The study was undertaken to evaluate the radiological and microscopic findings of radiation pneumonitis. All 50 rats, which were used for the experiment, received a single dose of 2,000 rads of radiation on right lung using 60 Co teletherapy. Chest teleroentgenograms were checked weekly for 13 weeks and microscopic examination of irradiated lung were performed monthly after irradiation. The resuls obtained are summerized as follows. 1. Inroentgenologic examination, radiation pneumonitis began to appear 4 weeks after irradiation and progressed withtime intervals and began to resolve after 11 weeks. 2. Chest roentgenologic findings were pneumonic consolidation,atelectasis, and pleural effusion of irradiated right lung with compensatory hyper-inflation of left lung. 3. Inthe microscopic examination, pneumonitis appeared 2 weeks after irradiation and progressed with time intervals andbegan to resolve after 8 weeks. 4. The microscopic findings were fibrin deposit in alveoli and increased septalcellularity, vascular thickening, edema and congestion. After septal fibrosis and hyperplasia of bronchialmucosa, suggseting early regenerative change of radiation fibrosis, were noted 12 weeks after irradiation.

Since Whitehead had described a circular hemorrhoidectomy in 1882, many surgeons adopted it for decades for patients with protruding anal deformity. After a few decades of performing Whitehead operation, devastating complications such as anal stricture, fecal incontinence, and wet anus with mucosal eversion had been reported on the literatures and then it was buried as an abandoned procedure by surgeons for a long period. Recently, a few prominent anal surgeons reported that they could avoid such devastating complications by introducing diverse modifications of the original Whitehead's operation. The authors analyzed 22 patients who had undergone original Whitehead circular hemorrhoidectomy with the technique of preserving most of the perianal skin and W-shaped circular incision during the period from 1991 to 1996, with special regard to the com plications such as anal stricture and anal mucosal eversion which have been debated on so far and reviewed the articles about these issues. In immediate postoperative period, suture failure and resultant non-surgery requiring, mild anal stricture were documented in 3 of the 22 cases(13.6%). On long-term follow-up with the mean period of 44 months (18~79 months) in 14 cases, except those 8 cases that were lost, with phone-call questionaire, 13 patients(93%) had quite normal anal functions. The authors would like to suggest that the original Whitehead's circular hemorrhoidec tomy is a valuable surgical technique to manage the protruding anal deformity if surgeons can avoid well known complications such as anal stricture and anal mucosal eversion by choosing a correct location of initial W-shaped incision to preserve as much perianal skin as possible.

To evaluate the radioprotective effects of 5-thio-D-glucose(5-TDG) on normal skin, an experimental study was carried out with total 140 mice, of which 30 mice were given 5-TDG followed by irradiation and 60 mice were given intraperitoneally (IP) two hours before irradiation, and the radiation doses were singly 2500, 3500 or 4500 radsrespectively. The skin changes of the irradiated right hind limb were inspected for 31 days and the skin scores were analized. The results are as follow; 1. Marked radioprotective effect was presented in the group of 5-TDG,1.5g/kg body weight, with 2500 rads irradiation. 2. Definite radioprotective effect was also revealed in the groupof 5-TDG, 1.5kg/ body weight, with 3500 rads irradiation, In the group of double dose 5-TDG, 3.0g/kg body weight, the radioprotective effect appeared to be enhanced. 3. In the group of 4500 rads irradiation with 5-TDG, 1.5g/kgbody weight, no radioprotective effect was noticed. 4. In addition to the radiosensitization effect on the hypoxiccells, the radioprotective effect of 5-TDG on the normal animal tissue suggests increased possiblility of itsclinical application.

Billiary cystadenoma and cystadenocarcinoma are rare neoplasms of the biliary ductal system. The authorspresent 2 cases of biliary cystadenoma and 3 cases of cystadenocarcinoma. The typical sonographic apearance ofthese tumors is a large intrahepatic gloular or ovoid thickwalled cystic mass which often containes multipleseptations with papillary growth and solid portion. Low-level internal echoes may be seen within the cystic mass.The characteristic CT findings of these tumors are multiloculated cystic mass, thick sepatations with papillaryprojection sand solid portion within the cystic mass. Angiographical characteristics of these tumors arehypovascular mass with frequent abnormal clusters of tumor vessels within the walls on arterial phase andaccumulation of contrast material along the wall or internal septation on delayed film.

Nine cases of gastrointestinal leiomyosarcoma were studied by ultrasound. Abdominal sonogram disclosed a largelobulated iso or hypoechoic mass containing a few or multiple central sonolucent cavitations in seven cases. Twocases arising from mesentery and abdominal wall were hypoechoic mass without central sonolucent area. Thesecentral sonolucent areas were condirmed to be necrotic or hemorrhagic cavities in the solid mass pathologically.Although not pathognomonic, a large lobulated exophytic gastrointestinal mass with central sonolucent areastrongly suggests the diagnosis of leiomyosarcoma.

Congenital vertical talus associating tarsal coalition, which is a very anomalous condition and causes severe rigid flat foot, is presented with literary reviews. This case was treated with soft tissue release and triple arthrodesis.

The treatment of open tibia fracture is challenging to Orthopedic Surgeon with its high infection rate and other complications. We had wound culture of open tibia fracture at emergency room in all cases, and treated using various External Fixators. The author analized the 134 cases of the open tibia fractures from March 1979 to February 1987. The average follow-up was one year. The results were as follws : 1. Type I open fracture was 23 cases, Type II 58 cases, Type IIIA 33, Type IIIB and Type IIIC was 10 cases each. The most frequent type of fracture was Type II, 58 cases by Gustilo classification. 2. The union time of Type I fracture was 6.5 months in average, and that of Type II and Type III was 9.5 months, 11.2 months. 3. Of those 52 cases whose initial culture was positive, 12 cases developed infection. The low infection rate would probably be due to combined administration of cephalosporin and firm application of external fixator to severe open tibia fractures.

Positron emission tomography (PET) with 2-[F-18]-Fluoro-2-deoxy-D-glucose (FDG) was performed in ninteen patients who had benign musculoskeletal tumors in order to determine if there was a relationship between histologic grade of tumor and FDG uptake of tumor. These patients had been evaluated previously with computed tomography (CT) or magnetic resonance (MR) imaging or both. The diagnoses were confirmed with incisional or excisional biopsy or by radiographic follow-up. Generally high-grade tumors had significantly greater uptake of FDG than low-grade lesions . Benign lesions such as giant cell tumor, fibrous dysplasia, and osteofibrous dysplasia showed significant elevation of SUV (Standardized Uptake Value) above 4.0. On the contrary. The current studies suggest the utility of FDG-PET imaging as an adjunct to CT or MR imaging in the evaluation of benign tumors. And results of PET suggest benign tumors with high SUV are histologically active lesion and tend to be locally aggressive.

To differentiate amebic and pyogenic liver abscesses, sonography(US) and CT findings of 21 amebic and 22 pyogenic liver abscesses were reviewed retrospectively. US was performed in all cases and CT in 15 cases of each group. Eleven cases of amebic abscesses had well defined margin which was observed in 10 cases of pyogenic abscessed. Amebic abscesses showed hyperechoic wall in 7 cases and calcification in 4 cases. Ten cases of pyogenic abscesses had mearby intrahepatic duct dilatation which was observed in 2 cases of amebic abscesses. Pneumobilia was seen in 6 cases of pyogenic abscesses. US findings of clear margin and hyperechoic wall, and CT findings of intrahepatic duct dilatation, pneumobilia, and calcification were helpful in differentation of amebic pyogenic liver abscesses.
