1.Three Cases of Sporotrichosis Confined to the Face.

We experienced 3 cases of sporotrichosis confined to the face. They were lymphatic type, pustular type and fixed ulcerative type of cutaneous sporotrichosis respectively. Two types of them except the lymphatic type were very difficult to be diagnosed correctly with clinical features. So, we reviewed the types of sporotrichosis and the problems in establishing the diagnosis. 3 cases are as follows: Case l. A 57 years old female second hand dealer had a ulceration measuring 2-2cm in diameter on the right dorsum of the nose after injuring with sharp rusty iron material. Soon after ulceration healed, 5 beaded nodules measuring 0.5-1cm in diameter newly developed on the right side of the maxillary area. Case II. A 55 years old female house wife had a suppurated ulcer with serous exudate, which looked like impetigenous lesions, on the left mandibular area, without any apparent trauma history. Case III. A 52 years old female had a rice grain sized papule, which was grown slowly in size, on the left maxillary area without any apparent trauma history. Skin graft was performed after excision of the nodule, because it was diagnosed as skin cancer at a local clinic. A large ulceration with necrosis of the tissues developed on the very site of skin graft performed, accompanied by a bean sized nodule below the ulceration.

3.The Prevalence and Surveillance of Deep Vein Thrombosis after Total Hip Arthroplasty.

PURPOSE: To establish the incidence of deep vein thrombosis among Koreans after primary total hip arthroplasty and the usefulness of color doppler ultrasonography for routine noninvasive screening for deep vein thrombosis. MATERIALS AND METHODS: We performed prospective, randomized routine venography and color doppler ultrasonography before and after operation in a consecutive series of 89 primary total hip arthroplasty in which no form of prophylaxis was used. RESULTS: Of the 89 cases, eleven (12.4%) were found to have deep vein thrombosis (three proximal veins and eight distal veins) by bilateral venography. Most of the thromboses except two were asymptomatic. No patient had a symptomatic pulmonary embolism. Statistical analysis of risk factors, age, gender, weight, height, fixation method, and duration of operation time, were performed and there was no statistical difference (P>0.05). The reliability of color doppler ultrasonography for detection of deep vein thrombosis was demonstrated. CONCLUSIONS: We found insufficient evidence to recommend routine thromboprophylaxis in primary total hip arthroplasty among Koreans. The authors recommend that routine bilateral color doppler ultrasonography screening be considered after primary total hip arthroplasty for detection of deep vein thrombosis

4.Experience of Microsurgery Using Dorsalis Pedis Artery

Since the introduction of surgical microscope in microvesael surgery by Jacobson and Suarez in 1960, many surgeons have succeeded replantation, transplantation of composite segment of tissues so called free fiap-free bone graft and toe to hand transfer. McCraw & Furlow reported successfully transfered dorsal foot flap using dorsalis pedis artery in 1975 and Cobett transfered great toe to band for reconstruction of the amputated thunb. The authors experienced six cases of microsurgery using dorsalis pedis artery durig the recent two years in the department of Orthopaedic surgery of Soon Chun Hyang College and results in this paper. 1. Four cases out of six were dorsalis pedis free flap, one case was second toe to thumb and tbe other one was reconstruction of an amputated thumb in one stage using iliac bone graft and dorsalis pedis flap. 2. One case out of four cases of dorsalis pedis free flap was performed for reconstruction of contracted first web and the other cases were performed for foot. 3. Five cases out of six were successfully transfeed, one case which was toe to thumb was failed. The cause. of fail was probably due to post-operative hematoma. 4. Composite tissue using dorsalis pedis artery is one of the good donor site for composite tissue transfer for not only skin defect and scar contracture of the hand and foot but also reconstruction of the amputated fingers because it has several advantages; an acceptable thickness, a constant arterial supply, venous drainage through the saphenous system, and constant innervation through the terminal branches of the superficial and deep peroneal nerve.

5.A Clinical Study of Foreign Body Aspiration in Children in Suwon-Kyungki Province.

PURPOSE: Foreign bodies in the airway of infant and children require prompt diagnosis and removal. To evaluate the pattern of clinical characteristics and disease course, we reviewed and analyzed a 6-year experience with pediatric airway foreign bodies in children. METHODS: We retrospectively reviewed the medical records of pediatric foreign body aspiration managed by pediatric and otorhinolaryngologic departments of Ajou University Hospital from November 1994 to October 2000. Nineteen cases of aspiration were collected and we analyzed their age, sex, symptoms, duration between onset of symptoms and diagnosis, initial simple radiographic findings, matierials of aspirations, anatomic location of foreign body, length of hospital day, and complications. RESULTS: The mean age was 2.7 years and male-to-female ratio of 1.8 : 1. In 11 cases, choking episode was identified by parents or patients. The correct diagnosis was made within first 24 hours of aspiration in 4 patients; while in 15 cases, the proper diagnosis was done after 24 hours. The most common presenting symptoms were cough (73.6%) and decreased breath sounds (57.9%) and the most common initial finding of simple radiographs was emphysema, presented in 68.4% of patients. Vegetable substances, particularly peanuts were the most common material of identified foreign body. Nine foreign bodies were in the right bronchus, 8 in the left bronchus and 2 in the subglottic area. The mean duration of admissions were 3.75 days in pateints who were diagnosed within 24 hours, however, 8.61 days in those with a delayed diagnosis (P=0.012). CONCLUSION: Taken together, the majority of accidental foreign body aspiration in airway was found under 3-year of age. Pediatricians and parents should be conscious to check for history of foreign body aspiration who have suspicious clinical features of aspiration, for early diagnosis and proper management without complications and long term morbidity.

