1.Ultrasound-Guided Intervention in Cervical Spine.

Interventional procedures around the cervical spine have been classically performed under the guidance of fluoroscopy with radiation hazards to patients and doctors. Even though under fluoroscopic guidance, vascular and nerve structures cannot be shown and there are actual risks for the patient. Nowadays, we can use high resolution image ultrasound around cervical spine procedures. Real time imaging is possible. Cervical root block, medial branch block and many other interventions can be performed under ultrasound guidance. In out-patient clinics, ultrasound is very helpful in management of cervical problems in differentiating the origin of pain and treatment for the pain. Ultrasound is radiation free, easy to use and the imaging can be performed continuously while the injectant is visualized in real-time, increasing the precision of injection. Importantly, ultrasound enables visualization of major nerves and vessels and thus leads to improved safety of cervical interventions by decreasing the incidence of injury or injection into nearby vasculature. We therefore performed a review to investigate the feasibility of performing cervical interventions under real-time ultrasound guidance.

2.A Clinical Analysis of the Breast Masses by Biopsy.

BACKGROUND : A discussion of the chronologic trends of operative indications of breast masses for proper management of breast masses is desirable. METHODS : A retrospective analysis of 882 patients with breast masses who underwent operations at Kangnam General Hospital from January 1986 to December 1997 was done. All the lesions were classified according to for age, size of the mass, and the pathologic result. Concurrently, the distributions of the different masses were retrospectively analyzed for each one-year time period. RESULTS : The most prevalent age group was the third decade (28.6%) followed by the fourth decade (25.3%) and the fifth decade (24.4%). The most common lesion was fibroadenoma (38.7%), followed by mammary dysplasia (22.6%) and malignancy (18.3%). In fibroadenoma, the most common pathologic type was intracanalicular (34.9%), and the juvenile type occurred in significantly younger patients compared to other types (19.2 vs 29.2 years of age). In mammary dysplasia, the most common pathologic type was fibrous (74.4%), and the epithelial hyperplasia and adenosis types were occurred in older patients compared to the other types (39.1 and 41.0 vs 33.5 and 35.4 years of age). As the chronologically changing pattern of the pathologic types, the number of mammary dysplasia had a decreasing tendency and fibroadenoma had a relatively increasing one, and the reason may be the effect of fine-needle aspiration cytology. CONCLUSIONS : The result of this study shows that the distribution of the disease in terms of age and frequency is almost the same as that of other reported results. The trend of the disease types in terms of the time period shows that the operation rate for fibroadenomas is increasing while the rate for mammary dysplasia is decreasing. This tendency might come from the decreasing use of surgical biopsy for mammary dysplasia.

3.Developmental Pattern and Treatment in Colon Diverticular Disease.

PURPOSE: Colon diverticula has continuously increased in Korea. This study is to identify the changes in the developmental patterns of colon diverticula and in the methods of treatment. RESULTS: A total of 66 patients who were diagnosed colon diverticular disease and hospitalized at Red Cross Hospital from April 1994 to March 2005 were enrolled. This retrospective assessment was performed by analyzing the hospital records of clinical characteristics such as demographics (age, sex), physical findings, distribution of diverticula, diagnosis methods, applicable treatments depending on severity and the stage of the disease, and complications and recurrences with whether the patient underwent a emergency or an elective operation. RESULTS: The median age was 51.8 years, with 37 males and 29 females. Of the 66 cases, right-side colon diverticula was found in 47 cases, and left side in 15 cases. Transverse and descending colon diverticula occurred concurrently in 2 cases, with 2 entire colon cases. The median number of diverticula per patient was 4.6. Of the 29 operation cases, a right colectomy was done in 10 along with 6 appendectomy and diverticulectomy cases. Postoperative complications occurred in 7 patients, of these 6 occurred in patients who had undergone an emergency operation, and 1 in a patients who had undergone an elective operation; 6 in right colon diverticular disease, and 1 in left color diverticular disease. CONCLUSIONS: Colon diverticular disease has increased with noticeable increases in left side and both-sides diseases. A one-stage operation is mainly performed for surgical treatment while fewer undergo a two-stage operation. Post-operative complications occurred in 7 cases. Exact preoperative diagnosis and an elective operation that allows bowel preparation are regarded as more crucial factors than the extension of the operation in decreasing the developmental rate and the severity of postoperative complications.

