1.Refractive state in Adult with Normal Visual Acuity.

Visual acuity refers to the spatial limit of visual discrimination. It is surely the single most significant measure of functional integrity of the eye. Normal visual acuity can be represented in hyperopia of less than 2 diopters or astigmatism of less than 1 diopter. The purpose of this study was to observe the prevalence, type and degree of of refractive errors in 338 eyes of 40-69 years of age with normal visual acuity(20/20 or more). The results were as follows: 1. In objective manifest refraction, 246 eyes(72.8%) had refractive error such as hyperopia(110 eyes), simple hyperopic astgmatism(61 eyes),compound hyperopic astigmatism(57 eyes) and simple myopic astigmatism(18 eyes). 2. In subjective manifest refraction, 132 eyes(39.1%) had refractive error such as hyperopia(51 eyes), simple hyperopic astigmatism(44 eyes), simple myopic astigmatism(24 eyes) and compound hyperopic astigmatism(13 eyes). 3. As to the type of astigmatism, 71 of 81 astigmatism were "against the rule" and the rest(10 eyes) "with the rule". Degree of astigmatism was less than 1.00 diopter. 4. A verage degree of refractive errors was 0.43 diopter.

3.The Effect of Augmentation with The Synthetic Polyester Ligament in Aculte Tear of The Anterior Cruciate Ligament Preliminary report

The development of the arthroscopic surgery technique presented a great method in the reconstruction of anterior cruciate ligament. There were many debates about the timing of reconstruction in the acute anterior cruciate ligament injury. However, it was regarded true that primary reconstruction in the acute phase have higher complication rate of arthrofibrosis than in the chronic phase. Many authors recommended delay of reconstruction at least 3 weeks in acute tear of anterior cruciate ligament. In contrast, some surgeons have tried to suture the ruptured anterior cruciate ligament accompanied by 'over-the-top' augmentation with the hamstring tendon or the artificial ligament arthroscopically. Since Jan. 1992, the authors have implanted the synthetic polyester ligament(ABC ligament; Surgicraft, U.K.) into the substance of ruptured ligament proper and fixed at the 'over-the-top' position without any procedure for repair of the ruptured anterior cruciate ligament. Our indication for this surgery is only acute rupture of anterior cruciate ligament with good stump condition. We report the clinical results of 22 cases at 20.3 months follow-up(mean) preliminarily. l. On measurement of pre-operative and post-operative differences between the normal and the affected side by Telos stress X-ray study and KT-2000 arthrometer. Differences were markedly improved from 5.6mm(mean: pre-op) to 1.9mm(mean: post-op) on Telos stress X-ray study(on 15 Kp stress) and both knee showed minimal differences on KT-2000 study(2.2mm on 9Kp, 2.5mm on maximum stress) at the time of follow-up. 2. The average Lysholm score was 82. By Clancy criteria, the 20 cases(91%) showed good and excellent results. 3. Second look arthroscopy was available in three cases. In two cases, there were marked fibrous proliferation around the ligament. The stability felt good on probing. We could observe healing-like appearance of ruptured ACL with minimal fibrous tissue proliferation in one another case.

4.Arthroscopy-assisted Reduction and Fixation in Fracture of Proximal tibial Condyle

In the treatment of proximal tibial condyle fracture, it should be considered that it can often accompany injuries of the collateral ligament, both cruciate ligament, and the menisci of the knee. Moreover it is necessary to restore anatomical congruity of the articular surface accurately. We performed reduction of the depressed articular fragment under monitoring of arthroscopy and fixed with cannulated cancellous screws and accompanied by bone graft under the control of image intensifier in 2 cases of Type I(Pure cleavage) and 3 cases of type II(cleavage with depression) fractures. The results were very satisfactory. Arthroscopy assisted reduction and fixation are very useful method in the treatment of proximal tibial condyle fracture because of (1) the minimized operative morbidity, (2) the ability to evaluate and manage associated intraarticular pathology simultaneously, (3) the rapid rehabilitation, (4) the achievement of good reduction comparable to open reduction.

Owing to the advancement of imaging techniques which include the CT scan, it became easier to evaluate fracture patterns of calcaneal fractures accurately. Moreover, it is possible to obtain good results with operative treatment as a consequence of the development of good operative equipment and new operative technique. In 1988, Regazzoni and Benirschke in 1990, recommended L-shaped extensive lateral approach for calcaneus which provide extensive exposure of calcaneus and so allow easier reduction and fixation. We carried out L-shaped extensive lateral approach in 11 cases from June, 1993 to April, 1994. This approach did not produce any skin problems and allowed excellent anatomical reduction and fixation. But we experienced some severe causalgia on the heel region in several cases. We tried to analyse the cause of pain and concluded that it was the damage to the lateral calcaneal branch of the sural nerve. We are reporting the problems of tbis approach.

