2.Two Cases of Lvmphomatoid Papulosis.

Lymphomatoid papulosis(LyP) is a chronic recurrent dermatosi characterized by involuting and recurring papules, plaques, and nodules showing histologic feaurs suggesting rnalignant lymphoma. On histologic ground, it is divided into two types. type A and type B. In type A, large atypical lymphocytes are the main cellular cornponent and most of these express CD 30(Ki-1) antigen, which has been p!reviously thought to be specific for Read Sternberg cells of Hodgkins disease. In type B, cerebriform mononuclear lymphocytes sirr la to those in mycosis fungoides predominates. We report two cases of lymphomatoid papulosis, type A and type B, respectively. The large atypical cells in type A LyP expressed Ki 1 antigen. Both case were treated with PUVA and the outcome of the treatment has been fair.

3.A Clinical study of the High Tibial Osteotomy in Osteoarthritic Knees with Varus Deformity

During the period of 5 years and 5 months, from January 1979 to May 1984, we treated 27 cases(17 patients) of osteoarthritis with genu varum deformity by high tibial osteotomy. The results were as follows: l. In the 27 cases of the high tibial osteotomy, 24 cases were female and 3 cases were male. 2. Of the etiology of the osteoarthritis with genu varum, 14 female patients had primary one and 3 male patient knees were related with infection(2 patients) and trauma (1 patient). 3. Preoperative varus angles, ranging from 0° to 20°, 6.4° in an average and postoperative valgus angles, ranging from 5° to 15°, 8° in an averge. 4. In a review of the high tibial osteotomy, good and fair results were noted in 26 cases (96.3%) of the knees after average 2 years of follow-up. 5. The high tibial osteotomy is found to be useful method in the treatrnent of the osteoarthritic knee with varus deformity.

5.A Clinical Analysis of Supination-outward-rotation Injuries of the Ankle

78 cases of ankle fracture are experienced and especially 25 cases of supination-outward rotation injuries treated during the period from January 1959 to October 1970 were analyed. 1) 17 cases of male and 8 cases of female, and 11 caces in right side and remaider in left side. 2) According to the Lauge-Hansens classification, 5 cases in type II, and 20 cases in type IV. 3) Conservative treatment was given in 8 cases and open reduction in 17 cases. 4) A comparison has been made between functional results treated by conservative treatment and open reduction; satisfactory results were obtained in 3 cases of type II and I case of type IV by conservative treatment. I case of type II and 14 cases of type IV by open reduction. 5) Time of complete healing by bony union were 7. 8 weeks in conservative treatment and 9. 3 weeks in open reduction. 6) Wound infection, necrosis of wound edge and pseudoarthrosis were noted as post-operative complications.

6.Experiences on Idiopathic Aseptic necrosis of Femoral Head

6 cases of idopathic asceptic necrosis of femoral head were experienced at the Department of Orthopedic Surgery. Age distribution ranged from 19 to 58 years and the ratio of sex was 3:3. Two of them had bilateral involvement of necrosis of femoral head. Two of them were primarily treated by means of arthrotomy, joint debridement and drilling of the necrotic area. Symptoms only temporarily relieved and required further surgical procedure. As a whole, arthrodesis in 2, cup arthroplasty in 1, Austin-Moor hip prosthesis in 2 and total hip prosthesis, McKee-Farrar type, in 1 case were performed. Four of the mobile operation of the hip restored satisfactory range of hip motions despite the types of surgery.

9.A Clinical Study on Fractures of the Tibial Plateau

A tibial plateau fracture was first described in the literature by Thamhayn, in 1852. It is a fracture of the proximal end of the tibia, usually involving an articular surface. The fracture is regarded as serious as it involves a major weight bearing joint and loss of function results in a high percentage disability of the extremity. Of the tibial plateau fractures treated at Hanyang University Hospital between June 1972 and August 1979, the 44 cases that could be followed for three months to two years have been analysed according to its cause, classification, treatment and result. Open reduction is indicated in anyone of or a combination of the following deformities as shown by roentgenograms: 1. a vertical depression in the plateau of greater than eight milimeters; 2. a horizontal displacement of the lateral or medial condyle of greater than four milimeters; 3. valgus or varus instability of the knee one or more milimeters in excess of that of the uninjured knee. Manipulation under anesthesia with skeletal traction is indicated in severe comminuted fracture that it is difficult to reduce with internal fixation, and open fracture that it is inevitable to delay operation. Plaster immobilization is indicated in the remaining situations except those of open reduction and skeletal traction. Anatomical considerations and a clinical end-result study of the 44 fractures emphasize the vulner-ability of the lateral tibial plateau to a valgus force on the extended knee. Among the 44 cases twenty-two (50 percent) were treated by plaster immobilization, 2 (4.5 percent) by skeletal traction, and 20 (45.5 percent) by operation. Thirty-nine cases (88.6 percent) out of 44 revealed the rating of “ACCEPTABLE” according to Hohl and Luck criteria.

10.A Case of Unsuspected Malignant Melanoma of the Choroid.

The authors presented a case of malignant melanoma of the choroid which had been unsuspected clinically. The patient, 38-year-old female, came to Korea General Hospital on Dec. 5-1973 because of blind painful eye and white pupil in the left side. And she was told at local clinic that she had retinal detachment in that eye about 3 years ago. We thought of this case as secondary glaucoma due to swollen lens behind at first. Therefore cryoextraction of the lens was performed for relieving the ocular pain, on Dec. 5-1973. Increased I.O.P., however, had been persisted and ocular pain had not been improved until the patient entered this hospital again on Jan. 25-1974. Enucleation was performed and choroidal malignant melanoma (spindle type) was proven by histopathological study. The patient was instructive to us that we should always remind the possibilities of intraocular tumor behind the opaque media, in cases of unilateral glaucomatous blind eye with shallow anterior chamber.
