6.Arthrolysis and Quadriceps Release in Stiff Knee: Report of Five Cases

Many procedures of quadricepsplasty were reported by Thompson (1944), Judet (1959), van Nes (1962), Nicoll (1963) and Hesketh (1963) in confining the indication of them for the strictly extra-articular origin of their causes. During the period of January 1979 to August 1979, 5 cases of stiff knee joints were surgically treated by means of arthrolysis and quadriceps release. The result were gratifying. The average gain of flexion was 119 degrees, and extension lag was absent except a case which had compression fracture of femoral condyle.

7.The Prevention of Posterior Displacement of Tibia During Treatment of Posterior Cruciate Ligament Injury Using Modified Quengel Hinge

In the treatment of posterior cruciate ligament injury, posterior displacerrient of tibia might be occured because of the direction of gravity due to weight of lower leg. To prevent this problem several methods have been introduced, but not settled yet. Therfore the author tried to solve this problem by applying Quengel brace that was firstly described by Mommsen in 1922 and perfected by Jordan using the correction of flexion contracture of the knee in hemophiliacs. But owing to the development of some problems in original hinge for prevention of posterior displacement of tibia after posterior cruciate ligament injury, we modified the design of the hinge that more effective anterior traction force should be operated on tibia. The authors applied Quengel brace including original and modified hinge to 21 cases of posterior cruciate ligament injury between June 1983 and May 1986. The followings were obtained. 1. By modification of the hinge, more effective anterior traction force operated on tibia without pressure sore or joint narrowing. 2. On clinical application of modified Quengel brace, posterior displacement of tibia were prevented effectively.

9.Clinical Study of Hypertrophic Patella after Treatment of Patellar Fracture

Authors treated 86 cases of patella fracture during 3 year period from June 1989 to March 1992 at the Dept. of Orthopaedic surgery, Soonchunhyang University. Forty cases of patella fracture showed hypertrophy of longitudinal and transverse diameter of patella during follow-up which were checked by simple radiographs. Completely documented cases were only forty-six, which were followed more than a year and preop. and postop. and final follow-up radiographs were reviewed. Authors measured longitudinal and transverse diameter of patella using preop. and postop and final simple radiographs to observe the morphological changes of patella after treatemnt. The purpose of this paper is to observe. 1. How much the patella was hypertrophied. 2. What kind of factors influence the hypertrophy of the patella, 3. What is the clinical correlation between the hypertrophy of patella and the clinical outcome. The results were as follows 1. Average hypertrophy of longitudinal and transverse diameter were 1.0±2.3mm and 2.5±1.2mm respectively. 2. Only fracture type(simple. vs comminuted.) was significantly related with the changes of longitudinal and transverse diameter of patella(p < 0.1), Other factors(cast immobilization period, operation method and age) show no significant relationship statistically(p>0. 5). And there is no relationship between longitudinal and transverse diameter statistically(p>0.4). 3. Those who showed change of longitudinal diameter between 0-10mm had good knee joint motion(99°±2). But those who showed shortening or lengthening over 10mm of longitudinal diameter had poor knee joint motion(37°±3, 60°±8 respectively). And change of transverse diameter was not related with the clinical result (p>0. 7).
