To compare the respiratory depressant effect of nalbuphine and morphine, six healthy male volunteers (mean age 26 yr) were given the nalbuphine at a dose of 0.15 mg/kg and morphine at a dose of 0.1 mg/kg intravenously, at intervals of 2 weeks in a double-blind laboratory study. Respiratory depression was monitored by ventilatory responses during CO2 rebreathing at 1 min, 30 min, 60 min and 90 min after injection. The displacement (mean intercept at Petco2 60 mmHg) and the slope of the linear relationship between minute ventilation with Petco2 were measured, There were no significant differences in the slope and the displacement of CO2 response curve between nalbuphine group and morphine group. Also, no significant differences in the slope and the displacement among 1 min, 30 min, 60 min and 90 min after injection of nalbuphine and morphine were noted. These findings demonstrate that the respiratory depressant effect of nalbuphine in similar to morphine at equianalgesic dose, and occurs within 10 min, and continues more than 90 min.

Iatrogenic Cushings syndrome is caused by administration of systemic steroids for some disorders, and is clinically characterized by truncal obesity, hypertension, fatigability, amenorrhea, hirsutism, abdominal striae, edema, glucosuria and osteoporosis. We report herein a case of iatrogenic Cushings syndrome in the treatment of atopic dermatitis. The patient, a 32-year-old male, had been diagnosed with atopic dermatitis and was intermittently treated with topical and systemic steroids for 12 years. He revealed facial edema, truncal obesity and striae distensae in both the axilla and abdomen. Steroids are one of the important agents in the treatment of atopic dermatitis but care must be taken in their administration.

We observed a 55-year-old man who had granuloma pyogenicum on the medial border of the right fifth toe which developed after scratching of the wound. Histopathological findings showed many newly formed capillaries that had prominant endothelial cells and showed varying degrees of dilation without appearance of epidermal collarette signs. We tried intralesional treatment with triamcinolone acetonide (10mg%, 1/week*3). After 3 sessions of treatment, the lesion dramatically regressed nearly to normal size.

Ecthyma Gangrenosum is a rapidly progressing skin infection charact,erized by edema, hemorragic bullae and necrosis. Ecthyma Gangrenosum occurs almost exclusively in severely imrnunocompromised patients during the course of pseudomanas aeruginosa septisemia. A 5 month-old boy was transferred to our department because of multiple well defined central necrotic black-colored large erythematous bullae. In the bacterial cultures of skin, stool and liver aspiration fluid, Pseudomonas aeruginosa was grown. In the abdominal ultrasonogram and computed tomogram, the multiple liver abscess, hepatomegaly and ascites were shown on both hepatic lobes. Herein we report a case of ecthyma gangrenosum associated with liver abscess.

Atopic dermatitis is a common chronic inflammatory skin disease, associated with marked inflammatory cells (of mast cells and eosinophils) and severe itching, which leads to clinical problems in the pediatric population. This study was designed to investigate the inhibitory effects of luteolin liposome solution, that is entrapped the hydrophobic luteolin (one of the flavonoids) into ethosome to improve its stability, by using hapten-induced atopic dermatitis animal model (NC/Nga mice).The luteolin liposome treated mice showed anti-inflammatory effect as evidenced by the lowering of erythema and edema in clinical observation, reduction of inflammatory cell infiltration and epidermal thickness in histopathological examination, when compared with TNCB induced controls. Luteolin liposome solution also reduced the serum IgE level which played important roles in the atopic dermatitis model. These results suggest that luteolin liposome solution has some merit in this formulation showing inhibitory effects for the atopic dermatitis.

The postanesthetic nausea and vomiting during the recovery period was regarded as one of the almost inevitable and undesirable sequelae of surgery and anesthesia. With the progress of anesthetic technics and agents the incidence of this complication has been reduced. The analysis of the incidence of postanesthetic nausea and vomiting that occurred in the postanesthetic recovery room was performed on 4239 patients who received general inhalation anesthesia from Nov. 1988 to Apr. 1989 in Seoul National University Hospital. The overall incidences of postanesthetic nausea and vomiting were 0.4% and 0.5% of 4239 patients, respectively. Although the use of isoflurane among anesthetics was followed by slightly lower incidence significant. The incidence was not significantly related to sex, operation site, and the department of surgery. The incidence decreased much more than before, but because nausea and vomiting can produce undesirable complications they should be carefully prevented and treated.

Since the first attemt of internal fixation not much more than a century ago, implants have come to play an important role in orthopaedic surgery. Yet little has been reported about complications which may result from implant surgery. During the period of May 1980 to April 1984, 32 cases of fixation failures were treated in the Department of Orthopaedic Surgery, Hallym College, Kang Nam Sacred Heart Hospital and results were obtained as follows: 1. The most common area involved femoral shaft, but the highest rate was distal femur. 2. The most common period of failure was between 2 to 4 months postoperatively. 3. The leading cause of failure was remaining defects at fracture site. 4. The authors recommend accurate reduction of fracture and bone graft, if necessary, adequate immobilization postoperatively and co-coperation of patient to avoid failure of implant.

Renovascular hypertension secondary to renal artery vascular disease is the most common form of surgically correctable hypertension. The common causes of renovascular hypertension are atherosclerosis, fibromuscular dysplasia, and Takayasu arteritis. Takayasu arteritis is a chronic nonspecific arteritis of unkown cause that is relatively prevalant in young female subjects. It has been well known that the pathologic feature of the disease consist predominantly of occulsive changes in the aorta and the origin of its major branches. The most important pathogenetic mechanism of hypertension seems to be through renal artery stenosis. We have experimented three hypertensive patients with Takayasu arteritis experienced. Basic diagnosis was established by angiographic study. This article presents surgical treatment methods of Takayasu arteritis with renovascular hypertension and brief review of literatures.

Cutaneous metastases from Internal carcinomas are relatively rare. We report a case of cutaneous metastasis believed t.o originate from primary lung carcinoma in a 69-year-old male patient. The patient presented with localized well defined multiple variable sized, hard eryt,hematous papules, nodules and pustules on the right chest, back and upper arm. Biopsy specimens from the nodules upper chest revealed scatt,ered islands of tumor cells and fi brosis of the dermis. To evaluate the role of p53 protein in the development of skin tumors, we examed p53 expres sion at protein level by imnunoperoxidase stain using monoclonal antimouse human p53 antibody on paraffin embeded tissues The patient died after four months from the appearance of cutaneous lesions.
