1.Alterations of Blood Cholinesterase Activity and Urinary Para-nitrophenol Excretion After Exposure to Organophorus Insecticides.

In order to evaluate the health hazard due to organ orphosphorus insecticides, we measured the blood cholinesterase activity and urinary para-nitrophenol among 56 exposed subjects. They are orchard workers, rice plant workers and smithion factory workers. The clinical symptoms were also checked by physicians. We also measured the blood cholinesterase activity and urinary para-nitrophenol excretion of 20 urban people and 15 rural people who had never been exposed to organophosphorus insecticides in order to compare them according to age, sex and geographical differences. And these results were also compared with those of exposed groups. The results obtained were as follows. 1. The normal plasma cholinesterase activity and cell cholinesterase activity were 0.861+/-0.148 delta pH/hr and 0.822+/-0.154 delta pH/hr. And normal para-nitrophenol in urine was 1.21+/-0.52 mg/liter. 2. No significant difference was existed in blood cholinesterase activities and urinary para-nitrophenol excretion according to sex, age and geographical difference. 3. The plasma cholinesterase activity and cell cholinesterase activity of orchard workers, rice plant workers and smithion factory workers were 0.682+/-0.189 delta pH/hr, 0.775+/-0.160 delta pH/hr, 0.754+/-0.123 delta pH/hr and 0.691+/-0.082 delta pH/hr, 0.756+/-0.117delta pH/hr. And significant decreases in blood cholinesterase activities were existed among orchard workers and smithion factory workers compared with control group. 4. The urinary para-nitrophenol excretions of orchard workers, rice plant workers and smithion factory workers were 1.33+/-0.66 mg/liter, 1.19+/-0.88mg/liter and 1.37+/-0.67mg/liter and there were no significant difference between exposed groups and control group. 5. The clinical symptoms complained during and after organophosphorus insecticides exposure were frequently ranked by headache (67.7%) and vertigo (64.5%) and muscular ataxia and weakness (51.6%).

4.A Clinical Study of the Patellar Fractures

Fractures of the patella are predominantly intra-articular fractures of a sesamoid bone. The options of treatment vary from closed treatment to total patellectomy, in the middle of the spectrum are osteosynthesis and partial patellectomy. In this series, we experienced 59 cases of patellar fractures from October 1974 to October 1983 at the Department of the Orthopedic Surgery, College of Medicine, Kyungpook National University. The results were as follows:1. Most fractures were encountered in man from twenties to forties. 2. The most common cause of fractures was traffic accident(34 cases, 59.7%), followed by slip down or falling down(15 cases, 26.3% ). 3. The most frequent configuration of fractures was the comminuted. 4. 36 cases were treated with osteosynthesis (32 cases), partial excision (1 case) or total excision (3 cases). Of the 32 cases treated with osteosynthesis, most were treated by modified tension band wiring (16 cases). And as compared with other fixation techniques, the best results were obtained by this method. 5. Modified tension band wire fixation has given accepatable results even in severely comminuted, displaced patellar fractures. Excision was sometimes unavoidable.

5.A Study on Superoxide Radical Formation, Catalase and Superoxide Dismutase Activities in Experimental Cerebral Infarction.

To observe the role of oxygen free radical and enzymatic scavengers in cerebral ischemia, an infarction model was made using transorbital occlusion of the middle cerebral artery in cats. The changes of the superoxide radical production and the activities of superoxide dismutase and catalase were measured. The results were as follows ; 1) The infarction of the left middle cerebral artery(MCA) territory was identified with intracardiac perfusion of a TTC solution after transorbital occlusion. 2) The superoxide radical activities after occlusion of the left MCA were not changed in all groups except for the decrease in 6 hours group of the right side compared to the control group(p<0.05). 3) The Mn-superoxide dismutase activities of the left side in the 12 hours group were significantly higher than those of the right side(p<0.01) and those in the control group(p<0.05). 4) The Cu, Zn-superoxide dismutase activities of the left side in the 3 hours group after occlusion of the left MCA were significantly higher than those in the control group(p<0.05). 5) The catalase activities of the left side in the 3 hours, 6 hours and 12 hours groups after occlusion of the left MCA were significantly higher than those of the right side(p<0.05). 6) The catalase activities of the left side in the 12 hours group after occlusion of the left MCA were significantly higher than those in the control group(p<0.05). The authors suggest that the enzymatic scavangers such as Mn-SOD, Cu, Zn-SOD and catalase increased in the infarcted brain, which means an involvement of free radicals in cerebral infarction.

