2.Cord Blood Leptin of Newborns with Normal Intrauterine Growth in 30-42 Weeks of Gestation.

PURPOSE: To evaluate leptin values in placental cord blood of newborns with normal intruterine growth after 30 weeks of gestation and to study the relationship of leptin with gestational age, birth size, ponderal index, BMI, placental weight, gender difference, Insulin-like Growth Factor-I(IGF-I) and Insulin-like Growth Factor binding protein-3(IGFBP-3). METHODS: Ninety healthy newborns(49 males and 41 females) with normal intrauterine growth were studied. They were classified into 4 groups according to the gestational age(GA):group I(30-33 weeks GA, n=17), group II(34-36 weeks GA, n= 24), group III(37-39 weeks GA, n=32) and group IV(40-42 weeks GA, n=17). At the time of delivery, newborn infant' weight, length, head circumference and placental weight were measured and BMI, ponderal index were calculated. A venous cord blood sample was collected at the time of delivery and serum leptin(RIA), IGF-I (IRMA) and IGFBP-3(IRMA) were measured. RESULTS: Cord blood leptin correlated positively with gestational age(r=0.730, P>0.01), body weight(r=0.686, P<0.01), length(r=0.419, P<0.01), head circumference(r= 0.498, P<0.01), BMI(r=0.750, P<0.01), ponderal index(r=0.704, P<0.01), placental weight(r=0.536, P<0.01), IGF-I(r=307, P<0.01) and IGFBP-3(r=0.736, P<0.01). Multiple linear regression analysis showed BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased progressively throughout gestation. Mean leptin values of group II(3.90+/-.28ng/mL) were significantly higher (P<0.01) than those of group I(1.92+/-.09ng/mL) and also values of group III(5.37+/-.52ng/mL) were higher than those of group II(P<0.05). But there was no significant difference between group III and group IV(6.45+/-.36ng/mL). Mean leptin values in females(5.07+/-.54ng/mL, n=41) were significantly higher than in males(4.07+/-.76 ng/mL, n=49) but there was no inter-gender difference in body weight, BMI, ponderal index, IGF-I, IGFBP-3. In addition, mean leptin values in females(7.45+/-.07 ng/mL) were significantly higher than in males(5.02+/-.09ng/mL) after 40 weeks of gestation. CONCLUSION: BMI, IGFBP-3 and sex(female) were independent predictors of leptin values. Leptin values increased significantly after 34 weeks of gestation and females had higher leptin values than males, especially in fullterm newborns. Leptin values in newborns had higher correlations with IGFBP-3 than with IGF-I as gestational age increased.

3.Ossification of Spinal Ligaments.

This authors reviewed the radiographs of 20,057 patients with spinl radiculomyelopathy from April 1983 August 1990. The incidence and distribtion of the ossification of the spinal ligaments were analized as following. 1) The overall incidence of spinal ligaments ossification was 3.6% among 20,057 patients. 2) The most frequently occurs at lumbar area(67.1%), and then cervical(27.5%) thoracic(0.4%) area as in orders. 3) The OPLL was most common type(2.19%) of these spinal ligaments ossification, and then OALL(0.7%), OLF(0.24%) as in orders. 4) OPLL (1) OPLL was most frequent at lumbar area(58.6%) and then cervical(37.0%), thoracic(0.4%) area as in orders. (2) The cervical OPLL was commonly found at C5 & C6 area and morphologically, the multiple segmented type was most common then continuous type, mixed type and single segmented type in orders. (3) The multiple segmented type was more frequently found at upper cervical area, but the continuous type was more frequent at lower cervical area. 5) OALL (1) OALL was most frequent at lumbar area(86.0%) but thoracic(1.4%) and cervecal area(5.3%) were relatively low in frequency. (2) Of the lumbar OALL, the L4/5 area was most frequent site(37.8%) but the L3/4(25.3%) and L5/S1 area(24.5%) also had relatively high occurance rate. 6) OLF (1) OLF was most frequent at lumbar area(92.7%), but the incidence of OLF was relativerly rare at cervical(2.1%) and thoracic area(5.2%). (2) Of the lumbar OLF, the L4/5 area was most common site(33.1), but L5/S1 area had also high occurance rate(27.2%).

