2.A Clinical Study of Colles' Fracture Treated by Closed Reduction and Sugar Tong Splint

We analyze ninty-seven cases of Colles fractures treated with closed reduction and sugar tong splint immobilization and followed for more than 1 year at Seoul National University from January 1981 to Decmber 1985. The results of this study were as follows; l. All of 97 cases were closed fractures and the incidence was high in females who aged over 6th decades. The main cause of this injury was slipping down accident comprising of 68.0%. 2. 63 cases (64.9%) were simple extra-articular fractures, and 34 cases (35.1%) were comminuted intra-articular fractures. 3. In a group of simple extra-articular fracture the functional end result was satisfactory in 88.9%, so closed reduction and sugar tong splint were thought to be a good procedure in this group. 4. In a group of comminuted intra-articular fracture the functional end result was satisfactory in 58.9% only. In the cases which show unacceptable loss of position during first two weeks of routine sugar tong splint immobilization we feel another technique is indicated, such as.pin and plaster immobilization or percutaneous pinning. 5. The main cause of unsatisfactory result was malunion, which was produced by loss of reduction position.

4.The Effects of Tumor Necrosis Factor - alpha on the Superoxide Radical and Superoxide Dismutase in Cultured Human Keratinocytes.

BACKGROUND: Superoxide dismutases (SOD) are important metallo-enzymes which scavenge and dismutase the superoxide free radical. They are thought to be the main enzymes in the antioxidant defense system. In the several cell ines the induction of Mn-SOD expression by tumor necrosis factor (TNF)-alpha which is a known inflammatory cytokine, suggests that Mn-SOD may play a role in the inflammatory process. However, this effect is occurred. OBJECTIVE: The present study was undertakento determine whether the induction of SOD expression by TNF-alpha, in the keratinocytes is occurred. METHODS: Primary keratinocyte cultures from neonatal foreskins were grown in complete MCDB 153 medium. Measurement of superoxide radical and SOD activity were performed according to Babior and Fridovich methods, respectively. RESULTS: 1. The release of superoxide radical were measured after 30, 60, 90 minutes exposure to 10ng/ms TNF-alpha, THE RESULTS SHOWS 2.94+/-0.30 NMOL/2x105, 3.55+/-0.52 NMOL/2x105, 4.13+/-0.70 nmol/2x105 respectivelyl The increase of superoxide radical release was detectable in a time dependent manner. 2. Total SOD activity without treatment of TNF-alpha at 24 hours was 18.46+/-2.85, Total SOD activities after treatment of 1, 10, 100 ng/ml TNF-alpha at 24hours were 36.47+/-6.80, 88.01+/-7.79, 94.50+/-8.97 respectively, which suggested that TNF-alpha induced increase of total SOD significantly (p<0.050. TNF-alpha stimulate total SOD induction in a dose and time dependent manner, although the effects were not outstanding. Mn-SOD activity after treatment of 100 ng/ml TNF-alpha at 48 hours was 84.12+/-14.83, which was approxdimately 10.5 fold of that of the untredated case. TNF-alpha also induced the Cu, Zn-SOD activity, but the effect was not so much as in the case of Mn -SOD. CONCLUSION: TNF-alpha induces production of total SODD activities, especially Mn-SOD in the human kerati ocytes, which may be to protect cells from inflammation mediated oxidative damage, namely production of total SOD protein, especially Mn-SOD in response to TNF-alpha induced and scavenged overproduced superoxide radical which in turn may result in protection of the keratinocytes.

5.Clinical Study on Congenital Heart Diseases in Korean Adult.

A clincal study was made on 383 adult patients with congenital heart disease who visited the Seoul National university Hospital during the period of March, 1961-July, 1979. 1. The incidence of congenital heart disease for the consecutive years was increasing in 1970s, especially in recent two years. 2. The sex incidence of congental heart disease was female 47.0%, male 53.0%, with male preponderance in tetralogy of Fallot, ventricular septal defect, coarctation of aorta, aortopulmonary window and female preponderance in atrial septal defect, patent ductus arteriosus. 3. Age distribution of congenital heart disease showed that 79.1% of all were below 30 years of age but atrial septal defect showed wide distribution below and above 30 years of age. 4. The incidences of each congenital heart disease were atrial septal defect 20.9%, tetralogy of Fallot 20.6%, ventricular septal defect 19.%, patent ductus arteriosus 12.0%, pulmonary stenosis 7.6%, trilogy of Fallot 1.8%, Ebstein's anomaly 1.8%, ventricular septal defect with pulmonary stenosis 1.3%, coarctation of aorta 1.3%, ventricular septal defect with aortic insufficiency 1.0%, transposition of great vessels 1.0%, in order and rare and various combined anomalies. 5. Average duration of illness in congenital heart disease was less than 15 years in majority, but duration from 11 years to 20 years was most common in tetralogy of Fallot. 6. Functional class of congenital heart disease is class II and I in majority, but class III was most common in tetralogy of Fallot. 7. Comparision of precatheterization diagnosis with postcatheterization diagnosis showed concordence in 92.2% and comparison of postcatheterization diagnosis with postoperative diagnosis showed concordence in 95.5% of cases. 8. Electrocardiographic findings in 5 major congenital heart disease were observed as follows; right ventricular hypertrophy 86.3%, right atrial enlargement 16.4% in tetralogy of Fallot, right ventricular hypertrophy 30.9%, incomplete right bundle branch block 27.9% in atrial septal defect, left ventricular hypertrophy 28.8%, right ventricular hypertrophy 10.6%, biventricular hypertrophy 10.6% in ventricular septal defect, left ventricular hypertrophy 63.2% in patent ductus arteriosus and right ventricular hypertrophy 70% in pulmonary stenosis. 9. Postoperative changes in electrocardiographic findings were observed in 36.1% of operated patients, of whom teteralogy of Fallot 61.7%, ventricular septal defect 48.0% were most common. Pstoperative electrocardiographic changes were complete right bundle branch block 58.3%, incomplete right bundle branch block 11.7%, myocardial ischemia 10%, nonspecific ST-T changes 10%, etc. in order.

