1.A Case of Intestinal Malrotation Complicated by Midgut Volvulus: Diagnosis with Abdominal CT Scan.

Intestinal malrotation is a constellation of a wide spectrum of embryologic failures of rotation and fixation of the gut, resulting in a narrow-based attachment of the mesentery and the presence of abnormal peritoneal (Ladd's) band. These abnormalities predispose to midgut volvulus, intestinal obstruction and internal herniation. We experienced a case of intestinal malrotation complicated by midgut volvulus in a 4-year-old girl who presente with abdominal pain, abdominal distension and melena. The diagnosis was made by abdominal CT scan, which showed "whirl sign." The diagnosis was confirmed by surgery. A brief review of literature ensues.

3.A Clinical Study of Diaphyseal Fractures of the Radius and Ulna

Diaphyseal fractures of the radius and ulna present high incidence of malunion and non-union because of difficulty in reduction and maintenance of two mobile, parallel bones in the presence of the pronating and supinating muscles which exert angulatory as well as rotational forces. Over the years various methods of open reduction and internal fixation or the closed method have been advocated, but open reduction, especially fixation with compression plate and screws resulted in more satisfactory results was proved recently by the major clinical reports. From March. 1977 to December. 1985, 59 cases of diaphysesl fractures of the forearm both bone in adult and children were managed in the Department of Orthopedic Surgery, Chonbuk National University Hospital and comparisions have been made between the conservative treatment group and operative group. The obtained results were as follows; 1. The most common csuse of injury was fallen on the outstretched hand in children and traffic accident in adult. 2. The most frequent site of the fracture was middle one-third(50.8%). 3. The most common pattern of the fracture was stable fracture(50.1%). 4. The applied methods for management of the fractures include cast immobilization after manual reduction or skeletal traction, K-wire fixation, compression plate and screws, intramedullary nailing, and combined. 5. Comparision of conservative treated group with operative treated group was as follows. 1) In roentgenologically, excluding those complicated by nonunion or infection, average bone union time of conservative group in children and adolescence was 10.2 weeks and in adult was 16.7 weeks, and of operative group was 13.9 weeks. 2) According to Grace and Eversmann's evaluation, Satisfactory results were shown in 80.0% of children and adolescence group treated by conservative method and 86.7% of adult group trested by compression plate and screws. 3) Among the various internal fixstion methods, compression plate and screws method was the best measure for rapid bone union and functional recovery in adult. 4) Any marked difference between each group was not seen in nonunion, but angulstory and rotational deformity were more common in conservative group and infection was developed 2 cases in operative group only. 6. There was no noticesble difference between each treatment in children and adolescence, but operative treatment, especially compression plate and screws, in adult was more superior to conservative treatment with. regard to bone union time and functional result.

4.Modality of malocclusion according to regional difference using HLD(handicapping labiolingual deviation), TPI(treatment priority index) and HMAR(handicapped malocclusion assessing record).

We tried to evaluate frequencies and severities of malocclusion of various socioeconomic areas according to growth and development. To obtain objective validity, we used Handicapping Labiolingual Deviation (HLD) suggested by Draker, Treatment Priority Index (TPI) by Grainger, Handicapped Malocclusion Assessing Record (HMAR) by American Dental Association and American Association of Orthodontist. Seoul and Kwangju were selected as an urban group, Ahnyang as an middle socioeconomic group and Hwasun near the Kwangju area as an rural group. 140 (male 63, female 73) of 5th grade in 'E' elementary school students in Seoul, 202 (male 101, female 101) of 'S' elementary school students in Kwangju, 207(male 105, female 102) of 'H' elementary school students in Hwasun, and 100(male 49, female 51) of 'M' elementary school students of in Ahnyang of all the same grade were analyzed and we obtained the results as follows; 1. TPI, HLD, and HMAR showed high coincidence and reproducibility between different observer. (p<0.01) 2. In comparison of TPI, HLD, and HMAR according to clinical decision, there was statistically difference between each other. (p<0.01) 3. As the severity of malocclusion increases there was high correlation between TPI and HLD, and between HLD and HMAR (p<0.05) 4. With respect to differences between areas in TPI and HLD, there was statistically difference between Ahnyang and other areas. 5. In Seoul as an urban group there was a high incidence of periodic checkup and history of orthodontic treatment than other area. 6. Mode of feeding and posture during sleeping did not affect the severity of malocclusion. As a conclusion, TPI, HLD and HMAR are so reproducible and coincident that they can be used as a guide on evaluating the frequency and severity of malocclusion and determining the priority of orthodontic treatment to determine the need and supply of orthodontic treatment. Furthermore these indices can provide objectively valid data for establishing public health problem solution.

6.Application of Family Treatment Camp in Treatment of ADHD.

