature of urachal adenocarcinoma was also made.

2.Milk-of-Calcium in a Pyelocaliceal Diverticulum; A Case Report.

The term "Milk of calcium" has been borrowed from a somewhat similar condition encountered in the gallbladder. The coexistence of stone in a caliceal diverticulum is well known but milk-of-calcium is unusual in this structure. Recently we experienced a case of milk-of-calcium in a pyelocaliceal diverticulum in a 44 year old man, so we report this case with a brief review of the literature.

3.Bifurcated Stent-Graft(Vanguard) for the Endovascular Treatment of Abdominal Aortic Aneurysm.

PURPOSE: The purpose of this study was to evaluate the safety, feasibility and effectiveness of an endoluminally-placed bifurcated stent-graft (Vanguard) for the treatment of infrarenal abdominal aortic aneurysm (AAA). METHODS: Transluminal endovascular stent-graft placements were attempted in 29 patients (28 male, mean age 69+/-7 years) with AAAs involving the common iliac arteries from Aug. 1997 to Jan. 1999. Endovascular therapy was performed in the cardiac catheterization laboratory with epidural anesthesia. One side of the femoral artery was opened by surgical cutdown for the bifurcated stent-graft entry and the other side was punctured percutaneously for the straight stent-graft. Computed tomography and/or intraarterial angiography were performed during an average follow-up of 10 months (2-18 months). RESULTS: Primary success rate was 75.9% (22 of 29 patients) and the overall success rate was 79.3% with successful correction of one perigraft leak. Twenty patients (69.0%) had significant coronary artery disease. There were two technical failure cases, the one was tortuous iliac vessel with spasm, the other was disconnection of the stent-graft connecting portion. Complications related to procedure occurred in 13.8% of patients (4 of 29 patients) and two of these four patients had procedure-related mortality because of acute renal failure following contrast overdose and sepsis after operation. CONCLUSIONS: Endovascular treatment of infrarenal AAA with bifurcated stent-graft (Vanguard<0A397>) is effective, feasible and relatively safe. However, further investigation for the outcome, complication and long-term follow-up are needed.

4.Transcatheter Double-Disc Occluder Closure of Patent Ductus Arteriosus.

We successfully closed the isolated patent ductusarteriosus(PDA) in 2 female patients using the Rashkind Double-Disc Occlusion system. nonsurgical closure of PDA usin plug was introduced by Porstmann in 1968 through transfemoral artery catheter and using umbrella by Rashkind in 1977 througth venous site. There has been continued improvement and simplification of the equipment as well as in the implantation technique. Nowaday, a multicenter study was conducted to test the safety and effectiveness of this interventioal method in States. Successful closure rate was high but some problems were reported such as embolization, abnormal deployment and residual shunt. Residual shunt was visualized by aortography in our both cases immediately after intervention but continuous murmur could not be heard in both cases. Oxygen step-up disappeared immediately after occluder implantation in both cases. Complete occlusion was expected with thrombus in near future.

6.Clinical Results of Bipolar Endoprosthesis

In 1974, bipolar endoprosthesis was first introduced by Bateman & Giliberty. It has theoretical advantages including minimizing acetabular wear, possible reduction of incidence of stem loosening, lessening of dislocation and easy revision procedure. Various methods for the treatment of fracture of the femur neck have been developed & performed, but it is still called “the unsolved fracture” in certain situation. But the role of it as primary treatment of fracture of the femur neck continues to be controversial. Current clinical results encourage its continued use and expansion of indication. Authors reviewed and analysed 21 cases of bicentric endoprosthesis and 1 case of Giliberty endoprosthesis those have been operated at the Department of Orthopaedic Surgery, National Medical Center from 1981 to 1983. Following results were obtained, l. According to disorder distribution, 14 cases were neglected fracture of femur neck, 2 old fracture, 1 idiopathic femoral head necrosis and 2 avascular necrosis & 1 non-union complicated by femoral neck fracture. 2. Among 20 cases, 8 cases were operated within 1/2 1 month after injury and 6 cases within 1 3months 3. Harris lateral appmach was used in 16 cases (80%) and Modified Gibson approach in 4 cases (20%). There were no significant differences in morbidity during operation and postoperative complication. 4. We used femoral stems those head diameter was 22 and 32mm(Charnley & Miiller type) and head pieces those diameter was 48-51 mm in male and 44–48 mm in Ifemale. 5. 3 cases were stiff in inner bearing without acetabular erosion. 6. In 6 cases more than 2 years followup 1 degree CE angle of Wiberg was increased in 2 cases of youngage respectively without associated hip joint pain. 7. Although inclination of head piece was vertical in 8 cases, fracture and dislocation were not found. 8. The circumference of head piece and opposite femoral head was measured and compared. There were 12 cases (mean 12.6 mm) that of head piece larger than opposite femoral head and 3 cases smaller than opposite femoral head. 9. There was 4 cases of complication (3 patients) and still no reoperation and revision converted to T.H.R. 10. With regard to average 18 months of follow-up, 7 cases of 81–90 Harris hip-rating score, 6 of 91–100, 4 of 71–80 and 3 of 61–70.

7.The Changes of the Immunoglobulin Levels of Prostatic Secretion in Bacterial and Nonbacterial Prostatitis.

