1.A Case of Ocular Ischemic Syndrome.

Ocular ischemic syndrome, characterized by rubeosis iridis, anterior uveitis, retinal arterial narrowing, retinal hemorrhage, cherry red spot, and neovascularization of the disc and/ or retina, is due to anterior and posterior segment ischemia caused by carotid artery obstruction. A 67-year-old man presented a typical ocular ishemic syndrome. The patient complained marked visual disturbance of the both eyes and headache for 15 days. On slit-lamp examination, there were no abnormal findings of the cornea, iris, and lens. Funduscopic finding revealed bilateral edema of the optic disc, marked attenuation of the arteriole with focal constriction, and dilatation of the veins. Fluorescein angiography showed prolonged arm-to-retinal circulation and retinal arteriovenous transit time, multiple microaneurysms, and staining retinal vessels of the both eyes. About 1+1/2 months later, iritis, rubeosis iridis, and cataract of both eyes were noticed. In carotid angiogram the left internal carotid artery was completely occluded, and the right internal carotid artery was incompletely occluded. About 7 months later, he died of CVA.

2.Changes in Na and K Concentrations in Human Milk: at different stage of lactation.

The concentrations of Sodium and Potassium were measured by flame photometer in the human for milk obtained at clostral(1st 5days postpartum), early transitional (2nd 5days postpartum), late transitional (11th to 29th day postpartum) and mature milk period (1to 15 months postportum) from 92 healthy nursing mothers who delivered at term. The results were summerized as follows: 1) The concentrations of Na and K at different stages of lactation showed the highest value in colostrum, tended to decrease therafter and maintained the lowest and nearly constant value in mature milk(.01 2) Average Na and K concentrations(Mean+/-S.D., mEq/L) at different stages of lactation were colostrum : 20.8+/-4.47, 15.9+/-3.17, early transitional milk : 15.3+/-4.73, 14.2+/-3.03, late transitional milk : 11.7+/-4.28, 12.8+/-3.40, and mature milk : 8.1+/-3.59, 11.2+/-2.88 respectively. 3) The content of Na and K among primi-and multiparae showed the highest value in colostrum, also tended to decrease therafter, and maintained the lowest value in mature milk. No statistical significance was found between the concentrations of Na and K at different stages of lactation between primi-and multiparae(p>.05). 4) Definite inverse relationship could be established between Na and K concentrations and days of lactation(Na:r=-0.6, p<.001, K:r=0.4, p<.001). 5) The Mean Na and K concentration in late transitional milk showed the most consistent percentage decrease that in colostrum.

3.A Case of Intraspinal Dernoid Cyst in a Child

Intraspinal tumors in children are of especially interest to orthopaedic surgeons for two reasons; First, musculoskeletal abnormalities, are often treated for long periods before the correct diagnosls is suspected; Second, important orthopaedic problems are encountered in almost everychild treated for intraspinal tumor, not only during the immediate postoperative management but all through subsequent growth. We report a case presentation of the intraspinai tumor, dermoid cyst, successfully treated with operation.

4.Multiple Intracranial Dural Sinus Thrombosis associated with Polycythemia: Case Report.

Cerebral venous thrombosis is commonly related to infectious processes, trauma, neoplasm, puerperium, and the use of oral contraceptives, etc., but this case was associated with a very rare condition, "polycythemia". This case was confirmed by hematologic study, brain C-T, conventional angiography, and digital subtraction angiography(DSA). The patient was treated by venesection, the use of urokinase and dicumarol, and lumbo-peritoneal shunt(L-P shunt), etc. Post-treatment DSA showed the complete recanalization of all dural sinuses except the left transverse sinus, suggesting normal variant.

5.A Case of Chronic Actinic Dermatitis.

Chronic actinic dermatitis was recently proposed as a syndrome including photosensitive eczema, photosensitivity dermatitis, actinic reticuloid, and persistent light reactivity. It is defined on the basis of 3 criteria, including clinically persistent eczematous eruption on sun-exposed skin, histologically consistent with chronic eczema, and photobiologically decreased MED to UVB. We report herein a 61-year-old man presented with 2-year history of erythematous to brownish lichenified papules and plaques on sun-exposed areas and shown decreased minimal erythema doses to both UVB and UVA. Biopsy specimen showed consistent with chronic eczema. He was treated with topical therapies of topical steroids and intralesional injection of triamcinolone, and oral anti-histamine, with avoidance of exposure to sun.

