1.Ultrasound Guided Biopsy of Malignant Focal Liver Lesions: Comparison of Automated Gun Biopsy with Fine Needle Aspiration Biopsy.

PURPOSE: To compare the efficacy and safety of ultrasound guided automated gun biopsy with fine needle aspiration biopsy of focal liver lesions. MATERIALS AND METHODS: We performed 32 automated gun(19.5G Autovac Biopsy Needle) biopsies and 27 fine needle aspiration(22G Westcott Style Biopsy Needle) biopsies in 48 patients with malignant focal liver lesion under the guidance of ultrasound. We compared the "positive for malignancy rate" and "pathologic diagnosis rate" of both methods with final diagnosis, and the rate of complications. RESULTS: The "positive for maiignancy rate" of automated gun biopsy and needle aspiration are 84.4%(27/32) and 66.7%(18/27) respectively(p=0.11). The "pathologic diagnosis rate" are 84.4%(27/32) and 55.6%(15/27) with significant statistical difference(p<0.05). And there is no difference of the rates of significant complications. CONCLUSION: When compared with the fine needle aspiration biopsy, ultrasound-guided automated gun biopsy is safe and more effective method for malignant focal liver lesion.

3.Intracranial meningeal Masson's hemangioma: CT and angiographic features

Masson's hemangioma is a rare benign vascualr condition with a papillary intravascular endothelial proliferation which may appear either as aprimary form or as a secondary form in a pre-existing vascular process.CT and angiographic features of 2 cases with Masson's hemangioma were presented. Both of them were locatedextra-axially in the posterior fossa. CT findings were not specific in both cases: One showed homogeneously enhancing mass, simulating meningioma. And the other demonstrated a multiolocular rim enhancing mass. However, the angiographic features were rather characteristic; Both cases showed persistent vascular poolings of contrast mediawhich were supplied form the meningeal vessels. Angiographic differential diagnosis of similar lesions in theposterior fossa is discussed.

4.Sonographic features of portal vein thrombosis

57 cases of portal vein thrombosis detected by ultrasonography were analized. The results were as follows: 1.Most common cause was hepatocellular carcinoma, showing 49 out of 57 cases(86%). 2. Sonographic findings revealedechogenic thrombus in the dilated portal vein. Echogenecity of thrombus was similar to the surrounding liverparenchyma, Thrombus was mainly located in the intrahepatic branch and main portal vein. 3. The location ofthrombus in hepatocellular carcinoma was related to the location of tumor, and frequently extended to the mainportal vein(68.9%). Incidence of thrombosis in hepatocellular carcinoma revealed 49 out of 157 cases, butprospective study showed 42 out of 84 cases(50%). Thrombosis was more common in large sized ill-defined diffuseinfiltrative hepatocellular carcinoma. 4. CT(only 15 cases analyzed) findings revealed low density in the portalvein in all the cases and periportal enhancement in 10 cases on post-enhanced scan. 5. Collateral circulationswere largely detected at the porta hepatis and gallblader fossa. The detection rate was lower on theultrasonography than on the CT or angiography.

5.Differentiation of amebic versus pyogenic liver abscess with US and CT.

To differentiate amebic and pyogenic liver abscesses, sonography(US) and CT findings of 21 amebic and 22 pyogenic liver abscesses were reviewed retrospectively. US was performed in all cases and CT in 15 cases of each group. Eleven cases of amebic abscesses had well defined margin which was observed in 10 cases of pyogenic abscessed. Amebic abscesses showed hyperechoic wall in 7 cases and calcification in 4 cases. Ten cases of pyogenic abscesses had mearby intrahepatic duct dilatation which was observed in 2 cases of amebic abscesses. Pneumobilia was seen in 6 cases of pyogenic abscesses. US findings of clear margin and hyperechoic wall, and CT findings of intrahepatic duct dilatation, pneumobilia, and calcification were helpful in differentation of amebic pyogenic liver abscesses.

