1.Atypical Case of Granuloma Annulare.

Granuloma annulare is a benign, usually self-limited chronic dermatasia of unknown cause, charaeterized by akin colored papules that may be arranaged in an annular configuraticn. Histopathologically, it is charicterized by camplete or incomplete foeal degeneration of collagen of chronic infliammatory cells. The authors obaerved an atypical case of granuloma annulare in a 4-year-old girl. She showed multiple papules and nodule on hands and feet. On inital examination, her skin lesions suggested histiacytosis, xanthanulama, rheumatoid nodule, or benign skin tumor. But the skin biopey revealed typica1 findings of granuloma awulare. Focal degeneration of colagen was confirmed by toluictine blue stain. Her skin lesions were almoatly cleared by skin biopsy and intralesional injectian of corticosteroid.

2.The Absent Lumbar Articular Process of the 4th Lumbar Vertebra: One Case Report

Unilateral absence of the articular process at a lumbar vertebra is a rare anomaly. The etiology of the congenital absence of articular process is not precisely determined yet, but it was explained as some of the possible embryologic bases. Absence of the articular process at the level of L5-Sl have been reported a few, but anomaly above L4 is rare. We are reporting a case of absence of articular process at the level of the 4th lumbar vertebra.

3.A Case of Multiple Basal Cell Epitheliomas Arising in An Epidermal Nevi.

Basal cell epithelioma arising from epidermis and its appendages is a common skin tumor. But multiple occurence of basal cell epitheliomas and that of arising in an epidermal nevi are extremely rsre. Since the first report by Toyama and Kojima in 1937, only few cases were reported in the world literatures. We observed a case of multiple basal cell epitheliomas arising in an epidermal nevi in a 54-year-old female patient who had rhomboidal verrucous patch on the right side of the cheek and arciform erythematous verrucous eruption from the right side of the eyebrow to the lower eyelid. Clinical features were not compatible with any type of basal cell epithelioma. But histological finding revealed basal cell epithelioma with epidermal nevi. Literatures were briefly reviewed.

4.Cerebral Aneurysms in Childhood and Adolescence.

The incidence of intracranal aneurysm is rare in childhood and adolescence. During the period from January 1982 through December 1989, 793 aneurysms were operated on the our department. Of these 8 aneurysms occurred 8 patients under age of 18 years. The average age was 11.6 years(range 4 to 18 years). There were 6 male and 2 females. Presenting sings and symptoms included SAH in 6 aneurysms and focal neurologic deficits in 2 aneurysms. All aneurysms were surgically treated : direct clipping was performed in 6, aneurysmorraphy in 1, trapping alone in 1. The postoperative result were excellent in 5, good in 2, and fair in 1. The rarity of such lesions in childhood and adolescence and their pathogenesis are reviewed briefly.

5.The Effect of Prior Dural Puncture with a 27G Spinal Needle on Epidural Anesthesia for Cesarean Section.

BACKGROUND: If started with segmental block, epidural anesthesia appears to have an inadequate sacral block in the early part of cesarean deliveries. A dural puncture using the combined spinal-epidural technique making an early sacral blockade might be a solution. METHODS: A prospective, randomized, double-blinded study was performed on forty patients who were divided into two groups (C, DP; n = 20, each). An epidural infusion of 25 ml of 2% lidocaine with epinephrine was given to the group C. A dural puncture with a 27G Whitacre needle was done before the epidural infusion in the group DP. Two-way sensory levels were checked for 15 min, just before the operation. The side effects and recovery variables were also compared between the groups. RESULTS: Initial wider cephalic block (P = 0.038, 0 min) followed by more caudal block (P = 0.023, 0.013; 5, 10 min after the completion of the epidural infusion, respectively) occurred in the DP group. After 10 min, there were no differences in both block levels. The number of dermatomes blocked was bigger in the DP group at 10 min after. Fewer patients in the DP group had nausea and/or vomiting (P = 0.044). CONCLUSIONS: Early spread of the sensory blockade, initial cephalic and then caudal, was due to the dural puncture by a 27G needle. This fast onset could make the parturients more comfortable in the early part of cesarean sections.

6.A Case of Traumatic Giant Aneurysm.

The authors are reporting traumatic giant aneurysm arising from supraclinoid portion of internal carotid artery. The patients was a 22-years old man who had suffered a major head injury in a traffic accident. On admission he was comatous and there were multiple abrasion over the body and severe hemorrhage comming from the left ear with open fracture in left parietal. 4 weeks later, computed tomographic scan demonstrated large well enhanced mass in the suprasellar region. Carotid angiography showed giant aneurysm fed by supraclinoid portion of internal carotid artery. After frontotemporal craniectomy, direct approach was performed using surgical microscope.

7.Comparision of Results after ACL Reconstruction between Bioabsorbable Interference Screw and Metallic Interference Screw Fixation.

