The purpose of this study was to verify the split-crest technique experimentally for successful implantation at alveolar bone having unfavorable condition. Using inferior border of the mandible of the canine, we made comparable study about the state of osseointegration between conventional technique and split-crest technique. We set experimental group which was implanted at inferior border of the mandible of the canine by split-crest technique using the fixture of 3.75 mm width and 8mm length, and set control group which was implanted by conventional technique at the counter area of the mandible. The experimental animal was sacrificed at 1, 4, 8 and 16 week. We observed the changing process of bone formation following implantation with stereoscopy, light microscopy, electron microscopy and fluorescent microscopy, and studied histomorphometrically. Histologic results were as follow : 1. In control group, a bit of new bone formation was initiated on a portion of bone defect area at 1 week. The initiation of osseointegration between fixture and new bone was seen at 4 week. New bone tissue with normal shape and structure formed and filled defect area at 8 week. But complete bone remodeling was attained at 16 week. 2. In experimental group, bone formation around fixture was going on actively. But the shape and structure of new bone area was more irregular than that of control group, and bone density was also lower than that of control group. Active new bone formation was still observed at 16 week. 3. The osseointegrated new bone was remodeled to cancellous bone having trabeculae and marrow space composed of compact lamellar bone. 4. In fluorescent microscopic analysis, active bone formation was seen between 4 weeks and 8 weeks at control group. Otherwise, along the all experimental period new bone formation was observed evenly at experimental group. 5. Both control and experimental group, normal osseointegration was accomplished without bone resorption which is essential factor in split-crest technique. As previous results, when implantation using split-crest technique at alveolar bone attenthaving unfavorable condition, early bone formation was delayed slightly, but we could get attentive results along the long term period.

The common bile duct and the duct of Wirsung cojoin at the level of the duodenum, forming the major. papilla of Vater. Existence of a double major papilla, i.e., two neighboring independent papillary structure:, is infrequent. In our endoscopy unit we have experienced one case of double papilla of Vater wherein canulation of the common bile duct and pancreatic duct could be accom plished through either orifice independently.

Numerous cause of the carpal-tunnel syndrome have been reported, but we could not find a reference for tuberculosis of the median nerve causing this syndrome. Therefore, we are presenting the case report of this rare condition as another cause of the carpaltunnel syndrome.

The Acromioclavicular joint is classified as a diarthrodial joint. Its stability is maintained by articular capsule, acromioclavicular ligament, articular disc, coraco clavicularligament(trapezoid and conoid ligament) and attached muscles. 17 cases of acromioclavicular dislocation which had been seen at Seoul Red Cross Hospital during the period of January 1975 through December 1977 were studied. The following observations were made. 1. This injury is more prevalent in male (82%) with the peak incidence in the 3rd and 4 th decades (53%). 2. Conservative treatment was given in 11 cases by adhersive strapping around the elbow and lateral one third of the clavicle reinforced by Velpeau dressing for 3 weeks, then followed by active exercise. 3. Operative treatment consist of modified Phemister technique in 3 cases, Mumford technique in 2 cases, and Dewar and Barington's technique in 1 case. 4. Results were evaluated in 4 groups as excellent, good, fair, and poor according to Gerald weitzman's classification(1975). 5. Overall results of more than good (satisfactory) was obtained in 72.7% of the 11 cases with conservative treatment and in 50% of the 6 cases with operative treatment.

We performed 75 cases of daryocystorhinostomy from Jan. 1988 to Dec. 1990. A routine dacryocystography was performed prior to operation in all cases. A modified T oti's method was applied as a surgical technique "H" shaped incision was done. Both anterior and posterior flaps were sutured adequately and vaseline guaze was used as a plugging agent for 7 to 10 days Success was achieved in 72(96.0%) of 75 cases. The causes of 3-failures were organized granulation tissue and trauma.

Two cases of hyperlipoproteinemia is reported, Hyperlipoproteinemia is characterized by various kinds of cutaneous xanthomatosis associated with or without arteriosclerotic changes due to increasing lipoprotein fraction of the serum. Case I . 37-year old housewife had multiple, pea to nut sized, various shaped, cutaneous and subcutaneous nodular masses has been developed last 3 years. Some of her family member have similar symptoms. Serum cholesterol level was 790 mg% and serum triglyceride level was 270 mg% after 14 hour fasting. Case II. 52-year old housewife was suffered from pea sized, multiple, subcutaneous nodular and cutaneous eruptions last 2 years. There was no family histories. Serum cholesterol level was 895 mg% and serum triglyceride level was 350 mg% after 14 hour fasting. Typical Touton giant cells were found in biopsy specimens taken from cutaneous lesons. ECG, X-ray, glucose toleranc test and other laboratory examinations showed within normal limits. Tne agarose electrophoresis pattern of serum lipoprotein fractions are very similiar with that of hyperlipoproteinemia type II. These two patients were treated with nicotinic and 1. 0 gm and clofibrate 2.0 gm daily by mouth and restricted animal fatty foods. Total serum cholesterol level was markedly decreasea 2 months after treatment in case I.
