Eighty one cases of the pelvic fractures treated at the orthopedic department of Pusan National University Hospital during the period from January 1960 to August 1970, were analyzed clinically. The results obtained were as follow: 1. The causes of fractures of the pelvic bone were mostly due to traffic accident (83%), industrial injury (7.4%), fall from a height (6%) and direct injury (3.7%). 2. 60% of the pelvic bone fractures were complicated by other bone fractures and soft tissue injuries. 3. The most common complication was the injury of the urinary system, and in neglected cases to treat it, severe sequellae such as stricture and urinary fistula were developed. 4. In cases of perineal laceration, it was possible to associate the infection to the adjacent hip joint by direct spread. 5. Open fracture associating severe soft tissue injury had poor prognosis. 4 cases out of 5deaths had open fractures. 6. The main cause of death was the hypovolemic shock due to intrapelvic bleeding. Therefore, immediate massive transfusion as life-saving measure was stressed.

4.Corticosteroid-Induced Avascular Necrosis of Femoral Head: Report of 5 Cases

Five cases of corticosteroid-induced avascular necrosis of femoral head which were experienced at the Department of Orthopaedic Surgery, Catholic Medical College, are reported with review of literature. All patients received corticosteroids for the treatment of rheumatoid arthritis. Prednisolone was the most common drug used, having been given to 3 patients. Two patients has received Dexacortisil alone or in combination with other corticosteroids. There was a considerable variation in the dosage, but in every case the dosage was in excess of physiological replacement. The total dosage administered in each patient was impossible to determine with any degree of accuracy. The duration of corticosteroid treatment varied. The shortest known period of treatment in this series was 12 months, the longest 60 months. The avascular necrosis noted in the eight femoral heads was treated conservatively in three and surgically in five. As a whole, metallic pinning in two cases, associating the fracture of the neck, cup arthroplasty in one and Austin-Moore hip prosthesis in two cases were performed. Five hips restored satisfactory range of motions, despite of the types of surgery.

5.Expression of bcl-2 Protein in Gastric Adenoma and Adenocarcinoma.

The bcl-2 oncoprotein confers a survival advantage to cells by blocking programmed cell death. Overexpression of bcl-2 probably plays a role in tumorigenesis, and the expression of the bcl-2 protein has been investigated in many kinds of tumors. However, there have been only a few reports on expression of bcl-2 in human gastric adenocarcinoma. The aim of this study was to investigate the relationship between the expression of bcl-2 protein and several clinical and pathological parameters such as age, tumor site, size, histological type, depth of invasion, Lauren's classification, and grade. Immunohistochemical staining using monoclonal bcl-2 protein antibody, clone 124, was performed on paraffin embedded specimens from 23 gastric adenomas and from 45 gastric adenocarcinomas. The results are as follows. 1. Variable intensity of epithelial staining was noted from case to case, although the lymphocytic component showed similar intensity in all examples. The staining was located at the gland and mucous neck region of non-neoplastic epithelium. 2. The more differentiated type of gastric adenocarcinoma showed the higher expression rate and intensity. 3. The relationship between the expression rates of bcl-2 protein and tumor grade (adenoma early gastric adenocarcinoma advanced gastric adenocarcinoma) was statistically significant. The reactivity in adenoma was somewhat stronger with a uniform pattern, while in adenocarcinoma it was much weaker with a heterogenous pattern. 4. Intestinal type carcinomas by Lauren's criteria showed a higher expression rate and intensity than diffuse type. These results suggest that the bcl-2 expression would be found in the early phase of gastric tumorigenesis, and the expression rate and intensity would decrease according to the tumor progression.

6.A Case of Cutaneous Hemorrhagic Bullous Eruptions in Lymphoma of the Small Intestine.

Authors experienced a 59-year-old male patient with the lymphoma of the small iritestine which involved the adjoining mesenteric lymph nodes, the parietaI pleura, and the skin. The skin lesions, simulating necrotizing angiitis, were deep purple colored, asymptomatic, tense, hemorrhagic bullae with soft consistency on the both thighs and left infraorbital area. He had extranodal type, and diffuse, poorly differentiated lymphocytic lymphoma by the Rappaport classification, and stage IV by the Ann Arbor staging classification. After about 50 days of clinical manifestations, he was died mainly due to massive intestinal bleeding.

All of them were rapidly aggrevated after previous incompleted surgical management (3 cases) or postponement 01 operation (1 case). 2. Three cases of them except adenoid cystic Ca. were recurred after orbital exenteration. 3. The authors were concluded that the poor prognosis is due to its nature of malignancy and delayed radical surgical management.

9.Stereotactic Mesencephalotomy for Cancer - Related Facial Pain.

Cancer-related facial pain refractory to pharmacologic management or nondestructive means is a major indication for destructive pain surgery. Stereotactic mesencephalotomy can be a valuable procedure in the management of cancer pain involving the upper extremities or the face, with the assistance of magnetic resonance imaging (MRI) and electrophysiologic mapping. A 72-year-old man presented with a 3-year history of intractable left-sided facial pain. When pharmacologic and nondestructive measures failed to provide pain alleviation, he was reexamined and diagnosed with inoperable hard palate cancer with intracranial extension. During the concurrent chemoradiation treatment, his cancer-related facial pain was aggravated and became medically intractable. After careful consideration, MRI-based stereotactic mesencephalotomy was performed at a point 5 mm behind the posterior commissure, 6 mm lateral to and 5 mm below the intercommissural plane using a 2-mm electrode, with the temperature of the electrode raised to 80degrees C for 60 seconds. Up until now, the pain has been relatively well-controlled by intermittent intraventricular morphine injection and oral opioids, with the pain level remaining at visual analogue scale 4 or 5. Stereotactic mesencephalotomy with the use of high-resolution MRI and electrophysiologic localization is a valuable procedure in patients with cancer-related facial pain.