5.Role of Redistribution and 24 Hour Reinjection Images to assess Myocardial Viability in Patients with Acute Myocardial Infarction.

PURPOSE: We evaluated the importance of redistribution and 24 hour reinjection images in Tl-201 SPECT assessment of myocardial viability after acute myocardial infarction (AMI). MATERIALS AND METHODS: We performed dipyridamole stress-4 hour redistribution-24 hour reinjection Tl-201 SPECT in 43 patients with recent AMI (4-16 days). The myocardium was divided into 16 segments and perfusion grade was measured visually with 4 point score from 0 to 3 (absent uptake to normal uptake). A perfusion defect with stress score 2 was considered moderate. A defect was considered severe if the stress score was 0 or l (absent uptake or severe perfusion decrease). Moderate defect on stress image were considered viable and segments with severe defect were considered viable if they showed improvement of 1 score or more on redistribution or reinjection images. We compared the results of viability assess-ment in stress-redistribution and stress-reinjection images. RESULTS: On visual analysis, 344 of 688 segments (50%) had abnormal perfusion. Fifty two (15%) had moderate perfusion defects and 292 (85%) had severe perfusion defects on stress image. Of 292 severe stress defects, 53 were irreversible on redistribution and reversible on reinjection images, and 15 were reversible on redistribution and irreversible on reinjection images. Two hundred twenty four of 292 segments (76.7%) showed concordant results on stress-redistribution and stress-reinjection images. Therefore 24 hour reinjection image changed viability status from necrotic to viable in 53 segments of 292 severe stress defect (18%). However, myocardial viability was underestimated in only 5% (15/292) of severe defects by 24 hour reinjection. CONCLUSION: The 24 hour reinjection imaging is useful in the assessment of rnyocardial viability. It is more sensitive than 4 hour redistribution imaging. However, both redistribution and reinjection images are needed since they complement each other.

6.The Protective Effects of Garlic against Carbon tetrachloride-induced Hepatotoxicity.

OBJECTIVES: The purpose of this study was to find the protective effects of garlic on the halogenated hydrocarbon induced hepatotoxicities, and the possible protection mechanisms involved. METHODS: Male Sprague-Dawley rats received garlic (0.5 %) or regular diet, for 4 weeks. This was followed by a single dose of corn oil (the controls), carbon tetrachloride (400mg/kg body weight) and trichloroethylene (2,000mg/kg body weight) being administered to each diet group. Blood samples were collected 24 hours following the administration, and the serum aspartate aminotransferase (AST) and alanine aminotransferase (ALT) activities measured. The liver samples were studied for their cytochrome P450 and CYP2E1 contents, lipid peroxidation and histopathology. RESULTS: The results for the group receiving the 0.5 % garlic diet showed a slight decrease of CYP2E1 expression compared with the regular diet group. Carbon tetrachloride was significantly decreased the CYP2E1 contents in both the regular and garlic diet groups, but the trichloroethylene remained unchanged. Garlic did not decrease the lipid peroxidation of the liver in the control group, but attenuated the increase of lipid peroxidation caused by carbon tetrachloride. Garlic attenuated the increase of both the serum AST and ALT activities caused by carbon tetrachloride. The histopathological observations also showed that garlic attenuated centrilobular necrosis and vacuolar degenerative changes significantly in the carbon tetrachloride treated group. Conclusions : The results indicate that garlic attenuates the carbon tetrachloride-induced hepatotoxicity, through the prevention of the metabolic activation and lipid peroxidation.

7.The Role of the Cerebellar Uvulonodulus on the Early Stage of Vestibular Compensation in Rats.