6.Prognostic Parameters in Moderate or Severe Diffuse Axonal Injury.

A retrospective study 40 patients with moderate or severe diffuse axonal injury which is defined as posttraumatic coma for over 24 hours without mass lesions or ischemic insults was conducted in order to identify prognostic parameters. The sign of hypothalamic damage and motor reactivities of 40 patients were reviewed and compared to the outcome. The results were as follows ; 1) The ratio of male to female was about 3 to 1 and the peak incidence was at the first decade. 2) Of abnormal brain stem signs, fever of central origin was observed at the early posttraumatic phase and correlated with nonrecovery of consciousness(P<0.05). 3) Of abnormal brain stem signs, abnormal ADH secretion was significantly correlated with nonrecovery of consciousness(P<0.005). 4) Abnormal motor reactivity to pain was significantly correlated with nonrecovery of consciousness(P<0.005). 5) In the group of recovery of consciousness, initial Glasgow coma scale in 40 patients with moderate or severe DAI was significantly correlated with their outcome(P<0.005).

9.Study on the lead exposure of workers in a litharge making industry.

In order to investigate the level of lead exposure of workers in litharge making industry and to evaluate how lead exposure, personal habit such as smoking and drinking affect the prevalence of lead related symptoms and other study variables, we investigate 114 workers(24 office workers and 90 lead exposed workers) in a litharge making industry. Study variables chosen were blood lead(PbB), zinc protoporphyrin in whole blood(ZPP), Hemoglobin(Hb), hematocrit (Hct), SGOT and SGPT. symptom questionnaires which had 15 lead exposure related symptoms were provided to all workers and filled up by themselves and reconfirmed by physician. The results obtained were as follows; 1. The mean value of PbB, ZPP and SGOT in lead exposed group were higher than those of non-exposed group, and there were no differences of means in other study variables. 2. The smoking and drinking rate of study subjects were 65.8% and 71.0% as a whole. Smoking rates were lower in non-exposed group than exposed group, but drinking rate were not. 3. There were no differences of mean values of study variables between smoker and non-smoker in non-exposed and exposed group, but there was a difference of mean value of SGOT between drinker and non-drinker in lead exposed group. 4. while the symptom prevalence of lead exposed group were higher in neuromuscular category than non-exposed group, those of non-exposed group were higher or same with exposed group in gastrointestinal and general symptom category. 5. The symptom prevalence of smoker were higher than non-smoker regardless of exposure. 6. The symptom prevalence of drinker were only higher in gastrointestinal symptom category than non-drinker. 7. In multiple stepwise regression analysis of lead related symptoms as dependent variable and blood lead, smoking habit, drinking habit and work duration as independent variables, drinking habit contributed to the gastrointestinal symptom category, whereas blood lead and smoking contributed to the neuromuscular symptom category. For the total symptoms work duration and smoking habit contributed significantly.

10.A study of the etiology of unilateral Class II, division 1 malocclusion.

The purpose of this study was to verify the class I molar relationship in skeletal class II and class II molar relationship in skeletal class I malocclusion with unilateral class II, division 1 malocclusion. The sample consisted of lateral cephalometric radiographs and upper and lower dental casts of 30 unilateral class II, division 1 malocclusion. The results of this study were as follows: 1. Skeletal class I malocclusion was 43%, and skeletal class II malocclusion was 57% in 30 cases of unilateral class II, division 1 malocclusion. 2. In the skeletal class II with unilateral class II, division 1 malocclusion, mandibular first molar on the class I side showed more mesial migration than the opposite side. 3. In the skeletal class I with unilateral class II, division 1 malocclusion, maxillary first molar on the class II side showed more mesial migration than the opposite side. 4. Midline deviation of upper or lower dental arch was 90% in 30 cases of unilateral class II, division 1 malocclusion.
