2.Radiologic study of spontaneous pneumothorax

The authors reviewed serial chest films of 113 patients of spontaneous pneumothorax which were treated byclosed tube thoracotomy during the period from May, 1979 till July, 1982 in CNUH. The resuls are as follows; 1.Male was more frequently affected than female, and the sex ratio was 3.5:1. 2. 71.1 per cent of patients were over31 years of age. 3. Of the 113 cases, 51.3 per cent were on the right, 48.7 per cent on the left. 4. The mostcommon underlying pulmonary disease was pulmonary tuberculosis(39.8), the next was belb or bullar (17.7%), and thelast was pneumonia(8.8%). 5. Pneumothorax was the most common type in pulmonary tuberculosis(66.7%), andpyopneumothorax in pneumonia(80.0%). 6. Among 103 cases in complete re-expansion of collapsed lung after closedthe thoracotomy, 39.5 per cent was expanded completely within 1 week in presence of visceral and/or parietalpleural thickening, and 86.2 per cent in none of it. According to the degree of pneumothorax, the more severedegree of pneumothorax, the more delay in re-expansion times. 7. Most common chest finding after re-expansion ofcollapsed lung was pleural thickening(51.4%).

3.Radiologic study on measurement of lumbosacral angle in backache patients

The radiologic findings of lumbosacral spine and measurement of lumbosacral angle were analysed in 238 with backache and 102 without backache which were visited at Chungnam National University from March 1980 to July 1981.The measurement of lumbosacral angle was based on a method of Fergson. The results obtained were as follows; 1. The age group of 18 to 29 years was most common in backache group and the male was affected more frequently than the female with the ratio of 1.9 to 1. 2. In patients with backache, the overall mean lumbosacral angles were 44.2±2.6 degrees in male and 35.8±2.0 degrees in female. In patients without backache, in control group, the overall mean lumbosacral angles were 32.6±0.7 degrees in male and 33.4±1.4degrees in female. 3. In control group, difference of means between male and female was about 1degrees in patients with backache, the overall mean lumbosacral angles were increased about 12degrees in male and 2degrees in female than control group. In patients with and without backache, no significant difference of lumbosacral angle between the 4 age groups was present. 4. In backache group, increased lordosis was more common and increased lumbosacral angle than the decreased lordosis. 5. In backache group, lumbosacral angle of abnormal radiologic findings in lumbosacral spine was significantly increased than control group. 6. In patients with backache, radiologic findings and its lumboscral angles were alumbosacral anomaly 56 cases (23.5%): 46.9 degrees, increased lumbar lordosis 46 cases (19.2%): 48.1 degrees, osteoarthritis 44 cases(18.5%) : 40.8 degrees, decreased lumbar lordosis 30 cases (12.6%): 29.9 degrees, in order, And these radiologic findings were similar with many other authors.

Intraspinal tumors in children are of especially interest to orthopaedic surgeons for two reasons; First, musculoskeletal abnormalities, are often treated for long periods before the correct diagnosls is suspected; Second, important orthopaedic problems are encountered in almost everychild treated for intraspinal tumor, not only during the immediate postoperative management but all through subsequent growth. We report a case presentation of the intraspinai tumor, dermoid cyst, successfully treated with operation.

5.Clinical Study on Hip Fractures

Fractures of the hip are usually classified first according to their anatomic location. Isolated fractures of the lesser or greater trochanter are not common and rarely requir surgery, Femoral neck fractures, intertrochanteric fractures, and subtrochanteric fractures are common categories that usually require open reduction and internal fixation. These fractures most often occur in elderly patients. The prognosis of these three major categories of hip fractures seems to be entirely different because of their anatomic characteristics. Seventy three cases of hip fractures were experienced at the Department of Orthopedic Surgery, Eul-Ji General Hospital over the period from Jan. 1975 to Aug. 1980 and the following results were obtained. 1. Of the 73 hip fractures, femoral neck fractures were 28 cases (38.5%), intertrochanteric fractures were 23 cases (31.5%), subtrochanteric fractures 11 cases (15%), and acetabular, fractures 11 cases (15%). 2. Traffic accident was the major cause of hip fractures in our series. 42 cases (57.2%) were caused by traffic accident, whereas 27 cases (37.0%) by a fall from the height. 3. Hip fractures occured most frequently in the age group from 31 to 50 years (53.5%). Male to female ratio was 2:1. 4. Cases caused by traffic accident visited hospital earlier than the others. 5. The most common type was Garden's type IV (53.6%) in the femoral neck fractures, Boyd-Griffins type Il (47.8%) in the intertrochanteric fractures, and Fielding's type II (54.5%) in the subtrochanteric fractures. 6. Metal devices used to fix hip fractures were Knowles pin, Jewett nail plate, compression hip screw, plate and screws, and Kuntscher nail. Internal fixation was done anatomical reduction has been achieved in stable fractures, and Dimon-Hughston reduction in unstable fractures. 7. Postoperative cast immobillzation was performed in 13 cases of 21 femoral neck fractures, 15 cases of 22 intertrochanteric fractures and 6 cases of 11 subtrochanteric fractures. The average duration of immobilization was 10 to 12 weeks. 8. Incidence of avascular necrosis in femoral neck fractures was 16.7%, nonunion in intertrochanteric fractures was 11.8%, and nonunion is subtrochanteric fractures was 12.5%.

7.A Study for Diagnostic Usefulness of Computer Assisted EEG Topography.

