1.Assessment of Sliding Hip Compression Plate in the Intertrochanteric Fracure of the osteoporotic Bone

The incidence of intertrochanteric fracture of femur in young adult is relatively high in our home than other countries. But recently the incidence in elderly patient is progressively increased due to prolongation of average life span and increased activity. Various devices has been developed and the result of treatment has been improved, but the morbidity after fracture in elderly patient remains high because of poor general condition and osteoporosis. We tried to assess the availability of SHCP(sliding hip compression plate) in the intertrochanteric fracture of the osteoporotic bone by the review of the 27 cases below Singh index III that we experienced from September, 1989 to June, 1993 at the Department of Orthopedic Surgery of Chonbuk National University Hospital. The results were as follows: 1. Mean age was 68 years. Type III in Tronzo classification was most common(51.9%). Singh index II was 17 cases(63%) and grade III was 10 cases(37%). 2. In 23 cases(88.8%), anatomical reduction was performed. The bony union was gained at average 15.8 weeks. The serious complications were 1 penetration and 1 osteoarthritis of hip. 3. Satisfactory results were obtained in 88% of patients by the functional class of Clawson DK.

2.Preventive Efficacy of Influenza Vaccination against to Influenza-like Illness among Eldery.

BACKGROUND: Influenza is a highly infectious viral illness. The epidemics of influenza occur from the late fall to the early spring nearly every year and are responsible for several hundred thousand death per year all over the world. Influenza vaccine is the primary method for preventing influenza and its more severe complications. The efficacy of influenza vaccination have been well known in developed countries. The influenza vaccination has been recommended as one of the tentative immunization schedule for indicated persons since 1997 in Korea. But there are still no available data about them, even though nearly 5 - 8 million doses of influenza vaccine were used in a winter season. PURPOSE: To assess the clinical efficacy of influenza vaccine against influenza-like illness(ILI) among Korean elderly(age over 65). METHODS: Primary study design - a prospective field trial. Study subjects and method - We prospectively observed the 1,488 vaccinee and 1,425 non-vaccinee in a community(Nonsan city) from November 1 1998 to March 31 1999. The vaccinee were those who randomly selected 100-200 per 13 sub-region(Myon) in Nonsan city, and the non-vaccinee were those who had not vaccinated until January 1999 among the Nonsan city elderly cohort. This cohort was consisted of 5,787 elderly(over 1/3 of total elderly population) who had ever visited the community health center or 13 public health offices in Nonsan city in 1998. We followed up these two groups two times-in January and April 1999 - by telephone survey. The questionnaire included the questions about the health behaviors, medical history, socioeconomic condition, medical security type, physical function status, medical facility utilization in the season, symptom and signs of influenza-like illness, number and type of the flu, hospitalization or not. We did throat swap to isolate the virus of 490 patient who had influenza -like symptoms in the same period. RESULTS: Influenza vaccination was significantly reduced the incidence of influenza -like illness(fever or chilling sensation and cough or sore throat) in the vaccinated. The preventive effect of influenza-like illness among influenza risk group was about 32%(95% CI, 20%-44%). The illness duration of main symptoms of influenza-like illness(fever, cough) was significantly shorter in the vaccinee. The consistency of the survey was noted quite high from the analysis of the result of test-retest(104 person). CONCLUSION: Influenza vaccination significantly reduced the incidence of influenza-like illness(32%) among elderly.

3.Anatomical Study of Musculus Pyramidalis in Korean Adults.

The report of the M. pyramidalis was studied in 51 cadavers of Korean adults (male 33, female 18). Origin, insertion and number of this muscle were observed, and the length and breadth were also measured. The results were as follows : 1. The right pyramidal muscle was absent in one male cadaver, and the left pyramidal muscle was absent in two male cadavers. 2. In one male cadaver, pyramidal muscles of both sides had bifurcated origin. 3. In Korean adults, the average length of this muscle was 6.72cm in right, 7.05cm in left, and the breadth was about 2.0cm in both sides. The morphological index was 31.10 in right, and 29.55 in left. 4. In Korean male, the average length of this muscle was 1.93cm in right, 7.42cm in left, and the breadth was 1.93cm in right and 1.92cm in left. The morphological index was 28.70 in right and 26.58 in left. 5. In Korean female, the average length was 6.26cm in right, 6.38cm in left. and the breadth was 2.13cm in right and 2.12cm in left. The morphological index was 35.57 in right and 34.63 in left. 6. Compared with other human races, the pyramidal muscle was less frequently wanting in Asians (3.9%), including the Korean adults, than wanting in European (19.9%), American Whites (20.4%) and Negroes (15.1%). 7. The morphological index of this muscle was little differences existing in male of the human races, but this index was higher in Korean female (35.0) than in Japanese female (24.4) and in American female Whites (25.8). The morphological index was 32.3 in American female Negroes.