PURPOSE: To analysis clinical results after arthroscopic ACL reconstruction between bioabsorbable interference screw fixation and metallic interference screw fixation. MATERIALS AND METHODS: We evaluated the results of arthroscopic ACL reconstruction with patellar tendon autograft between two groups, of which group 1 is used metallic interference screw, group 2 is used bioabsorbable interference screw. All of 53 patients underwent arthroscopic patellar tendon auto-grafts with one incision technique. A minimum 12 months follow-up is available on 31 patients in group 1 and 22 patients in group 2. The average age was 28.4 years(19 to 47 years) in group 1 and 29.7 years(18 to 55 years) in group 2. The average period, from trauma to operation, was 20.7 months in group 1 and 24.6 months in group 2. The average follow-up period was 24.1 months in group 1 and 13.1 months in group 2. In terms of the results of pivot shift test, Lachman test, anterior drawer test, Lysholm score, Arthrometer(KT-2000), Cybex test, simlpe X-ray and MRI findings of group 1 compared with group 2. RESULTS: At the final follow-up, KT-2000 test showed that average maximum manual side-to-side dif-ference(STSD) was 2.7mm for group 1 and 2.7mm for group 2. Average muscle power of injured quadri-ceps muscle compared with normal leg by Cybex test was average 82% for group 1 and 79% for group 2. Lysholm score was average 90 for group 1 and 89 for group 2. 3 cases of group 1 and 2 cases of group 2 were showed instability in physical examination, but there were no evidence of synovitis of knee joint or rerupture of anterior cruciate ligament in the follow-up MRI. CONCLUSIONS: No statistical difference was found between the bioabsorbable interference screw fixation group and the metallic interference screw fixation group. Short-term data support that bioabsorbable interference screw is a reasonable alternative to metallic interference screw.

8.A Case of Bilateral Macronodular Adrenocortical Hyperplasia

Cushing's syndrome associated with nodular adrenal glands will be divided into four main categories: adrenal adenoma, adrenal carcinoma, primary pigmented nodular adrenal dysplasia (PPNAD) and macronodular adrenal hyperplasia(MAH). The term macronodular adrenal hyperplasia is restricted to the presence of multiple nodules visible to the naked eye, ranging in size from 0.5 to 7.0 cm. We report a case of Cushings syndrome caused by bilateral macronodular adrenal hyperplasia (MAH). A 45-year-old man presented with Cushingoid features, hypertension and diabetes mellitus. Urine free cortisol was 449.9 mmol/day(27-276) and were not suppressed after administration of low-dose and high-dose dexamethasone. Plasma ACTH was very low(1.87 pmol/L(18)) and was not stimulated by administration of ovine CRH. In abdominal CT, both adrenal glands were markedly enlarged and nodular in appearance. Pituitary MRI showed no abnormal finding. Bilateral adrenalectomy was done. Histologic examination revealed multiple nodules and internodular hyperplasia. This case and other reports suggested that because of variable biochemical, radiologic and pathologic findings, macronodular adrenal hyperplasia represents a heterogeneous group of patients with varying degrees of adrenal autonomy.

9.Camprison of Diagnostic and Therapeutic scans in Patients with Differentiated Thyroid Cancer

Background: Whole body scan using 131-iodine is performed to detect local recurrence or metastasis after thyroidectorny in differenciated thyroid cancer patients. The sensitivity of this procedure is related to the dose of radiopharmaceutical administered. It was reported that 131I posttreatment whole body scan demonstrate foci of tracer uptake not previously observed in diagnostic scan in 10~30% of cases. Posttreatment scans were most likely to reveal new foci in young patients(<45) and patients who had previously received radioactive iodine therapy. Method: We observed the frequency of discordant posttreatment scans and analysed the clicnical significance in 33 differenciated thyroid cancer patients who were admitted for radioiodine ablation from June, 1995 to April, 1996. Results: In 7 cases(21.2%), post treatment scan demonstrated cme or more foci of uptake and revealed less sites of uptake than diagnostic scan in 3 cases(9.1%). In one case with elevated thyroglobulin level and negative diagnostic scan, post treatment scan revealed new uptake sites with thyroid bed and cervical 1ymph node. The sites of discordant uptake were cervical lymph nodes in 4 cases and rnediastinal lymph node in one case, lung in one case, thyroid bed and cervical lymph nodes in one case, 3 cases of 7 pts(43%), demonstrated ane or more foci of uptake in post treatment scan, had history of previous radioiodine treatent. Conclusion: Post treatment scan confirmed uptake into remnant and metaststic tissues identified on the corresponding low dose diagnostic scans. Scanning after high dose radioiodine treatment frequently demonstrated one or more foci of uptake, especially in patients with previous radioiodine treatment, which were not visualized on the diagnostic low dose scan. Treatment scan may be useful for detection of remnant tissue or metastatic lesion in patients with elevated thyroglobulin and negative diagnostic scan.

10.Multiple Intracranial High Density Foci after Brain Parenchymal Catheterization.

OBJECTIVE: To report an observational investigation of small high attenuated foci in computed tomography (CT) scan followed by brain parenchymal catheterization. METHODS: From January 2011 to March 2015, we retrospectively reviewed the 381 patients who had undergone brain catheterization in our clinic and enrolled the patients who had newly developed high attenuation foci in the postoperative CT scans. The brain CT scans were reviewed about the lesion location, Hounsfield Unit (HU) and the time of appearance. RESULTS: Twenty seven of 381 patients had high attenuation foci in CT scans after the procedure. The location of high density lesions was as follows: parenchyma in 9 (33.3%) cases, ventricle in 5 (18.5%), combined in parenchyma and ventricle in 13 (48.1%). The lesions were identified in the catheter tract in parenchymal type, and catheter-lodged frontal horn or choroid plexus in ventricular type. We could not find the calcific foci before the catheter removal, and those were found after removal in all cases. The time of appearance after the removal was variable from 0 to 14 days (mean 4.2, median 3). The regular rules of HU change in CT scans were not found as times go on. CONCLUSION: The high attenuation foci in CT scans were bone dust originated from skull during operation. Although these lesions did not make troubles, we should clean the operation field before the insertion of brain catheter and we may use another material, like Surgicel to seal up the burr hole instead of bone dust in the end of operation.