Unilateral adsence of a pulmonary artery is frequently undiagnosed. Unless this entity is recognized, a patient with a potentially curable lesion may become inoperable. The importance of considering unilateral absence of a pulmonary artery in the diffirential diagnosis of cyanotic congenital heart disease and pulmonary hypertension has become clear. We exeperienced a case of congenital isolated unilateral absence of it. pulmonary artery confirmed by clinical features, lung scan, echocardiogram and angiogram.

8.A Case of Desmoplastic Trichilemmoma.

Desmoplastic trichilemmoma is a rare variant of trichilemmoma characterized by typical features of trichilemmoma with abundant desmoplastic stroma. We report herein a 54-year-old man presented with 2-year history of a solitary bean sized verrucous nodule on the occipital scalp. Histologically, the tumor showed a well-defined lobular growth extending from the epidermis into the dermis with abundant central desmoplastic stroma. Epithelial lobules at the margins of the neoplasm showed the typical features of trichilemmoma composed of glycogen-rich clear cells and peripheral palisading columnar cells. In contrast, at the center the cells assume a more random pattern of cords and strands traversed by the hyaline stroma, mimicking invasive carcinoma. It was diagnosed desmoplastic trichilemmoma by the distinctive histologic appearance and totally excised.

9.Cytochrome P-450, Drug Metabolizing Enzymes and Arachidonic Acid Metabolism in Pig Ocular Tissues.

The eye is a relatively isolated organ and tends to retain or accumulate compounds within itself. Unless the eye possesses a mechanism for detoxifying drugs and chemicals that are carried to ocular tissues via the circulating blood, the compounds may cause visual impairment. The purposes of this experiment was judged by drug metabolizing action by cytochrome P-450 system, distribution and effect of metabolism of arachidonic acid and distribution of lipid peroxide formation and phospholipids in the lens, cornea and choroid-retina of pig ocular tissues. The results were as follows: 1. The activities of mixed-function oxidase, cytochrome P-450 and NADPH-cytochrome C reductase were the highest in the choroid-retina compared to the lens and cornea. 2. The activities of drug metabolizing enzymes, aryl hydrocarbon hydroxylase, 7-ethoxycoumarin-o-deethy lase and benzphetamine-N-demethy lase, heme oxygenase activity, and the activity of arachidonic acid metabolizing enzyme, cyclooxygenase were the highest in the choroid-retina compared to the lens and cornea. 3. Lipid peroxide formation was the highest in the cornea compared to the lens and choroid retina. 4. In distribution of phospholipids, sphingomyelin and phosphatidylcholine were the highest in the choroid-retina compared to the lens and cornea and phosphatidylethanolamine was the highest in the cornea campa red to the lens and choroid-retina. 5. Cytochrome P-450 isoenzyme of pig ocular tissues was confirmed in the same form of MAb(Monoclonal Antibody) PCN(pregenolone-16alpha-carbonitrile) 213-1.

10.A Case of Epithelioid Angiosarcoma of the Scalp.

Angiosarcoma is a rare malignant tumor of endothelial cell origin. Histopathologically, varied differentiation may be observed even within the same tumor. Epithelioid angiosarcoma is one subset of poorly differentiated angiosarcoma which is a rare variant. We report a case of epithelioid angiosarcoma of the scalp in a 59-year-old man. The patient had a painful solitary, 2x3 cm sized, dome-shaped, firm, erythematous to purplish nodule with crust on the vertex of the scalp. Histopathologically, the tumor was chiefly composed of solid sheets of atypical epithelioid cells with prominent eosinophilic cytoplasm, a large vesicular nuclei, and occasional intracytoplasmic vacuoles. Well to moderately differentiated areas with irregular vascular channel-like structures lined by atypical hyperchromatic endothelial cells were also seen in the peripheral areas of the tumor. Immunohistochemically, the tumor cells were positive with CD31 which is sensitive marker of endothelial cell.