6.A Case of Renal Hemangiopericytoma.

Hemangiopericytoma of the kidney is extremely rare, which is very much resembled a renal cell carcinoma. It tends to grow slowly but occasionally had demonstrated rapid infiltration and distant metastasis. The incidence and degree of its malignancy appears to be low. Herein we report a case of hemangiopericytoma which was developed from left kidney in 42 old man and treated with radical nephrectomy.

Magnetic Resonance Imaging(MRI) with 2.0 Tesla superconductive magnet developed by Korea Advanced Institute ofScience was performed in 25 patients with various abdominal diseases and compared with x-ray CT. MRI was obtainedwith spin echo technique using a variety of pulse sequence and various slice orientation including axial, sgittaland coronal section in order to evaluate the diagnostic value, limitation and to determine the optimal pulsesequency in various abdominal diseases. MRI demonstrated the capability of detecting the lesions shown on CT inall cases and also detected one case of diffuse hepatocellular carcinoma which was not seen on CT. MRI showedcapability of differentiation of various liver mass including hepatocellular carcinoma, hemangnioma and simplecyst. MRI showed better anatomical resolution of tumor in retroperitoneum and pelvis, however CT delineatedalimentary tract disease better than MRI did.

8.Comparision of Inclusion Technique with Open Technique in Patients with Bentall Procedures.

To understand the clinical results of aortic root replacement with either inclusion or open technique, we analysed 53 patients who underwent replacement of the aortic root with composite graft between October,1980, and May, 1995. Annuloaortic ectasia was the most common indication for operation(29 patients), follwed by aortic dissection(22 patients). Among 53 patients, 19(35%) had Marfan syndrome. Three patients died during hospitalization (Mortality: 5.5%). The follow up was possible in 48 patients(Follow-up rate; 94%,mean duration;37 months). The actuarial survival rate at 24 months was 95% in open technique group, and 87% in inclusion technique group. Late complications developed in 10 patients. Dissecting aneurysm in the remaining aorta was noted in 3 patients with inclusion technique, and a pseudoaneurysm from coronary artery anastomosis site developed in a patient with inclusion technique. In conclusion, there was no statistical differences in survival for 24 months between inclusion technique and open technique group. But late problems in the remaining aorta or death from unknown cause occurred with moderate frequency ; careful follow-up after aortic root replacement thought to be important for long term survival.

9.Combination Treatment of Osmidrosis by Liposuction and Endoscopic Shaver.

Many surgical procedures have been introduced for treatment of axillary osmidrosis, but some complications, such as symptom recurrence, hematoma, seroma, delayed wound healing, skin flap necrosis and scarring, are not solved yet. Recently introduced liposuction method has several advantages over previous surgical methods, such as increased skin flap vascularity, minimal scarring and bleeding, etc, but the sympton recurrence rate has been reported 10-30%. Liposuction method could remove apocrine glands in subcutaneous tissue effectively, but the glands firmly seated in subdermis remain as the cause of sympton recurrence. For this reason, we employed combination treatment of liposuction and endoscopic subdermal shaving method. From March 1997 to February 2001, a total 71 patients (24 men and 47 women) had been treated with this procedure for bilateral axillary osmidrosis. In our procedure, apocrine glands in subcutaneous tissue were removed by liposuction method and the glands in subdermis were removed by using 4.5 mm incisor blade of endoscopic shaver. Follow-up evaluation period was from 3months to 24 months, and the results were satisfactory. No one suffered from sympton recurrence. Postoperative scar was negligible (below 0.5 cm) and other complications were minimal.

10.A Study on Comparison of the Metabolites Related with Working Methods and Immunity of the Toluene diisocyanate-Exposed Workers.

