Agnogenic myeloid metaplasia (AMM) is a myeloproliferative disorder characterized by leukoerythroblastosis, tear-drop erythrocytes, extramedullary hematopoiesis with hepatosplenomegaly, and varying degrees of myelofibrosis. The mean age at presentation is about 60 years, and pediatric cases are rare. We experienced a case of AMM in a 9 months old female who was presented with pallor, huge splenomegaly and intermittent fever. Peripheral blood showed leukoerythroblastosis poikilocytosis, and tear drop cells. Bone marrow was difficult to aspirate, and biopsy specimen showed increased reticulin with decreased cellularity, which was compatible with myelofibrosis. We presented a case of AMM with brief review of the literatures.

BACKGROUND/AIM: Serial measurement of serum carcinoembryonic antigen was assessed to define its significance and to determine the adequacy in detecting recurrence after curative resection for colorectal cancer. METHODS: Six hundred forty-five patients with colorectal cancer underwent curative resection were included. The median follow-up period was 49 months (range, 24~94 months). Serum CEA was analyzed in accordance with location, histologic differentiation, stage of the tumor, recurrence and survival. RESULTS: The incidence of elevated preoperative serum CEA (> 6 ng/ml) was correlated with tumor stage (stage I vs. II, P=0.01; stage II vs. III, P=0.0001). Fifty five patients among 87 patients with recurrence (63.2%) had concomitant elevation of serum CEA, whereas 32 of 558 patients (5.7%) without recurrence showed a false-positive result. Measurement of serum CEA was more sensitive in patients with elevated preoperative serum CEA and liver metastases than in patients without elevated preoperative serum CEA and local recurrence (P=0.0397). The leading time of serum CEA between the first elevated serum CEA and the identification of recurrence was 3.5 months (range, 1~12 month). Tumor stage and preoperative serum CEA level were found to be significant prognostic variables by multivariate analysis. The overall 5-year survival rate in the normal preoperative serum CEA and the elevated group were 76% and 64% respectively (P=0.00019). CONCLUSION: Serum CEA seemed to be closely correlated with survival and to be an useful tool to detect recurrence after curative resection for colorectal cancer. The appropriate measurement of serum CEA might be suggested in stage II and III postoperatively: every three month for two years, every 6 month for succeeding 2 years, and annually thereafter. Monitoring of serum CEA in stage I could be individualized by preoperative serum CEA and clinical course.

Iatrogenic Cushings syndrome is caused by administration of systemic steroids for some disorders, and is clinically characterized by truncal obesity, hypertension, fatigability, amenorrhea, hirsutism, abdominal striae, edema, glucosuria and osteoporosis. We report herein a case of iatrogenic Cushings syndrome in the treatment of atopic dermatitis. The patient, a 32-year-old male, had been diagnosed with atopic dermatitis and was intermittently treated with topical and systemic steroids for 12 years. He revealed facial edema, truncal obesity and striae distensae in both the axilla and abdomen. Steroids are one of the important agents in the treatment of atopic dermatitis but care must be taken in their administration.

We observed a 55-year-old man who had granuloma pyogenicum on the medial border of the right fifth toe which developed after scratching of the wound. Histopathological findings showed many newly formed capillaries that had prominant endothelial cells and showed varying degrees of dilation without appearance of epidermal collarette signs. We tried intralesional treatment with triamcinolone acetonide (10mg%, 1/week*3). After 3 sessions of treatment, the lesion dramatically regressed nearly to normal size.

Idiopathic midline destructive disease(IMDD) is a subgroup of midline granuloma syndrome, which is a clinical entity characterized by a relentless uilcer ation of the upper air way involving the nose, the palate, and the face, with a fatal outcorne. A 32 year-old male had symptoms of nasal obstruct.ion and rhinorrhea for about 4 months prior to visiting the hospital ad was referred to our Department for evaluation of a verely destruct.ive skin lesions on his face. The patient, revealed unique clinopatholcgic featurs compat with IMDD. These were : 1) prescence of locally destructive lesion which were restricted to the upper respiratory tract. 2) abscence of systemic disease durin the follow up peroid, 3) The histologic features consisted of acute and chronic inflarnmation with a variable dearel of necrosis on the examination of repeated biopsy specimens, 4) inabiliy to demonst rate an infectious causative organisms on repeated laboratory tests.

In this experiment using 30 rabbits(40 normal eyes), the applicability of ocular adhesive, HISTOACRYL blue, as an implant beneath the scleral and conjunctival flap. Group 1 consisted of 10 left eyes. These eyes had undergone a fornix-based conjunctival flap and a triangular lamellar scleral flap without sclerectomy and served as a control on histologic examination. Group 2 consisted of 10 right eyes of the rabbits used in group 1. The Histoacryl was pasted on the sclera under the lamellar scleral flap and the conjunctiva. The pasted Histoacryl turned into a thin membrane. Group 3 consisted of 10 right eyes which had undergone a conventional trabeculectomy. Group 4 consisted of 10 right eyes. These eyes had undergone a trabeculectomy and placement of a Histoacryl membrane. The adhesive was applied closely to the limbus in 5 eyes and posteriorly away from the limbus in the remaining 5 eyes of group 4. At the eighth week after operation a filtering bleb was observed in all of the eyes in group In contrast to this, only two eyes showed filtering blebs in group 3. Filtering blebs were so confined over the Histoacryl membrane that their size and location was in accordance with the membrane. The vessels of the conjunctiva overlying the Histoacryl membrane were dilated and congested. This vascular congestion lasted long. The light microscopic examinations of the operation site showed a thick layer composed of macrophages, eosinophils, neutrophils, foreign body giant cells, and fibroblasts surrounding the Histoacryl membrane with cystic space between them. At the eight postoperative week, this wall remained thick measuring about 50-70 micro thick. From the result, Histoacryl appeared to be effective in preventing the fibrotic scarring down of the filtering bleb, but postoperative inflammatory changes indicated that the Histoacryl seemed to be rather unsuitable for clinical application.

We performed surgery for correction of abnormal head turn in 10 patients with congenital nystagmus. For 3 patients with a head turn of 30 degrees, a 40% augmented modified kestenbaum operation was performed. For 4 patients with a head turn of 45 degrees, a 40% augmented modified kestenbaum operation combined with a 3 - 4 mm recession of conjunctival and subconjunctival tissues was performed. Finally, for 3 patients with a head turn of 60 degrees, a 60% augmented modified kestenbaum procedure combined with a 3 - 4 mm recession of conjunctival and subconjunctival tissues was performed. At the final follow-up visit(averaging 5.9 months), 7 patients showed complete corrections of previous head turn and 3 patients showed a small residual head turn of less than 10 degrees to the same side of the preoperative head turn. The average amount of correction we obtained from the surgery described above were 26.7, 42.5 and 56.7 degrees for the patients who had had a preoperative head turn of 30, 45 and 60 degrees respectively. Among the 10 patients, 4 showed no change in corrected visual acuity, but 6 showed increased corrected visual acuity by over 1 Snellen line. From the above results, we concluded that if recession procedure of the conjunctival and subconjunctival tissues was combined with augmented modified Kestenbaum procedure, its effect for correction of abnormal head turn in a patient with congenital nystagmus could be enhanced.