6.Embryonal rhabdomyosarcoma in the abdominal cavity of an aged Sprague-Dawley rat.

We report a spontaneous embryonal rhabdomyosarcoma in the abdominal cavity of an aged (88-week-old) Sprague-Dawley rat. The animal had a firm lobulated 5 x 5 x 4.5 cm mass in the abdominal cavity that was whitish to tan with necrotic and hemorrhagic plaques. Microscopically, the mass contained nodules with spindle or globoid shaped neoplastic cells with abundant eosinophilic cytoplasm and round or elongated nuclei mixed with other spindle cells with a filamentous appearance and scanty cytoplasm. Multinucleated cells and cross-striations were also observed. The neoplastic cells were positive for vimentin, desmin, and alpha-smooth muscle actin, especially the small spindle cells.

7.Ozone Chemonucleolysis on the Lumbar Intervertebral Disc of the Rabbit.

OBJECTIVE: This study is aimed to evaluate the chemonucleolytic effect of medical ozone by investigating the change of the signal intensity on magnetic resonance imaging (MRI) scan after injection of medical ozone into the lumbar intervertebral disc of the rabbit. METHODS: A series of 21 intradiscal injections of 1ml of medical ozone(30microgram of ozone per ml of oxygen) was performed in 7 young adult rabbits. Two control series, which 20 intradiscal injections of 0.1ml of iodine contrast medium in 7 young adult rabbits and 21 intradiscal injections of 0.05ml of chymopapain in 7 young adult rabbits, were performed. T2 weighted MRI scan was performed pre-operatively and at 4 weeks after injection. RESULTS: T2-weighted MRI scan of intervertebral disc spaces demonstrated significantly decreased signal intensities in the series (n=21) of intradiscal injections of medical ozone at 4 weeks after injection, as compared with the control series of intradiscal injections of 0.1ml of iodine contrast medium. T2-weighted MRI scan of intervertebral disc spaces demonstrated significantly decreased signal intensities in the series(n=21) of intradiscal injections of chymopapain at 4 weeks after injection. Any neurologic deficit or complication was not found in all groups. CONCLUSION: Intradiscal injections of medical ozone into the lumbar intervertebral disc of the rabbits are proven to have chemonucleolytic effects.

8.Hepatic Masses: Differential Diagnosis with Two Phases Imaging by Using Spiral CT.

PURPOSE: We compared the contrast enhancement patterns of hepatocellular carcinomas(HCC), hemangiomas, and metastasis on two phase imagings to determine the value of two phase dynamic CT in the differential diagnosis. MATERIALS AND METHODS: Two phases spiral volumetric CT scan were obtained 45 seconds and 6 minutes after bolus injection of contrast material in 106 patients. A bolus of 100 mL of nonionic contrast material was administered intravenously at a injection rate of 2 mL/sec. RESULT:In Hepatocollular carcinoma, 47% were totally hyperdense and 30% were totally hypodense in early phase, and 82% were totally hypodense in late phase. In hemangioma, 90% were peripherally hyperdense in early phase, 52% were isodense and 27% were totally hyperdense in late phase. In metastatic liver malignancies, 65% were hypodense and 32% were peripherally hyperdense in early phase, and 79% were hypodense in late phase. CONCLUSION: Two phases spiral volumetric CT scaning is considered to be heplful in the differential diagnosis of hepatic tumors.

9.Effect of DHEA on Recovery of Muscle Atrophy Induced by Parkinson's Disease.

PURPOSE: The purpose of this study was to determine the effect of dehydroepiandrosterone (DHEA) on recovery of muscle atrophy induced by Parkinson's disease. METHODS: The rat model was established by direct injection of 6-hydroxydopamine (6-OHDA, 20 microg) into the left striatum using stereotaxic surgery. Rats were divided into two groups; the Parkinson's disease group with vehicle treatment (Vehicle; n=12) or DHEA treatment group (DHEA; n=22). DHEA or vehicle was administrated intraperitoneally daily at a dose of 0.34 mmol/kg for 21 days. At 22-days after DHEA treatment, soleus, plantaris, and striatum were dissected. RESULTS: The DHEA group showed significant increase (p<.01) in the number of tyrosine hydroxylase (TH) positive neurons in the lesioned side substantia nigra compared to the vehicle group. Weights and Type I fiber cross-sectional areas of the contralateral soleus of the DHEA group were significantly greater than those of the vehicle group (p=.02, p=.00). Moreover, extracellular signal-regulated kinase (ERK) phosphorylation significantly decreased in the lesioned striatum, but was recovered with DHEA and also in the contralateral soleus muscle, Akt and ERK phosphorylation recovered significantly and the expression level of myosin heavy chain also recovered by DHEA treatment. CONCLUSION: Our results suggest that DHEA treatment recovers Parkinson's disease induced contralateral soleus muscle atrophy through Akt and ERK phosphorylation.

10.A Case of Cervical Synovial Cyst Causing Myelopathy.

Synovial cysts occur typically in peripheral joints and tendon sheaths. They rarely occur intraspinally. Recently we have experienced a case of extradural synovial cyst in the cervical spine, causing a myelopathy which results in progressive motor weakness and sensory change.