The authors studied the diagnostic usefulness of the Topographic analysis of EEG, using Topography ststem 700 (San-ei), in evaluation of supratentorial focal cerebral lesions of 27 patients with various etiology, comparing with visual anslysis of EEG. Focal cerebral lesions, which were proven with brain C-T, were 11 cases of cerebral infarction, 6 of intracranial hematoma, 6 of cerebral gliolysis and 4 of others. The topography system displays the spatial distribution of activity in the classic delta, theta, alpha and beta frequency and computed mapping of EEG displays equipotential maps of square of roots of power spectra over each frequency band. For visual analysis of slow waves and background activity changes, Mayo classification system of EEG abnormality was used and for visual evaluation of topographic display, above system was also applied with some modification in order to compare with the data of visual analysis of conventional EEG. The results of the study were as follows; 1. While visual analysis of conventional anlysis of EEGs showed abnormality only in 13 cases (48.1%) of 27, topographic analysis showed abnormality in 22 cases (81.5%). Topographic analysis was more sensitive than than visual analysis of the EEG and topographic analysis was thought to be more sensitive in assessment of local slow waves as well as minor changes, especially slight asymmetry, of background EEG activity. 2. Topographic analysis showed higher concor dance rate (55.6%) to the brain C-T finding in lateralization of supratentorial focal cerbral lesion than that (44.4%) of visual analysis of the conventional EEG.

8.Study on the trematode parasites of the birds in Korea.

Authors carried out an investigation of the helminthic parasites of the wild birds in Korea. Most wild birds were collected from Kyoung Sang Do, Kyoung Kee Do and Seoul area from September 1970 to March 1973. A total of 70 wild birds, 19 species were examined, which comprised as follows; 10 Anas platyrhynchus, 10 Anas fulicata, 4 Anas poeciolorhyncha zonorhyncha, 8 Anser albifrone albifrone, 5 Anser cygonopsis cygnoid, 10 Aythya fuligula, 6 Gallicrex cinerea, 2 Porgana fusca, 2 Phisianum calichius korponri, 4 Sterptopilla orientalis, Podiceps cristatus, Larus crasirotatus, Malantta fusca, Garis artcus, Buetes buteo, Egretta alba, Starmus philpiens, Bombycilla granulus. Trematode parasites found from six species of the wild birds. Echinostomatidae has been found from intestine of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha, Anas fulcata. Notocatylus attenuatis found from intestine of the Podiceps cristatus, Anas fulcata. Catylurus japonicus was found from rectum of the Anas platyrhynchus platyrhynchus, Anas poeciolorhyncha zonorhyncha.

9.Expression of Apoptosis, bcl-2, and PCNA in Uterine Cervical Intraepithelial Neoplasia and Invasive Carcinoma.

This study was undertaken to know the extent of apoptosis, expression of bcl-2 and proliferating cell nuclear antigen (PCNA) in uterine cervical intraepithelial neoplasia (CIN; 15 cases) and invasive carcinoma (27 cases) and to evaluate them as a prognostic marker. Apoptosis was analysed by using the in situ apoptosis detection kit and bcl-2 and PCNA were detected by the immunohistochemical method. The results were as follows: Apoptotic indices (AI) in the invasive carcinoma (mean: 4.3) were 10-times higher than that in the CIN (mean: 0.43). Bcl-2 was expressed 60% of the cases in the dysplastic cells of the CIN II and CIN III, 33.3% of cases in the invasive carcinoma and not expressed in the CIN I except basal cells. The expression of the PCNA was increased by the grades of CIN and was strong in invasive carcinoma. The mean survival time of the patient with invasive carcinoma was significantly decreased in the higher AI index (above 4.3) than in the lower AI index (below 4.3). There was no significant correlation between the extent of apoptosis and the expression of bcl-2. According to the above results, AI are able to be used as an independent prognostic marker in the invasive cervical carcinoma, and bcl-2 and PCNA have an important role in the tumorigenesis of uterine cervical carcinoma.

10.High-Resolution CT Findings in Swyer-James Syndrome.

PURPOSE: The purpose of this study was to evaluate the high-resolution CT features of Swyer-James syndrome with special attention to the airway lesions and to determine the clinical utility of high-resolution CT compared with that of chest radiography. MATERIALS AND METHODS: In seven patients with Swyer-James syndrome, we retrospectively reviewed chest radiographs obtained during inspiration and expiration and high-resolution CT scans obtained in inspiration. The high-resolution CT appearance was evaluated and compared with that of chest radiography. RESULTS: On both chest radiographs and high-resolution CT, the affected lung volume was relatively diminished in four patients and normal in three patients. In all seven patients, chest readiographs showed hyperlucency of the lung which was unilateral in four and bilateral in three patients. Unilateral small hilum was seen in six patients and bronchiectasis was demonstrated in one patient on chest radiographs. The hyperlucent lung volume was not diminished on expiratory radiographs in all seven patients. In all patients, high-resolution CT demonstrated low attenuation regions of the lung either bilaterally(n=5) or unilaterally(n=2). Pulmonary vessels were markedly decreased in size and number in the lung parenchyma with low attenuation. Six patients had bronchiectasis on high-resolution CT, which were cylindrical or varicose in five and cystic in one. Bronchiolectasis was observed in three patients on high-resolution CT. CONCLUSION: The high-resolution CT findings are characteristic of Swyer-James syndrome. High-resolution CT is more sensitive than chest radiography in detecting regions of low attenuation and bronchiectasis and may be useful for the diagnosis of Swyer-James syndrome. Our results suggest that bronchiectasis is a frequently associated airway lesion of Swyer-James syndrome and bronchiolectasis may be associated in some cases.
