2.Medial Approach of Supracondylar Fracture of the Humerus in Children

The Supracondylar fracture of the humerus is the most common elbow fracture in children. In general, accurate anatomic reductio, the least regional trauma as possible and the maintenance of the reduction are necessary to obtain excellent results. Lateral and posterior approach is commonly used method for the treatment of supracondylar fracture of the humerus in children. But lateral and posterior approach has some troubles in reduction of fracture and Kirschner wire fixation. From March 1988 to February 1993, seventeen supracondylar fractures of the humerus were treated by means of medial approach and followed from 8 monts to 13 months with an average 10 months. There were some advantages in medial approach. Reduction was easy and ulnar nerve was not damaged by medial approach at insertion of Kirschner wire and no more another incision. The results obtained are as follows. Postoperative vascular impairment or Volkmann's ischemia was not complicated and neurologic deficits accompanied with injury were all recovered completely. According to Mitchell and Adams' criteria, all had satisfactory results.

6.Gottron's Acrogeria Syndrome: a case report.

Gottron's acrogeria syndrome is a rare genetic disease of unknown etiology with some features suggestive of accelerated aging. The typical clinical presentation consists of premature aging of the skin with atrophy of the cutis and the subcutis. After a brief review of the literature available on this syndrome, we report a case of Gottron s acrogeria syndrome in a 19 year-old man whose right rectus femoris muscle had a fibrotic contracture.

7.A Clinical Study of Diastasis of the Ankle Joint

Diastasis of the ankle joint was associated with extensive ligamentous rupture and resulted from external rotation or abduction injuries at the ankle. It might occur without associated fracture but was more frequently seen in cases of bimalleolar fracture in which there was fracture of the medial malleolus or rupture of the deltoid ligament with fracture of the lower third of the fibula. Thirty patients of diastasis of the ankle joint were admitted and treated by internal fixation with screw and bolt from January 1978 to December 1981. The following results were obtained: 1. The main cause of the injuries was falling from a height and the other causes were slipping down and sports injury in order. 2. The most common type by Lauge-Hansen classification was supination-external rotation type (20 cases, 67%). 3. Partial diastasis of the ankle joint or anterior type was 14 cases (47%) and complete diastasis or total type was 16 cases (53%). 4. In all cases, mixed oblique fibular fracture and medial instability were seen. 5. The good results were achieved in 24 patients (80%).

8.Stereotactic Surgery of Neurocysticercosis.

The authors had performed stereotactic surgery to diagnose and treat 2 cases of neurocysticercosis under the local anethesia. Generally, such the open surgery need a general anethesia which may increase the chance of brain parenchyml injury. However, the stereotactic surgery has the advantage of being an easy and precise procedure under local anethesia. The authors reviewed the literature and discussed the stereotactic surgery in the diagnosis, and treatment of neurocysticercosis.

9.The Normal Values of Optic Disc Parameters According to the Optic Disc Size.

We analysed the optic nerve head of 151 normal eyes with 8 confocal scanning laser ophthalmoscope (TopSSTM, Laser Diagnostic Technologies, Inc.), and obtained the normal values of each parameters. As the optic disc size increases, the cup shape, effective area, 1/2 depth area, C/D ratio, neuroretinal rim area, volume above, volume below, and 1/2 depth volume also increase. Therefore we obtained the normal values of these parameters according to the optic disc size. The entire subjects were divided into four groups according to the disc area, each group of 1. 5mm2-2.0mm2, 2.0mm2-2.5mm2, 2.5mm2-3.0mm2, and 3.0mm2-3.5mm2`, respectively. The normal values of other parameters such as contour variation, rnean contour depth, average depth, maximum depth, average slope, and maximum slope which are not affected by optic disc size were obtained without grouping. This may minimize the error in evaluating the optic disc parameters and may be useful in the early detection of glaucomatous optic disc changes.
