1.Postoperative Recurrence Rate of Superior-Inferior Sliding Conjunctival Flap and Mytomycin-C for the Treatment of Primary Pterygium.

One of the major problems after pterygium removal are recurrence and complications of the adjunctive therapy. The author compared the recurrence rate, interval and age of the primary pterygium after surgical treatment and with(group 1:94 eyes) or without(group 2: 101 eyes) supple-mentary mitomycin-C therapy. All of whom had been treated by one surgeon using a pterygim and subconjunctival tissue removal and superior-inferor, sliding conjunctival flap to cover the pterygium defect, In group 1, supplementary mitomycin-C was topicall used. In group 1, three pterygia(3.2%) were recurred. Recurrence were more Likey after ealy polstperative wound disruption with flap retraction(2 of 3 recurrences), relativy uoung pationts(all recurrences are under 50), and recurrence occured at a mean interval of 3.8 months after surgery. In group 2, three pterygia(3.0%) were recurred. Recurrencd were monre likely after early postoperative wound disruption with flap retraction(all recurrences). young patients(all recurrences are under 45), and recurrence occured at a mean interval of 3.1 months after surgery. Between two group no significant difference in recurrence rate, age, and interval so pterygium surgical procedure, operator and early postoperative conjunctival flap state are more important factors than adjunctive therapy in recurrence.

2.The Effect of Saponin on the Na+ - K+ Activated ATPase of Cattle Lens Epithelial Cell.

The effect of saponin on the Na+ - K+ ATPase of anterior capsule and epithelium of cattle lens has been investigated. The experiment also designed to determine the cation of saponin on the Na+ - K+ ATPase activity in anterior capsule and epithelium of cattle lens. The following results are observed. 1. The Na+ - K+ ATPase activity of cattle lens epithelial cell membrane is inhibited by low concentration of saponin, but increased by high concentration. The strongest activity showed at the dose of 50mg% saponin concentration. 2. The activating effect of saponin on the cattle epithelial cell membrane Na+ - K+ ATPase activity is inhibited by ouabain, but the stimulation of the Mg++ ATPase by high concentration of saponin is not inhibited by ouabain. 3. The activity ratio of Na+ - K+ ATPase by high concentration of saponin is increased by raising the sodium concentration but decreased by raising the potassium concentration. 4. The Na+ - K+ ATPase activity is increased by small amount of calcium but completely inhibited by over 5mM concentration of calcium.

3.The Irritancy of Hand Dishwashing Liquids on the Skin.

BACKGROUND: The regular use of soaps and synthetic detergents has been associated with skin dryness and aggravation of dermatological conditions. OBJECTIVE: Our purpose was to investigate the irritancy of 10 hand dishwashing liquids on normal skin. METHODS: The skin changes, responding to repeat patch testing with 10 hand dishwashing liquids, were measured by visual scoring. RESULTS: The results are summarized as follows; 1. Of the 16 healthy volunteers 8 had reactions. 5 women and 3 men, but there was no significant difference in irritancy between men and women. 2. The mean irritancy scores of the 10 hand dishwashing liquids ranged from 0.06 to 0.88. 3. A prominent increase in the degree of erythema in the first 24 hour patch test did not persist beyond exposure day 5, but after day 5, augmented responses of the erythema and scales, even fissuring occurred. 4. The pH of the test solutions of the hand dishwashing liquids ranged from 5.0 to 6.0, but there appeared to be no correlation between pH and irritancy in the dishwashing liquids tested. CONCLUSION: Our data suggests that regular exposure to hand dishwashing liquids may cause disturbances of the skin in a substantial proportion of individuals, and workers should use hand dishwashing liquids in lower concentrations than recommended and only for short periods with protective measures.

4.A Case of Subcutaneous fat Necrosis Associated with Pancreatitis.

We report a case of subcutaneous fat necrosis associated with pancreatitis that may be the first case in Korean hterature as for we know. The patient was a 41-year-old man wbo presented erythema nodesum-like erythematous subcufaneous nodules on both lower extremities. Histopathologic findings showed subcutaneoms focal fat necrosis and ghoat-like cells. Granular basophilic material was deposited in and around the necrotic fat cells and stained positively with von Kossa stain. The lesioas subsided gradwdly without any specific treatment, as the underlying pancreatitis was ameliorated.

5.The Expression of Keratinocyte Growth Factor mRNA in Dendritic Epidermal T Cell.

BACKGROUND: The keratinocyte growth factor(KGF) is a recently identified mitogen for epithelial cells produced by nomal stromal fibroblasts. ln the skin, KGF has been shown to stimulate keratinocyte proliferation and differentiatian. Dendritic epidermal T cells(DETC) are skin-specific members of the epithelial y 8 T-cell family that reside normally in the murine epidermis. The DETCs recognize antigen expressed by damaged or diseased neighboring keratinoctyes and consequently secrete cytokines sueh as IFN- y, lL-2, IL-4, GM-CSF. OBJECTIVE: The purpose of this study was to observe the expression of KGF mRNA in keratinocyte and DETC as well as to investigate the cytokine-mediated intercellular communication between kerati- nocyte and DETC. METHODS: Using a RT-PCR(reverse transcription-polymerase chain reaction), we examined the expression of KGF mRNA in keratinocyte and DETC, and compared the level of KGF mRNA between resting and activated DETC with Con-A (concanavalin A).

6.A Clinical Study of Breast Cancer.