6.Sonographic features of portal vein thrombosis

57 cases of portal vein thrombosis detected by ultrasonography were analized. The results were as follows: 1.Most common cause was hepatocellular carcinoma, showing 49 out of 57 cases(86%). 2. Sonographic findings revealedechogenic thrombus in the dilated portal vein. Echogenecity of thrombus was similar to the surrounding liverparenchyma, Thrombus was mainly located in the intrahepatic branch and main portal vein. 3. The location ofthrombus in hepatocellular carcinoma was related to the location of tumor, and frequently extended to the mainportal vein(68.9%). Incidence of thrombosis in hepatocellular carcinoma revealed 49 out of 157 cases, butprospective study showed 42 out of 84 cases(50%). Thrombosis was more common in large sized ill-defined diffuseinfiltrative hepatocellular carcinoma. 4. CT(only 15 cases analyzed) findings revealed low density in the portalvein in all the cases and periportal enhancement in 10 cases on post-enhanced scan. 5. Collateral circulationswere largely detected at the porta hepatis and gallblader fossa. The detection rate was lower on theultrasonography than on the CT or angiography.

8.A Comparison of Traditional Anterior Colporrhaphy and Cystocele Repair with Monofilament Polypropylene Mesh Repair (Gynemesh PSTM).

PURPOSE: Traditional anterior colporrhaphy repair can have a high recurrence rate. Therefore, the use of synthetic mesh to reinforce the anterior vaginal wall support has been proposed to prevent recurrence. The objective of our study was to compare the anatomic recurrence rates in patients that underwent the traditional anterior colporrhaphy with patients who add a monofilament polypropylene mesh repair(Gynemesh PSTM). MATERIALS AND METHODS: We reviewed the charts of patients who underwent transvaginal cystocele repair between January 2003 and June 2006. All patients had a physical examination and staging of the prolapse; the International Continence Society(ICS) system was used for the staging. Mid urethral sling operations were performed when stress incontinences was an associated feature. An anatomic recurrence was defined as an ICS stage 2 or greater anterior prolapse on the last recorded physical examination. The subjective patient satisfaction was also recorded. RESULTS: Seventy-one patients were available for study with a mean follow- up of 18.4 months. Thirty-eight patients underwent traditional anterior colporrhaphy, while 33 patients underwent cystocele repair using the monofilament polypropylene mesh(Gynemesh PSTM). Among the 71 patients, 9(12.7%) had recurrence of the cystocele. Based on the type of repair, 21.1%(8/38) of the patients with traditional repair had a recurrence compared to 3.0%(1/33) of the patients with the polypropylene mesh repair(p=0.03). With regard to satisfaction, 76.3%(29/38) of the patients with anterior colporrhaphy and 97.0%(32/33) with the mesh repair were satisfied with the outcome of the surgery(p=0.02). One patient had erosion of the mesh, at the midline, in front of the vaginal scar, at 12 months after the procedure. CONCLUSIONS: Our results suggest that the monofilament polypropylene mesh(Gynemesh PSTM) repairs improved the outcomes of anterior colporrhaphy with regard to the prevention of recurrent cystoceles.

9.A Case of Huge Tentorial Meningioma: Case Report.

10.The relationship between alcohol consumption and serum liver enzymes in men.

BACKGROUND: It is well known that heavy alcohol consumption may result in various lesions on multiple organs and tissue, especially liver. The aim of this study was to measure prevalence of raised liver derived enzymes in healthy men and possible association with alcohol consumption, smoking and body mass index. METHODS: All subjects were 450 men who received Adult Health Examination at Seoul Hospital of Konkuk medical center from May, 1998 to October, 1998. We carried out telephone survey and analyzed 360 men, excluding men with HBs Ag(+), hepatic problems within 6 months, medication drug that could change serum activity of hepatic enzyme or joints, myscles, cardiac problem and non responder to telephone survey. We examined prevalence of raised liver enzymes and its possible association with alcohol consumption, smoking, and body mass index, applying logistic regression analysis. RESULTS: In 20.3% of study subjects, we found raised levels of lver derived enzymes. If the alcohol consumption was higher than 270 g/week, the odds ratio(OR) for raised liver enzymes, adjusted for body mass index and smoking, increased ; serum gamma glutamyltransferase(gamma GTP)(OR: 9.48), serum aspartate aminotransferase(AST)(OR: 13.09), and serum alanine aminotransferase(ALT)(OR: 7.40). Smoking was not associated with the risk of raised liver enzymes and obesity(body mass index 25 kg/m2) showed a positive association with two enzymes; gamma GTP(OR: 1.92) and ALT(OR: 2.08). CONCLUSION: If alcohol consumption was higher than 270 g/week, the risk of raised liver enzymes increased dramatically and gamma GTP, AST were shown more sensitive than ALT in alcoholic liver disease.
