1.Mucinous cystadenoma of the liver with ovarian-like stroma: the need for complete resection.

Cystadenoma of the liver is a rare neoplasm. Although many cystadenomas are asymptomatic, symptoms can include abdominal pain, postprandial epigastric discomfort, and nausea. Dramatic changes in hepatic imaging techniques have been helpful for diagnosing cystic lesions of the liver, such as simple cyst, hydatid cyst, cystadenoma, cystadenocarcinoma, and metastatic neuroendocrine tumors. However, it remains difficult to differentiate cystadenoma from cystadenocarcinoma for multiseptated cystic hepatic lesions with papillary projection on computed tomography (CT) and magnetic resonance imaging (MRI). Here we report the case of a 47-year-old woman with several months of postprandial discomfort and abdominal fullness. CT and MRI revealed multiseptated cystic lesions with papillary excrescences. A left hemihepatectomy was performed. Histology showed a benign mucinous cystic tumor with ovarian-like stroma.

2.Correction of Secondary Alveolar Cleft with Gingival Mucoperiosteal Flap and Iliac Bone Grafting:Use of a Percutaneous Bone Biopsy Set.

Correction of alveolar cleft with bone grafting has become a well-established step in the integral management of cleft lip and palate patients. Secondary bone graft at mixed dentition is a widely accepted protocol and iliac bone graft is recommended as the gold standard by a multitude of cleft centers. However, grave morbidities of the iliac donor site have discouraged cleft surgeons from adopting this reliable method. In this study, percutaneous bone biopsy set was used to avoid the morbidities of conventional iliac bone graft. Iliac bone graft was performed on 20 patients with alveolar cleft during the period from January 1995 to February 1999. The tube saw of the bone biopsy set was introduced through a small incision of less than 1 cm, and cancellous bone cores were harvested from the iliac crest. After the pericoronal incision, a wide gingival mucoperiosteal flap was elevated at both sides of the cleft. A soft tissue pocket that was created by closure of the nasal and palatal lining was filled with harvested bone cores. Medial transposition and water-tight closure of both gingival mucoperiosteal flaps provided coverage of the bone graft. During the follow-up period from 5 to 50 months (mean, 29 months), completion of the goals of the alveolar bone graft was observed. There was no serious complication except for one case the exposure of the bone graft, and this was resolved spontaneously. Bone harvest with a percutaneous bone biopsy set enables early ambulation by reducing postoperative pain, and it may be faster and simpler than any other method introduced so far. Furthermore, blood loss was minimal, while the scar was small and acceptable. Iliac bone graft with percutaneous bone biopsy is a reliable method for correction of the alveolar cleft and it has many advantages over conventional iliac bone graft and other sources of bone.

5.A Clinical Study of Benign Epilepsy of Childhood with Centrotemporal Spikes.

Benign epiepsy of childhood with centrotemporal spikes (BECCT) is an electroclinical syndrome characterized by noctural seizure that remit spontaneoully before adulthood, and belong to idiopathic age and location related epilepsies. We reviewed the medical records to analyse the seizure the seizure pattern, and also inspect the EEG recording to identify topography of the epileptiform discharge of 24 patient who met the following criteria: 1) presence of nocturnal seizure or partial seizure confined to the face, 2) normal intecual and development, 3) normal background EEG feature, 4) monomormhic spikes or sharp wave that increase in the frequency during sleep if those state were achieved. The result as follow: 1) The age of seizure onset were 3 to 11 years old (7.08 1.89) and male to female ratio was 1, 4:1. 2) Past history of birth trauma, CNS infection and other diseases involving CNS, were not found. There was past history of febrile convulsoin 33.3% of patient and epilepsy in 8.3%. One of paient's sister had childhood abscence epilepsy and never a nocturnal partial seizure, also and had centrotemporal spikes in EEG. 3) In 91.7% of Seizures occurred during sleep, especially short after sleep onset and the symptomatology of seizure was shown 66.7% of oropharyngeal sign, such as hypersalivation and gargle sound, and 54.2% of hemifacial involvement, 33.3% of speech arrest, 25.5% of upper limb involement, 18.3% of lower limb involvement, and 45.8% of general seizure. 4) In 91.5% of patients had on location of epileptiform discharge that located central (37.5%) midtemporal (33.3%), parietal and frontal. We could observe tangental dipole in 16.7% of patients.

6.Observational Analysis of Korean Society of Spine Surgery Annual Congress: (2001-05) Free Paper: From Presentation to Publication.