4.Clinical Significance of DNA Ploidy in Epithelial Ovarian Malignancy.

Prognosis and biologic behavior of malignant ovarian tumors have been assessed by clinical staging, morphological grading and many other variables. Recently DNA ploidy measured by flow cytometry has been suggested as an additional important indicator of the tumor behavior and prognosis. The author measured DNA ploidy, S-phase fraction and DNA Index in 36 patients of epithelial ovarian tumors(17 were malignant, and 19 were borderline) by flow cytometric analysis of paraffin embedded tumor blocks. Comparing with FIGO stage, tumor grade, histologic type and others, the author investigated the clinical significance of the results of flow cytometric analysis. 7.A Clinical Study of the Patellar Fractures

Fractures of the patella are predominantly intra-articular fractures of a sesamoid bone. The options of treatment vary from closed treatment to total patellectomy, in the middle of the spectrum are osteosynthesis and partial patellectomy. In this series, we experienced 59 cases of patellar fractures from October 1974 to October 1983 at the Department of the Orthopedic Surgery, College of Medicine, Kyungpook National University. The results were as follows:1. Most fractures were encountered in man from twenties to forties. 2. The most common cause of fractures was traffic accident(34 cases, 59.7%), followed by slip down or falling down(15 cases, 26.3% ). 3. The most frequent configuration of fractures was the comminuted. 4. 36 cases were treated with osteosynthesis (32 cases), partial excision (1 case) or total excision (3 cases). Of the 32 cases treated with osteosynthesis, most were treated by modified tension band wiring (16 cases). And as compared with other fixation techniques, the best results were obtained by this method. 5. Modified tension band wire fixation has given accepatable results even in severely comminuted, displaced patellar fractures. Excision was sometimes unavoidable.

8.A Study of Motor Conduction Velocity of Radial Nerve: Comparision of Proximal and Distal Segments

The usage of electromyography is customized in the diagnosis of the neuromuscular disorder and the determination of motor conduction velocity is important in the diagnosis of the peripheral nerve lesion and in the posing of the site and severity of nerve damage. Although the radial nerve is frequently involved in generalized neuropathy and in entrapment syndrome, relatively fewer reports have appeared in the literature regarding the radial nerve. The purpose of this study is to determine the normal data of the motor conduction velocity of the proximal and distal segments of the radial nerve. The radial nerve fibers supplying the extensor indicis muscls muscle was stimulated at Erb's point, above the elbow and in the distal forearm and its muscle action potential was sampled. Seventy-four radial nerve were studied in thirty-seven healthy young subjects. The results obtained were as follows; 1. The mean proximal velocity was 70.7±6.8m/sec and the mean distal velocity was 57.6±4.3m/sec. 2. In any case tested, the proximal velcity was over 55m/sec. 3. The proximal velocity was faster than the distal velocity and the mean difference was 13.1m/sec In only 7 of the 74 nerves tested, the proximal velocity was slower than the distal velocity and the difference of Sm/sec was the most reversal. 4. The velocity in dominant limb was faster than that in nondominant limb. 5. There were no significant difference between the sexes.

10.Two Cases of Lvmphomatoid Papulosis.

Lymphomatoid papulosis(LyP) is a chronic recurrent dermatosi characterized by involuting and recurring papules, plaques, and nodules showing histologic feaurs suggesting rnalignant lymphoma. On histologic ground, it is divided into two types. type A and type B. In type A, large atypical lymphocytes are the main cellular cornponent and most of these express CD 30(Ki-1) antigen, which has been p!reviously thought to be specific for Read Sternberg cells of Hodgkins disease. In type B, cerebriform mononuclear lymphocytes sirr la to those in mycosis fungoides predominates. We report two cases of lymphomatoid papulosis, type A and type B, respectively. The large atypical cells in type A LyP expressed Ki 1 antigen. Both case were treated with PUVA and the outcome of the treatment has been fair.