Even though prostatitis remains a complex, often puzzling disease, studies during the last decade have clarified many of its features. Much of the prevailing confusion concerning prostatitis is attributable is attributable to imprecise methods of diagnosis. We measured the immunologic response of the EPS using radial immunodiffusion method in 14 bacterial prostatitis cases, 39 nonbacterial prostatitis cases and 24 normal volunteer controls. The results obtained in this study are as follows: 1. Among the 14 cases of chronic bacterial prostatitis, gram negative bacterias were cultured from VB, and/or EPS specimens in 12 cases (85.8%) , and gram positive bacterias in 2 cases (14.2%). 2. In normal group, the mean+/-S.D. of IgG, IgA and IgM an serum was 1,235+/-203.78mg/dl, 192.0+/-50.16mg/dl, 172.0+/-32.11mg/dl respectively, in chronic bacterial prostatitis 1,171.5+/-241.O6 mg/dl, 163.1+/-67.61mg/dl, 137.1+/-51.88mg/dl, in nonbacterial prostatitis 1,258.3+/-313.47 mg/dl, 205.0+/-78.37 mg/dl, l35.0+/-48.78mg/dl. There was no significant difference among the normal control group, chronic bacterial and nonbacterial prostatitis patients. 3. In the normal group, the mean+/-S.D. of lgG, IgA and IgM was 1.5+/-0.74 mg/dl 0.82+/-0.06mg/dl, 2.7+/-1.37mg/dl, in chronic bacterial prostatitis 9.1+/-6.39mg/dl, 6.1+/-5.09mg/dl, 3.3+/-1.37mg/dl, in chronic nonbacterial prostatiti 2.8+/-1.99mg/dl, 2.7+/-3.05mg/dl, 2.2+/-1.56mg/dl. The levels of IgG. 4. The ratio of IgG and IgA in prostatic secretion to immunoglobulin in serum was increased in chronic bacterial prostatitis significantly more than the normal control group(P < 0.05). and IgA in prostatic secretion were more increased in chronic bacterial prostatitis than the normal control group(P< 0.05) but there was no significant elevation of EPS immunoglobulin in non-bacterial prostatitis.(P> 0.05) 5. After antimicrobial therapy the level of IgG and IgA in prostatic secretion was progressively decreased in chronic bacterial prostatitis but there was no definite change in chronic nonbacterial prostatitis.

8.Congenital Ventral Curvature of the Penis: A Case Report.

Congenital penile curvature without chordee is a rare anomaly and a surgically correctable lesion which can usually distinguished from chordee and other acquired causes of penile curvature. The lesion is usually found in young men in the third decade of life. The curvature is usually present since birth but there are no complaints until puberty. Physical examination with the penis in the flaccid state is usually normal. We report a case of ventral curvature of the penis in 20-year-old-male. Correction of the penile curvature with Nesbit`s operation was Performed.

9.Femoral Neck Stress Fractures in South Korean Male Military Recruits

Background:
Femoral neck stress fractures (FNSFs) are rare but potentially disabling injuries if the diagnosis is missed or delayed and proper treatment is not provided. The aim of this study was to investigate and describe the characteristics and clinical course of FNSFs in South Korean male military recruits.
Methods:
Between May 2015 and October 2019, 16 fractures in 12 young male military recruits were reviewed. The characteristics of the fractures were ascertained by detailed analysis of the history and clinical course, as well as radiographs, bone scintigrams, and magnetic resonance images.
Results:
The median duration between endurance training and the development of hip pain was 5 weeks, while the median duration of pain before the patient sought medical attention was 3 weeks. Four patients (33.3%) exhibited bilateral fractures, and concomitant lesions involving the proximal tibia were found in 3 patients (25%). Fourteen of the 16 fractures (87.5%) were compression-type fractures, and surgery was performed for 7 hips. Complete union without malunion or osteonecrosis was achieved in all cases, and all cadets returned to their full activity levels in an average of 10 weeks.
Conclusions
We observed excellent prognosis of FNSFs. Our report highlights the importance of early reporting, detection, and treatment regarding the challenging management of FNSFs. When a military recruit reports hip pain, a FNSF should be considered; furthermore, the bilaterality of the fracture and the presence of concomitant lesions should also be investigated.
10.Pulmonary Embolism following Pelvic Lymphadenectomy for Staging Prostatic Carcinoma.

We reported a case of postoperative pulmonary embolism associated with pelvic lymphadenectomy for staging prostatic carcinoma. A 66-year-old man was admitted to our hospital complaining of dysuria. Transperineal prostatic biopsy under transrectal ultrasonographic guidance revealed evidence of malignancy. Abdominal CT and whole body scan demonstrated no abnormal findings. Staging pelvic lymphadenectomy was performed safely under general anesthesia. On the 6th postoperative day, the patient had sudden dyspnea and chest pain. EKG showed sinus tachycardia. Chest A-P showed some elevation of left diaphragm. Pulmonary perfusion scan revealed multiple cold area throughout both lung fields. A diagnosis of pulmonary embolism was made. Shock treatment and anticoagulant therapy were successfully performed. This case suggests that attention should be directed to pulmonary embolism as one of the postoperative complication of staging pelvic lymphadenectomy.