OBJECTIVES: Demands for comprehensive and intensive treatment programs for treatment of children with attention-deficit hyperactivity disorder (ADHD) who suffer from serious impairment in various psychosocial areas are increasing. The aim of this study was to evaluate the possibility of developing new types of camp programs focused on improving social adjustment of children and helping parents effectively manage their children's problem behaviors. METHODS: Fifteen children diagnosed as ADHD according to Diagnostic and Statistical Manual of Mental Disorders-IV (mean age 9.5 years) and their parents participated in this program. Eight consecutive camps were held at two-week intervals, from Jun 2007 to Oct 2007 in an adolescent training center located at Chungtaesan, in Korea. Each camp program included social skills training in the classroom and outdoor activities in the forest for children, as well as parent training for mothers. In addition, after the sixth camp, we conducted one parental session for fathers and a three-day booster family camp three months later. Finally, eleven families adhered to the program. RESULTS: All children, parents, and therapists rated questionnaires or checklists at baseline, intra-, and post-treatment. Parenting burden rated using the Parenting Stress Index showed a significant decrease, from 57.89 to 46.22 (p=.019). And, overprotection rated using the Parenting Bonding Instrument showed a significant decrease, from 16.56 to 12.44 (p=.046). However, no significant improvement in children's behavioral and emotional problems was observed. CONCLUSION: In this study, the consecutive ADHD family camp program was effective for empowerment of parental competency, but not for general improvement of children's behavioral and emotional symptoms.

8.Surgical Treatment of Supra and Intercondylar Fracture of the Femur.

We performed a retrospective analysis of the results of surgical treatment of sixteen patient with comminuted fracture of the distal femur from September 1993 to August 1995 and evaluated the efficacy of the angled blade plate as one of the fixation devices used. The fractures were limited to C2 and C3 fracture classifed according to the AO classification. After an average follow-up of 13 months (range, 9 to 24 months), all of the fractures had healed. Postoperatively, the arc of flexion was limited between 90degrees and 125degrees in eight, normal arc in five, and severely restricted arc of joint motion was present unexceptionally in three patients with deep infection. In angular deformity, average 5.8degrees of valgus and 2.4degrees posterior angulation were showed. The functional results by Sanders-Swiontkowski-Rosen-Helfet rating system were showed excellent in 3, good in 7, fair in 3, and poor in 3 cases. The overall results were seen to be excellent to good in ten cases (62.5%) but in twelve patients using the angled blade plate, excellent to good results were showed 10 patients (83.3%). We attribute the favorable results in our series to restoration of the articular surface through the modified extensile approach, sufficient stable fixation using the angled blade plate with conforming to the concept of bridge plate, routinely performing the bone graft for medial buttress and bone healing, and early mobilization of the extremity.

9.A statistical study of dental crowding with dental cast analysis.

Crowded group is composed of 60 subjects who visited Yonsei University for orthodontic treatment and has no history of orthodontic treatment. Noncrowded group is composed of 26 subjects who has no crowding, considered to have a normal occlusion and no history of orthodontic treatment. Currently available and approved analytic method was used. Interrelationship between tooth size and arch size, and it's correlation on tooth crowding was studied and the following results were obtained. 1. In comparison of sum of mesiodistal width, of crown size of teeth was greater in noncrowded group than that of crowded group on both arch. (p<0.01). 2. In comparison of arch lengths, the values measured from arch length 2 showed greater in non crowded group (p<0.01). Also in crowded group, arch length 1 showed greater value than arch length 2 on both arch (p<0.01). 3. In comparison of arch widths, upper interlateral (p<0.01) and upper intercanine width (p<0.05) showed greater value in noncrowded group on upper arch and intermolar width showed greater value in noncrowded group on both upper and lower arches (p<0.05). 4. In comparison of arch perimeters, arch perimeter 2 was greater value in noncrowded group on both upper and lower arches (p<0.01). 5. Dentoalveolar disproportion was greater in noncrowded group on both upper and lower arches (p<0.01). 6. In lower incisors MD/FL ratio, the central and lateral incisors were greater in crowded group, but statistial significance was only in lateral incisors (p<0.01) 7. The irregularity index of lower incisors showed greater value in crowded group (p<0.01).

10.A study of root resorption and alveolar bone changes during tooth movement after treatment with etidronate disodium.

Many studies has been conducted concerning prevention of unnecessary complications such as root resorption during orthodontic tooth movement under various mechanical forces. Nowadays, the cause of the root resorption is not thought to be confined only to mechanical forces. But the factor that affects bone metabolism are thought to be major one of the predisposing factors. The light microscope and scanning electron microscope were used to the effects of 60gm, and 100gm of tipping force on root resorption of cats, which were treated with Etidronate disodium. The results were as follows: 1. In the 60gm control group, hyalinization on the compression site of periodontal ligament appeared after first week and second week. In the 60gm experimental group, it appeared after first week with low frequency. In the 100gm control group it appeared with high frequency by first and second week while in 100gm experimental group, it appeared with low frequency. 2. In the 100gm control group, resorption of the cementum and the alveolar bone rapidly increased after second week. In the 60gm experimental group, resorption or formation of alveolar bone and cementum didn't appear all through the experimental period. 3. In the 100gm control group. formation of cementum and alveolar bone appeared after first week while in the 100gm experimental group, formation of cementum and alveolar bone appeared after second week and fourth week respectively. In the 60gm control group, formation of the cementum didn't appear all through the experimental period. 4. In the control group, the root resorption of 100gm group was higher than that of 60gm group after second week, while in experimental group, root resorption didn't appear regardless of the forces.