Among individual factors, age, frequency of communication with friends, religious activity, social activity, leisure activity, trust in neighbors, subjective stress level, depressive symptoms, and subjective health status were significantly associated with suicidal ideation. The results showed that the lower the regional deprivation level, the higher the suicidal ideation odds ratio. In terms of regional size, the most significant effects were found in rural areas.CONCLUSIONS: This study suggested that suicidal ideation in the elderly is associated with community factors, such as the regional deprivation index, as well as personal factors.

5.Management of endometrial polyps in infertile women: A mini-review

Considerable disagreement exists regarding whether endometrial polyps should be removed before attempting natural pregnancy and before pregnancy via intrauterine insemination (IUI) or in vitro fertilization (IVF). Through a literature review, we obtained information on the impact of endometrial polyps and polypectomy on fertility outcomes. Several observational studies have suggested that women with unexplained infertility may benefit from endometrial polypectomy for a future natural pregnancy. A few studies reported benefits from endometrial polypectomy in infertile women who plan to undergo IUI. However, no strong evidence supports polypectomy as a way to improve the pregnancy rate in infertile women who plan to undergo IVF or polypectomy during controlled ovarian stimulation for IVF. Although no studies have defined criteria for the polyp size that should be removed in infertile women, clinicians should be aware that small endometrial polyps (<10 mm) sometimes regress spontaneously. Endometrial polypectomy is currently justified in patients with repeated IVF failure, but more studies are needed to verify that endometrial polypectomy itself will eventually increase the pregnancy rate. Although several mechanisms by which endometrial polyps exert a negative effect on fertility have emerged, there is no consensus about the proper management of endometrial polyps in infertile women. Therefore, the management of endometrial polyps should be individualized depending on the patient's situation and clinician’s preference.

9.Mortality Rate and Early Prognostic Factors In Patients With Severe.

BACKGROUND & OBJECTIVES: Severe hemispheric infarction(SHI) reportedly has various range of high mortality. Recently it has been reported that more aggressive therapeutic intervention, such as decompressive hemicraniectomy or mild hypothermia might has potential benefits in the management of SHI. However, the mortality and the prognosis of Sill under the conservative treatment were rarely studied yet in Korea. METHODS: Sixty-six patients with CT proven SHI were subjected among 1649 acute stroke patients registered at the Hallym Stroke Data Bank since Jan. 1993. We analyzed the computerized databases for the mortality and compared inclusively demographic features, clinical characteristics, etiology, therapy modalities and CT findings of fatal group with those of survivors. RESULTS: Twenty-five patients were expired among 66 SI-il patients(mean age: 64.9+/-11.5, male to female ratio;37:29). The mean time to expire was 160.4 hours. The fatal group had a significantly higher incidence of cardioembolism. uncontrolled BP during the acute stage, and extensive infarction territory(MCA+ACA). Risk factors for ischemic stroke such as hypertension, hyperlipidemia. smoking, previous stroke history, diabetes mellitus. and old age were not related to mortality low densities and hyperdense MCA signs on brain imaging were not different between two groups. CONCLUSIONS: The early mortality rate of Sill under the conservative care was 38% in a referral hospital of Korea. intractable hypertension in the acute stage, cardioembolism and extent of infarct area may have predictable values of early mortality of SHI.

10.Embryonic development after exposure of mouse oocyte to various amount of ovarian endometriotic fluid.

This study assesses the fertilization and blastocyst-forming rate in mice cumulus-oocyte complexes (COCs) after the exposure of human ovarian endometriotic fluid. Endometriotic fluid was obtained from a single patient by aspiration at the time of a laparoscopic cystectomy and serially diluted. COCs were obtained from 46-week-old female BDF1 mice. After exposure to ovarian endometriotic fluid for five minutes, the COCs were washed three times and the oocytes were then fertilized by mice sperm. The fertilization and blastocyst formation rate and the proportion of hatching/hatched blastocyst in the four treatment groups were not inferior to those in non-exposure group.