7.Study on Superoxide Dismutase Activity in Scar Tissues.

BACKGROUND: Superoxide disrnutase(SOD) provides a protective defease mechanism against potential cytotoxicity of superoxide radical in the aerobic organism. Although human skin is constantly at risk for developing acute and chronic changes by ultraviolet radiation and phototoxic reactions with exogenous and endogenously procluced photosensitizing molecules, studies in SOD in the human skin are rare. OBJECTIVE: We measured the level of SOD activities in the scar tissues and the normal human skin specimens. This study was to investigate changes of SOD activity by age, sex, and regional differences of SOD activities in the scar issues and the normal skin. METHODS: Aut,hors assayed the level of SOD activit,ies in 32 scar tissues(male 8, female 24) and 11 normal human skin specimens(male 8, female 3), which were obtaine 3 from face/neck(17 and 3 specimens), forearm(only 4 scar tissues), trunk(10 and 8 specimens), and lower extremity(only 1 scar tissue). RESULTS: First, activities of total SOD, Cu, Zn-SOD, and Mn-SOL ere 18.93+5.49, 16.97+55.31, and 1.96+0.90 units/mg proteiii respectively in the scar tissues. Second activities of total SOD, Cu, Zn-SOD, and Mn-SOD were 17.27+7.09, 13.82+6.44, and 3.45+1.07 units/mg protein respectively in the normal skin. Third, the changes of total SOD, Cu, Zn-SOD, and Mn-SOD activities by age and sex were similar each other and three were no significant, differneces between age groups in total, Cu, Zn- SOD, and Mn-SOD activities. Fourth, in sun exposed area and unexposed area there were no significant differences in the scar tissues in SOD activities. But, SOD activite.(total, Cu, Zn, and Mn-SOD) in face/neck were higher than those in trunk and lower extremity in tae normal skin(P<0.05). CONCLUSION: These findings suggest that there are no differences in the intrinsic SOD activities by age and sex in the mature scar tissues and the normal skin. Differences between exposed and unexposed area in the normal skin are due to the induction of exogenous SOD activity by sun-light generation of superoxide radicals. In wound, increased production of leukocyte derived superoxide radicals is the main factor of increased level of SOD activity.

8.Effects of Converting Enzyme Inhibitor on the Left Ventricular Remodeling after Coronary Artery Reperfusion in Rats.

BACKGROUND: Angiotensin convertiong enzyme inhibitors have been shown to exert favorable effects on the left ventricular remodeling process associated with ventricular dilation after coronary occlusion. However, the effects of such therapy on global and regional left ventricular remodeling after coronart artery reperfusion have not been characterized, nor have such effects been assessed after exercise training. METHODS AND RESULTS: Female Sprague-Dawley rats(n=80) were randodmized into 4 groups at 5 days after 45 minutes of left coronary artery occlusion followed by reperfusion. Animals completion the experiment included : Untreated Sedentary group(n=20), Untreated with Swimming Exercise group(n=21), Captopril Treated Sedentary group(n=18) and Captoril Treated with Exercise group(n=21). At 3 weeks after randomization, global and regional morphologic changes of the left ventricle(LV) were examined from mid-ventricular transverse slices which were perfusion-fixed at a constant aortic pressure of 60mmHg and a left ventricular cavity pressure of 10mmHG. At rest and during exercise, compared to untreated rats, the captopril treated animals showed significantly decreased LV weight/tibial length ratio(LV/TL)(p<0.01),increased LV cavity area and dimension(both p<0.01), decreased total myocardial area and noninfarcted area(both p<30.001) and reduced wall thicknesses in the noninfarcted and infarcted regions(both p<0.001). Compared to treated and untreated dsedentary rats, exercise significantly increased LV/TL(p<0.05) and epicardial and endocardial areas in the infarcted zone(both p<0.05) and decreased transmurality(p<0.01). Exercise decreased LV cavity area in the captopril treated groups(42.3+/-10.4 vs. 40.4+/-6.0mm2),whereas exercise increased LV cavity area in the untreated groups(33.5+/-8.9 vs. 39.1+/-6.2mm2)(p<0.05). CONCLUSION: These findings provide evidence in rats for evidence in rats for exaggerated left ventricular dilation and supperssion of compensatory myocardial hypertrophy globally and in the infarct zone with 3 weeks of captopril treatment following coronary artery reperfusion with acute nontransmural myocardial infarction. In addition, the effects of captopril on LV dilation and suppression of global and regional hypertrophic response were partially reversible by swimming exercise.

9.A Case of Chronic Mucocutaneous Candidiasis.

A 6-year-old male patient had been suffering from angular cheilitis and paronychia with fragmentation and dystrophic change of the finger nails. Laboratory findings showed low serum iron level and anemia. Immunologic studies revealed defects in cell mediated immunity. KOH examination and culture of specimens from the lesions showed hyphal elements and growth of Candida albicans respectively. Concomitantly itraconazole and iron sulfate were administered. Four months after treatment he was free of any clinical evidence of the disease.