BACKGROUND AND OBJECTIVES: Many expeerimental evidences have beensupported that the vestibulocerebellum involved in recovery of vestibular compensation. The purpose of this study was to investigate effect of unilateral ablation of uvula and nodular lobes on early stage of vestibular compensation. MATERIALS AND METHODS: Adult Sprague-Dawley rats weighing 250~300 g were used and divided into control (CON) group receiving sham operation, unilateral labyrinthectomy (ULX) group, ULX plus left uvulonodulectomy (ULX+Lt-UNLX) group, and ULX plus right uvulonodulectomy (ULX+Rt-UNLX) group. UNLX was performed 1 hour following ULX. Spontaneous nystagmus (SN) and roll head tilt (RHT) were recorded for 3 days following ULX. Temporal change of c-Fos protein expression in the medial vestibular nuclei (MVN) was measured for 2 days following ULX. RESULTS: There were observed SN with contralateral direction to injured labyrinth and RHT toward ipsilateral side with flexion of ipsilateral limb and extenstion of contralateral one just after ULX. There was significant increase in SN frequency of ULX+Rt-UNLX group compared with that of ULX group. In addition, ULX+Rt-UNLX group resulted in severe postural asymmetry. ULX elicited prominent expression of c-Fos protein in contralateral MVN 2 hours but in ipsilateral one 6, 24 hours after ULX, respectively. In contrast, number of c-Fos positive neurons was higher in contralateral MVN. 24 hours after ULX higher expression of c-Fos protein in contralteral MVN was still remained in ULX+Rt-UNLX group but that in ipsilateral MVN was observed in ULX+Lt-UNLX group. CONCLUSION: These results suggest that UNLX have a different effect on the early stage of vestibular compensatory process and the uvulonodulus serves the restoration of balance between intervestibular nuclear activities after ULX by inhibition of vestibular nuclear complex through nhibitory Purkinje system.

BACKGROUND: Serum eosinophil cationic protein(ECP) level has been proposed as a indirect marker of eosinophilic inflammation of the airway in bronchial asthma. OBJECTIVE: To evaluate serum ECP against indirect clinical markers of disease, we compared bronchial obstruction, bronchial hyperresponsiveness and peripheral blood eosinophil counts, total IgE with serum ECP levels in patients with bronchial asthma and normal controls. METHOD: Fourty-two patients with bronchial asthma and twenty-six normal controls were enrolled. Measurement were made by spirometry, inhalation challenge with methacholine, peripheral blood eosinophil counts, total IgE and FEIA(fluoroenzymatic immunoassay) of serum ECP RESULT: Serum ECP levels were significantly higher in asthmatic patients than normal controls(p<0.0,5). Serum ECP levels were correlated with peripheral blood eosinophil counts(p<0.01, r=0.544) and bronchial hyperresponsiveness(PC,)(p<0.01, r=-0.456) in patients with bronchial asthma. Serum ECP levels were correlated with degree of bronchial obstruction(FEV, % to predicted value, FEV1/FVC%) in total subjects, but not in asthmatic patients. CONCLUSION: Serum ECP level may be used as indicator of disease activity in bronchial asthma and be helpful in differentiation between normal person and asthmatic patients on simple serological method. Further studies on the changes of serum ECP levels according to disease course and therapeutic responses are needed.

9.Adult-onset Kawsaki Disease Complicated by Splenic Infarction and Coronary Aneurysm.

Kawasaki disease or mucocutaneous lymph node syndrome is an acute inflammatory illness of childhood characterized by systemic panvasculitis. It presents with high fever, dramatic changes of the skin and mucous membranes, and lymphadenopathy. Adult-onset Kawasaki disease is rare and reports on coronary involvement in adult are even rarer. Herein, we report a case of adult-onset Kawasaki disease complicated by splenic infarction and development of coronary aneurysm even despite of treatment with intravenous gamma globulin. A 20-year-old man presented with fever, erytheatous rash, induration and desquamation of hands and feet, pulmonary edema and shock due to cardiomyopathy, splenic infarction, bilateral conjunctivitis, jaundice, and cervical lymphadenopathy. After Kawasaki disease was suspected, intravenous gamma globulin (2 g/kg once) and aspirin (6 g/day) were administered. On the 30th hospital day, transesophageal echocardiography showed one coronary aneurysm and coronary angiography showed three aneurysms. Eight months after the first admission, follow-up coronary angiography showed normalization of the previous coronary abnormalities.
