1.A Case of Appendage Tumor Combined with Eccrine Poroma , Clear Cell Hidradenoma and Syringocystadenoma Papilliferum as a Single Tumor.

Because most of the benign skin appendage tumors arise from pluripotential cells that form during adult life and possess the potentiality of differentiating in more than one direction, combinations of several tumor types can occur. We report a case of combined with eccrine poroma, clear cell hidradenoma, and syringocystadenoma papilliferum as a single tumor in a 41-year-old male who had suffered from a tumor on posterior neck for 5 years. The tumor was a relatively oval shaped pedunculated, and rubbery hard mass, measuring 3.5*3.0* 1.3cm in size. The outer surface was variegated and covered with hair. The diagnosis was confirmed by histopathological findings and surgical excision was done.

2.Texture Analysis of Supraspinatus Ultrasound Image for Computer Aided Diagnostic System.

OBJECTIVES: In this paper, we proposed an algorithm for recognizing a rotator cuff supraspinatus tendon tear using a texture analysis based on a histogram, gray level co-occurrence matrix (GLCM), and gray level run length matrix (GLRLM). METHODS: First, we applied a total of 57 features (5 first order descriptors, 40 GLCM features, and 12 GLRLM features) to each rotator cuff region of interest. Our results show that first order statistics (mean, skewness, entropy, energy, smoothness), GLCM (correlation, contrast, energy, entropy, difference entropy, homogeneity, maximum probability, sum average, sum entropy), and GLRLM features are helpful to distinguish a normal supraspinatus tendon and an abnormal supraspinatus tendon. The statistical significance of these features is verified using a t-test. The support vector machine classification showed accuracy using feature combinations. Support Vector Machine offers good performance with a small amount of training data. Sensitivity, specificity, and accuracy are used to evaluate performance of a classification test. RESULTS: From the results, first order statics features and GLCM and GLRLM features afford 95%, 85%, and 100% accuracy, respectively. First order statistics and GLCM and GLRLM features in combination provided 100% accuracy. Combinations that include GLRLM features had high accuracy. GLRLM features were confirmed as highly accurate features for classified normal and abnormal. CONCLUSIONS: This algorithm will be helpful to diagnose supraspinatus tendon tear on ultrasound images.

3.Ultrasonography in Neuromuscular Disorder.

High-resolution (HR) ultrasound, which has been progressing continuously in technology, has improved in aspect of spatial and contrast resolution. The HR ultrasonography is a noninvasive, readily applicable imaging technique, which could get static and dynamic image in real-time for various neuromuscular disorders, especially in entrapment neuropathy. It is also a reliable tool to detect dynamic muscle movements such as fasciculation as well as muscle atrophy in chronic myopathies or neuropathies. Although reliability of the HR ultrasonography has not been investigated in large series of patients, different neuromuscular disorders tend to show specific changes on the ultrasound, which can be helpful in differential diagnosis. The HR ultrasonography is an ideal tool for the clinical and research investigation of neuromuscular system complementary to electrodiagnostic studies. This review briefly describes applicability for various neuromuscular disorders with previous study results and the technical aspects of ultrasound and its physical principles.

Background: The On-X valve was recently introduced. It was the aim of this study to assess the safety and feasibility from the data derived from 28 patients who underwent aortic and/or mitral valve replacement with this prosthesis in National Medical Center. 5.The Comparison of Analgesic and Hemodynamic effects in Different Nitrous Oxide Concentration.

BACKGROUND: Nitrous oxide is a safe analgesics which has analgesic and sedative effects without loss of consciousness. The advantage of Nitrous oxide is that it has a rapid onset time and that we can modulate duration of its action. The purpose of this experiment is to obtain the safe and effective concentration of Nitrous oxide in Emergency Department as use of analgesics and sedative drugs. METHODS: Twenty volunteers were investigated according to double blind method. Nitrous oxide was administrated to volunteer subjects at different concentrations ; 33%, 50%, 67% each. Then we evaluated the Pain score, Blood pressure, Heart rate, Respiratory rate, Oxygen saturation and its side effects. RESULTS: Pain Score were 8.4+/-2.1 at 33%, 6.1+/-1.5 at 50%, and 3.65+/-2.2 at 67% of Nitrous oxide. Pain Score is decreased significantly following administration of Nitrous oxide at concentration of 50%, 67%(P<0.05). There was no difference between preadministration group and postadministration group. While no remarkable change in systolic blood pressure, heart rate, and oxygen saturation. Respiratory rate showed significant increase when 679o of Nitrous oxide was administrated. There was no side effect in administration of 33% of Nitrous oxide. When 50% of Nitrous oxide was administrated, there were dizziness 5(25%), paresthesia 2(10%), nausea 1(5%), palpitation 1(5%) and hyperethesia 1(5%). At 679o of Nitrous oxide, there were nausea 7(35%), dizziness 6(30%), paresthesia 5(25%), headache 5(25%), diplopia 2(10%), dysphoria 2(10%), salivation 1(5%), dyspnea 1(5%), nystagmus 1(5%), tinnitus 1(5%) and euphoria1 (5%). Incidence of side effect increased as the concentration of Nitrous oxide was increased. CONCLUSION: We can conclude that safe and effective concentration of Nitrous oxide is 50% as analgesics and sedative drugs in the emergency department.