Following recent advanced industrialization. the amount of polyurethane to use as thermal insulating materials, upholstery mattresses and packing materials in automotive and furniture industry is increasing world-widely, and the number of polyurethane-producing worker will be increased. Because the numerous organic solvents are used in polyurethane-producing factory, the workers in this work site is exposed to many organic solvents. Of the organic solvents. Toluene Diisocyanate(TDI) has many hazardous effects to human. The effects of TDI on human are the irritation to respiratory mucosa and gastrointestinal symptoms. Conjunctival irritation, dermal inflammation (redness, pain, vesicular formation) and gastrointestinal symptom(nausea, vomiting, abdominal pain) are reported just after short-term exposure of TDI. TDI is known to give rise to bronchial asthma, as the immune disorder. And because of strongly volatile characteristics of TDI, it is suggested as a more injurious material to human health, especially human immune system, than other organic solvents. Bronchial asthma inducing mechanism of TDI is not clearly known, but on the analogy of TDI-induced symptoms and recent studies, early-onset asthma is type I hypersensitivity reaction mediated by immunoglobulin E(IgE), and late-onset asthma is maybe type III hypersensitivity reaction by circulating IgG. And we know that the complicated human immune function is likely to move in such that mechanisms, there are not studies on immune indices evaluating the bronchial asthma-related immune function. The evaluation of change patterns of humoral immunity including IgE and IgG and cellular immunity including T-helper cell, T-suppressor cell and T-cytotoxic cell will be helpful to evaluate exposure degrees and prognosis in TDI-exposed workers. Because TDA(toluene diamine) as a biological exposure index of TDI becomes the focus of interest, we know that a study on the correlation between urinary TDA and air TDI and immunological indices will make a contribution to biological effect monitoring indicies. We examined human immunity indicators such as WBC. %Lymph (percentile of Lymphocyte in WBC). %T-cell(percentile of T-lymphocyte in total lymphocyte). CD4, CD8, C3, C4, IgA, IgG, IgM, IgE in peripheral blood to evaluate the health hazard of the TDI-exposed workers. And we examined TDA to evaluate correlation between exposure and effect. Total 90 subjects was selected, 45 workers who worked in the polyurethane-producing factories as an exposed group, and 45 cases who were office workers(10 cases), other blue collors(27 cases), and medical college students(8 cases) as a control group. And the results were as follows ; 1. The logarithm of IgE -Log10(IgE)+/-SD- in peripheral blood of a exposed group was significantly higher than a control group, 2 22+/-.62 in case group compared with 1.98+/-.53 in control group.(p<0.05) 2. IgA and IgM in the polyurethane-producing workers were 261.02+/-3.12 mg/dl, 151.97+/-9.64 mg/dl, respectively, and 292.77+/-00.45, 179.17+/-00.78 in control group. IgA and IgM was slightly lower in polyurethane-producing group than control (p>0.05). 3. WBC, %Lymph, %T-cell, C3, C4, CD4, CD8, CD4/CDB ratio and IgG in case group were 6,391.1 ea/ml, 37.53%, 59.54%, 76.68 mg/dl, 30.54 mg/dl, 0.76x10(9) ea/L, 0.63x10(9) ea/L, 1.39, and 1606.29 mg/dl, respectively, and 6,974.7 ea/ml, 35.12%, 59.64%, 71.95 mg/dl, 33.94 mg/dl, 0.80x109 ea/L, 0.61x10(9) ea/L, 1.39, and 1581.51 mg/dl in control group. There was no statistical sinificance between two groups. (p>0.05) 4. In the comparison of each other companies, average of individual urinary TDA in polyurethane paint manufacturing companies is higher than that of polyurethane sponge foaming companies. And, the concentration of 2,6-TDA which is a metabolite of well-vaporized 2,6-TDI is higher than that of 2,4-TDA in the polyurethane sponge foaming companies. But, the concentration of 2,4-TDA which is a metabolite of illvaporized but well skin-absorbed 2,4-TDI is higher in polyurethane paint manufactures. 5. There were no statistical significance in the correlations between individual urinary TDA and immunologic indices.
