4.A case report of mandibular prognathism treated with the labiolingual appliances.

A boy (Hellman dental age III B) with mandibular prognathism was treated by means of the labiolingual appliance and the chin cap. Following results were obtained; 1) Except the problem of development, most of treatment objectives were achieved after 8 months of active treatment. 2) The axial inclinations of upper and lower incisors were changed by the treatment. 3) The most notable change was the reduction of genial angle. 4) The patient has retained the good occlusal stability till after 18 months of retention.

5.Radiologic findings of rib tumors

Authors reviewed 21 cases of histologically confirmed rib tumor except metastasis and myeloma from Nov. 1973 to Dec. 1982 at Seoul National University Hospital. The resuls are as follows: 1. Major clinical findings are painand mass. 2. Pathologic diagnosis of rib tumors are 5 cases of fibrous dysplasia, 3 chondroma, 2 osteochondroma, 2desmoplastic fibroma, 3 chondroma, 2 osteochondroma, 2 desmoplastic fibroma, 5 osteosarcoma, 1 aneurysmal bonecyst, 1 eosinophilic granuloma, 1 cavernous hemangioma, 1 chondrosarcoma. 3. Radiologic findings are osteolyticchange and cortical expansion without marginal sclerosis in fibrous dysplasia, central location, well-definedosteolytic change with marginal sclerosis and no cortical destruction in desmoplastic fibroma, corticaldestruction, soft tissue mass and pleural effusion in malignant tumors.

6.A Case of Maffucci's Syndrome Associated with Spindle-cell Hemangioendothelioma.

A case of a 21-year old female with Maffucci's syndrome is presented where the patient developed multiple cutaneous spindle-cell hemangioendotheliomas. She had multiple pea to walnut sized, non-tender, normal skin colored or slight bluish nodules and bony mass-like lesions on the left upper extremity and hand. A histological examination of the lesions showed that they were composed of irregularly dilated, thin walled cavernous blood spaces containing phleboliths and collapsed vascular spaces separated by spindled fibroblastic cells. Radiologically, the bony lesions showed radiolucent densities with calcified spots within the second proximal phalanx and metacarpal bone of the left hand, which were consistent with enchondromas.

7.Eccrine Squamous Syringometaplasia.

An eccrine squamous syringometaplasia (ESS) is defined as a mature squamous metaplasia of the eccrine ducts. The clinical and pathological features of an ESS are presented. Syringometaplasia is a rare lesion, mostly occuring in the extremities, and as far as we know, no report on the ESS has been published in Korean literature. We experienced a case of an ESS occured in a 15 year-old male, who had a tender erythematous plaque in the right knee. The histologic examination revealed some scattered nests of metaplastic squamous epithelium in the deep dermis, associated with acute nonspecific panniculitis. The importance of the ESS is that it histologically simulates the well differentiated squamous cell carcinoma. The histopathologic findings were discussed and a brief review of the literature was made.

9.Crossed Cerebellar and Cerebral Cortical Diaschisis in Basal Ganglia Hemorrhage.

PURPOSE: The purpose of this study was to evaluate the phenomenon of diaschisis in the cerebellum and cerebral certex in patients with pure basal ganglia hemorrhage using cerebral blood flow SPECT. MATERIALS AND METHODS: Twelve patients with pure basal ganglia hemorrhage were studied with Tc-99m ECD brain SPECT Asymmetric index (AI) was calculated in the cerebellum and cerebral cortical regions as |CR-CL|/(CR-CL)x200, where CR and GL and the mean reconstructed counts for the right and left ROIs, respectively. Hypoperfusion was considered to be present when AI was greater than mean+2 SD of 20 control subjects. RESULTS: Mean AI of the cerebellum and cerebral cortical regions in patients with pure basal ganglia hemorrhage was significantly higher than normal controls (p<0.05): Cerebellum (18.68+/-8.94 vs 4.35+/-0.94, mean+/-SD), thalamus (31.91+/-10.61 vs 2.57+/-1.45), basal ganglia (35.94+/-16.15 vs 4.34+/-2.08), parietal (18.94+/-10.69 vs 3.24+/-0.87), frontal (13.60+/-10.8 vs 4.02+/-2.04) and temporal cortex (18.92+/-11.95 vs 5.13+/-1.69). Ten of the 12 patients had significant hypoperfusion in the contralateral cerebellum. Hypoperfusion was also shown in the ipsilateral thalamus (n=12), ipsilateral parietal (n=12), frontal (n=6) and temporal cortex (n=10). CONCLUSION: Crossed cerebellar diaschisis (CCD) and cortical diaschisis may frequently occur in patients with pure basal ganglia hemorrhage, suggesting that CCD can develop without the interruption of corticopontocerebellar pathway.

10.Dorsal Epidural Gas after Lumbar Microdiskectomy Treated with CT-guided Needle Aspiration

To present a case of unusual dorsal epidural gas (EG) accumulation after a simple lumbar microdiskectomy (MD), treated with computed tomography (CT)-guided needle aspiration.A 78-year-old woman underwent simple lumbar MD at the L3–4 level. One week after the operation, the patient complained of severe back pain radiating to the right thigh. Follow-up magnetic resonance imaging (MRI) and CT revealed huge EG formation at the dorsal L3–4 epidural space. Conservative treatment did not resolve the patient's pain. We performed CT-guided needle aspiration after 1 week of conservative treatment. The patient's pain fully resolved after aspiration, but it recurred 1 week later. Follow-up MRI and CT revealed reaccumulation of the dorsal EG at the L3–4 level. CT-guided needle aspiration was repeated, again leading to full pain resolution. Follow-up CT 6 months after the second aspiration showed no recurrent dorsal EG. The patient has been symptom-free for 1 year since the second aspiration. CT-guided needle aspiration is a safe and effective alternative to reoperation in the context of dorsal EG formation after MD.
