1.Metabolic Activities of Benign Musculoskeletal Tumors Using 2- F - 18 - Fluoro -2 - deoxy - D - glucose ( FDG ) Positron Emission Tomogrphy ( PET ) ( preliminary report ).

Positron emission tomography (PET) with 2-[F-18]-Fluoro-2-deoxy-D-glucose (FDG) was performed in ninteen patients who had benign musculoskeletal tumors in order to determine if there was a relationship between histologic grade of tumor and FDG uptake of tumor. These patients had been evaluated previously with computed tomography (CT) or magnetic resonance (MR) imaging or both. The diagnoses were confirmed with incisional or excisional biopsy or by radiographic follow-up. Generally high-grade tumors had significantly greater uptake of FDG than low-grade lesions . Benign lesions such as giant cell tumor, fibrous dysplasia, and osteofibrous dysplasia showed significant elevation of SUV (Standardized Uptake Value) above 4.0. On the contrary. The current studies suggest the utility of FDG-PET imaging as an adjunct to CT or MR imaging in the evaluation of benign tumors. And results of PET suggest benign tumors with high SUV are histologically active lesion and tend to be locally aggressive.

2.A case of osteoma cutis.

Osteoma cutis is a primary cutaneous ossification, which has no preceding trauma or skin disease and no evidence of Albrights hereditary osteodystrophy n the patient or his family. The lesion appears as hard, round to irregular, sharply defined tumor of varying size within the skin or subcutis, and color ranges from flesh-colored to purple or brown. We report herein a case of osteoma cutis in a 32-year-old female, who had a 1 x 1cm sized, asymptomatic, round, flesh colored, hard nodule on the right side of her forehead for 5 years. Histopathologic examination showed mature bone with many ostocytes, osteoblasts, cement lines and Haversian canals in the dermis.

3.A Brief Report on the Myocardial Damage Evaluated by Creatine Kinase MB Isoenzyme in Valvular Heart Disease.

During a 3 month period in 1984, 12 patients undergoing scheduled valvular replacement were Studied by CK and CK-MB isoenzyme to evaluate the myocardial damage during open heart operation. Total creatine kinase value was 58.3+/-25.6 u/L at control, 59.8+/-23.5 u/L at prebypass period, 85.5+/-49.6 U/L at during bypass period, 20.2+/-87.6 at postbypass period respectively. It began to rise in prebypass period and showed higtest level in postbypass period. CK-MB value was low in most cases below 20 U/L but in 2 patients postbypass period showed high level which above 40U/L. Initial isoenzyme activity was detected in 2 patients prior to anesthesia, in 2patients prior to bypass, in 5 patients during bypass, and in all others after termination of bypass. Considering above data we concluded that anesthetic management during prebypass period and myocardial preservation during bypass period required more proper and aggressive management despite of good operation results.

5.Thiopental-induced Hemolysis - A case report-.

We experidced recently a case which showed intravascular homolysis and acute renal insufficiency during open heart surgery. After various studies including immunohematologic study, urinalysis, CBC, transfusion reaction study, coombs test, we detected antibody against thiopental in the patient's serum. The end-point titer was 1:4. We concluded that it was thiopental-induced immune hemolysis so report this case along with references.

7.Pyogenic Arthritis in Renal Transplant Recipients.

Acute pyogenic arthritis in renal transplant patient is one of the serious problem resulting in significant morbidity and mortality. The purpose of this paper is to analyze seven patients in whom acute pyogenic arthritis developed following renal transplantation. Pyogenic arthritis developed in seven (0.7%) out of 958 renal transplant patients whose transplanted kidney functioned well for more than two years. The age of the patients ranged from 37 to 65 years with a mean of 48 years. All patients were male. Pyogenic arthritis developed between the period of one and 12 years (mean: 4.5+/-4.2 years) following transplantation. Causative organisms were Staphylococcus aureus (three), E. coli (two), Salmonella (one) and Acinetobacter bananii (one). Involved joints were four knees, one hip, one elbow and one finger. Two patients improved on a conservative regimen of bed rest and antibiotics and five patients required open drainage. However, pyogenic arthritis recurred in three years, four years and seven years after renal transplant in one patient. Looking at the probable risk factors of age, sex, immunosuppresants, diabetics and acute graft rejection, diabetics and older age (over 40 years old) are highly associated with post renal transplant pyogenic arthritis.

8.A Case of Pyogenic Granuloma on the Fissured Tongue.

Pyogenic granuloma is a benign vascular tumor related to trauma, infection, or hormonal changes. When it affects the oral cavity, the most frequent site is the gingiva and involvement of the tongue is very rare. Occurrence of pyogenic granuloma on the fissured tongue has not been reported yet. We present a rare case of pyogenic granuloma on the tongue in a 64 year old female patient in which the fissured tongue seemed to play important roles in the pathogenesis of occurrence of the pyogenic granuloma.

9.Thrombolysoangioplasty in chronic, long segmental occlusive arterial disease.

From July 1988 to September 1992, we experienced 15 cases of thrombolysoangioplasty (TLA) at Chonnam University Hospital. Among 15 cases, 5 had claudication (Clinical stage II according to Modified Fontaine Classification), pain at rest (III), and gangrene(IV). Duration of symptoms was 1-7 months except patients of clinical stage IV unable to guess occlusion age. The occlusion length was 5-10cm in 8 cases, 10-20cm in 4 cases, and above 30cm in 2 cases. In all cases, thromolytic therapy was performed with intraarterial urokinase infusion. The total amount of urokinase ranged from 300,000 IU to 2,000,000 IU and infusion time ranged from 2 to 50 hours except three cases infused bolus dose only. Complete successful TLA was defined as technical (less than 50% of residual stenosis) and clinical success. Partial success was defined as technical success but clinical failure. Follow-up angiography could be performed in 8 cases. Overall initial success rate was 86.6% (13/15). Among them. Complete success was in 11 cases and partial success was in 2 cases. Recurrence of disease was not noted on all cases(n=8). Severe complications, such as hemorrhage, did not occurred. TLA was considered to be effective and safe way to recanalized chronic long artery occlusion in lower extremity.

10.Detection of A-V shunting with CO2-DSA in hepatocellular carcinoma.

The presence of A-V shunting in hepatocellular carcinoma is an important factor for deciding the prognosis and in the management with transarterial chemoembolization. Twenty-four patients with hepatocellular carcinoma performed with CO2-DSA and iodinated-DSA were reviewed for the evaluation of visibility of A-V shunting. It was classified by the visibility into clearly visible, faintly visible and invisible. Also the authors evaluated neovascularity and tumor staining. And we checked side effects after the injection of CO2 gas during CO2-DSA. A-V shunting was noted in 19 cases(63%), which were clearly visible in 15 of 19 cases(79%) on CO2-DSA and in 7 of 19 cases(37%) on iodinated-DSA. In 3 cases, A-V shunting was noted only on CO2-DSA. CO2-DSA(17%)was inferior to iodinated-DSA in detection of neovasculaity and tumor staining. Side effects from CO2-DSA were abdominal pain(1 case) and chest discomfort(1 case) but improved within several minutes. In conclusion, CO2-DSA is a sensitive and effective method for the detection of A-V shunting in hepatoma.