STUDY DESIGN: A review of all presentations at the Korean Society of Spine Surgery(KSSS) congress from 2001 to 2005. OBJECTIVES: To determine the presentation to publication rate in peer-reviewed journals of abstracts presented at the KSSS congress and evaluate the consistency between the presented abstract and published full-text counterpart. SUMMARY OF LITERATURE REVIEW: The rate of publication for presentation at international meetings has been determined but there has been no study for the KSSS. MATERIAL AND METHODS: This study reviewed all podium presentations of the past KSSS Spring and Fall meetings from 2001 to 2005. A computer search for each abstract was performed. The final published journal, time to publication and publication rate were evaluated according to subspecialty. The published full-text article was compared with the original abstract and evaluated for consistency with respect to the author, material and method, and result. RESULTS: A total of 288 free-papers were identified, of which 167 of them were published as full-text articles. The mean time to publication was 12.15 months. The publication rates according to the subspecialty were basic 67%, lumbar 68%, cervical 64%, deformity 50%, trauma 53%, minimal 14% and others 51%, respectively. CONCLUSION: The presentation to publication rate of the Korean society of spine surgery podium presentations was lower than other international meetings but the articles showed a high consistency rate.

7.Detection of Micrometastases of Breast Cancer by Immunohistochemical Analysis of Cytokeratin.

PURPOSE: Axillary lymph node metastases are the single most important predictor of overall survival in patients with breast cancer. Micrometastases are defined by the American Joint Committee on Cancer as tumor foci less than or equal to 2 mm in greatest dimension. Recently, up to 30% of breast cancer patients were reported to have micrometastases. In this paper, to assess the rate of micrometastases in patients with stage I breast cancer, we attempted to determine the most useful marker of the micrometastases in node negative cases by routine histopathologic examination of regional lymph nodes and comparison of the results with the influencing factors on prognosis. MATERIALS AND METHODS: We performed immunohisto chemical staining for pancytokeratin, cytokeratin 7, cytokeratin 20 and CEA to identify which protein was the most useful marker for the detection of micrometastases in 86 node negative cases and determined the correlation between histological and clinical data. RESULTS: A total of 5 lymph nodes in 5 separate cases showed micrometastases among the total 1,296 lymph nodes and 86 cases. The rates of micrometastases of lymph nodes and cases were 0.38% and 5.8%, respectively. The tumor type of micrometastasis was infiltrating ductal carcinoma in all cases. None of the microme tastases cases showed any relationship with tumor grade, tumor size, expression of ER and PR, patient survival rate or recurrence rate. The most useful marker to detect micrometastases was pancytokeratin. CONCLUSION: The results of this study indicate that micrometastasis of axillary lymph nodes does not carry any independent prognostic significance.

8.Sedative - Analgesic Effect with Diazepam - Fentanyl for Extracorporeal Shock Wave Lithotripsy.

Extracorporeal shock wave lithotripsy(ESWL) for urinary calculi is usually performed under general anesthesia, regional anesthesia or intravenous anesthesia. We evaluated the sedativeanalgesic effeet and untoward effects of diazepam-fentanyl for ESWL. 60 patients were belonged to physical status 1 or 11 of ASA classification who injected diazepam(5~10 mg) and fentanyl(1.5 ug/kg) at 2 minutes were as follows The results were as follows; 1) Mean arterial pressure(MAP) was significantly decreased in 3-10 minutes after injection compared to baseline value. 2) Heart rate(HR) was statistically nonsignificant but slightly decreased from 2 minutes after injection. 3) Respiratory rate(RR) and arterial oxygen saturation(SaO2) were significantly decreased until 15 miuntes after injection but SaO was not decreased below 92.7% and RR was not decreased below 13 rates/minute. 4) Pain and movement during ESWL developed in 18 cases but repositioning and discon- tinuation of EWSL were not necessary. Episodes of desaturation(SaO2<90%) developed in 2 cases. 5) Postoperative dizziness developed in 24 cases. nausea and vomiting developed in a few cases. We concluded that intravenous administration of diazepam-fentanyl is more convenient and simpler than other anesthetic technique for ESWL.

10.Expression of Survivin and KAI-1 in Gastric Adenocarcinomas.

PURPOSE: The aim of this study was to investigate the impact of survivin expression and the decrease or loss of KAI-1 on the clinical stage and the survival rate in gastric adenocarcinomas. MATENRIALS AND METHODS: Expressions of survivin and KAI-1 were immunohistochemically determined in 40 cases of gastric adenocarcinomas. The survivin and KAI-1 expressions were also analyzed by using western blots in 14 cases among them. RESULTS: Resected gastric cancer specimens from 40 patients (intestinal type: 15 cases and diffuse type: 25 cases) were evaluated immunohistochemically. Survivin protein expressions were significantly higher in diffuse types (P=0.03) and in advanced clinical stages (UICC TNM II and III, P=0.02). In contrast, a decrease or loss of KAI-1 expression had no statistically significant correlation with the Lauren classification or the clinical stage. Survivin protein positivity was associated with an unfavorable prognosis. Decrease or loss of KAI-1 was associated with a shorter disease free survivial rate (P<0.01). The western blot data (n=14) indicated that neither survivin protein over-expression nor KAI-1 down-expression had an significant correlation with the Lauren classification or the clinical stage. CONCLUSION: In gastric carcinomas, survivin over-expression and decrease or loss of KAI-1 were associated with unfavorable prognosis, being independent prognostic factors along with the clinical stage and the disease free survival rate.