BACKGROUND: Breast cancer is the most frequent malignant neoplasm among women in the Orient. This cancer is also increasing in Korea due to westernization of the living pattern, increased recognition among the general population, and improved methods for early detection. In the surgical treatment of breast cancer, cosmetic conservation of the breast has drawn the patients' attention so that conservative surgery is increasing nowadays. The purpose of this study is to describe the current status of breast cancer management at our institution and to identify the survival and the recurrence rates according to the stage and the surgical method used. METHODS: We reviewed the medical records of 150 patients diagnosed as having breast cancer on pathologic examination at the Department of General Surgery, EulJi College of Medicine, from January 1991 to December 1996 and evaluated the clinical findings, the diagnostic method, operation, and the prognosis. RESULTS: The mean age of the patients was 49.1 years, and 34.0% of the patients was in the fifth decade. A palpable mass in the breast was the most frequent clinical symptom and sign (80.7%). The interval between symptom onset and diagnosis was about 6 months in 76.0% of patients. The size of the mass was 2~5 cm in 51.3% of the patients. A modified radical mastectomy was the most frequently employed surgical method (74.0%), but conservative surgery has increased lately. Seventy-ight patients (52.0%) were in stage II. The five-ear survival rates for stages I, II, III were 100%, 82.9%, and 55.0% (p=0.00006). The five-ear survival rates for 0, 1~3, 4~9, and more than 10 axillary-ode metastases were 85.9%, 55.3%, 53.6%, and 21.1%, respectively (p=0.0007). The five-ear survival rates for breast-conserving surgery and a modified radical mastectomy, for both stage of I and II, were 90.9% and 89.3% (p=0.8114), respectively, and the recurrence rates were 17.9% and 23.8% (p=0.88). CONCLUSIONS: The authors may conclude that breast-onserving surgery may offer an effective alternative to a modified radical mastectomy for patients with stage I & II breast cancer.

7.A Statistical Observation of the Ocular Injuries.

The author analysed statistically 221 cases of the ocular injuries, among 4056 out-and-in patients who visited to the Red Cross Hospital, during the period of 1979. 6. 1. ~ 1980. 5. 31. The incidence of ocular injuries was 5.4% of all patients. The incidence was exceedingly more common in male (76.5 %) and in the age of first to second decade (56.6 %). The ocular injuries were more common in the spring (33.1 %). Subconjunctival hemorrhage is a most common in the ocular injuries (21.8 %). Small flying particles, such as sand, dust, and iron piece, were most common causative agent of ocular injuries. At the first visit, 15.8 % of all cases were markedly decreased visual acuity and 2.5 % of all cases were not improved. The prognosis of the ocular injury is good with early adeguate management of trauma.

8.Coexistence of Porokeratosis of Mibelli with Linear Porokeratosis.

We report a case of porokeratosis of Mibelli coexistence with linear porokeratosis in a 28-year old male. He had multiple discrete reddish to brownish annular plaques with peripheral keratotic ridges on the left side of the neck and axilla, and such lesions extended in a linear fashion on the left side of the upper arm and posterior aspect of the lower extremity. The cutaneous lesions started on the left neck and axilla about 6 years prior to consultation. Two years after that, new lesions developed on the left side of the upper arm and lower extremity in a linear fashion. There was no family history of similar skin disorders. A histological examination showed cornoid lamellae in the horny layer. Hypogranulosis, dyskeratotic cells and perivascular mononuclear in-filtrations were also seen beneath the cornoid lamella.

9.Changes of Water Content in Senile Cataractous Lenses.

Changes in the state of water in senile cataractous lenses at the different stages of the maturation of cataract were investigated. Senile cataracts of 34 eyes were divided into 4 groups according to degree of lens opacity. Intracapsular cataract extraction was performed and wet weight, dry weight and water content of the cataractous lenses were measured. In incipient and immature cataract, wet weight was 197.4 +/- 20.6mg, dry weight 66.2 +/- 8.2mg and water content 130.6 +/- 15.2mg(66.2%). In mature and hyper mature cataracts, wet weight was 240.8 +/- 39.3mg, dry weight 53.4 +/- 8.3mg and water content 176.3 +/- 37.5mg(73.2%). Wet weight and water content were significantly increased and dry weight was significantly decreased with the maturation of cataract. Sexual differences of wet weight, dry weight and water content were not significant.

10.The Change of the Intraocular Pressure and Blood Pressure After Exercise.

The object of this study was to analyze the effects of exercise on intraocular pressure, blood pressure, and pulse rate. This study was performed in 60 eyes of 30 patients. The intraocular pressure, blood pressure, and pulse rate were taken before exercise, and after exercise for 5 minutes. The change of the intraocular pressure and blood pressure before and after exercise were compared using the t-test. The results of analysis were summarized as follows; 1. The mean of systolic blood pressure was 118.2 +/- 8.2 mmHg before exercise, 145.2 +/- 11.5mmHg after exercise. This change was statistically significant. 2. The mean of diastolic blood pressure was 77 +/- 6.1 mmHg before exercise, 90 +/- 8.3mmHg after exercise. This change was statistically significant. 3. The mean of pulse rate was 73 +/- 11.8/min. before exercise 103.3 +/- 14.4/min. after exercise. This change was statistically significant. 4. The mean of intraocular pressure was 17.41 +/- 1.6 mmHg in right eye 17.80 +/- 1.6rnmHg in left eye respectively before exercise, and 17.34 +/- 1.6 mmHg in right eye 17.64 +/- 1.6 mmHg in left eye respectively after exercise. This change was statistically insignificant. With the above results, we found no clear association between change of intraocular pressure and change of blood pressure after exercise.
