The Kikuchi's disease is a distinctive type of histiocytic necrotizing lymphadenitis that affects primarily the cervical lymphadenites of young adults and has a self-limited clinical course. Authors report the case of a 77-year old patient who presented with axillary and cervical, tender lymphadenopathy and who was diagnosed as having Kikuchi's disease upon excisional biopsy.

4.Expression of Glial Fibrillary Acidic Protein in Human Retinal Macroglia by Immunogold Labeling.

There are two types of retinal macroglia: astrocyte and Muller cell. The major component of iintermediate filament in astrocyte is GFAP, recognized as the primary marker of astrocyte. Muller cells do not physiologically express GFAP in normal developing retina. A striking increase in GFAP expression. however, is observed in Muller cells under certain pathological condition. It is also suggested that this kind of changes of Muller cells in human retina is related with aging, and may be an early feature of pathogenesis of Age related macular degeneration. The goal of this study is to investigate the involvement of the human retinal macroglia in normal aging by immunogold labeling for electron. The sections of aged neurosensory retina showed the evidence of expression of GFAP by Muller cells. The results suggest, therefore, that Muller cells are induced to express GFAP during aging.

5.Clinical Features and Histopathological Findings of Traumatic Cyclodialysis Clefts.

PURPOSE: Cyclodialysis clefts occur when the meridional ciliary muscle fibers become separated from the scleral spur, thereby providing a new drainage pathway of aqueous humor into the suprachoroidal space. Although the mechanism by which cyclodialysis lowers IOP is both of the hyposecretion of aqueous humor and the increased uveoscleral outflow, cyclodialysis clefts do not always cause hypotony. METHODS: The authors retrospectively analyzed the data of 9 eyes of 9 patients who had been diagnosed as traumatic cyclodialysis cleft by gonioscopic examination. RESULTS: Only 4 of the 9 eyes showed hypotony. This hypotony occurred 3 to 13 days (mean 7.3 days) after trauma. Conservative treatment combined with air injection was done in 4 patients with hypotony. In 3 eyes, IOP was normalized 7~10 days after the above treatment. The remaining one eye had an extensive cyclodialysis of 6 o clock and was treated with argon laser. But normalization of IOP was not achieved within the follow up period. Only cyclodialysis clefts of relatively small range were closed spontaneously or with only conservative management. Also posterior synechiae was found in one of the four eyes with hypotony, and was found in three of the four eyes without hypotony. CONCLUSION: Whether hypotony in cyclodialysis patients occur or not depends on the individual ability of scar formation in the cyclodialysis cleft against the anti-proliferative properties of aqueous humor.

6.A Case of Stent Embolizations into the Left Renal and Right Common Iliac Artery during Primary PTCA.

Coronary stent embolization is a rare event but may result in clinically relevant cardiac ischemia or peripheral embolization during the procedure. We report a case of systemic embolizations of two coil stents during the primary PTCA in acute myocardial infarction, who were treated successfully with a double wire helix technique and a gooseneck snare. Although in our experience this rare complication did not produce any clinical complications, care should be taken to prevent this possibility, especially in patients with significant vessel tortuosity, calcification, total occlusion, or mild stenosis proximal to the target lesion.

7.Expression of Neurofilament in Human Retinal Horizontal Cell by Immunogold Labeling.

Retinal horizontal cell is second-order neuron that integrates the information from photoreceptors over large retinal areas, mediating the lateral spread of visual signals to distant retina. The Neurofilament proteins, considered as neuronal markers, have been imunolocalized to mammalian retinal horizontal cess. However, the immunolabeling of Neurofilament in human, has been focused on the studies of visual pathway and large ganglion cells The goal of this study is to see whether human retinal horizontal cells are indeed neuronal nature or glial nature by immunogold labeling for electron microscopy. The sections of 65 year-old human retina showed the expression of Neurofilament by horizontal cells, which confirms the evidence of human retinal horizontal cell as neuronal nature.

8.The Effects of Arthroscopic Lavage Combined with Percutaneous multiple Drilling in Patients with Degenerative arthritis of knee.

42 knees with degenerative arthritis underwent arthroscopic lavage and 7 knees underwent arthroscopic lavage combined with multiple bone drilling in between 1994 and 1995 and followed average 12 months. Clinical evaluation using the hospita1 for specia] surgery knee scores (HSS scores) was done at last follow up and classified into 4 groups: excellent (90-99), good (80-89), fair (70-79) and poor (less than 70). The results were as follows: l. Of the 42 knees of the lavage group, 6 knees (16%) was excellent, 14 knees (33%) good, ]7 knees (40%) fair and 5 (11%) knees poor. Success group (Excellent & Good) were achived in 20 knees (49%). 2. Of 17 knees treated with arthroscopic lavage comhined with multiple bone drillig resulted in excellent in 9 knees (53%), good in 4 knees (23%), fair in 2 knees (12%) and poor in 2 knees (12%). Success group were achived in 13 knees (76%). 1n conclusion, we helieve that multiple bone drilling in conjunction with arthroscopic lavage has more favorable effect on the pain relief for degenerative osteoarthritis of the knee than arthroscopic lavage only.