6.Radiologic Assessment of Postoperative Stability in Unstable Intertrochanteric Fracture Using Lateral Radiograph.

PURPOSE: The purpose of this study was to compare the sliding distance of lag screw in patients with unstable femoral intertrochanteric fractures treated with intramedullary fixation using a cephalomedullary nail with a fixed angle between the neck and shaft of the femur in relation to reduction type by lateral radiographs. MATERIALS AND METHODS: Between January 2009 to October 2013, 86 cases (86 patients) with unstable femoral intertrochanteric fractures were treated with intramedullary fixation using a metal nail with a fixed neck-shaft angle and followed for at least 6 months. We used AO/OTA classification, and all cases were unstable fractures. Twenty cases were 31-A22, 54 cases were 31-A23, and 12 cases were 31-A3. There were 30 men and 56 women. Average patient age was 73.7 years (range, 47-97 years). We classified reduction types into three groups as postoperative lateral radiologic findings. Group 1 showed no displacement, group 2 showed anterior displacement of the femur neck, and group 3 showed posterior displacement of the femur neck. The radiological assessment compared the sliding distance of the lag screw between postoperative X-ray and last follow-up X-ray. RESULTS: Forty-two cases were in group 1, 22 cases were in group 2, and the other 22 cases were in group 3. There was no significant difference in the patient characteristics of each group. The sliding distances of the lag screw were 4.9±3.2 mm, 4.6±3.6 mm, and 8.5±4.9 mm, respectively, and group 3 showed a significant result (p<0.0001, p=0.024). CONCLUSION: In cases treated with intramedullary fixation using a cephalomedullary nail with a fixed neck-shaft angle, appropriate reduction with a lateral radiograph before screw fixation is needed to prevent excessive lag screw sliding.

Hyperlipidemia is a rare cause of pancreatitis. It has been believed that free fatty acids released from hydrolyzed serum chylomicrons or triglycerides and chylomicrons induce hyperlipidemic pancreatitis by damaging acinar cells and capillaries. Type I, IV or V hyperlipidemic (Fredrickson's classification) pancreatitides have distinctive features of increased and heightened serum chylomicron and triglyceride levels. In contrast, type IIb hyperlipidemia usually doesn't have increased chylomicrons. It is a dominant inherited genetic disorder and doesn't manifest the subjective symptom before combining vascular complications such as coronary artery disease. Only a few cases of type IIb hyperlipidemic pancreatitis have been reported. We experienced a male patient with recurrent hyperlipidemic pancreatitis combined with type IIb hyperlipidemia. We present the case report and a review of the literature of hyperlipidemic pancreatitis, especially cases in Korea.

8.Design of a medical image processing software for clinical-PACS.

Software modules for interactive display, manipulation and retrieval of medical images have been designed for a Picture Archiving and Communications System (PACS). The target of these modules is not for a high-end diagnostic workstation for radiologists, but for a PC-based low cost clinical workstation for a referring physician. This software is constructed based on a concept of an object-oriented language which is designed to be modular and expandable. It consists of several functional modules: (a) a communication module for image retrieval, (b) a standard module for the interpretation of the DICOM images, (c) a user interface module for the non-computer oriented clinicians and (d) a tool module for viewing and manipulating images as well as editing the annotation.

9.Design of Mobile Emergency Telemedicine System Based on CDMA2000 1X-EVDO.

Using a high bandwidth cellular communication network is appropriate to implement a high-quality mobile emergency telemedicine system. In Korea, the commercial service of CDMA2000 1X-EVDO has been providing since 2002. In this paper, we designed the system that transfers the biological signal and the video information of a patient simultaneously based on this CDMA2000 1X-EVDO network environment. In CDMA2000 1X-EVDO, the maximum speed of its reverse link can be observed within 153.6Kbps. Before the system design, several field tests had been performed using commercial CDMA2000 1X-EVDO reverse link with the UDP data segments. The test had been taken under several velocity and tunnel areas of Seoul. With the test result we implemented an efficient emergency telemedicine system fitted to the features of CDMA2000 1X-EVDO reverse link using UDP packets. Additional header information is added to the UDP packet data. With the header information the emergency system can transmits the ECG signal prior to the video data and controls the transmission error. The designed system has the ability to transmit both the biological signals and MPEG4 video of 640x480 spatial resolution at the same time. We set up the ambulance with this system and test it on the road.

10.Two Cases of Apathetic Hyperthyroidism associated with Peripheral Eosinophilia.

Peripheral eosinophilia is perpetually associated with many disease conditions like asthma, parasitic disease, IL-2 therapy, hypereosinophilic syndrome and eosinophilia/myalgia syndrome. Furthermore, peripheral eosinophilia may also be linked with Graves' disease, which is induced by thyroid-stimulating anti-TSH receptor antibodies and is related to type 2 helper T cell(Th2) predominant condition that is dependent on humoral immunity. In some of the cases of Graves' disease, thyrotoxicosis may induce peripheral eosinophilia associated with relative cortisol deficiency. In our present study, we present a case of two patients who were suffering from Graves' disease without any thyrotoxic symptoms and exhibited increased levels of peripheral eosinophil count. But, the count was observed to decreas in euthyroid state.
